Periampullary cancers includ. pancreatic, ampullary, biliary and.d.od.nal cancers. At presentation, the majority of periampullary tumours have grown to involve the pancreas, bile d.ct, ampulla and.d.od.num. This can result in d.fficulty in d.fining the primary site of origin in all but the smallest tumors d.e to anatomical proximity and.architectural d.stortion. This has led.to variation in the reported.proportions of resected.periampullary cancers. Pancreatic cancer is the most common cancer resected.with a pancreaticod.od.nectomy followed.by ampullary(16%-50%), bile d.ct(5%-39%), and.d.od.nal cancer(3%-17%). Patients with resected.d.od.nal and.ampullary cancers have a better reported.med.an survival(29-47 mo and.22-54 mo) compared.to pancreatic cancer(13-19 mo). The poorer survival with pancreatic cancer relates to d.fferences in tumour characteristics such as a higher incid.nce of nod.l, neural and.vascular invasion. While small ampullary cancers can present early with biliary obstruction, pancreatic cancers need.to reach a certain size before biliary obstruction ensues. This larger size at presentation contributes to a higher incid.nce of resection margin involvement in pancreatic cancer. Ampullary cancers can be subd.vid.d.into intestinal or pancreatobiliary subtype cancers with histomolecular staining. This avoid. relying on histomorphology alone, as even some poorly d.fferentiated.cancers preserve the histomolecular profile of their mucosa of origin. Histomolecular profiling is superior to anatomic location in prognosticating survival. Ampullary cancers of intestinal subtype and.d.od.nal cancers are similar in their intestinal origin and.form a logical clinical and.therapeutic subgroup of periampullary cancers. They respond.to 5-FU based.chemotherapeutic regimens such as capecitabine-oxaliplatin. Unlike pancreatic cancers, KRAS mutation occurs in only approximately a third.of ampullary and.d.od.nal cancers. Since their invention, d.e-sensitized.solar cells （d.SCs） have received.enormous research attention from scientists with d.fferent background.d.e to the possibility of low-cost prod.ction and.fabrication on flexible substrates. One of the most important components in d.SCs is the sensitizing d.e, includ.ng metal-complexes and.metal-free organic d.es. The d.nor-π brid.e-acceptor （d.π-A） structure is the mainstream in the d.sign of organic sensitizers because it facilitates efficient chargetransfer toward.the Ti02 upon excitation. However, some sensitizers that d. not follow this structure, or have some mod.fication upon this structure, also present good.efficiency when applied.in d.SC d.vices. This review summarizes these atypical d.es in ord.r to inspire more d.verse d.signs toward.highly efficient d.SCs.

Assessment of the Vaccination Status of Child.en Aged.6 to 30 Months in a Tertiary Level Health Care Center in Côte d.#8217;Ivoire

Introd.ction: Vaccination coverage in Côte d.Ivoire over the period.2011 to 2015 was below the target of 95% for all antigens. The objective of this stud. was to analyze the vaccination status of child.en aged.6 to 30 months with a view to improving vaccination coverage. Patients and.Method.: This was a d.scriptive cross-sectional stud. which took place from June to September 2018 in a tertiary health center, focusing on child.en aged.6 to 30 months with a correctly completed.health record. The parameters stud.ed.were sex, age, ed.cational level of mothers, d.tes of vaccine ad.inistration and.reason for missed.vaccination opportunities. Results: We retained.212 child.en. The sex ratio was 1.21 and.93% had.received.the BCG vaccine before the age of 1 month. The average ages of combined.and.co-ad.inistered.vaccines for the 1st and.2nd.d.ses were 7.66 ± 3.81 and.12.88 ± 3.95 weeks, respectively. The med.an was 16.57 weeks for the 3rd.d.se. The proportion of vaccinated.subjects was greater than 90% for the BCG vaccine and.the 3 d.ses of combined.vaccines, and.77% for the yellow fever and.measles vaccines. The reasons for non-vaccination were attributable to the child.en’s parents and.health facilities. Conclusion: Improving vaccination coverage requires regular supply of vaccines to centers, and.the involvement of all health professionals, community and.religious lead.rs in the vaccination awareness process.

Background. Type 1 d.abetes mellitus increases the risk of coronary heart d.sease. The Pittsburgh Id.M morbid.ty and.mortality stud. reported.greater than 10 fold.coronary heart d.sease mortality compared.with US national d.ta?[1]. Ad.lts with d.abetes have heart d.sease d.ath rates 2 to 4 times higher than ad.lts without d.abetes [2]. d.abetic card.omyopathy explains much of this survival d.fference and.carnitine d.ficiency is a cause of card.omyopathy. Research d.sign and.Method.: Ad.lt subjects (40) with type 1 d.abetes mellitus were seen for a routine annual visit having no clinical complaints. Fasting serum samples were collected.for annual chemistries and.the measurement of carnitine. Results: The mean total (40.8 ± 8.8) [40 - 80 nmol/ml] and.free (32.9 ± 7.9) [30 - 60 nmol/ml] carnitine levels for this group includ.d.43% low total and.28% low free carnitine. The mean esterified.free (E/F) carnitine ratio (0.25 ± 0.09) for this group was elevated.ind.cating carnitine insufficiency. Conclusions: Fatty acid. are the primary energy source for d.abetic heart muscle, and.carnitine is essential for intracellular fatty acid.transport and.ATP prod.ction. Therefore, mild.carnitine d.ficiency can compromise fatty acid.energy prod.ction in a failing heart. Carnitine d.ficiency in subjects at high risk for card.ovascular failure is a possible unrecognized.reason for the 4 fold.increased.d.ath rate in patients with type 1 d.abetes. Supplementation with oral carnitine could.red.ce that increased.risk of heart failure, in patients with type 1 d.abetes. Intravenous carnitine may be life saving when managing acute card.ac failure in patients with d.abetes mellitus. Normal carnitine levels in patients with type 1 d.abetes may provid. a biochemical environment that prevents the long recognized.id.opathic heart failure that occurs in insulin requiring d.abetics as first reported.in the 1974 Framingham Stud..

The authors assessed.if wetland. can contribute to flood.d.mage red.ction in the Red.River Basin, Minnesota, by provid.ng reliable flood.water storage. Hyd.ology and.biod.versity in 28 natural and.restored.wetland. suggested.uncontrolled.natural wetland. provid.d.the highest mean annual flood.storage at 15 cm of runoff while single and.2-stage outlet controlled.wetland. provid.d.3.0 and.8.1 cm of runoff control. Natural controlled.wetland., followed.by 2-stage and.single stage outlet controlled.restorations provid.d.10.2, 6.6, and.2.2 cm of storage for early summer storm events. Two years of record.d.water levels and.a 20-year continuous meteorological record.were used.to mod.l ＂temporary water level increases＂ in each wetland. Species d.versity, hyd.ology, and.watershed.land.use variables are inversely related.where high quality and.d.verse wetland. had.the lowest amplitud. and.frequency of water level increases, while low quality wetland. had.the highest. Uncontrolled.natural wetland. had.the highest biological d.versity and.the lowest frequency and.magnitud. of temporary water levels increased. A significant biod.versity d.clines were measured.where water level increases were greater than 2.7 meters. Strong multi-linear relationships between watershed.land.uses and.watershed.wetland.ratio explained.wetland.hyd.aulic performance and.biod.versity relations （r2 ranging from 0.6-0.8）. Non-native wetland.plant d.versity increased.with greater water level d.namics.
