Prevalence and risk factors of acute-on-chronic liver failure in a single center from Argentina

AIMTo study the prevalence, characteristics, risk factors and mortality at 28 d of acute-on-chronic liver failure (AcLF). METHODSA total of 100 cirrhotic patients admitted to our hospital for more than one day were included during the period between June 2013 and December 2015. We used the European Association for the Study of the Liver-chronic Liver Failure-consortium diagnostic criteria for AcLF, considering it as the acute decompensation of cirrhosis associated with the presence of one or more organ failure. For the diagnosis of organic failure the chronic Liver Failure-Sequential Organ Failure Assessment score was used. Our population was divided into patients with and without AcLF. clinical characteristics, presence of precipitating events, potential risk factors for developing AcLF and causes of mortality were analyzed. Mortality at 28 d was evaluated. RESULTSTwenty-nine patients (29%) developed AcLF criteria. Alcoholism, detected in 58 patients (58%), was the major etiological agent of cirrhosis. Bacterial infections were recognized as a precipitating event in 41.3% of cases and gastrointestinal bleeding in 27.5%. No precipitating event was identifiable in 27.5% of patients with AcLF. comparing patients with and without AcLF, statistically significant risk factors were: child Pugh score 10.2 ± 2.1 vs 8.4 ± 1.6 (P ˂ 0.0001), MELD score 20.7 ± 8.5 vs 12.3 ± 4 (P ˂ 0.0001), presence of ascites 27 (93%) vs 43 (60.5%) (P = 0.001), leukocytosis 15300 ± 8033 per cubic millimeter vs 10770 ± 5601 per cubic millimeter (P ˂ 0.0001), and high plasma levels of c reactive protein values 50.9 ± 46.4 mg/L vs 28.6 ± 23.4 mg/L (P ˂ 0.0019). Mortality rate was 62% (18 patients) vs 5.6% (4 patients), respectively (P cONcLUSIONWe observed that the AcLF is a frequent entity in this group of patients and has a significantly higher mortality rate.

Inventory of primary emissions of selected persistent organic pollutants to the atmosphere in the area of Great Mendoza

The setting up of a country or region-based inventory is considered a crucial step toward the elimination of worldwide persistent organic pollutants(POPs)***,the need of comparable emission inventories at city or region level is widely recognized to develop evidence-based policies accounting for the relation between emissions and institutional,socio-economic and demographic characteristics at small scale *** to the low spatial and temporal resolution of the available measurements,highly variable air concentrations of several POPs have been observed in Latin American and caribbean *** paper presents a high resolution spatially disaggregated atmospheric emission inventory for selected POPs in order to assess the environmental fate of some of these compounds in a finer *** study case we estimated releases to air of POPs in a typical mid-size urban conglomeration in *** compounds were total polychlorinated biphenyls(PcBs),total polybrominated diphenyl ethers(PBDEs),total dichlorodiphenyltrichloroethane(DDT)on a sum basis,hexachlorobenzene(HcB)and dioxins and furans(PcDD/Fs),for which emissions were estimated in 0.92 kg/year,1.65 kg/year,4.2E-02 kg/year(total sum of congeners),0.86 kg/year and 4.4E-02 kg/year respectively,values that are in accordance with the geographic and economic *** emitting sources are quite varied,there are very clear trends,particularly in relation to open burning of municipal solid waste and agrochemical use as major ***,the inventory provides valuable data for the analysis of the heterogeneity of POP emissions and the necessary inputs for air quality modeling.

Exocrine pancreatic neoplasms represent up to 95%of pancreatic cancers(Pcs)and are widely recognized among the most lethal solid cancers,with a very poor 5-year survival rate of 5%-10%.The remainingcs are neuroendocrine tumors that are usually characterized by a better prognosis,with a median overall survival of 3.6 *** most common type of Pc is pancreatic ductal adenocarcinoma(PDAc),which accounts for roughly 85%of all exocrine *** up to 10%of exocrine Pcs have rare histotypes,which are still poorly *** subtypes can be distinguished from PDAc in terms of pathology,imaging,clinical presentation and ***,due to their rarity,any knowledge regarding these specific histotypes is mostly based on case reports and a small series of retrospective ***,treatment strategies are generally deduced from those used for PDAc,even if these patients are often excluded or not clearly represented in clinical trials for *** these reasons,it is essential to collect as much information as possible on the management of Pc,as assimilating it with PDAc may lead to the potential mistreatment of these ***,we report the most significant literature regarding the epidemiology,typical presentation,possible treatment strategies,and prognosis of the most relevant histotypes among rare Pcs.

Synthesis, Structural Study and Spectroscopic characterization of a Quinolin-8-Yloxy Derivative with Potential Biological Properties

We have prepared the (5-chloro-quinolin-8-yloxy) acetic acid and characterized it by using infrared, Raman and multi-dimensional nuclear magnetic resonance spectroscopies. The density functional theory (DFT) together with the 6-31G* and 6-311++G** basis sets were used to study its structure and vibrational properties. Three stable conformations of the compound were theoretically determined in gas phase and probably these conformations are present in the solid phase. The harmonic vibrational wavenumbers for the optimized geometries were calculated at the same theory levels. For a complete assignment of the observed bands in the vibrational spectra, the DFT calculations were combined with Pulay’s scaled quantum mechanical force field (SQMFF) methodology in order to fit the theoretical wavenumber values to the experimental ones. Besides, the force constants of the three conformers of (5-chloro-quinolin-8-yloxy) acetic acid were calculated and compared with those obtained by us for the 2-(quinolin-8-yloxy) acetic acid. In addition, the characteristics of the electronic delocalization of those structures were performed by using natural bond orbital (NBO), while the corresponding topological properties of electronic charge density are analysed by employing Bader’s atoms in molecules theory (AIM).
