Increased hepatic expression of insulin-like growth factor-Ⅰreceptor in chronic hepatitis C

AIM： Although increased insulin-like growth factor-I receptor （IGF-IR） gene expression has been reported in hepatocellular carcinoma, studies assessing IGF-IR in chronic hepatitis C （CHC） and cirrhosis are scarce. We therefore aimed to evaluate IGF-IR and IGF-I rnRNA expression in liver from patient with CHC. METHODS： IGF-IR and IGF-I rnRNA content were determined by semi-quantitative RT-PCR and IGF-IR protein expression was determined by immunohistochemistry in hepatic tissue obtained from patients with CHC before （34 patients） and after （10 patients） therapy with interferon-α and ribavirin. ResULTS： An increase of IGF-IR rnRNA content was observed in hepatic tissue obtained from all CHC patients as well as from 6 cadaveric liver donors following orthopic transplantation （an attempt to evaluate normal livers） in comparison to normal liver, while no relevant modifications were detected in IGF-I mRNA content. The irnrnunohistochemical results showed that the raise in IGF-IR rnRNA content was related both to ductular reaction and to increased IGF-IR expression in hepatocytes. A decrease in IGF-IR rnRNA content was observed in patients who achieved sustained virological response after therapy, suggesting an improvement in hepatic damage. CONCLUSION： The up-regulation of IGF-IR expression in hepatocytes of patients with CHC could constitute an attempt to stimulate hepatocyte regeneration. Considering that liver is the organ with the highest levels of IGF-I, our finding of increased IGF-IR expression after both acute and chronic hepatic damage highlights the need for additional studies to elucidate the role of IGF-I in liver regeneration.

Etiology of Fungemia in Neonates: Four Decades of Case Reports

Objectives: To compare groups of etiologic agents for neonatal fungemia based on previously published case reports with regard to case outcomes. Methods: Secondary analysis of data from case reports published in scientific literature indexed in Medline, Scopus and LILACS databases, and starting from the year, full texts were registered until the year 2010. Results: Thirty-four species of fungi were isolated in 252 cases reported over a 40-year period. The distribution of fungal groups according to the development level of a case’s region of origin was very uneven (p Aspergillus sp.) in more developed countries. However, 73% of the chi-squared distribution corresponded to differences observed in the frequency of Candida sp. and non-albicans Candida. While the frequency of candidemia by C. albicans was progressively declining in the period from 1966 to 2010, reports of non-albicans candidemia and other non-Candida fungi became significantly (p albicans Candida or non-Candida fungi were isolated. Conclusion: fungi of the genera Candida and Aspergillus were the etiologic agents most frequently identified in published cases of neonatal fungemia;the outcome of death was more often related to cases where non-albicans Candida or non-Candida fungi were isolated.

Hydrological Processes and Climate Change in Hydrographic Regions of Brazil

The objective of this work is to assess the impacts of IPCC AR5 climate change scenarios on water resources and hydrological processes across the entire Brazilian territory. Hydrological simulations are carried out in total drainage area of about 11,535,645 km2 and average stream flow of about 272,460 m3/s. The study area consists of different climates and land covers such as the Amazon Forest, Northeast Semiarid, Brazilian Savannah, Pantanal wetlands and temperate climate in the South. The atmospheric forcing to drive the large-scale hydrological model MGB-IPH is derived from the downscaling of two global climate models, HadGEM2-es and MIROC5, by the Eta Regional Climate Model, at 20 km resolution. The Eta model provided the downscaling of the baseline (1961-1990) and three time-slices (2011-2040, 2041-2070 and 2071-2099). These projections adopted two emission scenarios, the RCP 4.5 and RCP 8.5. The change in the average and extremes of precipitation, evapotranspiration, rates of river discharge and soil moisture were assessed. The simulations showed the response of the hydrographic regions due to change of precipitation and potential evapotranspiration in the scenarios. Water availability decreases in almost the entire study area (exception for the South) and the major basins for hydroelectric power generation are affected. The Northwest, Amazon and a small area along the Northeast Atlantic coast exhibited intensification of the extremes discharges, where the anomaly is positive for high-flow (Q10) and negative for low-flow (Q95). The results highlight the most climatic sensitive regions in Brazil in terms of hydrological variables and water resources.

Enamel microabrasion: An overview of clinical and scientific considerations

Superficial stains and irregularities of the enamelare generally what prompt patients to seek dental intervention to improve their smile. These stains or defects may be due to hypoplasia, amelogenesis imperfecta, mineralized white spots, or fluorosis, for which enamel microabrasion is primarily indicated. Enamel microabrasion involves the use of acidic and abrasive agents, such as with 37% phosphoric acid and pumice or 6% hydrochloric acid and silica, applied to the altered enamel surface with mechanical pressure from a rubber cup coupled to a rotatory mandrel of a lowrotation micromotor. If necessary, this treatment can be safely combined with bleaching for better esthetic results. Recent studies show that microabrasion is a conservative treatment when the enamel wear is minimal and clinically imperceptible. The most important factor contributing to the success of enamel microabrasion is the depth of the defect, as deeper, opaque stains, such as those resulting from hypoplasia, cannot be resolved with microabrasion, and require a restorative approach. Surface enamel alterations that result from microabrasion, such as roughness and microhardness, are easily restored by saliva. Clinical studies support the efficacy and longevity of this safe and minimally invasive treatment. The present article presents the clinical and scientific aspects concerning the microabrasion technique, and discusses the indications for and effects of the treatment, including recent works describing microscopic and clinical evaluations.

The order Gobiiformes is made up of more than 2200 species,representing one of the most diverse groups among teleost *** biological causes for the tachytelic karyotype evolution of the gobies have not yet been fully *** we expanded cytogenetic data for the Eleotridae family,analyzing the neotropical species Dormitator maculatus,Eleotris pisonis,Erotelis smaragdus,and Guavina *** addition,a meta-analytical approach was followed for elucidating the karyotype diversification versus biological aspects(habitat and egg type)of the *** species *** and *** present 2n=46 acrocentric chromosomes(NF=46),*** 2n=46(36sm+4st+6a;NF=86),and ***,the most divergent karyotype,with 2n=52 acrocentric chromosomes(NF=52).Besides numeric and structural diversification in the karyotypes,the mapping of rDNAs and microsatellites also showed noticeable numerical and positional variation,supporting the high chromosomal evolutionary dynamism of these *** Gobiiformes,karyotype patterns which are more divergent from the basal karyotype(2n=46a)are associated with characteristics less effective to dispersion,such as the benthic *** adaptive characteristics,connected with the organization of the repetitive DNA content in the chromosomes,likely play a synergistic role in the remarkable karyotype diversification of this group.

Opportunistic Usage of Maritime VHF Band—Deployment Challenges for a New Regulatory Framework

similarly to other domains, maritime community requests for broadband services have been significantly increasing. Worldwide navigation footprint and the lack of practical alternatives to Satellite Communications (SATCOM) empower VHF band as the natural choice to support most of those demands. Nevertheless, the major challenge for an implementation of maritime broadband VHF services is unquestionably the spectrum availability and management. Eventually, the solution must include spectrum sharing, using a Cognitive Radio (CR) based approach, but unfortunately current regulatory framework and spectrum management regime are not appropriate for such concepts and emerging technologies. To overcome such constraints, it is necessary to address a whole field of regulatory and standardization issues in order to prepare an evolution towards a more flexible and dynamic approach to spectrum management and a transition that would ensure incumbents live operations and legacy systems. The required paradigm change encompasses a new policy definition, an enforcement mechanism implementation and a comprehensive transition plan. The presented analysis pretends to address the regulatory feasibility of a framework change, discusses its evolving process and points some challenges related with practical aspects associated to Quality of Service (QoS) enforcement metrics definition, centering the arguments in maritime VHF band.

Gastric Bypass with Long Pouch and Transit Bipartition for Endoscopic Access to the Remaining Stomach

Introduction: obesity has a complex and multifactorial etiology, difficult treatment and increasing incidence rates in recent decades. The treatment involves clinical and pharmacological approaches and, in case of lack of results, surgical interventions. Roux-en-Y gastric bypass (RYGB) is one of these surgical interventions in which the stomach is divided, creating a small pouch, and the remaining portion of the stomach become excluded and left without endoscopic access. Objective: to evaluate the results of modified RYGB with long pouch and endoscopic access to the remaining stomach. Materials and Methods: prospective clinical trial with sample selected among patients seen at the Alberto Rassi General State Hospital of Goiânia (HGG) and indicated for bariatric and metabolic surgery confirmed by the medical and multidisciplinary team. The study was conducted from January 2020 to August 2021. Clinical history and laboratory test results of the selected patients were collected through consultations with the medical and multidisciplinary team. Results: twelve participants were included in the study. Of these, 11 (91.7%) were women and the mean age was 46.3 years. The weight before surgery was 112.17 kg (92.00 - 150.00) and the Body Mass Index (BMI) was 44.89 (35.06 - 74.39). After 18 months of surgery, the mean weight was 80.77 kg (±11.92) and the mean BMI was 29.46 (±11.00), showing a significant reduction in both (p = 0.003 and p = 0.002, respectively). All patients underwent endoscopic evaluation of the pouch, remaining stomach and duodenum at 12 months postoperatively. The mean percentage of lost excess weight loss was 68.21%. Conclusion: We conclude that the proposed changes in RYGB (GBLP + GIB – Roux-en-Y gastric bypass with long pouch and gastrointestinal bipartition) did not compromise weight loss or control of type 2 diabetes and other comorbidities and proved to be a safe and effective alternative without gastroduodenal exclusion, enabling a better postop

Objective:To investigate the isolation of enterobacteria associated with Macrobrachium amazonicum(***) farming and evaluate the in vitro antimicrobial susceptibility of Vibrio ***:Strains were isolated from female *** prawns and environmental and hatchery *** assays were used to identify bacterial genera and those belonging to the genus Vibrio were submitted to further analyses for species identification,through Vitek 2 automated system and *** test was performed according to Clinical Laboratory Standards ***:The following genera of enlerobacteria were recovered:Enterobacter(n=11),Citrobacter(n=10),Proteus(n=2),Serratia(n=2),Kluyvera(n=2),Providencia(n=2),Cedecea(n=1),escherichia(n=1).Edwardsiella(n=1) and BulliauxeUa(n=1).As for Vibrio,three species were identified:Vibrio cholerae non-O1/non-O139(n=4).Vibrio vulnificus(***)(n=1) and Vibrio mimicus(n=1).Vibrio *** minimum inhibitory concentrations values within the susceptibility range established by Clinical Laboratory Standards Institute for almost all antibiotics,except for ***,which presented intermediate profile to ***:Enterobacteria do not seem to be the most important pathogens associated with *** farming,whereas the recovery of Vibrio *** larviculture,with emphasis on Vibrio cholerae and ***,deserves special attention due to their role as potentially zoonotic aquaculture-associated ***,the intermediate susceptibility of *** to ampicillin reflects the importance of monitoring drug use in prawn farming.

Bone Marrow Transplant Long-Term Survivors’ Satisfaction with Quality of Life: Comparison with a Control Group

Purpose: To evaluate the global quality of life (QoL) of survivors with 10-year or more post-transplant, and to identify risk factors that interfere with well-being. Methods: This is a prospective analytic transversal study with 214 survivors of Bone Marrow Transplant (BMT) and 264 healthy people identified among blood donors, treated as the control group, of both sexes, 18 years or older. The protocol includes a demographic-socioeconomic questionnaire, World Health Organization Quality of Life (WHOQOL) and the Karnofsky Performance Status Scale. Results: 53.7% of the survivor group members are satisfied with their QoL. A similar result can be found in the control group (54.2%). Chronological maturity, anxiety, sexual difficulty, and being a provider are factors that interfere negatively in the QoL of male survivors. In female survivors, the risk factors are anxiety, low educational level, not having a stable partner, being a provider, and not being Caucasian. Conclusions: Survivors are as satisfied with their QoL as the control group. QoL is understood as a perceptive process composed of objective (functional and relational capacity) and subjective phenomenon (perceptive composition).
