Pancreatic cancer is the second most common abdominal cancer in North America with an estimated 20%resectability at diagnosis,and overall 5-year survival of 5%.Pain is common in pancreatic cancer patients with 70%-80%suffering substantial *** plexus neurolysis(CPN)is a technique that can potentially improve pain control in pancreatic cancer while preventing further escalation of opioid *** is performed by injecting absolute alcohol into the celiac plexus neural network of *** review sets out to explore the current status of CPN in non-resectable pancreatic *** will examine:(1)the efficacy and safety of percutaneous-CPN and endoscopic ultrasound guided-CPN;(2)specific technique modifications including bilateral(vs central)injections and celiac ganglia neurolysis;and(3)the issue of CPN timing,early at pancreatic cancer diagnosis vs traditional late use as salvage therapy.

In recent years,deep learning methods have gradually come to be used in hyperspectral imaging *** of the peculiarity of hyperspectral imaging,a mass of information is contained in the spectral dimensions of hyperspectral ***,different ob jects on a land surface are sensitive to different ranges of *** achieve higher accuracy in classification,we propose a structure that combines spectral sensitivity with a convolutional neural network by adding spectral weights derived from predicted outcomes before the final classification ***,samples are divided into visible light and infrared,with a portion of the samples fed into networks during ***,two key parameters,unrecognized rate(δ)and wrongly recognized rate(γ),are calculated from the predicted outcome of the whole ***,the spectral weight,derived from these two parameters,is ***,the spectral weight is added and an improved structure is *** improved structure not only combines the features in spatial and spectral dimensions,but also gives spectral sensitivity a primary *** with inputs from the whole spectrum,the improved structure attains a nearly 2%higher prediction *** applied to public data sets,compared with the whole spectrum,on the average we achieve approximately 1%higher accuracy.

Purpose To determine the amount of familiarization sessions required by breast cancer survivors to achieve a reliable meas-urement of muscle function assessed using isokinetic *** Twenty-six breast cancer survivors performed three isokinetic knee extension tests separated by,at least,48 *** isokinetic testing protocol included one warm-up set of 10 submaximal knee extensions at 120°/s,followed by two sets of four maximal knee extensions at 60°/s,with 2-min rest interval between *** torque(PT),time to peak torque(TPT),angle of peak torque(APT),and average power(AP)of each trial was used for the assessment of testing *** change in the mean,typical error,coefficient of variation and intraclass correlation coefficients(ICC2.1)were calculated to determine test-retest *** For PT,change in mean was lower between trials 2 and 3 than between trials 1 and 2(4.18% and 13.18%,respec-tively),and ICC was greater between trials 2 and 3 than between trials 1 and 2(0.962 and 0.818,respectively).For TPT and APT,ICC was clinically acceptable only between trials 2 and 3(0.757 and 0.803,respectively).For AP,change in mean was clinically acceptable between trials 2 and 3(9.84%),while ICC met acceptable reliability between both,trials 1 and 2 and,trials 2 and 3(0.756 and 0.891,respectively).Conclusion At least one familiarization session is adequate to achieve reliable measurements of muscle function using isokinetic dynamometry,while avoiding the impact of learning effect of the measurements in breast cancer survivors.

Background： The global search for new ways to sequester carbon has already reached agricultural lands. Such land constitutes a major potential carbon sink. The production of high value timber within agroforestry systems can facilitate an in-situ carbon storage function. This is followed by a potential long term ex- situ carbon sinkwithin long lasting products such as veneer and furniture. For this purpose wild cherry （Prunus avium l.） is an interesting option for middle Europe, yielding high prices on the timber market. Methods： A total number of 39 wild cherry were sampled in 2012 and 2013 to assess the leafless above ground biomass. The complete trees including stem and branches were separated into 1 cm diameter classes. Wood and bark from sub-samples were analysed separately and nutrient content was derived. Models for biomass estimation were constructed for all tree compartments. Results： The smallest diameter classes possess the highest proportion of bark due to smaller cross sectional area. Tree boles with a greater amount of stem wood above 10 cm in diameter will have a more constant bark proportion. Total branch bark proportion also remains relatively constant above *** measurements of 8 cm. A balance is evident between the production of new branches with a low diameter and high bark proportion offset by the thickening and a relative reduction in bark proportion in larger branches. The results show that a single tree with an age of 17 and 18 years can store up to 85 kg of carbon within the aboveground biomass portion, an amount that will increase as the tree matures. Branches display greater nutrient content than stem sections per volume unit which can be attributed to a greater bark proportion. Conclusions： Using the derived models the carbon and the nutrient content of above-ground woody biomass of whole trees can be calculated. Study Objective: We studied the overall efficacy of fiberoptic aided intubation using three different supraglottic airways (SGA) as intubation conduits with a standard endotracheal tube (ETT) to determine which, if any, is superior as an intubation conduit. Design: After induction of general anesthesia, subjects were randomized to one of three groups: Air-Q IlATM, lMA Classic ExcelTM, and lMA UniqueTM. Subjects were intubated with a fiberoptic aided technique with continuous ventilation with FiO2 = 1.0 through one of these SGAs. The primary endpoint was the overall efficacy of the intubation procedure. In addition, the following data were collected: demographic data, intubation times, grade of view of the larynx, and a visual analog scale (VAS) score of difficulty as determined by the primary anesthesiologist performing the procedure. Data were analyzed using a Kruskal-Wallis one-way analysis of variance and Post hoc analysis was done using Dunn’s Multiple Comparison Test. Results: 126 total subjects were studied. Intubation success rates were 100%, 87.8%, and 95% with the Air-Q IlATM, lMA Classic ExcelTM, and lMA UniqueTM respectively. There was no significant difference among the three different SGAs when comparing the times to place the SGA (T1), the true intubating time (T2), the time to remove the SGA (T3), or the total time (T4). Data were also stratified by the grade of view of the larynx;all grade I views, grade II views, and grade III views were grouped together regardless of the type of the SGA used. The grade I view of the larynx group had significantly faster true intubation times (T2 = 75.1 sec, p = 0.01) and significantly lower VAS scores (VAS = 1.9, P = TM provides the best view of the larynx and is the easiest one to use as an intubation conduit.

Over the last 30 years there have been numerous worldwide investigators involved in cancer research. Billions of dollars have been spent on drug development and cancer research;however, with all of the new agents and modalities of treatment, we have honestly not significantly improved the overall survival of the Stage IV cancer patient. There is and will not be a magic bullet treatment, thus the extensive title of this paper. We are convinced that unless we use multiple innovative therapies in combination with conventional treatment, we will never truly defeat this disease. We have attempted to address this problem by presenting in detail some of these complex mechanisms involved in tumorigenesis, progression, escape, and metastasis. Most investigators have their own special area of interest, but if we are to conquer this scourge, we must develop an extensive, multifaceted, comprehensive approach. Hopefully this article will contribute to awareness and further insight into this very serious and complicated problem, so we can improve quality of life and improve the survival of the Stage IV cancer patient.

In this paper, we present a comparative study of Obama's closing statements in the presidential debates, in particular, the first and third debates with Mitt Romney in 2012. Obama was considered to have done poorly in the first debate, but very well in the third debate. A systemic-functional analysis of Obama's closing statements in these two debates reveals the lexicogrammatical and rhetorical choices which contribute to the functional and aesthetic value of Obama's debate performance.
