Outcomes following minimally invasive dorsal cheilectomy for hallux rigidus:A systematic review

BACKGROUND Cheilectomy of the 1^(st)metatarsophalangeal joint(MTpj)is one of the most common procedures for the management of hallux ***,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDC)for the management of hallux *** To evaluate outcomes following MIDC for the management of hallux *** During November 2023,the pubMed,EMBASE and Cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDC for the management of hallux *** Six studies were *** total,348 patients(370 feet)underwent MIDC for hallux rigidus at a weighted mean follow-up of 37.9±16.5 *** distribution of patients by Coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 patients,27.1%),Ⅱ(112 patients,52.3%),Ⅲ(44 patients,20.6%).Three studies performed an additional 1^(st)MTpj arthroscopy and debridement following *** intra-articular bone debris was observed in 100%of patients in 1 *** weighted mean American orthopedic foot and ankle society score improved from a preoperative score of 68.9±3.2 to a postoperative score of *** complication rate was 8.4%,the most common of which was persistent joint pain and ***-two failures(8.7%)were ***-three secondary procedures(8.9%)were performed at a weighted mean time of 8.6±3.2 months following the index *** This systematic review demonstrated improvements in subjective clinical outcomes together with a moderate complication rate following MIDC for the management of hallux rigidus at short-term follow-up.A moderate reoperation rate at short-term follow-up was *** marked heterogeneity between included studies and paucity of high quality comparative studies limits the generation of any robust conclusions.

Background:There exist few maximal oxygen uptake(VO_(2max))non-exercise-based prediction equations,fewer using machine learning(ML),and none specifically for older *** direct measurement of VO_(2max)is infeasible in large epidemiologic cohort studies,we sought to develop,validate,compare,and assess the transportability of several ML VO_(2max)prediction ***:The Baltimore Longitudinal Study of Aging(BLSA)participants with valid VO2_(max)tests were included(n=1080).Least absolute shrinkage and selection operator,linear-and tree-boosted extreme gradient boosting,random forest,and support vector machine(SVM)algorithms were trained to predict VO_(2max)*** developed these algorithms for:(a)the overall BLSA,(b)by sex,(c)using all BLSA variables,and(d)variables common in aging ***,we quantified the associations between measured and predicted VO_(2max)and ***:The age was 69.0±10.4 years(mean±SD)and the measured VO_(2max)was 21.6±5.9 mL/kg/*** absolute shrinkage and selection operator,linear-and tree-boosted extreme gradient boosting,random forest,and support vector machine yielded root mean squared errors of 3.4 mL/kg/min,3.6 mL/kg/min,3.4 mL/kg/min,3.6 mL/kg/min,and 3.5 mL/kg/min,*** quartiles of measured VO_(2max)showed an inverse gradient in mortality *** VO_(2max)variables yielded similar effect estimates but were not robust to ***:Measured VO_(2max)is a strong predictor of *** ML can improve the accuracy of prediction as compared to simpler approaches but estimates of association with mortality remain sensitive to *** studies should seek to reproduce these results so that VO_(2max),an important vital sign,can be more broadly studied as a modifiable target for promoting functional resiliency and healthy aging.

The risk of prostate cancer on incidental finding of an avid prostate uptake on 2-deoxy-2-[^(18)F]fluoro-D-glucose positron emission tomography/computed tomography for non-prostate cancer-related pathology:A single centre retrospective study

Objective:To review the risk of prostate cancer(pCa)in men with incidentally reported increased intraprostatic uptake at 2-deoxy-2-[18F]fluoro-d-glucose positron emission tomography/computed tomography(18F-FDG pET/CT)ordered at Department of Urology,The Wesley Hospital,Brisbane,QLD,Australia for non-pCa related ***:Retrospective analysis of consecutive men between August 2014 and August 2019 presenting to a single institution for 18F-FDG pET/CT for non-prostate related conditions was *** were classified as benign,indeterminate,or malignant depending of the results of prostate-specific antigen(pSA),pSA velocity,biopsy histopathology,and three-Tesla(3 T)multiparametric MRI(mpMRI)prostate Imaging Reporting and Data System score,or gallium-68-prostate-specific membrane antigen(68Ga-pSMA)pET/CT ***:Three percent(273/9122)of men demonstrated 18F-FDG avidity within the ***-five percent(231/273)were further investigated,including with pSA tests(227/231,98.3%),3 T mpMRI(68/231,29.4%),68Ga-pSMA pET/CT(33/231,14.3%),and prostate biopsy(57/231,24.7%).Results were considered benign in 130/231(56.3%),indeterminate in 31/231(13.4%),and malignant in 70/231(30.3%).pCa was identified in 51/57(89.5%)of the men who proceeded to biopsy,including 26/27(96.3%)men with prostate Imaging Reporting and Data System scores 4-5 mpMRI and six men with a positive 68Ga-pSMA pET/*** most common Gleason score on biopsy was greater than or equal to 4+5(14/51,27.5%).68Ga-pSMA pET/CT was concordant with the 18F-FDG findings in 26/33(78.8%).All 13 men with a positive concordant 18F-FDG,3 T mpMRI,and 68Ga-pSMA pET/CT had pCa on *** was no statistically significant difference in the 18F-FDG maximum standardized uptake value between the benign or malignant groups(5.7 vs.6.1;p=0.580).Conclusion:In this study,after an incidental finding of an avid intraprostatic lesion on 18F-FDG pET/CT,70 of the 231 cases(30.3%;0.8%of the entire cohort)had results c

Tropomyosin receptor kinase B(TrkB)signaling plays a pivotal role in dendritic growth and dendritic spine formation to promote learning and *** activity-dependent release of brain-derived neurotrophic factor at synapses binds to pre-or postsynaptic TrkB resulting in the strengthening of synapses,reflected by long-term ***,the association of postsynaptic density protein-95 with TrkB enhances phospholipase Cγ-Ca^(2+)/calmodulin-dependent protein kinaseⅡand phosphatidylinositol 3-kinase-mechanistic target of rapamycin signaling required for long-term *** this review,we discuss TrkB-postsynaptic density protein-95 coupling as a promising strategy to magnify brain-derived neurotrophic factor signaling towards the development of novel therapeutics for specific neurological disorders.A reduction of TrkB signaling has been observed in neurodegenerative disorders,such as Alzheimer's disease and Huntington's disease,and enhancement of postsynaptic density protein-95 association with TrkB signaling could mitigate the observed deficiency of neuronal connectivity in schizophrenia and *** with brain-derived neurotrophic factor is problematic,due to poor pharmacokinetics,low brain penetration,and side effects resulting from activation of the p75 neurotrophin receptor or the truncated TrkB.T1 *** TrkB agonists and antibodies that activate TrkB are being intensively investigated,they cannot distinguish the multiple human TrkB splicing isoforms or cell type-specific *** TrkB–postsynaptic density protein-95 coupling provides an alternative approach to specifically boost TrkB signaling at localized synaptic sites versus global stimulation that risks many adverse side effects.

背景：证据显示，与临床常规降脂比较，对稳定性冠状动脉疾病患者施行强化降脂降低低密度脂蛋白胆固醇（low—density lipoprotein cholesterol，LDL—C）可进一步获益。目的：比较2种降脂治疗策略对既往有心肌梗死（MyocardialInfarction，MI）病史患者心血管事件危险的影响。设计、地点及研究对象：IDEAL研究是一项前瞻性、随机、开放、盲法终点评价试验。试验于1999年3月至2005年3月在北欧190个心脏病急救单位和专家诊所进行，中位随访时间为4．8年。试验人选8888例有急性MI病史的患者，年龄≤80岁。干预：患者随机分配接受大剂量阿托伐他汀（80mg／d；n=4439）或常规剂量辛伐他汀（20mg／d；n=4449）治疗。主要终点：主要冠脉事件的发生定义为冠脉死亡、证实有非致命性急性MI或心脏骤停复苏。结果：治疗期间，辛伐他汀组平均LDL—C水平为104mg／dL（SE，0．3），阿托伐他汀组为81mg／dL（SE，0．3）。辛伐他汀组有463例（10．4％）患者发生了主要冠脉事件，阿托伐他汀组为411例（9．3％）（风险比[hazardratio，HR]，0．89；95％CI，0．78—1．01；p=0．07）。两组发生非致命性MI者分别为321例（7．2％）和267例（6．0％）（HR，0．83；95％CI，0．71—0．98；p=0．02）。但是另外两个一级终点组分的发生率两组末见显著差异。两组发生主要心血管事件者分别为608例和533例（HR，0．87；95％CI，0．77—0．98；p=0．02）。辛伐他汀组发生任何一种冠脉事件的患者有1059例，阿托伐他汀组898例（HR，0．84；95％CI，0．76—0．91；p〈0．001）。两组发生非心血管死亡者分别为156例（3．5％）和143例（3．2％）（HR，0．92；95％CI，0．73—1．15；p=0．47）。各种原因所致死亡：辛伐他汀组374例（8．4％），阿托伐他汀组366例（8．2％）（HR，0．98；95％CI，0．85—1．13；p=0．81）。阿托伐他汀组患者凶非严重性不良事件而停药的比例较高；因转氨酶升高分别导致43例（1．0％）和5例（0．1％）患者停药（p（0．001）。两组罕见严重肌病及横纹肌溶解。结论：研究表明，在既往有MI的患者，强化降低LDL—C并不能显著降低主要冠脉事件的一级终点，但是能降低其他复合二级终点及非致命性急性MI的危险。心血管死亡率或全因死亡率没有差异。强化降低LDL—C可使MI患者获益，而且不增加非心血管死亡率或其他严重不良反应。

Background:Newly diagnosed breast cancer patients experience symptoms that may affect their quality of life,treatment outcomes,and *** and managing breast cancer-related symptoms soon after diagnosis is *** purpose of this study was to investigate the associations between health-related fitness(HRF)and patient-reported symptoms in newly diagnosed breast cancer ***:This study utilized baseline data from the Alberta Moving Beyond Breast Cancer Cohort Study that were collected within 90 days of *** measures included peak cardiopulmonary fitness(peak volume of oxygen consumption(VO_(2peak))),maximal muscular strength and endurance,flexibility,and body *** measures included depression,sleep quality,and *** multivariable logistic regression was performed for ***:Of 1458 participants,51.5%reported poor sleep quality,26.5%reported significant fatigue,and 10.4%reported moderate *** multivariable-adjusted models,lower relative VO_(2peak)was independently associated with a greater likelihood of all symptom measures,including moderate depression(ppoor sleep quality(p=0.009),significant fatigue(p=0.008),any symptom(pple symptoms(ppeak)demonstrated threshold associations with all symptom measures such that all 3 lower quartiles exhibited similar elevated risk compared to the highest *** strength of the threshold associations varied by the symptom measure with odds ratios ranging from-1.5 for poor sleep quality to-3.0 for moderate depression and multiple ***,lower relative upper body muscular endurance was also independently associated with fatigue in a dose-response manner(p=0.001),and higher body weight was independently associated with poor sleep quality in an inverted U pattern(p=0.021).Conclusion:Relative VO_(2peak)appears to be a critical HRF component associated with multiple patient-reported symptoms in newly diag

In many songbird species,birdsong features phonological syntax,meaning that the units within their vocal se-quences are ordered in a non-random way that adheres to a *** such syntactical patterns have been richly described in many species,comparatively little is known about how those patterns contribute to song achieving its important *** each of song’s main functions,territorial defense and mate attraction,evidence of a role for syntax is *** species for which syntax has been thoroughly described is the Hermit Thrush(Catharus guttatus),which presents song types from their repertoires in a semi-predictable order and,in doing so,rapidly cycle up and down the frequency *** objective of the present study was to explore the importance of song syntax in the Hermit Thrush through a within-subject examination of how measures of syntax,such as the predictability of song type order within song sequences,shift over the breeding *** hypothesized that,if such syntactical characteristics are important to breeding behaviour,they would be most prominent at the start of the breeding season when activity associated with territory establishment and mate attraction is most *** revealed that,as predicted,the rigidness of song type ordering within se-quences was highest at the start of the season and declined *** song type sequences were most predictable at the vitally important early part of the breeding season fit our hypothesis that this aspect of song syntax is important to song’s functions related to territory establishment and/or mate *** work will clarify whether that role relates to one of song’s two main functions or serves song transmission in some broader way.

The Soft X-ray Imager(SXI)is part of the scientific payload of the Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)*** is a joint science mission between the European Space Agency(ESA)and the Chinese Academy of Sciences(CAS)and is due for launch in *** is a compact X-ray telescope with a wide field-of-view(FOV)capable of encompassing large portions of Earth’s magnetosphere from the vantage point of the SMILE *** is sensitive to the soft X-rays produced by the Solar Wind Charge eXchange(SWCX)process produced when heavy ions of solar wind origin interact with neutral particles in Earth’s *** provides a mechanism for boundary detection within the magnetosphere,such as the position of Earth’s magnetopause,because the solar wind heavy ions have a very low density in regions of closed magnetic field *** sensitivity of the SXI is such that it can potentially track movements of the magnetopause on timescales of a few minutes and the orbit of SMILE will enable such movements to be tracked for segments lasting many *** is led by the University of Leicester in the United Kingdom(UK)with collaborating organisations on hardware,software and science support within the UK,Europe,China and the United States.
