Ischaemic heart disease(IHD)is a major cause of morbidity and mortality *** there have been major advances in this field,these patients are still a higher risk *** such,strategies to mitigate risk and tailor secondary prevention measures are of the utmost *** rehabilitation(CR),encompassing several domains including exercise training,cardiovascular risk factor optimization,nutritional and psychological assessments,as well as other ancillary interventions has shown to be one of the pillars in the contemporary management of patients with ***,CR is associated with several benefits in this population,ranging from functional capacity to improvements in *** this,there are still several issues concerning the optimal application of CR which are still not fully ascertained,such as lack of referral and completion,as well as questions related to programme design(particularly among patients with multiple comorbidities).In this review,we aim at presenting a pragmatic overview on the current role of CR in the management of individuals with IHD,while also discussing some of the caveats in the current data,as well as future concepts which could help improve the uptake and personalization of this pivotal time-tested intervention.

Cyst fluid glucose: An alternative to carcinoembryonic antigen for pancreatic mucinous cysts

Pancreatic cystic lesions(PCLs) have been increasingly recognized in clinical practice. Although inflammatory cysts(pseudocysts) are the most common PCLs detected by cross-sectional imaging modalities in symptomatic patients in a setting of acute or chronic pancreatitis, incidental pancreatic cysts with no symptoms or history of pancreatitis are usually neoplastic cysts. For these lesions,it is imperative to identify mucinous cysts(intraductal papillary mucinous neoplasms and mucinous cystic neoplasms) due to the risk of their progression to malignancy. However, no single imaging modality alone is sufficient for a definitive diagnosis of all PCLs. The cyst fluid obtained by endoscopic ultrasound-guided fine needle aspiration provides additional information for the differential diagnosis of PCLs. Current recommendations suggest sending cyst fluid for cytology evaluation and measurement of carcinoembryonic antigen(CeA) levels. Unfortunately, the sensitivity of cytology is greatly limited, and cyst fluid CeA has demonstrated insufficient accuracy as a predictor of mucinous cysts. More recently, cyst fluid glucose has emerged as an alternative to CeA for distinguishing between mucinous and nonmucinous lesions. Herein, the clinical utility of cyst fluid glucose and CeA for the differential diagnosis of PCLs was evaluated.

Concrete Compressive Strength estimation by Means of Nondestructive Testing: A Case Study

estimation of the mechanical properties of concrete in an existing structure is possible with the results of surface hardness and ultrasound tests (nondestructive tests (NDTs)). For both the use of correlation curves is necessary, it is established between NDT and the results of the compressive strength of test specimens or extracted from structure. The objective of this study is to produce correlations between the results of surface hardness and ultrasound NDTs and the compressive strength of the structural concrete in the bleachers of a soccer stadium in the city of Cianorte, which is located in the northwest part of the state of Paraná, Brazil. This concrete structure, which is approximately 26 years old, has some defects, such as corrosion, concrete segregation and cracks. Concrete spalling in one of the slabs has recently raised some concern. Another significant issue is the absence of records regarding concreting of the bleachers’ structure. Therefore, mapping the reinforcement was initially performed according to the results of a surface hardness test, as recommended by standard [1]. An ultrasound test was simultaneously performed according to standard [2] for the same points employed in the surface hardness test. The concrete specimens were extracted according to the recommendations of standard [1] to determine compressive strength, perform the NDT and construct the correlation curves for the results. A total of 26 concrete specimens were obtained from all structures of the bleachers. From the methodology and the results of the study, highly reliable equations were obtained from the correlation curves among the compressive strength of the concrete specimens and the values of the surface hardness index and the ultrasound wave propagation velocity.

We consider the problem K（x）Uxx = utt , 0 〈 x 〈 1, t 〉 0, with the boundary condition u（O,t） = g（t） e LZ（R） and ux（O,t） = 0, where K（x） is continuous and 0 〈α≤ K （x）〈＋∞. This is an ill-posed problem in the sense that, if the solution exists, it does not depend continuously on g. Considering the existence of a solution u（x, .） e H2（R） and using a wavelet Galerkin method with Meyer multiresolution analysis, we regularize the ill-posedness of the problem. Furthermore we prove the uniqueness of the solution for this problem.

Influence of PVD-duplex-treated, Bionic Surface Structures on the Wetting Behavior for Sheet-Bulk Metal Forming Tools

Bionic surface structures, inspired by the flora, were developed for Sheet-Bulk Metal Forming （SBMF） in order to locally control the friction condition by adjusting the wetting behavior. Five bionic structures were micromilled on ASP 2023 in annealed as well as hardened and tempered conditions. Subsequently, the structured surfaces were plasma-nitrided and coated with a CrA1N thin film. The influence of the treatment method on the structural geometry was investigated with the aid of a scanning electron microscope and 3D-profilometer. The wetting behaviors of water and deep drawing oil （Berufluid ST6007） on bionic surfaces were evaluated using contact angle measurements. The resulting micro-milled structures exhibit an almost identical shape as their bionic models. However, the roughness of the structured surfaces is influenced by the microstructure. The combination of plasma-nitriding and Physical Vapor Deposition （PVD） leads to an increase in roughness. All bionic struc- tures possess higher contact angles than that of the unstructured surfaces when wetted by water. This can be explained by the fact that the structural elevations block the spreading. When the bionic surfaces are wetted by deep drawing oil, the lubricant spreads in the structural cavities, leading to smaller contact angles. Furthermore, the anisotropy of the structure has an influence on the wetting behavior.

Biliary stone causing afferent loop syndrome and pancreatitis

We report the case of an 84-year-old female who had a partial gastrectomy with Billroth-Ⅱ anastomosis 24 years ago for a benign peptic ulcer who now presented an acute pancreatitis secondary to an afferent loop syndrome. The syndrome was caused by a gallstone that migrated through a cholecystoenteric fistula. This is the first description in the literature of a biliary stone causing afferent loop syndrome.

BACKGROUND： Fibrolamellar hepatocellular carcinoma （FLHCC） is a rare disease with an indolent behavior. Its prognosis is better than that of patients with hepatocellular carcinoma. The authors present their experience with resection of FLHCC. MeTHODS： Twenty-one patients with FLHCC were treated at our institution between 1990 and 2012. Of these patients, 14 were subjected to resection of the tumor. Patient demographics, medical history, results of imaging studies and laboratory tests, surgical data, and pathologic findings were evaluated. ReSULTS： The median age of the patients at the diagnosis of the tumor was 20 years and 14 patients were female. None of the patients had tumor-associated chronic liver disease or cirrhosis. The mean tumor size was 12.8 cm （range 6-19） and 18 patients had a single liver nodule. Fourteen patients were subjected to hepatectomy and six of them had lymph node metastases resected. Pathologic evaluation revealed that 5 （35.7%） patients had major vascular invasion. Tumor recurrence was seen in 8 patients （66.7%）, during a follow-up. The median survival time for patients who were subjected to resection was 36 months. The 5-year overall survival rate and disease free survival rate were 28.0% and 8.5%, respectively. Univariate analysis showed that vascular invasion was the only variable associated with the disease free survival ***： Despite an aggressive treatment, patients with FLHCC presented unexpected low survival rates. It seems that an underestimated malignant behavior is attributed to this disease, and that the forms of adjuvant treatment should be urgently evaluated.

In this review, we recall how stars contribute to the search for dark matter and the specific role of the Sun. We describe a more complete picture of the solar interior that emerges from neutrino detections, gravity and acoustic mode measurements of the Solar and Heliospheric Observatory （SOHO） satellite, becoming a reference for the most common stars in the Universe. The Sun is a unique star in that we can observe directly the effect of dark matter. The absence of a signature related to Weakly Interacting Massive Particles （WIMPs） in its core disfavors a WIMP mass range below 12 GeV. We give arguments to continue this search on the Sun and other promising cases. We also examine another dark matter candidate, the sterile neutrino, and infer the limitations of the classical structural equations. Open questions on the young Sun, when planets formed, and on its present internal dynamics are finally dis- cussed. Future directions are proposed for the next decade： a better description of the solar core, a generalization to stars coming from seismic missions and a better under- standing of the dynamics of our galaxy which are all crucial keys for understanding dark matter.

etiology of Fungemia in Neonates: Four Decades of Case Reports

Objectives: To compare groups of etiologic agents for neonatal fungemia based on previously published case reports with regard to case outcomes. Methods: Secondary analysis of data from case reports published in scientific literature indexed in Medline, Scopus and LILACS databases, and starting from the year, full texts were registered until the year 2010. Results: Thirty-four species of fungi were isolated in 252 cases reported over a 40-year period. The distribution of fungal groups according to the development level of a case’s region of origin was very uneven (p Aspergillus sp.) in more developed countries. However, 73% of the chi-squared distribution corresponded to differences observed in the frequency of Candida sp. and non-albicans Candida. While the frequency of candidemia by C. albicans was progressively declining in the period from 1966 to 2010, reports of non-albicans candidemia and other non-Candida fungi became significantly (p albicans Candida or non-Candida fungi were isolated. Conclusion: fungi of the genera Candida and Aspergillus were the etiologic agents most frequently identified in published cases of neonatal fungemia;the outcome of death was more often related to cases where non-albicans Candida or non-Candida fungi were isolated.

Relative peripheral refraction across 4 meridians after orthokeratology and LASIK surgery

Background:To characterize the axial and off-axis refraction across four meridians of the retina in myopic eyes before and after Orthokeratology(OK)and LASIK ***:Sixty right eyes with a spherical equivalent(M)between−0.75 to−5.25 D(cylindererwent LASIK(n=26)or OK(n=34)to treat *** and off-axis refraction were measured with an open-field autorefractometer before and after stabilized ***-axis measurements were obtained for the horizontal(35°nasal and temporal retina)and vertical(15°superior and inferior retina)meridians,and for two oblique directions(45–225°and 135–315°)up to 20°of *** refractive profile was addressed as relative peripheral refractive error(RPRe).Results:OK and LASIK post-treatment results showed an increase of myopic relative refraction at several eccentric *** the four meridians evaluated,the M component of the pre-treatment RPRe values was not statistically different(p>0.05)from the post-treatment RPRe within 30°and 20°of the central visual field after LASIK and OK,*** results demonstrated that the treatment zone warrants an optimal central field of ***:The present study gives an overview of RPRe after refractive corneal reshaping treatments(OK and LASIK)across vertical,horizontal and two oblique meridians *** allows a 3D representation of RPRe at the retina and shows that the myopic shift induced by both treatments is more relevant in horizontal directions.
