Factors associated with time to laparoscopic cholecystectomy for acute cholecystitis

AIM: To determine patient and process of care factors associated with performance of timely laparoscopic cholecystectomy for acute cholecystitis. METhODS: A retrospective medical record review of 88 consecutive patients with acute cholecystitis was conducted. Data collected included demographic data, co-morbidities, symptoms and physical findings at presentation, laboratory and radiological investigations, length of stay, complications, and admission service (medical or surgical). Patients not undergoing cholecystectomy during this hospitalization were excluded from analysis. hierarchical generalized linear models were constructed to assess the association of pre-operative diagnostic procedures, presenting signs, and admitting service with time to ***: Seventy cases met inclusion and exclusion criteria, among which 12 were admitted to the medical service and 58 to the surgical service. Mean ± SD time to surgery was 39.3 ± 43 h, with 87% of operations performed within 72 h of hospital arrival. In the adjusted models, longer time to surgery was associated with number of diagnostic studies and endoscopic retrograde cholangio-pancreatography (ERCP, P = 0.01) as well with admission to medical service without adjustment for ERCP (P h magnetic resonance cholangiopancreatography (MRCP) and computed tomography (CT) scans experienced the longest waits for surgery. Impact of a single bout of high-intensity interval exercise and short-term interval training on interleukin-6, FNDC5, and METRNL mRNA expression in human skeletal muscle

Background: Exercise promotes numerous phenotypic adaptations in skeletal muscle that contribute to improved function and metabolic capacity. An emerging body of evidence suggests that skeletal muscle also releases a myriad of factors during exercise, termed "myokines". The purpose of this study was to examine the effects of high-intensity interval training(hIIT) on the acute regulation of the mRNA expression of several myokines, including the prototypical myokine interleukin-6(IL-6), and recently identified myokines fibronectin type III domain-containing protein 5(FNDC5)(irisin) and meteorin-like protein(METRNL).Methods: Both before and after a 20-day period of twice-daily high-volume hIIT, 9 healthy males(20.5 ± 1.5 years performed a standardized bout of high-intensity interval exercise(hIIE; 5 × 4 min at ~80% pretraining peak power output) with skeletal muscle biopsy samples(vastus lateralis) obtained at rest, immediately following exercise, and at 3 h ***: Before training, a single bout of hIIE increased IL-6(p h recovery when compared to rest. Following 20 days of hIIT, IL-6 and FNDC5 mRNA were increased at 3 h recovery from the standardized hIIE bout when compared to rest(both p higher following training(p here was an overall increase in FNDC5 mRNA post-training(main effect of training, p human skeletal muscle(1) an acute bout of hIIE can induce upregulation of skeletal muscle IL-6 mRNA both before and after a period of intensified hIIT;(2) Resting and overall FNDC5 mRNA expression is increased by 20 days of hIIT; and(3) METRNL mRNA expression is responsive to both acute hIIE and short-term intense hIIT. Future studies are needed to confirm these findings at the protein and secretion level in humans.

While emerging data suggest nucleotide oligomerization domain receptor 1(NOD1),a cytoplasmic pattern recognition receptor,may play an important and complementary role in the immune response to bacterial infection,its role in cancer metastasis is entirely ***,we sought to determine the effects of NOD1 on ***1 expression in paired human primary colon cancer,human and murine colon cancer cells were determined using immunohistochemistry and immunoblotting(WB).Clinical significance of NOD1 was assessed using TCGA survival data.A series of in vitro and in vivo functional assays,including adhesion,migration,and metastasis,was conducted to assess the effect of NOD1.C12-iE-DAP,a highly selective NOD1 ligand derived from gram-negative bacteria,was used to activate ***130,a specific NOD1 inhibitor,was used to block C12-iE-DAP *** knockdown(KD)of NOD1 in human colon cancer cells(hT29)was constructed with shRNA lentiviral transduction and the functional assays were thus ***,the predominant signaling pathway of NOD1-activation was identified using WB and functional assays in the presence of specific kinase *** data demonstrate that NOD1 is highly expressed in human colorectal cancer(CRC)and human and murine CRC cell ***,we demonstrate that this increased NOD1 expression negatively impacts survival in patients with ***,we identify NOD1 activation by C12-iE-DAP augments CRC cell adhesion,migration and *** effects are predominantly mediated via the p38 mitogen activated protein kinase(MAPK)*** is the first study implicating NOD1 in cancer metastasis,and thus identifying this receptor as a putative therapeutic target.

Increased presence of effector lymphocytes during helicobacter hepaticus-induced colitis

AIM： To identify and characterize drosophila mothers against decapentaplegic （SMAD）3-dependent changes in immune cell populations following infection with he- Iicobacter hepaticus （h. hepaticus）. METhODS： SMAD3/ （n = L9） and colitis-resistant SMAD3＋/ （n = 24） mice （8-10 wk of age） were in- fected with/-/, hepaticus and changes in immune cell populations [T lymphocytes, natural killer （NK） cells, T regulatory cells] were measured in the spleen and mesenteric lymph nodes （MsLNs） at 0 d, 3 d, 7 d and 28 d post-infection using flow cytometry. Genotype-dependent changes in T lymphocytes and granzyme B＋ cells were also assessed after 28 d in proximal colon tissue using immunohistochemistry. RESULTS： As previously observed, SMAD3＋, but not SMAD3＋/- mice, developed colitis, peaking at 4 wk post-infection. No significant changes in T cell subsets were observed in the spleen or in the MsLNs between genotypes at any time point. however, CD4＋ and CD8＋/ CD62L＋＋ cells, an effector T lymphocyte population, as well as NK cells （NKp46/DX5＋） were significantly higher in the MsLNs of SMAD3/ mice at 7 d and 28 d post-in- fection. In the colon, a higher number of CD3＋ cells were present in SMAD3＋ compared to SMAD3＋/- mice at base- line, which did not significantly change during infection. however, the number of granzyme B＋ cells, a marker of cytolytic lymphoo/tes, significantly increased in SMAD3＋ mice 28 d post-infection compared to both SMAD3＋/- mice and to baseline values. This was consistent with more severe colitis development in these animals. CONCLUSION： Data suggest that defects in SMAD3 signaling increase susceptibility to h. hepaticus-induced colitis through aberrant activation and/or dysregulation of effector lymphoo/tes.

Characterizing and modeling the precipitation of Mg-rich phases in Al 5xxx alloys aged at low temperatures

Al 5083 alloys(5.25 at.% Mg) of different tempers(h131 and h116) were aged at low temperatures(50and 70?C) for 41 months. Scanning transmission electron microscopy(STEM), energy-dispersive X-ray spectroscopy(EDS), and atom probe tomography(APT) were applied to characterize precipitates formed in the sensitized samples. Experimental results revealed that the size of Mg-rich precipitates increased with aging time at 70?C for both alloys. APT results showed that Mg-rich precipitates of different Mg concentrations and morphologies formed in Al matrix and on the interface of Al matrix/pre-existing particles. In addition, a model based on local equilibrium of chemical potential and multi-class precipitates number evolution was adopted to predict the multiphase precipitation process in the Al-Mg binary system. The overall trend of precipitate radius and number density predicted by the model matched well with the experimental results. Moreover, modeling results revealed that nucleation and coarsening occurred faster in Al 5083 h131 than in Al 5083 h116 when aged at same temperature. The high density of dislocations and the pipe diffusion mechanism of dislocations can be used to explain such behavior.

Objective:Inflammatory serum markers have proven to be a powerful predictive tool of patient prognosis in cancer treatment for a wide variety of solid organ malignancies,predominantly in the context of localized *** this study we evaluated the preoperative neutrophil-to-lymphocyte ratio(NLR)as a predictive tool in patients with metastatic clear cell renal cell carcinoma(RCC).Methods:Sixty-four patients with metastatic clear cell RCC undergoing nephrectomy were *** patients with preoperative NLR were included for survival *** were categorized into high and low NLR score determined by plotting the NLR ROC *** analysis was ***:Median age was 60.8 years(38.2-81.2).Median follow-up time was 8.1 months(0.1-106.3).Fuhrman grade distribution was:2(3.1%)grade 1,6(9.4%)grade 2,24(37.5%)grade 3 and 32(50.0%)grade *** NLR score was 3.5(1.4-31.0).NLR4 was associated with decreased overall survival compared toNLRhowed NLR4 as an independent predictor of survival(hazard ratio(hR)2.41,95%CI 1.05-5.50,p=0.03).Conclusion:Elevated preoperative NLR is associatedwith poor prognosis in patients withmetastatic kidney *** NLR is a useful tool,which can predict prognosis,stratify patients for postoperative surveillance,and help guide decisions for therapy.

Understanding the way individuals are interconnected in social networks is of prime significance to predict their collective *** a large-scale dataset from a knowledge-sharing website,this paper presents an exploratory investigation of the way to depict structural diversity in directed networks and how it can be utilized to predict one’s online social *** capture the structural diversity of an individual,we first consider the number of weakly and strongly connected components in one’s contact neighborhood and further take the coexposure network of social neighbors into *** show empirical evidence that the structural diversity of an individual is able to provide valuable insights to predict personal online social reputation,and the inclusion of a coexposure network provides an additional ingredient to achieve that *** synthetically controlling several possible confounding factors through matching experiments,structural diversity still plays a nonnegligible role in the prediction of personal online social *** work constitutes one of the first attempts to empirically study structural diversity in directed networks and has practical implications for a range of domains,such as social influence and collective intelligence studies.

Employment and Patient Satisfaction after Liver Transplantation

Background and Aims:This study serves to revisit the effects of liver transplantation(LT)on employment in an era of improving survival outcomes post-transplant,and to identify areas of improvement in the transplant process to better optimize post-LT employment and patient ***:Prospectively,patients who had undergone LT at a single tertiary LT center were surveyed in person and by *** outcomes included employment rate pre-and post-LT,annual salary,weekly hours worked,barriers to re-employment,and patient ***:Responses were collected and analyzed from 121 patients who underwent ***-LT,68(56.1%)reported full-time employment,13(10.7%)part-time employment,and 40(33.1%)***-LT,26(21.4%)reported continued full-time employment,18(14.9%)part-time employment,and 77(63.6%)*** weekly work hours decreased post-LT(16.1 h/week vs.39.9 h/week).Mean annual salaries decreased post-LT(17 earning salary≥$40,000 vs.56 earning salary≥$40,000).These outcomes differed from patient pre-LT expectations,with 81.0%of previously employed patients believing they would return to employment,resulting in decreased patient *** working physically demanding jobs pre-LT were less likely to return to *** cited for lack of return to full employment included early fatigue and difficulty regaining physical ***:Re-employment rates remain low post-LT,which is particularly true for patients working physically active *** is a significant barrier to re-employment and increased physical rehabilitation post-LT may prove to be *** should be given realistic expectations about return to employment prior to their LT.
