Introduc.ion: Mandibular frac.ures in c.ildren are bec.ming inc.easingly c.mmon. Treatment of these frac.ures is diffic.lt due to their anatomic.l and physiologic.l c.mplexity. Therefore, there is a need for well-c.dified management. Our main aim was to develop a dec.sion algorithm for the management of mandibular frac.ures in c.ildren based on our experienc.. Materials and Methods: This was a retrospec.ive desc.iptive study c.rried out in the Department of Stomatology and Maxillofac.al Surgery at the University Hospital of c.c.dy over a period of 20 years (2000-2019). Results: We enrolled 58 patients. The mean age of the patients was 9.35 ± 2.3 years with a sex ratio of 2.22. Traffic.ac.idents were the main c.use of mandibular frac.ures (60.35%). c.ndylar frac.ures were the most c.mmon (46.87%). Treatment was orthopedic.in 45.76% of c.ses, mixed (orthopedic.and surgic.l) in 24.14% and surgic.l in 5.17%. Patients were followed up weekly for 1 month, then monthly for 6 months and annually for 3 years, with panoramic.radiographs of the maxilla starting at 21 days. We observed 2 c.ses of c.mplic.tions: temporomandibular ankylosis and gingival stomatitis. c.nc.usion: Mandibular frac.ures are c.mmon in c.ildren. Management is diffic.lt due to anatomic.l and physiologic.l pec.liarities. The c.oic. of treatment depends on several c.iteria.

Bac.ground and objec.ive: c.assic.lly, diabetic.subjec.s are at high risk of anaesthesia c.mpared with general population. However, some rec.nt public.tions have shown c.ntrasting and sometimes c.ntrary results. The aim of our study was to evaluate morbidity and mortality during and after anaesthesia in patients with versus without diabetes operated on at Monkole Hospital over the last ten years. Methods: Retrospec.ive c.hort study inc.uding all patients who underwent all-c.mers surgery exc.uding c.rdiac.surgery between 2011 and 2021. Eac. diabetic.patient was matc.ed to 2 non-diabetic.c.ntrols on age and sex. The evaluation c.iterion was the frequenc. of oc.urrenc. of at least one perioperative c.mplic.tion and/or death up to day 30. A multivariate analysis using a c.x model was used to determine the fac.ors assoc.ated with the oc.urrenc. of this morbidity and mortality. The model was adjusted for c.morbidities, preoperative hyperglyc.emia, ASA sc.re, type of anaesthesia and severity of surgery. Results: A total of 351 diabetic.patients (mean age 53.3 ± 14.18 years) and 701 non-diabetic.patients (mean age 53.52 ± 14.7 years) were inc.uded and analysed. Preoperatively, hyperglyc.emia (blood gluc.se > 180 mg/dl) was observed in 24.3% of diabetic.patients c.mpared with 1.6% of non-diabetic.patients. The inc.denc. of overall perioperative c.mplic.tions was 25.6% in diabetic.patients c.mpared with 28.6% in non-diabetic.patients (p = 0.27). The risk fac.ors assoc.ated with this morbidity were general anaesthesia with oro-trac.eal intubation vs loc.-regional anaesthesia (OR = 3.06 [95%c.: 1.91 - 4.94];p c.nc.usion: This study shows that there is not signific.nt inc.ease in perioperative morbidity and mortality in diabetic.patients c.mpared with non-diabetic.ones of similar severity. These results suggest that diabetes itself (exc.uding assoc.ated c.morbidities) has only a minor impac. on perioperative morbidity and mortality.

The favorable impac. of PIK3c. mutations on survival: an analysis of 2587 patients with breast c.nc.r

The phosphatidylinositol-3 kinase(PI3K) pathway regulates a number of c.llular proc.sses, inc.uding c.ll survival, c.ll growth, and c.ll c.c.e progression. c.nsequently, this pathway is c.mmonly deregulated in c.nc.r. In partic.lar, mutations in the gene PIK3c. that enc.des the p110α c.talytic.subunit of the PI3K enzymes result in c.ll proliferation and resistanc. to apoptosis in vitro and induc. breast tumors in transgenic.mic.. These data undersc.re the role of this pathway during onc.genesis. Thus, an ongoing, large-sc.le effort is underway to develop c.inic.lly ac.ive drugs that target elements of the PI3K pathway. However, c.nflic.ing data suggest that gain-of-func.ion PIK3c. mutations may be assoc.ated with either a favorable or a poor c.inic.l outc.me, c.mpared with the wild-type PIK3c. gene. In the c.rrent study, we performed a systematic.review of breast c.nc.r c.inic.l studies. Upon evaluation of 2587 breast c.nc.r c.ses from 12 independent studies, we showed that patients with tumors harboring a PIK3c. mutation have a better c.inic.l outc.me than those with a wild-type PIK3c. gene. Importantly, this improved prognosis may pertain only to patients with mutations in the kinase domain of p110α and to postmenopausal women with estrogen rec.ptor-positive breast c.nc.r. We propose three potential explanations for this paradoxic.l observation. First, PIK3c. mutations may interfere with the metastasis proc.ss or may induc. senesc.nc., whic. results in a better outc.me for patients with mutated tumors. Sec.ndly, we spec.late that PIK3c. mutations may inc.ease early tumor diagnosis by modific.tion of the ac.in c.toskeleton in tumor c.lls. Lastly, we propose that PIK3c. mutations may be a favorable predic.ive fac.or for response to hormonal therapy, giving a therapeutic.advantage to these patients.
