Herbal hepatoxicity from Chinese skullcap: A case report

The use of herbal supplements has increased considerably over the last decade. We report a case of an elderly woman who began taking Move Free Advanced for arthritis, which in addition to glucosamine and chondroitin, contained two herbal ingredients, Chinese skullcap and Black Catechu. Our patient presented with significant cholestasis and hepatitis which significantly improved after discontinuation of the supplement. Since neither the patient nor the treating physician recognized this supplement as a potential hepatotoxin, she resumed taking the supplement and again suffered from considerable hepatotoxicity. Liver biopsy at that time was consistent with acute drug induced liver injury. She, once again, recovered after discontinuation of the supplement. Review of the literature confirms that Chinese skullcap has been implicated as a possible hepatotoxic agent which was demonstrated in this case.

AIM:To study the effects of probiotic metabolites on maturation stage of antigen-presenting immune ***:Ganeden Bacillus coagulans 30(GBC30) bacterial cultures in log phase were used to isolate the secreted metabolite(MET) fraction.A second fraction was made to generate a crude cell-wall-enriched fraction,by centrifugation and lysis,followed by washing.A preparation of MET was subjected to size exclusion centrifugation,generating three fractions:< 3 kDa,3-30 kDa,and 30-200 kDa and activities were tested in comparison to crude MET and cell wall in primary cultures of human peripheral blood mononuclear cell(PBMC) as a source of antigen-presenting mononuclear *** maturation status of mononuclear phagocytes was evaluated by staining with monoclonal antibodies towards CD14,CD16,CD80 and CD86 and analyzed by flow ***:Treatment of PBMC with MET supported maturation of mononuclear phagocytes toward both macrophage and dendritic cell *** biological activity unique to the metabolites included a reduction of CD14+ CD16+ pro-inflammatory cells,and this property was associated with the high molecular weight metabolite *** were also seen for the dendritic cell maturation markers CD80 and *** CD14dim cells,an increase in both CD80 and CD86 expression was seen,in contrast to a selective increase in CD86 expression on CD14bright *** co-expression of CD80 and CD86 indicates effective antigen presentation to T cells and support of T helper cell *** selective expression of CD86 in the absence of CD80 points to a role in generating T regulatory ***:The data show that a primary mechanism of action of GBC30 metabolites involves support of more mature phenotypes of antigen-presenting cells,important for immunological decision-making.

We describe a case of pelvic narrowing in a patient with osteogenesis imperfecta (OI) type III and complications consisting of deep venous thrombosis, hydronephrosis and faeculomas. The literature is reviewed and the management is discussed. Conclusion: Monitoring of OI type III patients for pelvic narrowing and complications is recommended.

Concerning the stability problem of functional equations, we introduce a general （m, n）- Cauchy-jensen functional equation and establish new theorems about the Hyers-Ulam stability of general （m, n）-Cauchy jensen additive mappings in C^＊-algebras, which generalize the result＇s obtained for Cauchy-jensen type additive mappings.

BACKGROUND Missing occult cancer lesions accounts for the most diagnostic errors in retrospective radiology reviews as early cancer can be small or subtle,making the lesions difficult to *** is the most effective technique for reducing these events and can be economically implemented with the advent of artificial intelligence(AI).AIM To achieve appropriate AI model training,a large annotated dataset is necessary to train the AI *** goal in this research is to compare two methods for decreasing the annotation time to establish ground truth:Skip-slice annotation and AI-initiated *** We developed a 2D U-Net as an AI second observer for detecting colorectal cancer(CRC)and an ensemble of 5 differently initiated 2D U-Net for ensemble *** model was trained with 51 cases of annotated CRC computed tomography of the abdomen and pelvis,tested with 7 cases,and validated with 20 cases from The Cancer Imaging Archive *** sensitivity,false positives per case,and estimated Dice coefficient were obtained for each method of *** compared the two methods of annotations and the time reduction associated with the *** time differences were tested using Friedman’s two-way analysis of *** Sparse annotation significantly reduces the time for annotation particularly skipping 2 slices at a time(P<0.001).Reduction of up to 2/3 of the annotation does not reduce AI model sensitivity or false positives per *** initializing human annotation with AI reduces the annotation time,the reduction is minimal,even when using an ensemble AI to decrease false *** Our data support the sparse annotation technique as an efficient technique for reducing the time needed to establish the ground truth.

Fluorescence sensors based on a trifluoroacetophone compound doped in ethyl cellulose (EC) thin films have been developed for the detection of methanol, ethanol, and 2-propanol (isopropanol, PriOH) vapors. Thin-film sensors are prepared with 4-dibutylamino-4’-(trifluoroacetyl)stilbene (Chromoionophore IX or CIX) as the fluorescent dye and its solution in EC was spin-coated onto glass slides. The luminescence intensity of the dye (555 nm) is quenched when exposed to alcohol vapor. Tested in the range of ca. 0 - 1.5?× 104 ppm (wt) for MeOH and EtOH, and ca. 0 - 2.3 × 104 ppm for PriOH, the sensors gave detection limits of 9, 13, 21 ppm and quantification limits of 32, 43, and 70 ppm, respectively. To enhance the sensitivity of the sensors, TiO2 particles have been added to the films to induce Mie scattering, which increases the incident light interaction with the sensing films. The sensors in this work have been designed to work in a multianalyte platform for the simultaneous detection of multiple gas analytes.
