Factors associated with increased incidence of severe toxicities following yttrium-90 resin microspheres in the treatment of hepatic malignancies

AIM: To further define variables associated with increased incidences of severe toxicities following administration of yttrium-90 (90Y) ***: Fifty-eight patients undergoing 79 treatments were retrospectively assessed for development of clinical and laboratory toxicity incidence following 90Y administration. Severe toxicity events were defined using Common Terminology Criteria for Adverse Events version 4.03 and defined as grade ≥ 3. Univariate logistic regression analyses were used to evaluate the effect of different factors on the incidence of severe toxicity events. Multicollinearity was assessed for all factors with P ded due to multicollinearity were included in a multivariate logistic regression model for each measurement of severe ***: Severe (grade ≥ 3) toxicities occurred following 21.5% of the 79 treatments included in our analysis. The most common severe laboratory toxicities were severe alkaline phosphatase (17.7%), albumin (12.7%), and total bilirubin (10.1%) toxicities. decreased pre-treatment albumin (OR = 26.2, P = 0.010) and increased pre-treatment international normalized ratio (INR) (OR = 17.7, P = 0.048) were associated with development of severe hepatic toxicity. Increased pre-treatment aspartate aminotransferase (AST; OR = 7.4, P = 0.025) and decreased pre-treatment hemoglobin (OR = 12.5, P = 0.025) were associated with severe albumin toxicity. Increasing pre-treatment model for end-stage liver disease (MELd) score (OR = 1.8, P = 0.033) was associated with severe total bilirubin toxicity. Colorectal adenocarcinoma histology was associated with severe alkaline phosphatase toxicity (OR = 5.4, P = 0.043).CONCLUSION: Clinicians should carefully consider pre-treatment albumin, INR, AST, hemoglobin, MELd, and colorectal histology when choosing appropriate candidates for 90Y microsphere therapy.

dear Editor,Gut microorganisms process food in animal guts and release many metabolic by-products,which are predicted to influence host physiological processes such as energy and lipid ***,we investigate how succi-nate,a TCA cycle intermediate that is a major predicted release product of gut bacteria in drosophila,influences the nutritional physiology of its drosophila *** ad-ministered succinate as a dietary supplement to microbe-free drosophila,and quantified key nutritional *** succinate significantly reduced fly lipid levels by up to~50%.This response was not replicated in paral-lel experiments conducted with dietary fumarate supple-ment,indicating that it could not be attributed to a gen-eral effect of TCA *** hypothesize that microbe-derived succinate may contribute to the reduced lipid content of drosophila bearing gut bacteria,relative to axenic *** generally,this study high-lights the importance of microbial-derived metabolites as regulators of host metabolism.

Coupling a Gas Chromatography Unit to an Inductively Coupled Plasma Mass Spectrometer

Although the eminent threat of a terrorist group detonating an improvised nuclear device (INd) in an urban environment is low, it is crucial that countries develop modern nuclear forensic capabilities to expedite response in a post-detonation scenario. In particular, new instruments need to be created to shorten dissolution time, expedite chemical separation, and improve forensic analysis of the nuclear melt glass that is created during the detonation of the device. To expedite this process, an instrument was designed to thermally couple a gas chromatograph (GC) to a time-of-flight inductively coupled plasma time-of-flight mass spectrometer (ICPTOFMS) In order to couple these two instruments, another instrument was designed to provide an isothermal atmosphere between the GC and TOFICPMS to expedite rapid gas separations processes. By using gas separations instead of the current wet chemistry processes, the required separation and analysis time of the melt glass significantly decreases. The new instrument would also provide a more detailed analysis of the elemental and isotopic composition of the melt glass. By completing these tasks simultaneously, this significantly decreases the required time to conduct these separations and improves the elemental and isotopic analysis.

The effect of the equatorially symmetric zonal winds of Saturn on its gravitational field

The penetration depth of Saturn’s cloud-level winds into its interior is unknown.A possible way of estimating the depth is through measurement of the effect of the winds on the planet’s gravitational *** use a self-consistent perturbation approach to study how the equatorially symmetric zonal winds of Saturn contribute to its gravitational *** important advantage of this approach is that the variation of its gravitational field solely caused by the winds can be isolated and identified because the leading-order problem accounts exactly for rotational distortion,thereby determining the irregular shape and internal structure of the hydrostatic *** assume that（i）the zonal winds are maintained by thermal convection in the form of non-axisymmetric columnar rolls and（ii）the internal structure of the winds,because of the Taylor-Proundman theorem,can be uniquely determined by the observed cloud-level *** calculate both the variation △J_n,n=2,4,6...of the axisymmetric gravitational coefficients J_n caused by the zonal winds and the non-axisymmetric gravitational coefficients △J_（nm） produced by the columnar rolls,where m is the azimuthal wavenumber of the *** consider three different cases characterized by the penetration depth 0.36 R_S,0.2 R_S and 0.1 R_S,where R_S is the equatorial radius of Saturn at the 1-bar pressure *** find that the high-degree gravitational coefficient （ J_（12）＋△J_（12）） is dominated,in all the three cases,by the effect of the zonal flow with ｜△J_（12）/J_（12）｜〉100%and that the size of the non-axisymmetric coefficientsdirectly reflects the depth and scale of the flow taking place in the Saturnian interior.

Time-series prediction of shellfish farm closure: A comparison of alternatives

Shellfish farms are closed for harvest when microbial pollutants are *** pollutants are typically present in rainfall runoff from various land uses in *** currently use a number of observable parameters(river flow,rainfall,salinity)as proxies to determine when to close *** have proposed using the short term historical rainfall data as a time-series prediction problem where we aim to predict the closure of shellfish farms based only on ***-series event prediction consists of two steps:(i)feature extraction,and(ii)prediction.A number of data mining challenges exist for these scenarios:(i)which feature extraction method best captures the rainfall pattern over successive days that leads to opening or closure of the farms?,(ii)The farm closure events occur infrequently and this leads to a class imbalance problem;the question is what is the best way to deal with this problem?In this paper we have analysed and compared different combinations of balancing methods(under-sampling and over-sampling),feature extraction methods(cluster profile,curve fitting,Fourier Transform,Piecewise Aggregate Approximation,and Wavelet Transform)and learning algorithms(neural network,support vector machine,k-nearest neighbour,decision tree,and Bayesian Network)to predict closure events accurately considering the above data mining *** have identified the best combination of techniques to accurately predict shellfish farm closure from rainfall,given the above data mining challenges.

目的:观察自发性糖尿病小鼠模型中,糖尿病肾病早期近端小管肾素-血管紧张素系统(RAS)两条轴[血管紧张素转换酶(ACE)-血管紧张素ii(Ang ii)-血管紧张素ii 1型受体(AT1)经典轴和ACE2-Ang(1-7)-Mas受体(MasR)新轴]的变化特征及对肾脏结构和功能的影响。方法:应用雄性自发性1型糖尿病Akita小鼠(以C57BL/6背景的雄性非糖尿病小鼠为正常对照)和自发性2型糖尿病db/db小鼠(以C57BLKS/J背景的雄性db/m非糖尿病小鼠为正常对照),饲养至16周龄时处死小鼠,测定相应生理指标。收集尿液,检测尿液中Ang ii和Ang(1-7)的含量;收取肾脏组织行病理切片染色观察肾脏结构变化;免疫组化法观察肾脏组织血管紧张素原(Agt)、ACE、ACE2和MasR的表达部位及变化;Western blot和RT-qPCR测定近端肾小管的上述蛋白及mRNA的变化。结果:两种糖尿病小鼠16周龄时,肾小球滤过率和尿白蛋白/肌酐比值显著增加(Pii排泄显著增加(P<0.01),而ACE和MasR的蛋白和mRNA表达水平显著下调(P<0.01),尿Ang(1-7)含量显著减少(P<0.01)。结论:在自发性糖尿病小鼠中,近端小管RAS在糖尿病肾病早期被激活,激活的RAS可能参与糖尿病肾病早期肾小球高滤过、肾小球和肾小管肥大及近端小管损伤。

Platelets are small, versatile blood cells that are critical for hemostasis/thrombosis. Local platelet accumulation is a known contributor to proinflammation in various disease states. However, the anti-inflammatory/immunosuppressive potential of platelets has been poorly explored. Here, we uncovered, unexpectedly, desialylated platelets (dPLTs) down-regulated immune responses against both platelet-associated and -independent antigen challenges. Utilizing multispectral photoacoustic tomography, we tracked dPLT trafficking to gut vasculature and an exclusive Kupffer cell-mediated dPLT clearance in the liver, a process that we identified to be synergistically dependent on platelet glycoprotein Ibα and hepatic Ashwell–Morell receptor. Mechanistically, Kupffer cell clearance of dPLT potentiated a systemic immunosuppressive state with increased anti-inflammatory cytokines and circulating Cd4+ regulatory T cells, abolishable by Kupffer cell depletion. Last, in a clinically relevant model of hemophilia A, presensitization with dPLT attenuated anti-factor ViiI antibody production after factor ViiI ( infusion. As platelet desialylation commonly occurs in daily-aged and activated platelets, these findings open new avenues toward understanding immune homeostasis and potentiate the therapeutic potential of dPLT and engineered dPLT transfusions in controlling autoimmune and alloimmune diseases.

This review summarizes the rapporteur report on tropical cyclone(TC)intensity change from the operational perspective,as presented to the 10th International Workshop on TCs(IWTC-10)held in Bali,Indonesia,from dec.5–9,*** accuracy of TC intensity forecasts issued by operational forecast centers depends on three aspects:real-time observations,TC dynamical model forecast guidance,and techniques and methods used by *** rapporteur report covers the progress made over the past four years(2018–2021)in all three *** review focuses on the progress of dynamical model forecast *** companion paper(Part ii)summarizes the advance from operational *** dynamical model forecast guidance continues to be the main factor leading to the improvement of operational TC intensity ***,we describe recent advances and developments of major operational regional dynamical TC models and their intensity forecast performance,including HWRF,HMON,COAMPS-TC,Met Office Regional Model,CMA-TYM,and newly developed *** performance of global dynamical models,including NOAA's GFS,Met Office Global Model(MOGM),JMA's GSM,and IFS(ECMWF),has also been improved in recent years due to their increased horizontal and vertical resolution as well as improved data assimilation *** challenging cases of rapid intensification are presented and discussed.
