Vaginal Birth after Cesarean after Zavanelli Maneuver: A Woman’s Right to Choose

Introduction: Vaginal birth after cesarean (VBAC) plays an essential role in lowering cesarean rates. Despite endorsement, trial of labor after cesarean (TOLAC) attempt rates remain low, in part due to fear of lawsuits. Zavanelli maneuver is a last resort procedure in the management of shoulder dystocia. We discuss a case of a woman determined to have a vaginal birth after her prior birth was complicated by shoulder dystocia requiring a Zavanelli maneuver. Her physicians were reluctant to allow her a TOLAC given her prior obstetric history. Case: A 34-year-old para 1 with prior cesarean delivery due to shoulder dystocia that required Zavanelli maneuver presents determined to pursue VBAC in her current pregnancy. She considered her delivery route options and addressed her modifiable risk factors. She consulted with multiple perinatologists who agreed that a TOLAC was reasonable, however she had to travel more than 70 miles (from Pennsylvania to New Jersey) to find an obstetrical practice and hospital willing to consider VBAC. She transferred care and the remainder of her prenatal course was uncomplicated. She went into labor at 41 weeks and had a successful VBAC without complication. In a thank you letter to her obstetrician, she described her birth experience as euphoric. Conclusion: This case illustrates how a woman’s choice of delivery route may be impacted by fear of litigation. Local providers focused on her prior delivery instead of her overall improved risk profile. Delivery route decisions should be based on a thorough evaluation of all risk factors and individualized to meet the reproductive goals of each woman. .

Landslide Research at the British Geological Survey:Capture,Storage and Interpretation on a National and Site-Specific Scale

Landslide research at the British Geological Survey （BGS） is carried out through a number of activities, including surveying, database development and real-time monitoring of landslides. Landslide mapping across the UK has been carried out since BGS started geological mapping in 1835. Today, BGS geologists use a combination of remote sensing and ground-based investigations to survey landslides. The development of waterproof tablet computers （BGS＇SIGMAmobile）, with inbuilt GPS and GIS for field data capture provides an accurate and rapid mapping methodology for field surveys. Regional and national mapping of landslides is carried out in conjunction with site-specific monitoring, using terrestrial LiDAR and differential GPS technologies, which BGS has successfully developed for this application. In addition to surface monitoring, BGS is currently developing geophysical ground-imaging systems for landslide monitoring, which provide real-time information on subsurface changes prior to failure events. BGS＇s mapping and monitoring activities directly feed into the BGS National Landslide Database, the most extensive source of information on landslides in Great Britain. It currently holds over 14 000 records of landslide events. By combining BGS＇s corporate datasets with expert knowledge, BGS has developed a landslide hazard assessment tool, GeoSure, which provides information on the relative landslide hazard susceptibility at national scale.

Estimation of bioclimatic variables of Mongolia derived from remote sensing data

Control of modifiable risk factors and major adverse cardiovascular events in people with peripheral artery disease and diabetes

Land Suitability Evaluation for Agricultural Cropland in Mongolia Using the Spatial MCDM Method and AHP Based GIS

The purpose of this study was to prepare a cropland suitability map of Mongolia based on comprehensive landscape principles, including topography, soil properties, vegetation, climate and socio-economic factors. The primary goal was to create a more accurate map to estimate vegetation criteria (above ground biomass AGB), soil organic matter, soil texture, and the hydrothermal coefficient using Landsat 8 satellite imagery. The analysis used Landsat 8 imagery from the 2016 summer season with a resolution of 30 meters, time series MODIS vegetation products (MOD13, MOD15, MOD17) averaged over 16 days from June to August 2000-2016, an SRTM DEM with a resolution of 30 meters, and a field survey of measured biomass and soil data. In total, 6 main factors were classified and quality evaluation criteria were developed for 17 criteria, each with 5 levels. In this research the spatial MCDM (multi-criteria decision-making) method and AHP based GIS were applied. This was developed for each criteria layer’s value by multiplying parameters for each factor obtained from the pair comparison matrix by the weight addition, and by the suitable evaluation of several criteria factors affecting cropland. General accuracy was 88%, while PLS and RF regressions were 82.3% and 92.8%, respectively.

Demonstration of Pilot Scale Large Aperture Parabolic Trough Organic Rankine Cycle Solar Thermal Power Plant in Louisiana

During the calendar year of 2012 the University of Louisiana at Lafayette in conjunction with CLECO Power LLC (CLECO) has constructed and commissioned a pilot scale parabolic trough solar thermal power plant for the first time in Louisiana. The large aperture trough (LAT) solar collectors were provided by Gossamer Space Frames and are coupled with an organic Rankine cycle (ORC) power block provided by ElectraTherm, Inc. for study of the feasibility of cost-effective commercial scale solar thermal power production in Louisiana. Supported by CLECO and providing power to the existing CLECO grid, the implementation of state-of-the-industry collector frames, mirrors, trackers, and ORC power block is studied under various local weather conditions which present varied operating regimes from existing solar thermal installations. The solar collectors provide a design output of 650 kWth and preliminary actual performance data from the system level is presented. The optimal size, configuration and location for such a plant in the given solar resource region are being studied in conjunction with CLECO’s search for optimal renewable energy solutions for the region. The pilot scale size of the facility and implementation of the simpler ORC allow remote operation of the facility and flexibility in operating parameters for optimization studies. The construction of the facility was supported by the Louisiana Department of Natural Resources, the U.S. Department of Energy, and CLECO. The continued operation of the plant is supported by CLECO Power LLC and the University of Louisiana at Lafayette.

Numerical Model and Performance Validation of a Small-Scale Concentrating Solar Thermal Power Plant in Louisiana

A simplified numerical model of a small-scale (25 - 100 kWe) parabolic concentrating solar power (CSP) plant is presented that can be utilized during the planning stages for a CSP plant, utilizing only simplified information that would be available at the preliminary stages of a project. This is important because existing models currently used for planning purposes, such as the System Advisor Model (SAM) from the National Renewable Energy Laboratory (NREL), do not cover small-scale CSP plants. The model can be used to predict real-time performance, or it can be used with TMY data to estimate annual performance. The model was validated using performance data from an operating small-scale CSP power plant, which is a unique contribution of this work. The results showed that the model correlated well with actual operating measurements for all seasons of the year, and provided a useful tool for planning of future small-scale CSP plants.

Greenhouse gas （GHG） emissions from the surface soils and surface water receiving animal excreta may be important components of the GHG balance of terrestrial ecosystems, but the associated processes are poorly documented in tropical environments, especially in tropical arid and semi-arid areas. A typical sylvo-pastoral landscape in the semi-arid zone of Senegal, West Africa, was investigated in this study. The study area （706 km2 of managed pastoral land） was a circular zone with a radius of 15 km centered on a borehole used to water livestock. The landscape supports a stocking rate ranging from 0.11 to 0.39 tropical livestock units per hectare depending on the seasonal movements of the livestock. Six landscape units were investigated （land in the vicinity of the borehole, natural ponds, natural rangelands, forest plantations, settlements, and enclosed plots）. Carbon dioxide （CO2）, nitrous oxide （N2O） and methane （CH4） fluxes were measured with static chambers set up at 13 sites covering the six landscape units, and the 13 sites are assumed to be representative of the spatial heterogeneity of the emissions. A total of 216 fluxes were measured during the one-year study period （May 2014 to April 2015）. At the landscape level, soils and surface water emitted an average 19.8 t C-CO2 eq/（hm2.a）（CO2： 82%, N2O： 15%, and CH4： 3%）, but detailed results revealed notable spatial heterogeneity of GHG emissions. CO2 fluxes ranged from 1148.2 （±91.6） mg/（m2.d） in rangelands to 97,980.2 （±4,861.7） mg/（m2.d） in surface water in the vicinity of the borehole. N2O fluxes ranged from 0.6 （±0.1） mg/（m2.d） in forest plantations to 22.6 （±10.8） mg/（m2.d） in the vicinity of the borehole. CH4 fluxes ranged from -3.2 （±0.3） mg/（m2.d） in forest plantations to 8788.5 （±2295.9） mg/（m2.d） from surface water in the vicinity of the borehole.
