Comparison between Diastolic Subendocardial Tissue Pressures Measured Directly or Calculated from Pressure-Flow Relations

Changes in intramyocardial tissue pressure modulate the relationship between coronary pressure and flow during the cardiac cycle. The present study compared the relation between measured and calculated diastolic subendocardial tissue pressure and coronary pressure at zero flow in anesthetized dogs after modulation of either coronary sinus (i.e. Fogarty catheter) or left ventricular intracavity (i.e. volume loading) pressure. experiments were conducted in anesthetized, instrumented dogs;coronary pressure flow relations were constructed during pharmacologic vasodilatation and intramyocardial tissue pressure was measured using micromanometer pressure sensors. elevated coronary sinus pressures did not affect subendocardial pressure-flow relations signifying that diastolic tissue pressure within this layer is the effective coronary back pressure. Higher left ventricular intracavity pressure did not affect either diastolic subendocardial tissue pressure or pressure flow relations within this layer. Results show a direct linear relation (y = 1.106x - 0.652;r2 = 0.59. P = 0.001) between measured and calculated diastolic subendocardial tissue pressure and coronary pressure at zero-flow over a wide range of pressures after either LV systemic or coronary sinus pressure modulation. Knowledge of back pressure in the subendocardium is useful for the evaluation of efficacy of cardiac interventions on myocardial perfusion particularly at the level of the microcirculation.

Cancer chemoprevention through dietary avonoids: what's limiting?

Flavonoids are polyphenols that are found in numerous edible plant species. Data obtained from preclinical and clinical studies suggest that specific flavonoids are chemo-preventive and cytotoxic against various cancers via a multitude of mechanisms. However, the clinical use of flavonoids is limited due to challenges associated with their e ective use, including(1) the isolation and purification of flavonoids from their natural resources;(2) demonstration of the e ects of flavonoids in reducing the risk of certain cancer, in tandem with the cost and time needed for epidemiological studies, and(3) numerous pharmacokinetic challenges(e.g., bioavailability, drug–drug interactions, and metabolic instability). Currently, numerous approaches are being used to surmount some of these challenges, thereby increasing the likelihood of flavonoids being used as chemo-preventive drugs in the clinic. In this review, we summarize the most important challenges and e orts that are being made to surmount these challenges.

Detection of SARS-CoV-2 RNA in wastewater is a promising tool for informing public health decisions during the COVID-19 ***,approaches for its analysis by use of reverse transcription quantitative polymerase chain reaction(RT-q PCR)are still far from standardized *** characterize inter-and intra-laboratory variability among results when using various methods deployed across Canada,aliquots from a real wastewater sample were spiked with surrogates of SARS-CoV-2(gamma-radiation inactivated SARS-CoV-2 and human coronavirus strain 229e[HCoV-229e])at low and high levels then provided"blind"to eight *** estimates reported by individual laboratories were consistently within a 1.0-log_(10) range for aliquots of the same spiked *** laboratories distinguished between low-and high-spikes for both *** expected,greater variability was observed in the results amongst laboratories than within individual laboratories,but SARS-CoV-2 RNA concentration estimates for each spiked condition remained mostly within 1.0-log_(10) *** no-spike wastewater aliquots provided yielded non-detects or trace levels(ene copies/mL)of SARS-CoV-2 *** appear linked to methods that included or focused on the solids fraction of the wastewater matrix and might represent in-situ SARS-CoV-2 to the wastewater ***-229e RNA was not detected in the no-spike ***,all methods yielded comparable results at the conditions *** behavior of SARS-CoV-2 and spiked surrogates in wastewater should be considered to evaluate method effectiveness.A consistent method and laboratory to explore wastewater SARS-CoV-2 temporal trends for a given system,with appropriate quality control protocols and documented in adequate detail should succeed.

Current methods for assessing the environmental impact of point source discharges in the United States are reviewed. The emphasis of many assessments of damage from pollution is shifting from chemical or technological standards to biological standards, i.e., the protection of biological integrity in the receiving system. The importance of integrating information from chemical measurements, toxicity tests, and field surveys in impact assessment is re-emphasized. Ways in which environmental professionals can improve methods for assessing damage from pollution and its control are discussed.

The “opioid crisis” has had a tremendous impact not only on its victims, but also on the practice of medicine, pain patients, and society in general. Unfortunately, efforts to “stem the tide” have not been successful at reducing overdose deaths. Counterbalancing the many ardent efforts to eliminate overdose deaths (such as the current widespread availability and use of opioid-receptor antagonists such as naloxone) is influx of the illicit fentanoids (i.e., fentanyl and analogs). In addition to their high-potency, the fentanoids differ in surprising ways from more “traditional” opioids such as morphine and heroin. This uniqueness contributes to a reduced effectiveness of opioid receptor antagonists in the treatment of opioid overdose. Further greatly complicating overdose treatment is polysubstance abuse (e.g., an opioid plus a benzodiazepine). The non-opioid in the combination is not responsive to an opioid-recep- tor antagonist, which imparts additional challenges. Thus, the new reality introduces complications that negatively impact efforts to reverse “opioid” overdose. exploring Factors Influencing Species Natural Regeneration Response Following Harvesting in the Acadian Forests of New Brunswick

In the Acadian Forest Region of northeastern North America, forest managers are under increasing public pressure to restore the forest to a more historic, natural condition by reducing in clearcutting and promoting partial-cut treatments that more closely emulate historic, local natural disturbance regimes. However, although numerous studies on the effects of partial-cutting on forest regeneration response have been conducted in surrounding temperate and boreal forest ecosystems, there are few studies that directly explore responses to various forms of harvesting within the Acadian Forest ecosystem, with its unique mixture of northern hardwoods and boreal forest species. Here, we conducted one of the first retrospective studies on forest regeneration following a variety of harvesting methods in the Acadian Forest using univariate and multivariate regression trees to assess regeneration response in 50 naturally-regenerating, harvested forest sites in New Brunswick, Canada. Our study shows that regeneration was highly influenced by harvest type, overstory composition, and environmental conditions as reflected by ecoregion classification. Canopy opening size (as controlled by harvest method) significantly influenced the dominance of regenerating species. The presence of conspecific overstory trees increased the likelihood of their regeneration following disturbance, supporting the direct-regeneration hypothesis, especially for species with limited seed dispersal (e.g., sugar maple (Acer saccharum Marsh.) and American beech (Fagus grandifolia ehrh.). Despite reported problems elsewhere in eastern North America, neither American beech nor balsam fir (Abies balsamea (L.) Mill.) constituted significant competition for the desired species on a broad scale, but the presence of beech was a significant deterrent for yellow birch (Betula alleghaniensis Britt.).

Objective:To evaluate the efficacy of multiparametric magnetic resonance imaging(mp-MRI)using Prostate Imaging Reporting and Data System version 2.0(PI-RADSv2)definitions in detecting organ-confined prostate ***:All patients who underwent radical prostatectomy between January 1,2014 and December 30,2014 were *** underwent mp-MRI within 180 days before *** with prior pelvic irradiation or androgen deprivation therapy were *** embedded,whole-mount histopathology was centrally reviewed and correlated with imaging for tumour location,Gleason score(GS)and ***:There were 39 patients included,of which 35(90%)had mp-MRI done post-biopsy.A total of 93 cancer foci were identified on whole-mount pathology,of which mp-MRI detected 63(68%).Of those detected by mp-MRI,14 were PI-RADS 3(n=6 for GS 6,n=8 for GS 7,no GS≥8)and 49 were PI-RADS 4e5(nZ7 for GS 6,nZ33 for GS 7,and nZ9 for GS≥8).There were 30(32%)cancer foci missed by mp-MRI(n=15 for GS 6,n=13 for GS 7 and n=2 for GS≥8).A lesion classified as PI-RADS 4e5 predicted a higher grade cancer on pathology as compared to PI-RADS 3(for GS 7 lesions,odds ratio[OR]=3.53,95%CI:0.93e13.45,p=0.064).The mp-MRI size detection limit was 20 mm2 and 100 mm2 for 50%and 75%probability of cancer,*** associating with radiological and pathologic stage,the weighted Kappa value was 0.69(pe sensitivity and positive predictive values for this study were 68%(95%CI:57%e77%)and 78%(95%CI:67%e86%),***:In this predominantly post-biopsy cohort,mp-MRI using PI-RADSv2 reporting has a reasonably high diagnostic accuracy in detecting clinically significant prostate cancer.

PIK3CA基因激活突变所导致的一系列发育障碍表型谱被统称为PIK3CA基因相关过度生长综合征群(PIK3CA-related overgrowth spectrum,PROS)。临床表现分型众多,各型分类见表1A。由于基因变异发生在受精卵形成后,临床表型为嵌合体,因此PIK3CA基因变异在外周血中的阳性检出率很低,通常需要非血液标本或通过组织活检来发现超低频嵌合体。PIK3CA基因激活变异的作用为促生长,变异导致的表型取决于基因变异发生的时间、组织细胞类型、变异细胞的分布以及变异的激活强度。
