背景手术后恶心、呕吐（pONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者pONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的pONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生pONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（p〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，p〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（p〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，p〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少pONV，主要是通过减少不良反应而获得。

目的:普那布林靶向作用于肿瘤新生和原生血管,并通过RAS-JNK通道诱导肿瘤凋亡。本研究对普那布林和多西他赛联合用药(DN)与多西他赛单独用药(D)进行比较。方法:之前经历标准化疗的晚期NSCLC患者,被随机分至DN组和D组,21天治疗周期中,第1天多西他赛(75 mg/m,静注),DN组:第1和8天还给普那布林(静注,剂量:30或20 mg/m,简称30或20剂量组)。经历1次或2次既往化疗的ⅢB/Ⅳ期NSCLC患者入组,既往给D或血管阻断剂不入组。主要终点是总生存期(OS)。结果:172名患者被随机分为2个剂量组,其中163名进行治疗:30剂量组(DN:50;D:55)和20剂量组(DN:40;D:18)。30剂量组:DN组OS(月,90%CI)为8.7(6.6,12.6),D组7.5(6.3,10.5);客观缓解率:DN:14.0%对D:14.5%);DOR(月,90%CI):DN:12.7(4.0,13.9)对D:1.5(1.1,3.1),p=0.049。20剂量DN组的这些参数没30剂量组好。事后亚组分析:DN组经历1次既往化疗并有大肺肿瘤(>3 cm)患者,生存期可能更长。该亚组OS(月,95%CI)为DN组11.5(7.1,15.1)对D组7.8(4.1,17.4)。最常见不良反应为恶心,乏力,腹泻,便秘和厌食。30剂量组中DN组多西他赛初始剂量降低10%,而D组降低18.2%,所以DN组耐受性可能比D组更好。DN组中性粒细胞减少症发病率显著低于D组(30剂量组:8.0%对36.4%,p<0.001;20剂量组:7.5%对22.2%)。结论:经历1次既往化疗并有大肺肿瘤患者中,DN组OS比D组长,这发现与普那布林的靶向血管作用机制一致。计划在大肺肿瘤患者中进行Ⅲ期临床。

Self-reported and behavioural impulsivity in anorexia nervosa

AIM To examine how self-reported and behavioural impulsivity are related in anorexia nervosa(AN).METHODS Twenty-four females with AN and 25 healthy controls(HC) participant in the ***-reported impulsivity was assessed with the Barratt Impulsiveness Scale(BIS-11).The scale yields three second-order factors:Attentional,motor and *** impulsivity was investigated with the continuous performance test(CpT),a computer-based task of sustained attention in which numbers are flashed briefly on screen and participants are required to click the mouse when the same number appears *** rate of commission and omission errors can be used a measure of behavioural *** AN participants self-reported increased attentional [AN:20.67(3.64),HC:13.88(2.91),p = 0.001] and reduced motor impulsivity [AN:11.55(2.28),HC:14.08(2.78),p = 0.002].The rate of omission or commission errors on the CpT did not differ between groups(p > 0.05).BIS-11 and CpT measures did not significantly correlate,but attentional impulsivity was related to negative mood states in AN(depression:r = 0.52,p = 0.010,anxiety:r = 0.55,p = 0.006,stress:r = 0.57,p = 0.004).CONCLUSION The discrepancy between self-reported and behavioural impulsivity are discussed in terms of perfectionism in ***,it is suggested that improving negative mood states may resolve this inconsistency in AN.

Risk profile and outcomes of aortic valve replacement in octogenarians

AIM: To investigate the patient characteristics, relationship between the Logistic EuroSCORE (LES) and the observed outcomes in octogenarians who underwent surgical aortic valve replacement (AVR). METHODS: Two hundred and seventy three octogenarians underwent AVR between 1996 and 2008 at Bristol Royal Inf irmary. Demographics, acute outcomes,length of hospital stay and mortality were obtained. The LES was calculated to characterize the predicted operative risk. Two groups were def ined: LES ≥ 15 (n = 80) and LES patients with LES ≥ 15, 30 d mortality was 14% (95% CI: 7%-23%) compared with 4% (95% CI: 2%-8%) in the LES p (p pite the increase in number of operations from 1996 to 2008, the average LES did not change. Only 5% of patients had prior bypass surgery. The LES identifi ed a low risk quartile of patients with a very low mortality (4%, n = 8, p p had an excellent outcome and the high risk group had a poor outcome after surgical AVR. CONCLUSION: It may be better treated with transcatheter aortic valve implantation.

背景丙泊酚用于患者自控镇静（patient-controlled sedation，pCS）可解决中度镇静治疗窗窄的问题，但之前的研究存在方法学的不足。本研究中，我们使用瑞芬太尼＋丙泊酚经pCS给予，我们发现与芬太尼＋咪达唑仑相比，瑞芬太尼＋丙泊酚更方便使用。方法将50例择期结肠镜检的患者随机（使用盲法）分为咪达唑仑＋芬太尼组（MF组）或丙泊酚＋瑞芬太尼组（pR组），药物均经pCS给予。研究中评估项目有：镇静起效时间、恢复时间、患者满意度、护士满意度、镜检医师满意度、是否需要麻醉医师干预。结果与MF组相比，pR组患者进入镇静状态与恢复明显较快（p〈0．0001）。在pR组中，恢复时间短于操作时间。两组间患者满意度、护士满意度、镜检医师满意度相同。pR组中，有2例患者因为动脉氧饱和度低于设置的安全终点而需要麻醉医师干预。结论在适当的监护条件下，丙泊酚＋瑞芬太尼用于pCS比咪达唑仑＋芬太尼更有优势。

Total pancreatectomy and islet autotransplantation: A decade nationwide analysis

AIM: To investigate outcomes and predictors of inhospital morbidity and mortality after total pancreatectomy(Tp) and islet autotransplantation. METHODS: The nationwide inpatient sample(NIS) database was used to identify patients who underwent Tp and islet autotransplantation(IAT) between 2002-2012 in the United States. Variables of interest were inherent variables of NIS database which included demographic data(age, sex, and race), comorbidities(such as diabetes mellitus, hypertension, and deficiency anemia), and admission type(elective vs nonelective). The primary endpoints were mortality and postoperative complications according to the ICD-9 diagnosis codes which were reported as the second to 25 th diagnosis of patients in the database. Risk adjusted analysis was performed to investigate morbidity predictors. Multivariate regression analysis was used to identify predictors of in-hospital ***: We evaluated a total of 923 patients who underwent IAT after pancreatectomy during 2002-2012. Among them, there were 754 patients who had Tp + IAT. The most common indication ofsurgery was chronic pancreatitis(86%) followed by acute pancreatitis(12%). The number of patients undergoing Tp + IAT annually significantly increased during the 11 years of study from 53 cases in 2002 to 155 cases in 2012. Overall mortality and morbidity of patients were 0% and 57.8 %, respectively. postsurgical hypoinsulinemia was reported in 42.3% of patients, indicating that 57.7% of patients were insulin independent during hospitalization. predictors of inhospital morbidity were obesity [adjusted odds ratio(AOR): 3.02, p = 0.01], fluid and electrolyte disorders(AOR: 2.71, p p p p + IAT is a safe procedure with no mortality, acceptable morbidity, and achieved high rate of early insulin independence. Obesity is the most significant predictor of in-hospital morbidity.

1,引言双酚A(BpA)是一种内分泌干扰物,在人类样本中广泛检出。然而,游离的BpA与共价结合的BpA在母婴之间的传递情况研究很少,并且游离BpA在人类血清中的存在情况一直具有争议。本研究对匹配的母亲血清与脐带血清中BpA,BpA代谢物和BpA替代物的含量水平进行测定,评估胎儿体内BpA与BpA代谢物的来源,并对游离BpA与共价结合BpA的胎盘转移情况进行了比较。2,方法与样品本实验的血清样本分析应用了固相萃取方法和UpLC-MS/MS技术。本实验研究的61对匹配母亲静脉血与脐带血采集于中国石家庄市和北京市。母亲静脉血于产前2或3天收集,脐带血在产后立刻收集。所有新生儿均为健康体质无遗传性疾病。3,结果与讨论质量控制实验显示在样品的收集和储存过程中没有目标分析物质的污染,但样品前处理后观测到一定的BpA背景,在血清含量定量计算中扣除这一背景值。总BpA、游离BpA、BpA硫酸盐(BpA-S)和BpA葡萄糖醛酸苷浓度几何均数(ng/mL)在母亲血清中分别为0.307,0.124,0.062和0.018,脐带血清中分别为0.239,0.083,0.084和0.041。母亲血清中总BpA含量显著高于脐带血清中的总BpA(ppA含量具有显著相关关系(ppA与脐带血清中总BpA(ppA-S(p=0.01,r=0.18)有一定的相关性,其他母亲和脐带血清中相同形式的BpA及BpA代谢物之间没有显著的相关性。4,结论母亲血液中游离的BpA可能是本实验中胎儿体内总BpA的产前暴露重要来源之一。胎儿的代谢系统通过将游离BpA转化为BpA-S而导致脐带血中游离BpA含量水平下降。对母亲样本中游离BpA和总BpA的同时监测,特别是在怀孕的早期阶段,可以为暴露风险评估提供更加可靠的信息。

Linked to major volcanic eruptions around 536 and 540 CE, the onset of the Late Antique Little Ice Age has been described as the coldest period of the past two millennia. The exact timing and spatial extent of this exceptional cold phase are, however, still under debate because of the limited resolution and geographical distribution of the available proxy archives. Here, we use 106 wood anatomical thin sections from 23forest sites and 20 tree species in both hemispheres to search for cell-level fingerprints of ephemeral summer cooling between 530 and 550 CE. After cross-dating and double-staining, we identified 89Blue Rings(lack of cell wall lignification), nine Frost Rings(cell deformation and collapse), and 93Light Rings(reduced cell wall thickening) in the Northern Hemisphere. Our network reveals evidence for the strongest temperature depression between mid-July and early-August 536 CE across North America and Eurasia, whereas more localised cold spells occurred in the summers of 532, 540–43, and548 CE. The lack of anatomical signatures in the austral trees suggests limited incursion of stratospheric volcanic aerosol into the Southern Hemisphere extra-tropics, that any forcing was mitigated by atmosphere-ocean dynamical responses and/or concentrated outside the growing season, or a combination of factors. Our findings demonstrate the advantage of wood anatomical investigations over traditional dendrochronological measurements, provide a benchmark for Earth system models, support cross-disciplinary studies into the entanglements of climate and history, and question the relevance of global climate averages.

Background: Happy hypoxia is a new feature found in COVID-19 patients. It consists of the presence of severe hypoxemia but normal breathing rate. Failure to identify this hypoxia may have negative consequences on the survival of the patient. The objective of the present study was to measure the frequency of patients with happy hypoxia and to evaluate their survival at the Kinshasa University Hospital (KUH). Methods: This was a historical cohort of 141 hospitalized patients with COVID-19 at KUH from March 23 to June 15, 2020. Happy hypoxia was defined as oxygen saturation below 90% without dyspnea. Socio-demographic data, co-morbidities, follow up time of hospitalization and outcomes were studied. Survival was assessed using the Kaplan Meier curve. Results: Out of 141 hospitalized patients with COVID-19, 79 (56%) patients were at the severe or critical stage and 9 (6.4%) had a happy hypoxia on admission. patients who had happy hypoxia on admission were generally older than 60 years of age (55.6%) (p = 0.023). Comparison of survival curves, based on the presence or absence of happy hypoxia, shows a statistically significant difference (p = 0.001). The presence of happy hypoxia reduces survival. Conclusion: The frequency of happy hypoxia among COVID-19 patients was low. Survival was reduced in patients with happy hypoxia. prehospital pulse oximetry could serve as an early warning signal for the detection of happy hypoxemia in COVID-19 patients.
