The diagnosis of Chronic Pancreatitis （CP） is based on the detection of abnormal structure or function of the diseased pancreas. The pancreatic function tests more accurately determine the presence of CP than tests of structure, especially for early stage disease. The function tests can be divided into two categories： non- invasive and invasive. The invasive ＂tube＂ tests can reliably detect mild, early CP, but are only available at a few referral centers and tend to be poorly tolerated by patients. The non-invasive tests are easy to obtain, but tend to perform poorly in patients with early, mild disease. Therefore, no one test is useful in all clinical situations, and a detailed understanding of the rational, pathophysiologic basis, strengths, and limitations of various tests is needed. This review highlights the role of various pancreatic function tests in the diagnosis of CP including fecal fat analysis, fecal elastase, fecal chymotrypsin, serum trypsin, the secretin stimulation test, the cholecystokinin （CCK） stimulation test, the combined secretin-CCK stimulation test, the intraductal and endoscopic secretin stimulation tests, and the functional magnetic resonance imaging of the pancreas after secretin stimulation.

The Fine Structure Constant (eFSC) Model attempts to give a classical definition to a magical number that underlies much of quantum physics. The Fine Structure Constant (α) value equal to 137.03599206 represents a dimensionless constant that characterizes the strength of the electromagnetic (EM) interaction between subatomic charged particles. Python-generated property counts for the twin prime force F{139/137} show that the adjusted ratio gives a value of α = 137.036. This implies a mathematical framework underlying this constant is based on twin prime numbers and set theory. This study attempts to demonstrate a proof of concept that a hierarchy of fractional twin prime (αii) forces replicates the quantum nature of the universe and is aligned with the Standard Model of Particle Physics. An expanded eFSC Model demonstrates that twin prime forces and their property sets are mathematically viable substitutes for nuclear reactions, as demonstrated for the Beta-minus decay of neutrons into protons. Most significantly, the positive and negative prime numbers define these nuclear reactants and products as positive or negatively charged ions. Furthermore, the eFSC Model provides new insights regarding the hierarchy of EM forces underlying the quantum nature of the universe.

Dear Editor,gut microorganisms process food in animal guts and release many metabolic by-products,which are predicted to influence host physiological processes such as energy and lipid ***,we investigate how succi-nate,a TCA cycle intermediate that is a major predicted release product of gut bacteria in Drosophila,influences the nutritional physiology of its Drosophila *** ad-ministered succinate as a dietary supplement to microbe-free Drosophila,and quantified key nutritional *** succinate significantly reduced fly lipid levels by up to~50%.This response was not replicated in paral-lel experiments conducted with dietary fumarate supple-ment,indicating that it could not be attributed to a gen-eral effect of TCA *** hypothesize that microbe-derived succinate may contribute to the reduced lipid content of Drosophila bearing gut bacteria,relative to axenic *** generally,this study high-lights the importance of microbial-derived metabolites as regulators of host metabolism.

We have investigated the optical properties of laterally aligned Si nanowire （SiNW） arrays in order to explore their potential applicability in transparent electronics. The SiNW array exhibited good optical transparency in the visible spectral range with a transmittance of -90% for a NW density of -20-25 per 10 μm. In addition, polarization-dependent measurements revealed a variation in transmittance in the range of 80%-95% depending on the angle between the polarization of incident light and the NW axis. Using the SiNWs, we demonstrated that transparent transistors exhibit good optical transparency （greater than 80%） and showed typical p-type SiNW transistor characteristics.

This article presents a case of low-grade cribriform cystadenocarcinomas(LgCCC),a rare salivary gland tumor manifesting in the infratemporal fossa(ITF).The lesion in this case is unique in its location,histopathology,and management in that the tumor resection was performed using an exclusively endoscopic,endonasal *** case highlights the expanding application of endoscopic skull base techniques to address an indolent,slow-growing malignancy of the ITF.

Background:While the optimal combination of whole-brain radiotherapy (WBRT), stereotactic radiosurgery (SRS) and surgical resection in the treatment of brain metastases, is controversial, the addition of SRS to surgical resction of solitary metastasis may enhance local control while potentially minimizing toxicity associated with adjuvant WBRT. This study seeks to determine whether pre-operative SRS to the lesion versus post-operative SRS to the resection bed may reduce irradiation of adjacent normal brain tissue. Methods:A retrospective study of 12 patients with 13 surgically resected cerebral metastases was performed. The pre-operative contrast-enhancing tumors and post-operative resection cavities plus any enhancing residual disease were contoured to yield the gross target volume (gTV). In turn these gTV''''s were uniformly expanded by 3-mm to generate the pre-operative, as well as post-operative planning target volume (PTV.) For each lesion, a 7-static-conformal-beam, non-coplanar plan utilizing 6 MV photons was generated to encompass the PTV within the 85％isodose line. Excess normal brain volume irradiated was defined as the volume outside the gTV receiving the prescribed dose. Results:When lesions were divided into two groups-group A (pre-operative gTV''''sgroup B (pre-operative gTV''''s>15 cc, n=4)-the average volume of normal brain irradiated was significantly smaller if pre-operative SRS was used for treatment of lesions in group A (9.5 vs. 16.8 cc, paired t-test, p=0.0045). In contrast, this volume was smaller for group B lesions if post-operative SRS was used for treatment of these lesions (27.6 vs. 51.2 cc, p=0.252). A comparison of groups with respect to mean volume differences between pre-and post-operative SRS was significantly different (two-sample t-test p=0.016). gTV and the difference between pre-and post-operative volume were highly correlated (Pearson correlation=?0.875, p<0.0001). Factors associated with increased incidence of severe toxicities following yttrium-90 resin microspheres in the treatment of hepatic malignancies

AIM: To further define variables associated with increased incidences of severe toxicities following administration of yttrium-90 (90Y) ***: Fifty-eight patients undergoing 79 treatments were retrospectively assessed for development of clinical and laboratory toxicity incidence following 90Y administration. Severe toxicity events were defined using Common Terminology Criteria for Adverse Events version 4.03 and defined as grade ≥ 3. Univariate logistic regression analyses were used to evaluate the effect of different factors on the incidence of severe toxicity events. Multicollinearity was assessed for all factors with P g Pearson correlation matrices. All factors not excluded due to multicollinearity were included in a multivariate logistic regression model for each measurement of severe ***: Severe (grade ≥ 3) toxicities occurred following 21.5% of the 79 treatments included in our analysis. The most common severe laboratory toxicities were severe alkaline phosphatase (17.7%), albumin (12.7%), and total bilirubin (10.1%) toxicities. Decreased pre-treatment albumin (OR = 26.2, P = 0.010) and increased pre-treatment international normalized ratio (INR) (OR = 17.7, P = 0.048) were associated with development of severe hepatic toxicity. Increased pre-treatment aspartate aminotransferase (AST; OR = 7.4, P = 0.025) and decreased pre-treatment hemoglobin (OR = 12.5, P = 0.025) were associated with severe albumin toxicity. Increasing pre-treatment model for end-stage liver disease (MELD) score (OR = 1.8, P = 0.033) was associated with severe total bilirubin toxicity. Colorectal adenocarcinoma histology was associated with severe alkaline phosphatase toxicity (OR = 5.4, P = 0.043).CONCLUSION: Clinicians should carefully consider pre-treatment albumin, INR, AST, hemoglobin, MELD, and colorectal histology when choosing appropriate candidates for 90Y microsphere therapy.
