Background: Cervical cancer is the fourth most common cancer among women globally, the second most common cancer in Nigeria and the most common cause of cancer-related death in Africa. In 2020, World Health Organization in its updated guidelines recommended cervical cancer screening using HPV DNA, HPV mRNA tests and subsequent treatment as appropriate. However, in resource-poor settings, Visual Inspections with Acetic Acid (VIA), Lugol’s Iodine (VILI) and subsequent treatment of precancerous lesions with thermal ablation remain the practical approaches. Objectives: To determine the prevalence of precancerous cervical lesions and associated risk factors among Women Living with HIV (WLHIV). Methods: A retrospective study on sexually active WLHIV aged 16 - 55 years screened for cervical cancer using VIA and VILI within 16 months period in Faith Alive Hospital Jos. Data were analyzed using IBM-SPSS 26. Sociodemographic characteristics of the study participants and the screening results were presented in frequency tables, and logistic regression was performed to determine risk factors of pre-cancerous lesions. Results: 1113 women were screened for cervical cancer using VIA/VILI. 994 (89.3%) were negative, 101 (9.1%) were positive for precancerous lesions, and 18 (1.6%) were suspicious of cervical cancer. The mean age of clients with pre-cancerous lesions was 41.32 ± 9.89 years. A higher positivity yield (69.4%) was found in ages between 36 and ≥55 years while a less positivity yield (30.6%) was found in age’s ≤ 35 years. History of STI had 1.64 fold risk association with precancerous lesions. Conclusion: Our study demonstrated a high prevalence of precancerous cervical lesions among WLHIV;bimodal age distribution for cancer-suspicious lesions and risk associated with STI. Thus, a “screen-and-treat” approach to cervical cancer prevention by VIA and thermal ablation in resource-poor settings should be undertaken until widespread HPV testing to triage clients is feasible.

Land suitability analysis is a prerequisite to achieving optimum utilization of available land resources. Hence, a study on land resource inventory for agricultural land use planning was conducted in the Northern Transition Zone of India to determine land capability and develop a suitability map for wheat and sorghum-based on physical and climatic factors of production using remote sensing and GIS techniques. Detailed soil survey information was used for this exercise. Four series (Singhanhalli, Mugli, Bogur and Venkatapur series) were identified and mapped into seventeen mapping units. Land capability classification showed that a greater portion of the study area belonged to class III followed by class IV with limitations of erosion, wetness and varying soil properties. Four land capability classes viz., II, III, IV, and VI, and seven subclasses viz., IIsf, IIItsf, IVs, IVt, IVts, IVtsf and VIt were identified. Major limitations of these subclasses were slope, erosion, depth, texture, coarse fragments, pH, organic carbon and base saturation. Soil suitability assessment revealed that the soils are moderately suitable to permanently not suitable. About 234 ha (31.6%) is moderately suitable, 494 ha (65.0%) marginally suitable and 10.2 ha (1.3%) permanently not suitable for wheat;while 78.5 ha (10.3%) is moderately suitable, 633.4 ha (82.3%) marginally suitable and 32.6 ha (4.3%) permanently not suitable for sorghum respectively. The moderate, marginal and permanent non-suitability was due to moderate, severe and very severe limitations respectively. However, it is possible to achieve potential yield of the crops in the study area if these limitations are addressed.

Heart failure is common in older people and its prevalence is *** Heart ＇omics＇ in AGEing（HOMAGE） project aims to provide a biomarker approach that will improve the early diagnosis of heart failure.A large clinical database,based on（1） prospective population studies or（2） cross-sectional,prospective studies or randomized controlled trials（RCTs） of patients at risk for or with overt cardiovascular disease will be constructed to determine most promising ＇omics＇-based biomarkers to identify the risk of developing heart failure and/or *** studies,patient cohorts and RCTs are eligible for inclusion in the common database,if they received ethical approval to obtain and share data and have baseline information on cardiovascular risk ***,the HOMAGE database includes 43,065 subjects,from 20 studies in eight European countries,including healthy subjects from three population studies in France,Belgium and Italy（n = 7,124）,patients with heart failure（n = 4,312） from four cohorts in the UK,Spain and Switzerland and patients at high risk for cardiovascular disease（n = 31,629） in 13 *** is anticipated that more partners will join the consortium and enlarge the pooled *** large merged database will be a useful resource with which to identify candidate biomarkers that play a role in the mechanism underlying the onset and progression of heart failure.

Virus entry mediated by hepatitis B virus envelope proteins

Infection Prevalence of <i>Borrelia burgdorferi</i>in Ticks Collected from Songbirds in Far-Western Canada

Worldwide, wild birds play a vital role in the dispersal of ticks that harbour tick-borne pathogens, including Borrelia burgdorferi, the Lyme disease bacterium. Using PCR testing, we found 124 (31%) of 405 ticks (4 species), which were collected from 21 species of birds in far-western Canada, to be infected with B. burgdorferi. Transstadial transmission of B. burgdorferi occurred from larva to nymph, plus nymph to adult, in the avian coastal tick, Ixodes auritulus, collected from songbirds in British Columbia (B.C). Collectively, all 3 motile life stages (larva, nymph, adult) of this tick had an infection prevalence of 31% for B. burgdorferi, which suggests vector competency. A Pacific Wren was highly infested with I. auritulus immatures, and 20 (44%) of 45 ticks (2 nymphs, 43 larvae) were infected with B. burgdorferi. This heavy infestation shows the high potential to initiate a new population of ticks and to disseminate Lyme spirochetes. Epidemiologically, B. burgdorferi-infected I. auritulus larvae collected from the Spotted Towhee, Swainson’s Thrush, Pacific Wren, and fox Sparrow suggest that these avian hosts act as reservoirs for B. burgdorferi. In this study, the western blacklegged tick, Ixodes pacificus, and Ixodes spinipalpis played a limited role in the enzootic transmission cycle of B. burgdorferi along coastal B.C. We document the first record of I. spinipalpis on a bird in Alberta. Because songbirds widely disperse Lyme disease vector ticks, primary health providers and the general public must be vigilant that outdoors people may be bitten by B. burgdorferi-infected ticks throughout far-western Canada.

Temporal trends in 6-minute walking distance for older Japanese adults between 1998 and 2017

Analysis of Adult Nephrectomies at the Komfo Anokye Teaching Hospital, Kumasi, Ghana

Objective: To evaluate open nephrectomies performed in adults over a five year period at the Komfo Anokye Teaching Hospital (KATH) and to compare our experience with findings in the literature. Materials and Methods: This was a prospective, cross-sectional, hospital based study of all adults undergoing nephrectomy from October 2012 to September 2017 at KATH. We obtained data on patient demographics, clinical presentation, laboratory and imaging results, laterality of the disease, and indication for nephrectomy, complications and histopathology of nephrectomy specimen. The data was analyzed using PASW Statistics for Windows, Version 19.0. Chicago: SPSS Inc. Results: Thirty three (33) adult nephrectomies were performed over the study period. There were 18 males (54.50%) and 15 females (45.50%) with an age range of 16 years to 80 years. The modal age range for renal malignancies was 31 - 40 years accounting for 36.40% of adult nephrectomies. Flank pain (75.80%), haematuria (54.60%) and flank mass (51.50%) were the predominant presenting complaints. Malignant renal tumours accounted for 22 (66.70%) of the nephrectomies followed by neglected Pelvi-Ureteric Junction Obstruction (PUJO)—7 (21.20%);two (6.10%) nephrectomies were due to trauma and one (3.00%) for emphysematous pyelonephritis. Conclusion: Renal cell carcinoma constitutes the main indication for adult nephrectomy at KATH followed by neglected PUJO. Most of the patients with renal tumours were young and also presented late.

Etiologies and Prognostic Factors of Dyspnea in Infants at the University Hospital Center (CHU) of Bouaké (Ivory Coast)

Identify the epidemiological characteristics, etiologies and evolutionary aspects of dyspnea in infants. This was a retrospective study of infants hospitalized for dyspnea from January 1 to December 31, 2020. The parameters studied were sex, age, origin, vaccination status, existence of underlying pathology. Underlying, the diagnosis and the evolutionary modalities. Data analysis and processing were possible using Word, Excel and EPI info version 7 software. We retained 152 infants. The sex ratio was 1.34 and the median age was 4 months. Vaccines according to expanded immunization program (EPI) were up to date in 76.32%. The main antecedents with risk identified were malnutrition, hypotrophy at birth, interventricular communication. The pathologies observed were low acute respiratory diseases in 90.79%, ENT diseases in 04.60% and cardiac diseases in 03.95%. The median length of hospitalization was 4 days. Infants who died accounted for 15.13%. The median age of infants who died was 4 months. The median time to onset of death was 1.63 days. The risk factors for death were age < 6 months (p = 0.003;CI [1.27;9.33]), outdated vaccines (p = 0.012;CI [1.18;5.17]), history with risk (p = 0.031;CI [1.02;4.54]). Dyspnea in infants remains a concern in our service. Reducing mortality involves developing procedures for the management of lower respiratory ailments, continuous staff training and strengthening the technical platform.
