AIM: To examine the accuracy of the aspartate aminotransferase (AST)/Platelet Ratio Index (APRI) and FIB-4, in predicting longitudinal changes in liver histology in hepatitis C virus (HCV) ***: Patients that underwent repeat liver biopsies at least 1 year apart from 1999 to 2007 were identified. Liver fibrosis was staged on needle core biopsies evaluated by a single expert liver pathologist. Only laboratory values within 3 mo of the liver biopsies were ***: Thirty-six patients met the inclusion criteria with 50% stage 1 on initial biopsy, 25% stage 2, and 22% stage 3. Nineteen of 36 (53%) had progression of fibrosis on repeat biopsies, while 16 (44%) showed no change in stage, and one (3%) showed improvement. Patients that showed progression of fibrosis had significantly higher alanine aminotransferase and aspartate aminotransferase levels than the group that did not show progression. A significant correlation was seen between change in stage of fibrosis and change in APRI (r2 = 0.39, P = 0.00001) and a change in FIB-4 (r2 = 0.31, P = 0.00004). A change in APRI (ΔAPRI) of 0.18 had 80% positive predictive value (PPV) and 67% nega- tive predictive value (NPV) for progression of fibrosis. A change in FIB-4 (ΔFIB-4) of 0.39 had 75% PPV and 75% NPV for predicting progression of fibrosis. CONCLUSION: ΔAPRI and ΔFIB-4 parallel changes in fibrosis progression, and could be useful tools for clinicians in following patients with active chronic HCV infection.

Hepatocellular carcinoma(HCC)-related mortality is high because early detection modalities are hampered by inaccuracy,expense and inherent procedural *** there is an urgent need for minimally invasive,highly specific and sensitive biomarkers that enable earlydisease detection when therapeutic intervention remains practical. Successful therapeutic intervention is predicated on the ability to detect the cancer early. Similar unmet medical needs abound in most fields of medicine and require novel methodological approaches. Proteomic profiling of body fluids presents a sensitive diagnostic tool for early cancer detection. Here we describe such a strategy of comparative proteomics to identify potential serum-based biomarkers to distinguish high-risk chronic hepatitis C virus infected patients from HCC patients. In order to compensate for the extraordinary dynamic range in serum proteins,enrichment methods that compress the dynamic range without surrendering proteome complexity can help minimize the problems associated with many depletion methods. The enriched serum can be resolved using 2D-difference in-gel electrophoresis and the spots showing statistically significant changes selected for identification by liquid chromatographytandem mass spectrometry. Subsequent quantitative verification and validation of these candidate biomarkers represent an obligatory and rate-limiting process that is greatly enabled by selected reaction monitoring(SRM). SRM is a tandem mass spectrometry method suitable for identification and quantitation of target peptides within complex mixtures independent on peptidespecific antibodies. Ultimately,multiplexed SRM and dynamic multiple reaction monitoring can be utilized for the simultaneous analysis of a biomarker panel derived from support vector machine learning approaches,which allows monitoring a specific disease state such as early HCC. AIM To determine if end-stage renal disease (ESRD) is a risk factor for post endoscopic retrograde cholangio-pancreatography (ERCP) adverse events (AEs). METHODS we performed a retrospective cohort study using the Nationwide Inpatient Sample (NIS) 2011-2013. we identified adult patients who underwent ERCP using the International Classification of Diseases 9^(th) Revision (ICD-9-CM). Included patients were divided into three groups: ESRD, chronic kidney disease (CKD), and control. The primary outcome was post-ERCP AEs including pancreatitis, bleeding, and perforation determined based on specific ICD-9-CM codes. Secondary outcomes were length of hospital stay, in-hospital mortality, and admission cost. AEs and mortality were compared using multivariate logistic regression *** There were 492175 discharges that underwent ERCP during the 3 years. The ESRD and CKD groups contained 7347 and 39403 hospitalizations respectively, whereas the control group had 445424 hospitalizations. Post-ERCP pancreatitis (PEP) was significantly higher in the ESRD group (8.3%) compared to the control group (4.6%) with adjusted odd ratio (aOR) = 1.7 (95% CI: 1.4-2.1, ~aP was associated with significantly higher ERCP-related bleeding (5.1%) compared to the control group 1.5% (aOR = 1.86, 95%CI: 1.4-2.4, ~aP with adjusted mean difference (aMD) = 5.9 d (95% CI: 5.0-6.7 d, ~aP with higher hospital mortality. Careful selection and close monitoring is warranted to improve outcomes.

目的评价经颈静脉肝内门腔分流术（TIPS）专用覆膜支架（Viatorr支架）在TIPS中的应用价值。方法回顾性分析37例在美国俄勒冈州健康生命科技大学Dotter介入放射学研究所采用Viatorr支架行’nPs治疗的患者资料，随访时间为（15．2±9．3）个月（3～42个月）。TIPS指征包括门静脉高压相关性的急慢性消化道出血，经药物及内镜治疗无效者17例；顽固性肝源性胸、腹水18例，***综合征2例。采用配对t检验比较手术前后门腔静脉压力差（PSG）的变化，以Kaplan-Meier曲线分析支架开通率。结果37例共置入41枚Viatorr支架，其中3枚直径为8mm，38枚为10mm，支架带膜长度为4～8cm，无相关手术并发症。PSG由术前的（22．4±8．4）mmHg（1mmHg=0．133kPa）降为（8．1±3．2）mmHg，差异有统计学意义（t=12．754，P〈0．01）。17例出血患者术后出血均停止，1例于术后17个月复发。18例严重顽固性腹水及肝性胸水患者中，4例术后腹水不消退，其余14例随访期间有2例腹水复发。2例（5．4％）发生分流道阻塞，Kaplan-Meier曲线分析结果显示1年的开通率为97．0％。术后1个月内无病死患者，2例分别于术后3个月及15个月死于多器官功能衰竭，晚期病死率为5．4％，死亡前1周内复查支架均通畅。肝移植患者5例（13．5％）。结论Viatorr支架能明显提高TIPS术后开通率，选择合适的支架，采用正确的释放技术能进一步提高疗效，但长期效果评价尚需验证。

BACKGROUND Endoscopic ultrasound(EUS)-guided main pancreatic duct(PD)access may be used when conventional endoscopic retrograde cholangiopancreatography(ERCP)techniques *** use of a percutaneous transluminal angioplasty balloon(PTAB),originally developed for vascular interventions,can be used to facilitate transmural(e.g.,transgastric)PD access and to dilate high-grade pancreatic *** To describe the technique,efficacy,and safety of PTABs for EUS-guided PD *** Patients who underwent EUS with use of a PTAB from March 2011 to August 2021 were retrospectively identified from a tertiary care medical center supply *** included 3-4 French angioplasty catheters with 3-4 mm balloons designed to use over a 0.018-inch *** primary outcome was technical *** outcomes included incidence of adverse events(AEs)and need for early *** A total of 23 patients were identified(48%female,mean age 55.8 years).Chronic pancreatitis was the underlying etiology in 13(56.5%)patients,surgically altered anatomy(SAA)with stricture in 7(30.4%),and SAA with post-operative leak in 3(13.0%).Technical success was achieved in 20(87%)*** AE rate was 26%(n=6).All AEs were mild and included 1 pancreatic duct leak,2 cases of post-procedure pancreatitis,and 3 admissions for post-procedural *** patients required early *** EUS-guided use of PTABs for PD access and/or stricture management is feasible with an acceptable safety profile and can be considered in patients when conventional ERCP cannulation fails.
