National trends in resection of the distal pancreas

AIM:To investigate national trends in distal pancreatectomy(Dp) through query of three national patient care ***:From the Nationwide Inpatient Sample(NIS,2003-2009),the National Surgical Quality Improvement project(NSQIp,2005-2010),and the Surveillance Epidemiology and End Results(SEER,2003-2009) databases using appropriate diagnostic and procedural codes we identified all patients with a diagnosis of a benign or malignant lesion of the body and/or tail of the pancreas that had undergone a partial or distal *** of laparoscopy was defined in NIS by the International Classification of Diseases,Ninth Revision correspondent procedure code;and in NSQIp by the exploratory laparoscopy or unlisted procedure current procedural terminology *** SEER,patients were identified by the International Classification of Diseases for Oncology,Third Edition diagnosis codes and the SEER program Code Manual,third edition procedure *** analyzed the databases with respect to trends of inpatient outcome metrics,oncologic outcomes,and hospital volumes in patients with lesions of the neck and body of the pancreas that underwent operative ***:NIS,NSQIp and SEER identified 4242,2681 and 11 082 Dp resections,***,laparoscopy was utilized in 15%(NIS) and 27%(NSQIp).No significant increase was seen over the course of the *** was performed for malignancy in 59%(NIS) and 66%(NSQIp).Neither patient Body mass index nor comorbidities were associated with operative approach(p = 0.95 and p = 0.96,respectively).Mortality(3% vs 2%,p = 0.05) and reoperation(4% vs 4%,p = 1.0) was not different between laparoscopy and open *** complications(10% vs 15%,p pital costs [44 741 dollars,interquartile range(IQR) 28 347-74 114 dollars vs 49 792 dollars,IQR 13 299-73 463,p = 0.02] and hospital length of stay(7 d,IQR 4-11 d vs 7 d,IQR 6-10,p paroscopy was *** and two year sur

Cyclooxygenase-2 plays a central role in the genesis of pancreatitis and associated lung injury

BACKGROUND: The exact mechanism by which cyclooxy- genase-2 (COX-2) promotes inflammation in pancreatitis in obscure. This study was undertaken to investigate the role of COX-2 inhibition in an animal model of pancreati- tis , a disease process characterized by a systemic inflamma- tory response and ensuing neutrophil-mediated lung injury. METHODS: pancreatitis was induced in 24 Sprague-Daw- ley rats by intraperitoneal injection of 20% L-arginine (500 mg/100 g body weight). The animals were randomized into 3 groups (8 rats in each group); controls and rats with pancreatitis intravenously resuscitated with either normal saline (0.9% NaCl 3 ml/kg) at 24 and 48 hours or COX-2 inhibitor (parecoxib 1 mg/kg). pancreatic and lung inju- ries were assessed histologically. Lung injury was assessed utilizing wet;dry ratio and myeloperoxidase activity to in- dicate pulmonary neutrophil infiltration. A Western blot was used to determine COX-2 protein expression in pancrea- tic tissue. RESULTS: The animals treated with COX-2 inhibitors dis- played significantly less pancreatic and lung injuries than their normal saline counterparts. Histological pancreatic and lung injury scores were significantly reduced (p p. Lung wet: dry ratios were sig- nificantly improved and pulmonary neutrophil infiltration was attenuated in the COX-2 group (pprotein expression. CONCLUSION: This study shows, for the first time in a rat model, that adjuvant COX-2 inhibition significandy attenu- ates the severity of both pancreatitis and its associated sys- temic inflammatory response and end-organ injury.

The worldwide distribution and extensive genetic diversity of the whitefly,Bemisia tabaci,has long been ***,the levels of separation within *** and the nomenclature of the various genetic groups have been a subject of *** phylogenetic analyses indicate that *** is a complex composed of 28 morphologically indistinguishable *** this article,we first review the debate and difficulties associated with ***＇s taxonomy and systematics,and argue for the need to apply the biological species concept in order to elucidate ***＇s *** summarize the accumulated genetic and behavioural data on reproductive incompatibilities evident amongst phylogenetic mtCOI groups of *** studies have been conducted with 14 of the 28 putative species covering 54 reciprocal inter-species pairs,and observations on mating behaviour have been conducted for seven species *** from both crossing trials and behavioural observations indicate a consistent pattern of reproductive isolation among the putative *** then discuss the technical and conceptual complexities associated with crossing experiments and behavioural observations designed to reveal reproductive ***,we elaborate on a strategy for further clarifying the pattern of reproductive isolation between *** groups and propose future research directions on the systematics of this complex.

The effect of particle size and amount of inoculum on fungal treatment of wheat straw and wood chips

Background： The aim of this study was to optimize the fungal treatment of lignocellulosic biomass by stimulating the colonization. Wheat straw and wood chips were treated with Ceriporiopsis subvermispora and Lentinula edodes with various amounts of colonized millet grains（0.5, 1.5 or 3.0 % per g of wet weight of substrate） added to the substrates. Also, wheat straw and wood chips were chopped to either 0.5 or 2 *** of the fungal treatment after 0, 2, 4, 6, or 8 wk of incubation was determined by changes in chemical composition, in vitro gas production（IVGp） as a measure for rumen degradability, and ergosterol content as a measure of fungal ***： Incomplete colonization was observed for C. subvermispora treated wheat straw and L. edodes treated wood chips. The different particle sizes and amounts of inoculum tested, had no significant effects on the chemical composition and the IVGp of C. subvermispora treated wood chips. particle size did influence *** treatment of wheat straw. The L. edodes treatment of 2 cm wheat straw resulted in a more selective delignification and a higher IVGp than the smaller particles. Addition of 1.5 % or 3 % L. edodes inoculum to wheat straw resulted in more selective delignification and a higher IVGp than addition of 0.5 % ***： particle size and amount of inoculum did not have an effect on C. subvermispora treatment of wood chips. At least 1.5 % L. edodes colonized millet grains should be added to 2 cm wheat straw to result in an increased IVGp and acid detergent lignin（ADL） degradation.

photocatalytic degradation of textile dyeing wastewater through microwave synthesized Zr-AC,Ni-AC and Zn-AC

The novel zirconium oxide, nickel oxide and zinc oxide nanoparticles supported activated carbons（Zr-AC, Ni-AC, Zn-AC） were successfully fabricated through microwave irradiation method. The synthesized nanoparticles were characterized using XRD, HR-SEM, XpS and BET. The optical properties of Zr-AC, Ni-AC and Zn-AC composites were investigated using UV–Vis diffuse reflectance spectroscopy. The photocatalytic efficiency was verified in the degradation of textile dyeing wastewater（TDW） in UV light irradiation. The chemical oxygen demand（COD） of TDW was observed at regular intervals to calculate the removal rate of COD. Zn-AC composites showed impressive photocatalytic enrichment, which can be ascribed to the enhanced absorbance in the UV light region, the effective adsorptive capacity to dye molecules, the assisted charge transfer and the inhibited recombination of electron-hole pairs. The maximum TDW degradation（82% COD removal） was achieved with Zn-AC. A possible synergy mechanism on the surface of Zn-AC was also designed. Zn-AC could be reused five times without exceptional loss of its activity.

purpose:To examine the effects of a school-based karate intervention on academic achievement,psychosocial functioning,and physical fitness in children aged 7-8 ***:Twenty schools in 5 different European countries(2 second-grade classrooms per school)participated in a cluster randomized controlled trial(Sport at School trial).participants were assigned to either a control group,which continued with their habitual physical education lessons,or to an intervention group,which replaced these lessons with a 1-year karate intervention(Karate Mind and Movement program).A total of 721 children(344 girls and 377 boys,7.4±0.5 years old,mean±SD)completed the study,of which 333 and 388 were assigned to the control group and intervention group,*** included academic performance(average grade),psychosocial functioning(Strengths and Difficulties Questionnaire for parents),and different markers of physical fitness(cardiorespiratory fitness,balance,and flexibility).Results:The intervention provided small but significant benefits compared to the control group for academic achievement(d=0.16;p=0.003),conduct problems(d=-0.28;p=0.003),cardiorespiratory fitness(d=0.36;pp=0.015).There was a trend towards significant benefits for flexibility(d=0.24;p=0.056).No significant benefits were observed for other variables,including psychosocial difficulties,emotional symptoms,hyperactivity/inattention,peer problems,or prosocial behaviour(all p>0.05).Conclusion:A 1-year school-based karate intervention was effective in improving academic achievement,conduct problems,and physical fitness in primary school *** results support the inclusion of karate during physical education lessons.

Distinct risk factors for early and late blood transfusion following pancreaticoduodenectomy

Background: The International Study Group of pancreatic Surgery(ISGpS) has defined two periods of postpancreatectomy hemorrhage, early(24 h). A previously published Blood Usage Risk Score(BURS) aimed to predict early and late blood transfusion. The primary aim of this study was to define risk factors for early and late blood transfusion after pancreaticoduodenectomy. Secondary aims were to assess the predictive accuracy of the ***: In this retrospective observational study, multivariable analyses were used to identify independent risk factors for both early and late blood transfusion. The predictive ability of the BURS was then assessed using a receiver operating characteristic(ROC) curve ***: Among 628 patients, 99(15.8%) and 144(22.9%) received early and late blood transfusion, respectively. Risk factors for blood transfusion differed between early and late periods. preoperative anemia and venous resection were associated with early blood transfusion whilst Whipple’s resection(as opposed to pylorus preserving pancreaticoduodenectomy), lack of biliary stent and a narrow pancreatic duct were predictors of late blood transfusion. The BURS was significantly predictive of early blood transfusion,albeit with a modest degree of accuracy(AUROC: 0.700, p p = 0.360). Late blood transfusion was independently associated with increasing severity of postoperative pancreatic fistula(pOpF)(OR: 1.85, 3.18 and 9.97 for biochemical, types B and C pOpF,respectively, relative to no pOpF).Conclusions: Two largely different sets of variables are related to early and late blood transfusion following pancreaticoduodenectomy. The BURS was significantly associated with early, albeit with modest predictive accuracy, but not late blood transfusion. An understanding of pOpF risk allows assessment of the need for late blood transfusion.

Failure of p-selectin blockade alone to protect the liver from ischemia-reperfusion injury in the isolated blood-perfused rat liver

AIM： To determine if blockade of p-selectin in the isolated blood-perfused cold ex vivo rat liver model protects the liver from ischemia-reperfusion injury. METHODS： The effect of p-selectin blockade was assessed by employing an isolated blood-perfused cold ex vivo rat liver with or without p-selectin antibody treatment before and after 6 h of cold storage in University of Wisconsin solution. RESULTS： In our isolated blood-perfused rat liver model, pre-treatment with p-selectin antibody failed to protect the liver from ischemia-reperfusion injury, as judged by the elevated aspartate aminotransferase activity. In addition, p-selectin antibody treatment did not significantly reduced hepatic polymorphonuclear leukocyte accumulation after 120 min of perfusion. Histological evaluation of liver sections obtained at 120 min of perfusion showed significant oncotic necrosis in liver sections of both ischemic control and p-selectin antibody-treated groups. However, total bile production after 120 rain of perfusion was significantly greater in p-selectin antibody-treated livers, compared to control livers. No significant difference in p-selectin and ICAM-1 mRNAs and proteins, GSH, GSSG, and nuclear NF-kB was found between control and p-selectin antibody-treated livers. CONCLUSION： In conclusion, we have shown that blockade of p-selectin alone failed to reduced polymorphonuclear leukocyte accumulation in the liver and protect hepatocytes from ischemia-reperfusion injury in the isolated blood-perfused cold-ex vivo rat liver model.
