BACKGROUND Cytoreductive surgery(CRS)with hyperthermic intraperitoneal chemotherapy(HIPeC)for peritoneal carcinomatosis can be performed in two ways:Open or closed abdominal *** To evaluate the impact of HIPeC method on post-operative and long-term survival *** Patients undergoing CRS with HIPeC from 2000-2017 were identified in the United States HIPeC collaborative ***-operative,recurrence,and overall survival outcomes were compared between those who received open vs closed *** Of the 1812 patients undergoing curative-intent CRS and HIPeC,372(21%)patients underwent open HIPeC and 1440(79%)underwent closed *** was no difference in re-operation or severe complications between the two *** HIPeC had higher rates of 90-d readmission while open HIPeC had a higher rate of 90-d *** multi-variable analysis,closed HIPeC technique was not a significant predictor for overall survival(hazards ratio:0.75,95%confidence interval:0.51-1.10,P=0.14)or recurrence-free survival(hazards ratio:1.39,95%confidence interval:1.00-1.93,P=0.05)in the entire *** findings remained consistent in the appendiceal and the colorectal *** In this multi-institutional analysis,the HIPeC method was not independently associated with relevant post-operative or long-term *** technique may be left to the discretion of the operating surgeon.

Ascorbic acid and low-volume polyethylene glycol for bowel preparation prior to colonoscopy:A meta-analysis

AIM: To evaluate the benefits of low-volume polyethylene glycol(PeG) with ascorbic acid compared to fulldose PeG for colonoscopy preparation. MeTHODS: MeDLINe, Cochrane Central Register of Controlled Trials and Database of Systematic Reviews, CINAHL, Pub Med, and recent abstracts from major conferences were searched(January 2012). Only randomized-controlled trials on adult subjects comparing lowvolume PeG(2 L) with ascorbic acid vs full-dose PeG(3 or 4 L) were included. Meta-analysis for the efficacy of low-volume PeG with ascorbic acid and full-dose PeG were analyzed by calculating pooled estimates of number of satisfactory bowel preparations as well as adverse patient events(abdominal pain, nausea, vomiting). Separate analyses were performed for each main outcome by using OR with fixed and random effects models. Heterogeneity was assessed by calculating the I2 measure of inconsistency. Rev Man 5.1 was utilized for statistical ***: The initial search identified 242 articles and trials. Nine studies(n = 2911) met the inclusion criteria and were analyzed for this meta-analysis with mean age range from 53.0 to 59.6 years. All studies were randomized controlled trials on adult patients comparing large-volume PeG solutions(3 or 4 L) with low-volume PeG solutions and ascorbic acid. No statistically significant difference was noted between lowvolume PeG with ascorbic acid and full-dose PeG for number of satisfactory bowel preparations(OR 1.07, 95%CI: 0.86-1.33, P = 0.56). No statistically significant difference was noted between low-volume PeG with ascorbic acid and full-dose PeG for abdominal pain(OR 1.09, 95%CI: 0.81-1.48, P = 0.56), nausea(OR 0.70, 95%CI: 0.49-1.00, P = 0.05), or vomiting(OR 0.99, 95%CI: 0.78-1.26, P = 0.95). Prevalence and Antimicrobial Susceptibility Patterns of escherichia Coli and Salmonella Typhi Isolated from Meat and Fish Samples from Selected Markets in a Metropolis and District in Ghana

Antimicrobial resistance has become a major challenge to the treatment and prevention of infections resulted in high morbidity and mortality globally. The inappropriate or abuse of antibiotics in animal farming is a key factor and thus led to the emergence of bacteria resistance and subsequent transfer of resistance genes to humans through the food chain. This study was to determine the prevalence of Salmonella typhi (S. typhi) and escherichia coli (e. coli) isolated from various meat and fish samples and their susceptibility patterns against five commonly used antibiotics in Ghana (Ciprofloxacin 5 μg, Amoxicillin-clavulanic acid 20/10 μg, Imipenem 10 μg, Tetracycline 30 μg and Ceftazidime 30 μg). A total of 105 meat and fish samples were obtained from Tema and Prampram markets and bacteria isolation was carried out using appropriate selective microbial culture media and various biochemical methods for identification. The susceptibility patterns were determined using the Kirby-Bauer disk diffusion method and the results were interpreted using the CLSI 2020 guidelines. The results revealed a total of 56 bacterial isolates comprising 14 e. coli (25%) and 42 S. typhi (75%) isolated from the meat and fish samples. The antibiogram study showed a high resistance rate (88.64%) of S. typhi isolates to amoxicillin-clavulanic acid and tetracycline (97.73%). A moderate susceptibility of the isolates was obtained with imipenem (53.27%). All the e. coli isolates were resistant to tetracycline (100%) and demonstrated 78.57% and 50% resistance to amoxicillin-clavulanic acid and ceftazidime respectively. A total of 78.57% of the e. coli isolates and 68.18% of S. typhi isolates showed multidrug resistance. The multiple antibiotic resistance (MAR) index for all the isolates ranged from 0.2 to 1.00 with two S. typhi isolates and one e. coli isolate having a MAR index of 1.00 signifying total resistance to all the 5 antibiotics tested. Optimizing perioperative Crohn's disease management: Role of coordinated medical and surgical care

AIM: To investigate rates of re-establishing gastroenterology care, colonoscopy, and/or initiating me-dical therapy after Crohn's disease(CD) surgery at a tertiary care referral ***: CD patients having small bowel or ileocolonic resections with a primary anastomosis between 2009-2012 were identified from a tertiary academic referral center. CD-specific features, medications, and surgical outcomes were abstracted from the medical record. The primary outcome measure was compliance rates with medical follow-up within 4 wk of hospital discharge and surveillance colonoscopy within 12 mo of surgery. ReSULTS: eighty-eight patients met study inclusion criteria with 92%(n = 81) of patients returning for surgical follow-up compared to only 41%(n = 36) of patients with documented gastroenterology follow-up within four-weeks of hospital discharge, P ed with more timely postoperative medical follow-up included younger age, longer length of hospitalization, postoperative biologic use and academic center patients. In the study cohort, 75.0% of patients resumed medical therapy within 12 mo, whereas only 53.4% of patients underwent a colonoscopy within 12 mo of ***: Our study highlights the need for coordinated CD multidisciplinary clinics and structured handoffs among providers to improve of quality of care in the postoperative setting.

Adsorptive removal and dynamic interaction of three different pharmaceutical pollutants,namely rifampicin(RIF),streptomycin(STM),and ibuprofen(IBU)onto chitosan were systematically investigated using a batch adsorption technique at different processing *** this study,chitosan was derived from mud-crab shells,as an innovative way to use the waste from marine foods as *** kinetics,intraparticle diffusion,mechanism,and thermodynamics of the adsorption were systematically evaluated and analyzed using kinetic models,Boyd mass transfer and WebereMorris intraparticle diffusion models,Langmuir,Freundlich,DubinineRadushkevich,and Temkin isotherm models,and the Gibbs *** adsorption isotherm of the larger molecules,RIF and STM,could be explained by the Langmuir isotherm model,in contrast,that of IBU,which is a much smaller molecule,followed the Freundlich isotherm *** maximum adsorption capacity of RIF,STM,and IBU on chitosan was estimated to be 66.91 mg g^(-1),11.00 mg g^(-1),and 24.21 mg g^(-1),respectively,which are higher compared to those on a variety of agricultural wastes,suggesting that this biopolymer is a potential practical and economical adsorbent to remove the pharmaceutical compounds from *** adsorption mechanism of the pharmaceutical compounds on chitosan is proposed based on the vibrational spectroscopic analyses,XRD patterns,and DSC thermograms of the biopolymer before and after adsorption process.
