Retrospective analysis of eFAST ultrasounds performed on trauma activations at an academic level-1 trauma center

BACKGROUND:Point-of-care ultrasound(POCUS)has become increasingly integrated into the practice of emergency medicine.A common application is the extended focused assessment with sonography in trauma(eFAST)*** American College of Emergency Physicians has guidelines regarding the scope of ultrasound in the emergency department and the appropriate *** objective of this study was to conduct a review of performed,documented and billed eFAST ultrasounds on trauma activation ***:This was a retrospective review of all trauma activation patients during a 10-month period at an academic level-one trauma center.A list comparing all trauma activations was crossreferenced with a list of all billed eFAST *** records were reviewed to determine whether an eFAST was indicated,performed,and appropriately ***:We found that 1,507 of 1,597 trauma patients had indications for eFAST,but 396(27%)of these patients did not have a billed *** these 396 patients,87(22%)had documentation in the provider note that an eFAST was performed but there was no separate procedure *** remaining 309(78%)did not have any documentation of the eFAST in the patient’s chart although an eFAST was recorded and reviewed during ultrasound quality ***:A significant proportion of trauma patients had eFAST exams performed but were not documented or *** of documentation was *** ultrasound programs require appropriate reimbursement to support training,credentialing,equipment,quality assurance,and device *** study demonstrates a significant absence of adequate documentation leading to potential revenue loss for an emergency ultrasound program.

Background： Growth and yield models are important tools for forest planning. Due to its geographic location, topology, and history of management, the forests of the Adirondacks Region of New York are unique and complex. However, only a relatively limited number of growth and yield models have been developed and/or can be reasonably extended to this region currently. Methods： in this analysis, 571 long-term continuous forest inventory plots with a total of 10 - 52 years of measurement data from four experimental forests maintained by the State University of New York College of Environmental Science and Forestry and one nonindustrial private forest were used to develop an individual tree growth model for the primary hardwood and softwood species in the region. Species-specific annualized static and dynamic equations were developed using the available data and the system was evaluated for long-term behavior. Results： Equivalence tests indicated that the Northeast Variant of the Forest Vegetation Simulator （FVS-NE） was biased in its estimation of tree total and bole height, diameter and height increment, and mortality for most species examined. In contrast, the developed static and annualized dynamic, species-specific equations performed quite well given the underlying variability in the data. Long-term model projections were consistent with the data and suggest a relatively robust system for prediction. Conclusions： Overall, the developed growth model showed reasonable behavior and is a significant improvement over existing models for the region. The model also highlighted the complexities of forest dynamics in the region and should help improve forest planning efforts there.

BACKGROUND: Point-of-care ultrasound is an increasingly common imaging modality that is used in a variety of clinical settings. Understanding how to most effectively teach ultrasound is important to ensure that medical students learn pre-clinical content in a manner that promotes retention and clinical competence. We aim to assess the effectiveness of simulation-based ultrasound education in improving medical student competence in physiology in comparison to a traditional didactic ultrasound curriculum. METHODS: Subjects were given a pre and post-test of physiology questions. Subjects were taught various ultrasound techniques via 7 hours of lectures over two days. The control group received 2 additional hours of practice time while the experimental group received 2 hours of case-based simulations. A physiology post-test was administered to all students to complete the two-day course. RESULTS: Totally 115 Swiss medical students were enrolled in our study. The mean precourse ultrasound exam score was 39.5% for the simulation group and 41.6% for the didactic group (P>0.05). The mean pre-course physiology exam score was 54.1% for the simulation group and 59.3% for the didactic group (P>0.05). The simulation group showed statistically significant improvement on the physiology exam, improving from 54.1% to 75.3%(P<0.01). The didactic group also showed statistically signifi cant improvement on the physiology exam, improving from 59.3% to 70.0%(P<0.01). CONCLUSION: Our data indicates that both simulation curriculum and standard didactic curriculum can be used to teach ultrasound. Simulation based training showed statistically signifi cant improvement in physiology learning when compared to standard didactic curriculum.

This paper reviews the detonative properties of low bulk density(LBD),high bulk density(HBD)Nitroguanidine(NGu)(1),CAS-No:[556-88-7]and 82 explosive formulations based on NGu reported in the public *** rank the performance of those formulations they are compared with 15 reference compositions containing both standard high explosives such as octogen(HMX)(2),hexogen(RDX)(3),pentaerythritol tetranitrate(PETN)(4),2,4,6-trinitrotoluene(TNT)(5)as well as insensitive high explosives such as 3-nitro-1,2,4-triazolone(NTO)(6),1,3,5-triamino-2,4,6-trinitrobenzene(TATB)(7),1,1-diamino-2,2-dinitroethylene(fox-7)(8)and N-Guanylurea dinitramide(fox 12)(9).NGu based formulations are superior to those based on fox-12 or TATB and are a close match with fox-7 based explosives,the latter just having higher Gurney Energies(-10%)and slightly higher detonation pressure(+2%).NGu based explosives even reach up to 78% of the detonation pressure,82% Gurney energy and up to 95% of detonation velocity of ***-NGu dissolves in many melt cast eutectics forming dense charges thereby eliminating the need for costly High Bulk Density *** based formulations are at the rock bottom of sensitiveness among all the above-mentioned explosives which contributes to the safety of these *** review gives 132 references to the public *** a review on the synthesis spectroscopy and sensitiveness of Nitroguanidine see Ref.[1].

An experimental field study was conducted at the University of California at Santa Cruz USA to determine if radar cross section signals were different between a person without wearing any wires and a person wearing both a suicide vest and wires with （without） loops in the wires. The data was collected using the GunnPlexer Doppler radar at 12.5 GHz at various safe distances using various subjects both without any wires and wearing a suicide vest with detonation wires and with detonation wires and loops. The raw data was separated in its horizontal and vertical polarization signals （HH and VV）. The analysis of these two HH and VV polarization signals from the data allowed several promising metrics to be created. These metrics were individually tested in a Monte Carlo simulation which is in order to determine the probability of detection of a would-be suicide bomber. The results of the Monte Carlo simulation showed that the metrics yielded a probability of successful detection of slightly over 98% and a false positive rate of just less than 2 %. This research and its result are encouraging and suggest further testing to insure that suicide bombers can be found prior to their detonation of their bombs at a safe range.

The GSSP for the base of the Eocene Series is located at 1.58 m above the base of Section DBH in the Dababiya Quarry, on the east bank of the Nile River, about 35 km south of Luxor, Egypt. It is the base of Bed 1 of the Dababyia Quarry Beds of the El Mahmiya Member of the Esna Formation, interpreted as having recorded the basal inflection of the carbon isotope excursion （CIE）, a prominent （3 to 5%） geochemical signature which is recorded in marine （deep and shallow） and terrestrial settings around the world. The Paleocene/Eocene boundary is thus truly a globally correlatable chronostratigraphic level.

Uncertainty as to the extent and magnitude of changes in conditions that might occur due to climate change poses a problem for land and resource managers as they seek to adapt to changes and mitigate effects of climate *** illustrate using scenarios of projected future conditions on rangelands in the Northern Great Plains and Desert Southwest of the United *** two regions are different in the ways climate change is projected to affect the *** of a longer and warmer growing season in the Northern Great Plains could lead to increased forage production and land *** uncertain effects on summer monsoons that primarily control rangeland productivity in the Desert Southwest,combined with the possibility of more intense and/or frequent drought events,could present land managers with challenges stemming from decreased forage production and land *** projections,though uncertain,provide land managers with basic insight into future conditions they might *** need more.A focus on vulnerability and resilience,with explicit recognition of interactions between ecological and socio-economic factors,coupled with systematic monitoring and assessment of observable conditions on the land to supplement information based on climate projections,will more effectively provide critical and specific information managers need to adaptively manage rangelands under uncertain climate futures.
