The a.pheta.ine deriva.ive 3,4-methylenedioxymetha.pheta.ine (MDMa., a.so ca.led ecsta.y, is a.neurotoxin widely consumed a.ong young people tha. ha. increa.ed in recent yea.s beca.se it is a.recrea.iona. drug, of which immedia.e effects a.e known such a. a.grea.er sensa.ion of well-being, extroversion, increa.ed sensory perception. However, its long-term effects ha.e been described very little in the medica. litera.ure, including da.a.e to the hea.t, centra. nervous system, kidney, etc. One of its little-known effects is hepa.otoxicity, of which few ca.es a.e known a.socia.ed with fulmina.t hepa.itis, which is a.ra.idly deteriora.ing condition tha. is genera.ly a.socia.ed with a.syndrome of multiple orga. dysfunction a.d dea.h. Therefore, it is very importa.t to know this type of da.a.e in the short a.d long term. The following ca.e is of a.39-yea.-old ma. who ca.e to our service due to ja.ndice syndrome a.d the only history of MDMa.consumption, who a. the da.s went by met the criteria.for fulmina.t liver fa.lure, with da.a.e to multiple orga.s (orga. dysfunction syndrome).

Dieula.oy’s lesions a.e ra.e va.cula. ma.forma.ions of the ga.trointestina. tra.t. a.Dieula.oy’s lesion is a. a.erra.t vessel tha. does not reduce in ca.iber when it extends from the submucosa.to the mucosa. Da.a.e to this a.tery ca. ca.se severe a.d intermittent a.teria. bleeding from sma.l va.cula. stumps tha. a.e difficult to visua.ize. Furthermore, these ca.a.trophic bleeding episodes frequently result in hemodyna.ic insta.ility a.d the need for tra.sfusion of multiple blood products. Recently, uremic syndrome ha. been identified a. a.risk fa.tor for ga.tric mucosa. lesions. We present two clinica. ca.es of a.ute digestive bleeding due to Diela.oy lesion with chronic kidney disea.e a. the ma.n ca.se, where two different thera.ies were performed endoscopica.ly. We concluded with the results of our pa.ients tha. the best thera.y wa. the a.plica.ion of the hemosta.ic hemoclip on the injury vs the injection with a.rena.ine on the wound site. Uremia.is identified a. a.risk fa.tor for upper ga.trointestina. bleeding in pa.ients with pre-existing Dieula.oy’s lesion, a. well a. a.higher incidence of new bleeding.
