Benefit of Growth Hormone Replacement in Adults Older than 60 Years

Objective: Benefits of replacement therapy in growth hormone deficiency (GHD) are well documented in younger and middle-aged patients. The aim of our investigation was to prove the benefit of GH replacement for patients older than 60 years especially in terms of health-related quality of life (HRQoL) of age as well. Design: Data of 743 consecutively recruited patients (394 men, 349 women) with GHD aged 20 - 49 (n = 606) and 60 - 69 (n = 137) years enrolled from KIMS Germany (Pfizer International Metabolic Database) were compared. Treatment effects over the 12 months dose-finding and the subsequent phase up to three years were analysed using mixed models. Serum insulin-like growth factor I (IGF-I), fasting blood glucose, fasting serum total cholesterol and low-density lipoprotein cholesterol (LDL-C) as well as body mass index (BMI) at baseline and at last visit were studied. HRQoL was assessed using the Quality of Life-Assessment of Growth Hormone Deficiency in Adults (QoL-AGHDA). Results: For both age groups and genders the IGF-I level and standardized IGF-I increased over the dose-finding phase. In women, the overall QoL-AGHDA score at the baseline examination was 8.7 (95% CI: 7.7 - 9.7) and decreased to 6.3 (95% CI: 5.1 - 7.6) at the end of the dose-finding phase (p < 0.001). In men, the corresponding values were 8.8 (95% CI: 7.8 - 9.8) and 6.4 (95% CI: 5.1 - 7.6;p < 0.001) without differences between the age groups. The therapy benefit for elderly was supported by the non-impairment after the dose-finding phase. In total cholesterol, LDL-C and fasting blood glucose, no significant changes were detected, whereas an increase in BMI did not differ between age groups. Conclusion: We could show positive effects of GH replacement on HRQoL in patients older than 60 years of age. Therefore, GH replacement should be considered in elderly GHD adults without difference compared to younger age groups.

This recently published cross-sectional study that included 99 men from Massachusetts General Hospital Fertility Center, Boston, MA, USA, reports that a high intake of saturated fats was inversely associated with sperm concentration and total sperm count and a high intake of omega-3 polyunsaturated fats （PUFAs） was positively associated with sperm morphology. Although the study is based upon a highly selected group of males and is of cross-sectional nature, the findings are of interest, since poor semen quality is a frequent problem in some parts of the world and identification of modifiable risk factors is highly wanted.

Coronal magnetic fields evolve quasi-statically over long timescales and dynamically over short *** of now there exist no regular measurements of coronal magnetic fields,and therefore generating the coronal magnetic field evolution using observations of the magnetic field at the photosphere is a fundamental requirement to understanding the origin of transient phenomena from solar active regions(ARs).Using the magneto-friction(MF)approach,we aim to simulate the coronal field evolution in the solar AR *** MF method is implemented in the open source PENCIL CODE along with a driver module to drive the initial field with different boundary conditions prescribed from observed vector magnetic fields at the *** order to work with vector potential and the observations,we prescribe three types of bottom boundary drivers with varying free-magnetic *** MF simulation reproduces the magnetic structure,which better matches the sigmoidal morphology exhibited by Atmospheric Imaging Assembly(AIA)images at the pre-eruptive *** found that the already sheared field further driven by the sheared magnetic field will maintain and further build the highly sheared coronal magnetic configuration,as seen in AR ***-driven MF simulation is a viable tool to generate the coronal magnetic field evolution,capturing the formation of the twisted flux rope and its eruption.

The evidence is scarce on the association between age at puberty and semen quality. A cohort of 320 Danish men aged 18-21 years enrolled in the ＂Healthy Habits for Two＂ birth cohort provided self-reported data on pubertal indicators and delivered semen and blood samples. The results indicated an association between older age at pubertal development and lower semen quality and altered reproductive hormones concentrations as measured in young adult life. Men who had their first nocturnal emission, start of pubic hair growth and first voice break episode when older than 15 years had 37.0%, 45.0% and 32.7% lower sperm concentration; 37.8%, 44.2% and 29.1% lower total sperm count; 7.4%, 13.4% and 15.3% lower testosterone concentration; and 21.3%, 1.5% and 3.7% lower inhibin B concentration, respectively, compared with the men who were younger than 13 years at their first pubertal indicators. Only few of the results were statistically significant, but similar tendencies were seen in several of the reproductive parameters suggesting an association between the timing of pubertal development and reproductive health later in life.

Threat recognition and response in an avian brood-parasite host from New Caledonia

Nest predation and avian brood parasitism are the main sources of nest failure in many passerine *** predators threaten both brood and parents,whereas brood parasites pose only a danger to eggs or *** fan-tailed gerygone Gerygone flavolateralis from New Caledonia is subjected to high rates of nest predation by the New Caledonian crow Corvus moneduloides(responsible for about 20-40%of predation)and moderate rates of brood parasitism by the shining bronze-cuckoo Chalcites lucidus(parasitizing about 18%of nests),which also depredates nests that are too advanced for parasitism(13%of nests).To test if fan-tailed gerygones are able to discriminate predators from brood parasites,we presented 3 bird models at active gerygone nests:a brood parasite/small nest predator(shining bronze-cuckoo),a large nest predator(crow),and a small non-native bird(common chaffinch Fringilla coelebs),which is unknown to the gerygone,as a *** assessed the response of adult gerygones to the presentation of each model by measuring the minimum approach distance,number of alarm calls,number of attacks,and time to first nest visit after the presentation(latency).Adult gerygones often attacked the cuckoo,approached but never attacked the chaffinch and always avoided the *** was shorter after an attack response and during brooding,but similar among *** did not find any link between the cuckoo model presentation and later ejection of cuckoo *** conclude that adult fan-tailed gerygones discriminate between different models and respond accordingly to the level of threat but do not show awareness of parasitism risk and increase of nestling ejection rates following exposure to the cuckoo model.

A community-based epidemiological study of elevated serum alanine aminotransferase levels in Kinmen, Taiwan

AIM: To explore any gender-related differences in prevalence of and condition-associated factors related to an elevated serum alanine aminotransferase (ALT) level amongst residents of Kinmen, Taiwan. METHODS: A total of 11 898 of a potential 20 112 regional residents aged 30 years or more completed a related questionnaire that was carried out by the Yang-Ming Crusade between 1991 and 1994 inclusively, with blood samples being collected by public nurses. The overall questionnaire response rate was 59.3% (52.4% for males and 66.0% for females). RESULTS: The prevalence of an elevated serum ALT level for this sub-population was found to be 7.2%, the prevalence revealing a statistically significant decrease with increasing population age (P<0.0001). Males exhibited a greater prevalence of elevated serum ALT level than did females (9.4% vs 5.3%, P<0.0001). Using multiple logistic regression analysis, in addition to male gender, a younger age, greater waist circumference, presence of type-2 diabetes and hyperuricemia were the significant factors associated with an elevated serum ALT level for both males and females. Gender-related differences as regards associated factors were also revealed. For males, obesity was significantly related to an elevated serum ALT level (OR = 1.28, 95%CI: 1.00-1.66) but this was not so for females (OR = 1.09, 95%CI: 0.84-1.42). Hypertriglyceridemia (OR = 1.80, 95%CI: 1.36-2.39) and hyperuricemia (OR = 1.61, 95%CI: 1.03-2.52) were significantly related to elevated serum ALT levels only for females. CONCLUSION: Several gender-related differences were noted pertaining to the prevalence of and relationship between obesity, hypertriglyceridemia and hyperuricemia and elevated serum ALT level in the present study.

Timing of pubertal development in boys born with cryptorchidism and hypospadias:a nation wide cohort study
