Opioid Prescriptions in Women: Is the data Accurate?

Background: In 2012, U.S. health care providers wrote more than 259 million opioid prescriptions, which is twice as many as in 1998. Approximately 1 in 10 women report the use of opioids for pain management during pregnancy. The Centers for disease Control and Prevention (CdC) estimated that between 2008 and 2012, 39% of reproductive-aged women on Medicaid had filled a prescription for opioid medication each year, as did 28% of women with private insurance. The opioid epidemic extends to the state of New Jersey (NJ);however, limited data is available regarding opioid prescriptions among Medicaid and private insurance patients within the state. Objective: Evaluate opioid prescriptions filled in reproductive-aged women presenting in labor at a community teaching hospital in suburban New Jersey. Methods: We performed a retrospective cohort study using data obtained from patient records and the New Jersey Prescription Monitoring Program (NJPMP) database. We enrolled 200 patients that were admitted in labor between May 2015 and May 2016. data was collected from reproductive-aged women during the one year preceding labor admission. We compared our findings to national data reported by the CdC using Chi-square analysis. Maternal demographic data were extracted from patient records and included age, insurance status (private insurance, Medicaid, and no insurance), race, and ethnicity. The primary outcome was opioid prescriptions filled. Results: Of the 200 women admitted in labor, 129 had private insurance, 63 had Medicaid, and 8 had no insurance. We found that 5.4% (7/129) of patients with private insurance, 4.8% (3/63) of patients with Medicaid, and 12.5% (1/8) of patients with no insurance filled opioid prescriptions. Overall, 5.5% (11/200) of women filled opioid prescriptions during the study period. Opioid prescriptions confirmed via NJPMP were significantly lower than rates reported by the CdC in Medicaid (4.8% vs. 41.4%, p-value 0.001) and private insurance (5.4% vs.

Home Range Size and Overlap of the Small Nocturnal Schlegel’s Japanese Gecko (Gekko japonicus), Introduced into a City Park in Korea

Studying the home range of an organism is important in understanding its ecology. due to being cryptic, few studies have been conducted on the home range studies of small, nocturnal geckos. We conducted radio-tracking surveys for 23 individuals to estimate the home range size and home range overlap of the Schlegel’s Japanese gecko(Gekko japonicus) previously introduced into a suburban city park. Individuals were commonly found in artificial structures(buildings and accessory structures) and on nearby natural trees. daily moved distance was positively correlated with home range size. Minimum convex polygon(MCP) home range was 97.8 m^2 for females and 99.5 m^2 for males, on average. Gekko japonicus moved farther daily distances and used wider MCP and Kernel 95 home ranges in breeding season compared to non-breeding season, while the size of Kernel 50 home range did not differ between seasons. Both daily moved distance and home range size were not significantly different between sexes. In the breeding season, MCP and Kernel 50 home ranges of each gecko overlapped with 32.4% and 13.8% of remaining geckos, respectively. Our results not only show that 1) G. japonicus uses both artificial structures and adjacent natural trees as microhabitat, but also suggest that 2) G. japonicus is non-territorial, but has a core habitat that is shared with few other individuals, and 3) the reproductive system of G. japonicus is polygamous.

Consistent differences in behavior between individuals, otherwise known as animal personalities, have become a staple in behavioral ecology due to their ability to explain a wide range of phenomena. Social organisms are especially serviceable to animal personality techniques because they can be used to explore behavioral variation at both the individual and group level. despite the suc- cess of personality research in social organisms generally, and social Hymenoptera in particular, social wasps （Vespidae） have received little to no attention in the personality literature. In the pre- sent study, we test Polistes metricus （Vespidae; Polistinae） paper wasp queens for the presence of repeatable variation in, and correlations （＂behavioral syndromes＂） between, several commonly used personality metrics： boldness, aggressiveness, exploration, and activity. Our results indicate that P. metricus queens exhibit personalities for all measured traits and correlations between differ- ent behavioral measures. Given that paper wasps have served as a model organism for a wide range of phenomena such as kin selection, dominance hierarchies, mate choice, facial recognition, social parasitism, and chemical recognition, we hope that our results will motivate researchers to explore whether, or to what degree, queen personality is important in their research programs.

Evaluation of coarse-grained CFd-dEM models with the validation of PEPT measurements

Computational Fluid dynamics coupled with discrete Element Method (CFd-dEM) is a commonly used numerical method to model gas-solid flow in fluidised beds and other multiphase systems. A significant limitation of CFd-dEM is the feasibility of the realistic simulation of large numbers of particles. Coarse-graining (CG) approaches, through which groups of multiple individual particles are represented by single, larger particles, can substantially reduce the total number of particles while maintaining similar system dynamics. As these three CG models have not previously been compared, there remains some debate, however, about the best practice in the application of CG in CFd-dEM simulations. In this paper, we evaluate the performance of three typical CG methods based on simulations of a bubbling fluidised bed. This is achieved through the use of a numerical validation framework, which makes full use of the high-resolution 3d positron emission particle tracking (PEPT) measurements to rigorously validate the outputs of CFd-dEM simulations conducted using various different coarse-graining models, and various different degrees of coarse-graining. The particle flow behaviours in terms of the particle occupancy field, velocity field, circulation time, and bubble size and velocity, are comprehensively analysed. It is shown that the CG simulation starts to fail when the size ratio between the bed chamber and the particles decreases to approximately 20. It is also observed, somewhat surprisingly, that the specific CG approach applied to interparticle contact parameters does not have a substantial effect on the simulation results for the bubbling bed simulations across a wide range of CG factors.

Life after Addiction—Post-Operative Pain Management in an Obstetrical Patient on Long-Term Buprenorphine Therapy

A G6P2032 female, prior cesarean x3 with history of opioid addiction maintained on buprenorphine presented for scheduled repeat cesarean section. Pre-operatively, her maintenance dose of medication was held secondary to concerns for partial agonist effect. Post-operative pain control was suboptimal with the patient ultimately proceeding to withdrawal. doses of hydromorphone were titrated to 10 mg every 3 hours to avoid further withdrawal. Review of expert opinion after discharge recommended against holding buprenorphine therapy in the post-operative period. Pain management options include maintenance therapy with additional doses of opioid and non-opioid pain relieving medications.
