A total of 2261 (808 male, 1453 female) consecutive patients attending contact dermatitis clinics were patch tested to isoeugenol and its derivatives listed i n the EUInventory of Fragrance Ingredients. Positive reactions were found to iso eugenol in 40, transisoeugenol in 40, isoeugenyl acetate in 19, isoeugenyl benzo ate in 4, isoeugenyl phenylacetate in 16, isoeugenyl methyl ether in 6 and benzy l isoeugenyl ether in 2 patients. There was a concomitant reaction to isoeugenol in 36/40 of those positive to transisoeugenol, 13/19 of those to isoeugenyl ace tate, 3/4 of those to isoeugenyl benzoate and 15/16 of those to isoeugenyl pheny lacetate but in none of those 6 positive to isoeugenyl methyl ether and in neith er of those 2 positive to benzylisoeugenyl ether. Concomitant contact allergy be tween isoeugenol and its derivatives may occur through chemical cross-reactivit y or local skin metabolism of the derivatives. It is more commonly observed with the esters rather than the ethers. Isoeugenyl acetate has been proposed as an a lternative to isoeugenol, but there is a high degree of concomit an treactivity with isoeugenol.

Advances in molecular diagnostics have led to the feasibility of DNA-based prenatal testing in families at risk for recurrence of severe forms of both dystrophic and junctional epidermolysis bullosa. In this report, we describe prenatal testing in a woman who previously had a child affected with Herlitz junctional epidermolysis bullosa. However, in her second pregnancy, she was found to have dichorionic diamniotic twins. DNA analysis of a pathogenic mutation and informative intragenic polymorphisms (LAMB3 gene) predicted one fetus to be affected and the other unaffected. selective termination of the affected fetus was performed, and pregnancy with the unaffected fetus was continued, leading to full term delivery of a healthy girl with no skin blisters. This is the first reported case of DNA analysis in a twin pregnancy at risk of Herlitz junctional epidermolysis bullosa, with successful diagnosis and selective termination of one affected twin.

This descriptive study aimed to determine the level of acceptability of the Instructional Guide in Physical Education 2(Advanced swimming skills and Life-saving Techniques).The respondents of the study were the content experts currently teaching PE(Physical Education)in other universities in Iloilo and Bacolod City,Philippines together with the BsMT(Bachelor of science Marine Transportation)students who used the instructional guide throughout the duration of the *** instrument used to evaluate the materials was adopted from Terunez(2008)and included seven evaluation criteria to include physical aspect,language,content,objectives,instructions,learning activities,and evaluation *** statistical tools employed were mean,standard deviation,and *** results of the study revealed that the level of acceptability of the instructional guide in physical aspect was“High”which means that the material was suited to the level of the intended *** significant difference was noted in the evaluation result of the content experts and student *** can be concluded that the development of Instructional Guide in Physical Education 2 is deemed to be highly beneficial in enhancing *** is recommended that this instructional material will be utilized by other maritime institutions offering courses in Advanced swimming and Life-saving Techniques.

希望(hope)是积极心理调适(positive psychological adjustment)的有力预测因子,希望具有希望效能(hope agency)和希望路径(hope pathways)两种结构。前人研究显示,两种希望结构对积极心理调适具有不同的效果。本研究关注希望对多种族民族(multiethnoracial)个体积极心理调适的预测效果。多种族民族个体因为多重文化身份而经历着种族歧视(ethnoracial discrimination)和社会孤独,从而面临着更多的积极心理调适问题。研究通过对203个(女:164)多种族民族身份美国大学生进行量表研究,探讨希望对于积极心理调适的预测作用,并检验在控制了种族歧视和社会支持(social support)后,希望是否仍可以单独预测积极心理调适,且两种希望结构是否起到不同的效果。使用一般性种族歧视量表(GEDs)测量种族歧视,社会网络量表(sNL)中的两个问题测量社会支持(家人&朋友),希望量表(Hs)测量希望的两个结构(希望效能&希望路径),主观活力量表(sVs)和生活满意度量表(sWLs)测量活力和生活满意度。结果显示,希望效能和希望路径与活力和生活满意度呈现显著正相关。层次回归结果显示:a.种族歧视单独负向预测生活满意度,无法预测活力;b.社会支持单独正向预测活力和生活满意度;c.控制了社会人口学变量、种族歧视和社会支持后,希望仍能单独正向预测活力和生活满意度;d.希望效能是活力的有力正向预测因子,而希望路径则更能有力正向预测生活满意度。本研究是首个强调希望对于多种族民族个体的重要作用的研究,结果可为对多种族民族个体进行积极心理调适的干预提供经验,即与消除种族歧视和增加社会支持相比,建立希望对多种族民族个体来说是更为现实易行的方法。

Focal dermal hypoplasia (Goltz) syndrome is a rare genetic disorder characterized by cutaneous, ectodermal and meso dermal defects. We present a case in which painful, exophytic granulation tissue has been the main symptom over the past 15 years. After unsatisfactory results with a number of treatment modalities including topical steroids, silver-nitrate applications, cryotherapy, curettage, excision and pulsed-dye laser, we achieved significant benefit with curettage in combination with photodynamic therapy. Although impaired wound healing has been described in focal dermal hypoplasia, this is, to our knowledge, the first time that pyogenic granuloma-like lesions have been reported.

Objective: To determine whether metformin or placebo could,in conjunction with healthy lifestyle counseling,decrease serum testosterone levels and related aberrations in adolescents with hyperandrogenism,hyperinsulinemia,and polycystic ovarian syndrome. Design: Randomized,placebo-controlled,double-blind trial. setting: Pediatric university teaching hospital. Participants: Twenty-two adolescents aged 13 to 18 years with hyperinsulinemia and polycystic ovarian syndrome. Intervention: Participants were randomly assigned to take a 12-week course of either metformin or placebo. Main Outcome Measures: Pretreatment and posttreatment oral glucose tolerance tests,fasting lipid profiles,and clinical measurements. Results: There was a significant decline in mean serum testosterone concentration with metformin (-38.3 ng/dL) compared with placebo (-0.86 ng/dL) (95% confidence interval,-∞ to -0.29 for the mean difference between groups). At completion,the relative risk of menses was 2.50 times higher in the metformin group compared with the placebo (95% confidence interval,1.12 to 5.58). Measures of insulin sensitivity,including insulin area under the curve and HOMA (homeostasis model assessment)-,demonstrated improvement only with metformin,but these did not reach statistical significance. High-density lipoprotein cholesterol levels increased by 6.98 mg/dL with metformin vs a decrease of -2.33 mg/dL with placebo (95% confidence interval,0.78 to 18.23 for the mean difference between groups). There were no significant changes in body mass index,hirsutism,triglyceride levels,or total and low-density lipoprotein cholesterol levels. Conclusion: Metformin significantly lowered total testosterone concentrations,increased the likelihood of menses,and improved high-density lipoprotein cholesterol levels without affecting measures of insulin sensitivity or body weight.

Background: Despite having a positive patch test reaction to para-phenylenediamine (PPD), some patients continue to dye their hair, while others are forced to give up or abandon this practice. This difference in patient behaviour could be due to the degree of sensitization. Objectives: To establish whether the ability to continue dyeing hair in PPD allergic patients is related to the strength of patch test reaction. To note differences in other clinical features in relation to the strength of patch test reaction. Methods We analysed retrospectively the patch test records of 400 sequential PPD-positive patients for the strength of patch test reaction (+, ++, +++) and different clinical features. Data were analysed using Cochran-Mantel-Haenszel χ2 tests. Results: There was a strong linear relationship between the strength of patch test reaction and continuation with hair dyeing. Patients were more likely to report a history of hair dye reaction with increasing strength of patch test reaction. There was no difference in strength of patch test reaction in relation to age, site of rash, occupation (hairdressing) or history of atopic eczema. Overall concomitant reactivity with related aromatic amine allergens (benzocaine,N-isopropyl-Nphenyl-para-phenylenediamine, para-aminobenzoic acid) was infrequent. Conclusions: Patients with stronger patch test reactions (++, +++) are more likely to have a clear history of reacting to hair dye and are less likely to still be dyeing their hair.
