Digital radiograph(DR)guided bedside IVC filter placements in patients with intracranial pressure monitors

Purpose:The purpose of this study is to report a single center experience with portable digital radiographically(DR)guided bedside IVC filters placed in intensive care unit(ICU)patients with high ICP and elevated head of bed(HOB).Materials and methods:A retrospective chart review was conducted on all bedside IVC filters placed from January 1,2010 to September 16,2020.Patients with high ICP and elevated head of bed requirements were included.Charts were reviewed for filter type,common femoral vein(CFV)access,filter location,pre procedure imaging,pre and post filter ICPs,glascow coma scale,number of radiographs taken,and filter removal.ICPs were obtained 1 h prior to procedure and 2 h post procedure and analyzed with a paired T test.Filters were placed by reviewing prior CT scan for IVC size,caval variants,renal and iliac veins and vertebral body landmarks.Then,CFV access was obtained and a Bentson wire was advanced 30-40 cm.A radiograph was used to confirm adequate position of the of the wire.The filter sheath was advanced and serial radiographs were used to position the filter sheath at the final predetermined position below the renal veins and above the iliac bifurcation.The filter was deployed,and a radiograph was obtained to confirm filter positioning.Results:A total of 9 DR guided bedside IVC filters were placed(4 Denali,3 Option elite,2 Celect).Indications included prophylactic placement(n=8)and acute DVT(n=1).The average patient age was 35.8 years(range:18-56 years)CT abdomen and pelvis was used to assess for the level of renal veins in all patients(n=9).No caval variants were encountered on pre-procedural planning.The average pre,intraprocedural,and post procedure intracranial pressure was 16 mmHg,13 mmHg,and 16 mmHg,respectively.Confirmation of placement after final placement was available in 7 patients(4 DR,2 CT and one fluoroscopic examination).Two non-procedural related deaths occurred.Technical success,defined as successful placement of IVC filter at the predetermined level,was achieved in 100%of pati

Comparative outcomes of Inferior Vena Cava filters placed at bedside using digital radiography versus conventional fluoroscopy

Purpose:To retrospectively assess the outcomes of Inferior Vena Cava(IVC)filters placed in critically ill patients in the ICU at bedside using digital radiograph(DR)guidance with previous cross-sectional imaging for planning,compared to IVC filters placed by conventional fluoroscopy(CF).Method and materials:The cohort consisted of 129 IVC filter placements;48 placed at bedside and 81 placed conventionally from July 2015 to September 2016.Patient demographics,indication,radiation exposures,access site,procedural duration,dwell time,and complications were identified by the eMR.IVC Filter positioning with measurements of tip to renal vein distance and lateral filter tilt were performed when cavograms or post placement CTs were available for review.Statistical analysis was performed using Stata IC 11.2.Results:Technical success of the procedure was 100% in both groups.Procedural duration was longer at the bedside lasting 14.5+/-10.2 versus 6.7+/-6.0 min(pe bedside DR group had a median radiation exposure of 25 mGy(15-35)and the CF group had mean radiation exposure of 256.94 mGy+/-158.6.There was no significant difference in distance of IVC tip to renal vein(p=0.31),mispositioning(p=0.59),degree of filter tilt(p=0.33),or rate of complications(p=0.65)between the two groups.Conclusion:IVCF placement at the bedside using DR is comparable to CF with no statistical difference in outcomes based on IVCF positioning,degree of lateral tilt or removal issues.It decreased radiation dose,but with overall increased procedural time.

Purpose: This study investigated the anatomical and histological characteristics of the rat eustachian tube(e-tube)and the feasibility of eustachian tubography in a rat model.Materials and methods: Fifteen male Wistar rats were used in this study, and the bilateral e-tubes of each rat were examined. Ten e-tubes were used for anatomical studies, another ten for histological analysis, and the other ten for eustachian tubography. Five rats were euthanized and decapitated, and ten e-tubes were dissected to describe the anatomy of the e-tube. Ten e-tube specimens obtained from five other rats were sectioned to investigate etube histology. eustachian tubography was performed on the bilateral e-tubes of the other five rats using the trans-tympanic approach.Results: The rat e-tubes consisted of bony and membranous parts. Cartilage and bone tissue covered only the bony part. The e-tubes’ mean diameter and overall length were 2.97 mm and 4.96 mm, respectively. The tympanic orifices’ mean diameter was 1.21 mm. The epithelium of e-tubes was mainly composed of pseudostratified ciliated and goblet cells. eustachian tubography was successfully performed on both sides of the e-tube for each rat.The technical success rate was 100%, the average running time was 4.9 min, and no procedure-related complications occurred. On tubography images, the e-tube, tympanic cavity, and nasopharynx could be identified because of the visualization of bony landmarks.Conclusion: In this study, we described the anatomical and histological features of rat e-tubes. With the aid of these findings, e-tube angiography was successfully performed using a transtympanic approach. These results will facilitate further investigation of e-tube dysfunction.

Avian responses to an extreme weather event:The case of the‘Filomena’snowstorm in Madrid(central Spain)

Cold spells and severe weather may have detrimental effects in wild birds,but owing to the stochastic nature of these events,it is not always possible to measure their impacts.In January 2021,a strong snowstorm known as‘Filomena’affected Spain,mainly the region of Madrid(Central Spain),which was fully covered with snow and suffered freezing temperatures for a fortnight.Using bird surveys conducted in the winter-spring of 2020 and 2021(before and after Filomena)at same locations,we addressed the possible impact of the cold spell in three widespread gamebirds:Wood Pigeon(Columba palumbus),eurasian Magpie(Pica pica)and Red-legged Partridge(Alectoris rufa).No significant abundance differences were found between years for none of the species and hunting pressure was only significantly reduced for Partridges,hence this species may have been negatively affected by Filomena.Results suggest that in response to Filomena,Pigeons conducted partial migration in search of better conditions,while Magpies aggregated in certain areas of Madrid and Partridges endured the harsh conditions within their home ranges.Our results highlight the importance to alleviate negative impacts of extreme weather events in wild birds through management actions,especially for sedentary species.

Design and selection of advanced protection schemes have become essential for reliable and secure operation of networked microgrids.Various protection schemes that allow correct operation of microgrids have been proposed for individual systems in different topologies and connections.Nevertheless,protection schemes for networked microgrids are still in devel-opment,and further research is required to design and operate advanced protection in interconnected systems.Interconnection of these microgrids in different nodes with various intercon-nection technologies increases fault occurrence and complicates protection operation.This paper aims to point out challenges in developing protection for networked microgrids,potential solutions,and research areas that need to be addressed for their development.First,this article presents a systematic analysis of different microgrid clusters proposed since 2016,including several architectures of networked microgrids,operation modes,components,and utilization of renewable sources,which have not been widely explored in previous review papers.Second,the paper presents a discussion on protection systems currently available for microgrid clusters,current challenges,and solutions that have been proposed for these systems.Finally,it discusses the trend of protection schemes in networked microgrids and presents some conclusions related to implementation.IndexTerms—Adaptive eprotection,microgrid cluster,microgrid,multiple microgrid,networked microgrid,real-time simulation,smart grid.

Correlative species distribution models(SDMs)are important tools to estimate species’geographic distribution across space and time,but their reliability heavily relies on the availability and quality of occurrence data.estimations can be biased when occurrences do not fully represent the environmental requirement of a species.We tested to what extent species’physiological knowledge might influence SDM estimations.Focusing on the Japanese sea cucumber Apostichopus japonicus within the coastal ocean of east Asia,we compiled a comprehensive dataset of occurrence records.We then explored the importance of incorporating physiological knowledge into SDMs by calibrating two types of correlative SDMs:a naïve model that solely depends on environmental correlates,and a physiologically informed model that further incorporates physiological information as priors.We further tested the models’sensitivity to calibration area choices by fitting them with different buffered areas around known presences.Compared with naïve models,the physiologically informed models successfully captured the negative influence of high temperature on A.japonicus and were less sensitive to the choice of calibration area.The naïve models resulted in more optimistic prediction of the changes of potential distributions under climate change(i.e.,larger range expansion and less contraction)than the physiologically informed models.Our findings highlight benefits from incorporating physiological information into correlative SDMs,namely mitigating the uncertainties associated with the choice of calibration area.Given these promising features,we encourage future SDM studies to consider species physi-ological information where available.

Background:Previous studies have reported associations of specific maternal and paternal lifestyle factors with offspring’s cognitive development during early childhood.This study aimed to investigate the prospective associations between overall parental lifestyle and offspring’s cognitive performance during adolescence and young adulthood in China.Methods:We included 2531 adolescents aged 10-15 years at baseline in 2010 from the China Family Panel Studies.A healthy parental lifestyle score(ranged 0-5)was constructed based on the following five modifiable lifestyle factors:Smoking,drinking,exercise,sleep,and diet.Generalized estimating equation models were used to examine the association between baseline parental healthy lifestyle scores and offspring’s fluid and crystallized intelligence in subsequent years(2012,2014,2016,and 2018).Results:Offspring in the top tertile of parental healthy lifestyle scores performed better in overall fluid intelligence(multivariable-adjusted β=0.53,95%confidence interval[CI]:0.29-0.77)and overall crystallized intelligence(multivariable-adjusted β=0.35,95%CI:0.16-0.54)than those in the bottom tertile of parental healthy lifestyle scores.The results were similar after further adjustment for the offspring’s healthy lifestyle scores and persisted across the subgroups of parental socioeconomic status.Additionally,maternal and paternal healthy lifestyle scores were independently associated with better offspring’s cognitive performance,with significant contribution observed for paternal never-smoking,weekly exercise,and diversified diet.When both parents and offspring adhered to a healthier lifestyle,we observed the highest level of the offspring’s overall crystallized intelligence.Conclusions:Our study indicates that parental adherence to a healthier lifestyle is associated with significantly better offspring’s cognitive performance during adolescence and early adulthood,regardless of socioeconomic status.These findings highlight the potential cognitive benefits of promoting healthy lif

Metamizole(MZ)is a widely used anti-inflammatory drug.Due to its common use,this contaminant is found in sewage and rivers.In order to reduce the contamination produced by the MZ,we fabricated in this work a photocatalytic composite using recycled polyethylene(RPe)and the CaLaCoO_(9)(LCCO)perovskite.Those nanoparticles had a microplate-like morphology and sizes of 1.4–5.5μm according to the analysis of microscopy.The photocatalytic properties of the LCCO powders were evaluated under ultraviolet-visible(UV-Vis)irradiation and found a removal efficiency of 96%.When the RPe+LCCO composite was employed for the photocatalytic degradation of MZ,a maximum degradation of 92.5%was obtained.The influence of the pH on the photocatalytic activity was also studied and found that an initial pH=3 produced a total degradation of MZ after 240 min of UV-Vis irradiation.Moreover,three reuse cycles were carried out for the pure LCCO powders and for the RPe+LCCO composites and found that the maximum loss of degradation was 5%.Furthermore,scavenger experiments demonstrated that the super oxide and hydroxyl radicals are formed during the photocatalytic reaction and were responsible for the degradation of MZ.Additionally,the X-ray photoelectron-spectroscopy and Raman analysis demonstrated the formation of defects(oxygen vacancies),those ones delayed the electron-hole recombination,which in turn,enhanced the degradation of the MZ.Thus,the studies performed in this work proved that composites made with recycled plastics and LCCO perovskites are a low-cost and feasible alternative for the cleaning of water sources polluted with pharmaceutical compounds.

The effectiveness of monitoring and early-warning systems for ground deformation phenomena,such as sinkholes,depends on their ability to accurately resolve the ongoing ground displacement and detect the subtle deformation preceding catastrophic failures.Sagging sinkholes with a slow subsidence rate and diffuse edges pose a significant challenge for subsidence monitoring due to the low deformation rates and limited lateral strain gradients.In this work,we satisfactorily illustrate the practicality of the Brillouin optical time domain analysis(BOTDA)to measure the spatial-temporal patterns of the vertical displacement in such challenging slow-moving sagging sinkholes.To assess the performance of the approach,we compare the strain recorded by the distributed optical fiber sensor with the vertical displacement measured by high-precision leveling.The results show a good spatial correlation with the ability to identify the maximum subsidence point.There is also a good temporal correlation with the detection of an acceleration phase in the subsidence associated with a flood event.

ephedroid macrofossils have been widely documented in Cretaceous deposits,including numerous from the Lower Cretaceous Yixian Formation of Ne China.However,few ephedroid macrofossils have been reported from South America.Herein,we describe a new plant of the family ephedraceae,Arlenea delicata gen.et sp.nov.,from the Lower Cretaceous Crato Formation of the Araripe Basin,Northeast Brazil,based on the vegetative and reproductive structures.It has the typical morphological characteristics of ephedroid plants,including fertile reproductive branches,opposite phyllotaxy,terminal female cones,a sympodial branching system,longitudinally striated internodes,and swollen nodes.Our new finding is unusual in having inner chlamydosperms subtended by two pairs of bracts,reproductive units connected to branches through swollen receptacles and a smooth seed surface.This new ephedroid taxon from the Crato Formation increases our understanding of plant diversity of this group during the early Cretaceous.Furthermore,the general morphology(fleshy bracts and enlarged receptacles)of this new fossil discovery indicates that seeds of this plant may have been dispersed by animals such as pterosaurs(mainly the Tapejaridae)and birds(enantiornithes and Ornituromorpha).If true,this would explain the cosmopolitan distribution of ephedraceae in the Lower Cretaceous.
