Echovirus 18 Infection Is Associated with Crohn’s Disease

Background: Enterovirus (EV) can cause gastroenteritis and are known to replicate in Peyer’s patches of terminal ileum. EV has been found in the intestinal specimens from immunocompromised patients with regional enteritis, and another study demonstrates the presence of enterovirus in the resected terminal ileum of immunocompetent pediatric patients with Crohn’s disease (CD). Cluster outbreaks of CD have also been reported in the literature but the cause of the disease remains elusive. Materials and Methods: A small cluster of pathologically proven CD occurred in our geographic area in 2004-2005, concurrently with an epidemic of Echovirus 18 (E18) meningitis. Serum samples of these and other CD patients and control subjects were tested for neutralizing antibodies of 11 common typeable enteroviruses and Echovirus 18;and tissue samples of CD patients, and terminal ileum and colon biopsies of normal controls were tested for the presence of viral capsid protein 1 (VP1) by immunoperoxidase staining. Results: Immunoperoxidase staining demonstrated VP1 in a rare epithelial granuloma, in diseased muscle of terminal ileum and also in colon biopsies of CD patients. Significantly elevated E18 neutralizing antibody was found in patients with pathologically-proven Crohn’s disease, as compared to control subjects. Conclusion: In this small observational study, EV VP1 is consistently demonstrated in tissue samples of CD patients as compared to control subjects;and neutralizing antibody for E18 was found in all of the patients with available serum samples. Larger cross-sectional studies will be needed to define the role of E18 in this chronic disease.

Crop nutrition has a significant effect on disease incidence, resistance or tolerance of various crops. There is currently a lack of reliable recent UK-based research about the effect of copper, zinc and boron on disease incidence, green leaf area （GLA）, green leaf retention （GLR） and grain yield of winter wheat, spring wheat and spring barley. Data analysis showed that these trace elements had positive effects on GLR. These positive effects may have been due to the role of copper, zinc and boron in the production of defence related compounds （phenolics and lignin）, which may have reduced the disease incidence resulting in prolonged GLR. Grain yield was significantly enhanced by the application of these trace elements on the crop grown on high pH calcareous soil, which can be partly attributed to enhanced GLR. Also trace elements have a positive role in reproductive growth, flowering and male fertility. On average, zinc was found to be the most consistent trace element in terms of enhancing GLR and grain yield. Across all trials, it was noted that for every 10% increase in GLA from trace elements, grain yields increased by 4.2% in 2012-2013, by 4.4% in 2013-2014 in winter wheat and by 3.9% in spring wheat in 2014. These are remarkably consistent and indicate that increasing GLA by 10% by early dough stage was associated with a yield improvement of about 4%. These trace elements also had a positive effect on grain protein content （GPC）. This research concluded that the trace elements had positive effects in enhancing GLA and yield. It can be speculated that with the use of these trace elements, there may be more scope for using less robust or reduced rates of fungicides to control foliar diseases, which may help to maximize farm profits.

目的对临床致病性不动杆菌进行种属鉴定,并比较3种方法(44℃生长试验、特异性引物PCR扩增和16SrRNA测序)对鲍曼不动杆菌鉴定的特异性、准确性及应用价值。方法收集临床致病不动杆菌56株,首先接种在胰蛋白胨大豆肉汤中44℃进行培养,观察是否可以生长;其次,提取56株不动杆菌基因组DNA,用两组不同的特异性引物进行PCR扩增鉴定;最后,用通用引物对56株不动杆菌的16SrRNA进行测序鉴定。结果 56株不动杆菌中有17株可在44℃生长。经特异性引物PCR扩增鉴定,发现8株鲍曼不动杆菌和3株13TU型不动杆菌均可在44℃中较早较快生长。金标准16SrRNA测序确定56株不动杆菌中含有9个种,其中鲍曼不动杆菌和13TU型不动杆菌的鉴定结果与特异性引物PCR扩增法相同。结论应用分子生物学方法鉴定鲍曼不动杆菌具有准确、高效且可重复性高的优点,尤其是特异性PCR扩增法,可作为较难诊断的鲍曼不动杆菌的首选鉴定方法。

Background: Immune checkpoint inhibitors produce prolonged responses in select non-small cell lung cancer (NSCLC) patients, however the identification of patients most likely to benefit is difficult. Pretreatment derived neutrophil-to-lymphocyte ratio (dNLR) is an easily calculated marker available in routine clinical care that has shown prognostic value in many cancer treatment settings, but its association with survival in NSCLC patients treated with immune-checkpoint inhibitors is less understood. Patients and Methods: We retrospectively reviewed 72 NSCLC patients receiving either nivolumab or pembrolizumab between 3/1/15 and 3/1/17 with a median follow-up time of 5.1 months. Patients were compared using Cox proportional hazards models to detect an association between pretreatment dNLR 3 vs ≥3 on overall survival (OS), progression-free survival (PFS) and overall response rate. Results: Median age was 65 (range: 41 - 86), 65% were male, 40% received ≥ 2 prior systemic therapies and 14% had an Eastern Cooperative Oncology Group Performance Status (ECOG PS) ≥ 2. Pretreatment dNLR ≥ 3 was independently associated with shortened OS (median 3.6 vs 8.5 months;HR: 5.4;95% CI: 2.0 - 14.6;p = 0.001) and PFS (median 2.1 vs 3.4;HR: 2.3;95% CI: 1.1 - 4.8;p = 0.027). Conclusion: Pretreatment dNLR ≥ 3 was independently associated with inferior survival in NSCLC treated with immune checkpoint inhibitors in routine practice. Prospective verification of this marker is warranted as it could serve as an inexpensive and widely-available marker for identifying NSCLC patients most likely to benefit from PD-1 inhibitors.
