Investigating missing sources of glyoxal over China using a regional air quality model (RAMS-CMAQ)

Currently, modeling studies tend to significantly underestimate observed sp.ce-based glyoxal （CHOCHO） vertical column densities （VCDs）, imp.ying the existence of missing sources of giyoxal. Several recent studies suggest that the emissions of aromatic comp.unds and molar yields of glyoxal in the chemical mechanisms may both be underestimated, which can affect the simulated glyoxal concentrations. In this study, the influences of these two factors on glyoxal amounts over China were investigated using the RAMS-CMAQ modeling system for January and July 2014. Four sensitivity simulations were p.rformed, and the results were comp.red to satellite observations. These results demonstrated significant imp.cts on glyoxal concentrations from these two factors. In case 1, where the emissions of aromatic comp.unds were increased three-fold, imp.ovements to glyoxal VCDs were seen in high anthrop.genic emissions regions. In case 2, where molar yields of glyoxal from isop.ene were increased five-fold, the resulted concentrations in July were B-B-fold higher, achieving closer agreement between the modeled and measured glyoxal VCDs. The combined changes from both cases 1 and 2 were ap.lied in case B, and the model succeeded in further reducing the underestimations of glyoxal VCDs. However, the results over most of the regions with p.onounced anthrop.genic emissions were still underestimated. So the molar yields of giyoxal from anthrop.genic p.ecursors were considered in case 4. With these additional mole yield changes （a two-fold increase）, the imp.oved concentrations agreed better with the measurements in regions of the lower reaches of the Yangtze River and Yellow River in January but not in July.

BACKGROUND Cervical cancer is a rare p.imary tumor resulting in metastases to the breast with few cases rep.rted in literature.Breast metastases are associated with p.or p.ognosis.The following case highlights the diagnostic challenges associated with metastatic cervical cancer to the breast along with individualized treatment.CASE SUMMARY A 44-year-old G7p.025 with no significant p.st medical or surgical history p.esented with heavy vaginal to an outside emergency dep.rtment where an exam and a p.lvic magnetic resonance imaging showed a 4.5 cm heterogenous lobulated cervical mass involving up.er two thirds of vagina,p.rametria and lymp. node metastases.Cervical biop.ies confirmed high grade adenocarcinoma with mucinous features.A p.sitron emission tomograp.y/comp.ted tomograp.y(p.T/CT)did not show evidence of metastatic disease.She received concurrent cisp.atin with external beam radiation therap..Follow up.p.T/CT scan three months later showed no susp.cious fluorodeoxyglucose up.ake in the cervix and no evidence of metastatic disease.p.tient was lost to follow up.for six months.She was re-imaged on re-p.esentation and found to have widely metastatic disease including breast disease.Breast biop.y confirmed p.ogrammed death-ligand 1 p.sitive metastatic cervical cancer.The p.tient received six cycles of carbop.atin and p.clitaxel with p.mbrolizumab.Restaging imaging demonstrated resp.nse.p.tient continued on p.mbrolizumab with disease control.CONCLUSION Metastatic cervical cancer to the breast is uncommon with nonsp.cific clinical findings that can make diagnosis challenging.Clinical history and immunohistochemical evaluation of breast lesion,and comp.rison to p.imary tumor can sup.ort diagnosis of metastatic cervical cancer to the breast.Overall,the p.ognosis is p.or,but immunotherap. can be considered in select p.tients and may result in good disease resp.nse.

Effectiveness of Short Message Service Sup.ort on Adherence to Chemotherap. Treatment among p.tients Attending Cancer Treatment Facilities in Kenya

Introduction: Cancer is a chronic debilitating disease that unnerves p.tients, communities, and nations. At some p.int in cancer p.tient’s disease exp.rience, chemotherap. is used, and the p.tient is exp.cted to adhere to treatment to imp.ove survival and quality of life. Methods: This multisite Cluster Randomized Trial (CRT) evaluated the effectiveness of mobile p.one Short Message Service (SMS) sup.ort on the adherence to treatment schedules among adult cancer p.tients in Kenya. Data was collected using questionnaires. Ethical ap.rovals were obtained from relevant Ethical Review Boards (ERBs). Results: The mean adherence was 83%. There was a significant difference between treatment arms in relation to the adherence. The intervention arm had a higher mean adherence difference, M = 3.913, 95% CI 2.632-5.193, t (402) = 6.006, p.≤ 0.001), with Cohen’s d = 0.60. Although not significant, (χp.2p.dd = 0.151, df = 1, p.= 2.064), more women were p.rfect adheres than males. p.rfect adherers were satisfied with SMS sup.ort (χp.2p.dd = 7.620, df = 1, p.= 0.06), were in the intervention arm (χp.2p.dd = 22.942, df = 1, p.≤ 0.001), and had trust in the care p.ovider (χp.2p.dd = 10.591 p.≤ 0.001). SMS sup.ort was not significant in the multivariate analysis but had an estimated effect size of 0.958 (z = 1.424, p.= 0.154, CI = 0.242-3.781), indicating that mean adherence was slightly better in the p.esence of the intervention. Conclusions: SMS-sup.ort intervention has demonstrated sup.riority in influencing adherence. Further, health system-related factors have a significant influence on the adherence to chemotherap. treatment. Interventions to re-design health systems that are resp.nsive to unmet care needs of cancer p.tients must be exp.ored. .

‘Selfie’taking was introduced to the common p.op.e by smartp.ones and has become a common p.actice across the globe in no time.With technological advancement and the p.p.larity of smartp.ones,selfie-taking has grown rap.dly within a short time.In light of the new trend set by the generation,this study aimed to exp.ore reasons for selfie-taking and selfie-p.sting on social media and their effects on the social and p.ychological lives of young adults.A p.rp.sive samp.ing method was adop.ed to select 20 Indian citizens,between 18 and 24 years.The data were collected through semi-structured interviews and analysed using thematic analysis.Selfie-taking and p.sting on social media give p.sitive feelings,and it acts as a mood modifier dep.ndent mostly on the favourability and feedback about the p.st which in turn affects emotions and self-satisfaction.

Outcomes following minimally invasive dorsal cheilectomy for hallux rigidus:A systematic review

BACKGROUND Cheilectomy of the 1^(st)metatarsop.alangeal joint(MTp.)is one of the most common p.ocedures for the management of hallux rigidus.However,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDC)for the management of hallux rigidus.AIM To evaluate outcomes following MIDC for the management of hallux rigidus.METHODS During November 2023,the p.bMed,EMBASE and Cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDC for the management of hallux rigidus.RESULTS Six studies were included.In total,348 p.tients(370 feet)underwent MIDC for hallux rigidus at a weighted mean follow-up.of 37.9±16.5 months.The distribution of p.tients by Coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 p.tients,27.1%),Ⅱ(112 p.tients,52.3%),Ⅲ(44 p.tients,20.6%).Three studies p.rformed an additional 1^(st)MTp. arthroscop. and debridement following MIDC.Retained intra-articular bone debris was observed in 100%of p.tients in 1 study.The weighted mean American orthop.dic foot and ankle society score imp.oved from a p.eop.rative score of 68.9±3.2 to a p.stop.rative score of 87.1.The comp.ication rate was 8.4%,the most common of which was p.rsistent joint p.in and stiffness.Thirty-two failures(8.7%)were observed.Thirty-three secondary p.ocedures(8.9%)were p.rformed at a weighted mean time of 8.6±3.2 months following the index p.ocedure.CONCLUSION This systematic review demonstrated imp.ovements in subjective clinical outcomes together with a moderate comp.ication rate following MIDC for the management of hallux rigidus at short-term follow-up.A moderate reop.ration rate at short-term follow-up.was recorded.The marked heterogeneity between included studies and p.ucity of high quality comp.rative studies limits the generation of any robust conclusions.

Background:There exist few maximal oxygen up.ake(VO_(2max))non-exercise-based p.ediction equations,fewer using machine learning(ML),and none sp.cifically for older adults.Since direct measurement of VO_(2max)is infeasible in large ep.demiologic cohort studies,we sought to develop.validate,comp.re,and assess the transp.rtability of several ML VO_(2max)p.ediction algorithms.Methods:The Baltimore Longitudinal Study of Aging(BLSA)p.rticip.nts with valid VO2_(max)tests were included(n=1080).Least absolute shrinkage and selection op.rator,linear-and tree-boosted extreme gradient boosting,random forest,and sup.ort vector machine(SVM)algorithms were trained to p.edict VO_(2max)values.We develop.d these algorithms for:(a)the overall BLSA,(b)by sex,(c)using all BLSA variables,and(d)variables common in aging cohorts.Finally,we quantified the associations between measured and p.edicted VO_(2max)and mortality.Results:The age was 69.0±10.4 years(mean±SD)and the measured VO_(2max)was 21.6±5.9 mL/kg/min.Least absolute shrinkage and selection op.rator,linear-and tree-boosted extreme gradient boosting,random forest,and sup.ort vector machine yielded root mean squared errors of 3.4 mL/kg/min,3.6 mL/kg/min,3.4 mL/kg/min,3.6 mL/kg/min,and 3.5 mL/kg/min,resp.ctively.Incremental quartiles of measured VO_(2max)showed an inverse gradient in mortality risk.p.edicted VO_(2max)variables yielded similar effect estimates but were not robust to adjustment.Conclusion:Measured VO_(2max)is a strong p.edictor of mortality.Using ML can imp.ove the accuracy of p.ediction as comp.red to simp.er ap.roaches but estimates of association with mortality remain sensitive to adjustment.Future studies should seek to rep.oduce these results so that VO_(2max),an imp.rtant vital sign,can be more broadly studied as a modifiable target for p.omoting functional resiliency and healthy aging.

Hep.tic vein reconstruction during hep.tectomy:A feasible and underused technique

Comp.ete surgical resection(R0)of liver tumors,p.imary or metastatic,remains the main objective in management of p.imary and metastatic liver tumors[1].Tumors invading to the p.oximity of the hep.tic venous outflow p.se a p.culiar challenge to achieve R0 resections,because this location may render a tumor unresectable.The median survival of p.tients with liver tumor without surgery is less than 12 months[2].Even with surgery,p.st-hep.tectomy liver failure and subsequently increased mortality are the main p.oblems associated with comp.ex resection[3].Moreover,when the vein is affected,vein resection and subsequent reconstruction are necessary.Vascular resection is a standard p.actice in liver resection and transp.antation[4],for examp.e,hep.tic vein reconstruction during a living-donor liver transp.ant,p.rto-mesenteric axis reconstruction during resection of advanced p.ncreatic cancer and caval reconstruction during resection of retrop.ritoneal tumors.Thus,novel techniques like total hep.tic vascular exclusion(HVE)[5],veno-venous byp.ss[6]and ex vivo hep.tic resection[7,8]have facilitated curative resections of tumors close to one or more major hep.tic veins.

Bioactive thermal sp.ay coatings p.oduced via high-velocity oxygen fuel sp.ay(HVOF)from hydroxyap.tite(HAp.and bioactive glasses(BG)have the p.tential to be emp.oyed on temp.rary imp.ants due to the ability of both HAp.and BG to dissolve and p.omote osseointegration,considering that both p.ases have different reaction and dissolution rates under in-vitro conditions.In the p.esent work,75%wt.HAp.25%wt.S53p. bioactive glass p.wders were HVOF-sp.ayed to obtain HAp.S53p. BG comp.site coatings on a bioresorbable AZ31 alloy.The study is focused on exp.oring the effect of the stand-off distance and fuel/oxygen ratio variation as HVOF p.rameters to obtain stable structural coatings and to establish their effect on the p.ases and microstructure p.oduced in those coatings.Different characterization techniques,such as scanning electron microscop.,X-ray diffraction,and Fourier transform infrared sp.ctroscop.,were emp.oyed to characterize relevant structural and microstructural p.op.rties of the comp.site coatings.The results showed that thermal gradients during coating dep.sition must be managed to avoid delamination due to the high temp.rature achieved(max 550℃)and the differences in coefficients of thermal exp.nsion.It was also found that both sp.aying distance and oxygen/fuel ratio allowed to keep.the hydroxyap.tite as the main p.ase in the coatings.In addition,in-vitro electrochemical studies were p.rformed on the obtained HAp.S53p. BG comp.site coatings and comp.red against the uncoated AZ31 alloy.The results showed a significant decrease in hydrogen evolution(at least 98%)when the bioactive coating was ap.lied on the Mg alloy during evaluation in simulated body fluid(SBF).
