Retrospective Case Series of Porous Titanium Cages in Oblique Lumbar Interbody Fusion Surgery Assessing Subsidence, Fusion and Functional Outcomes

Purpose: Implant subsidence is a possible complication of spinal interbody fusion. We aim to evaluate porous titanium cages subsidence, fusion and functional outcomes in patients subjected to oblique lumbar interbody fusion (OLIF) with these novel devices. Methods: Our institutional review board approved a single-center experience which included 60 patients who underwent OLIF from June 2018 to June 2020 utilizing the porous titanium implants. Data was collected in accordance with the Declaration of Helsinki, and written informed consent was obtained. Imaging studies including radiographs 1, 3, 6 and 12 months and computed tomography (CT) scan at 6 months obtained during routine postoperative follow-up visits, were studied for signs of implant subsidence, fusion and clinical parameters to determine the effectiveness of surgery such as Oswestry disability index (ODI). Results: Radiographic subsidence occurred in 1 out of 89 porous titanium interbody cages (1.1%). No subsidence was observed in the posterior screws and rods fixation group (N = 57). However, one case of subsidence occurred in the lateral plate fixation group (N = 3). The subsidence occurred in an osteoporotic elderly patient operated for adjacent segment disease, and she was later revised with posterior instrumentation using cemented screws and rods. She had an uneventful recovery. Fusion rates were evaluated under CT scan at 6 months with a rate of 88%. In terms of clinical outcomes, ODI decreased significantly from 20.3 preop to 10.7 postop with a P-value Conclusions: In our study, the subsidence rate was lower than previously reported in the literature. Also, we had good fusion rates at 6 months likely due to the porous titanium cages use. We had no subsidence in the posterior instrumented group and one case in the lateral fixation group with improved clinical outcomes.

It takes two: potential therapies and insights involving microglia and macrophages in glioblastoma

Microglia and macrophages, two myeloid cell lineages with different origins, make up the majority of immune cells present in glioblastoma (GBM). However, much of the literature does not distinguish between microglia and macrophages, despite a growing body of evidence that demonstrates key structural and functional differences between the cell types. Furthermore, the current M1/M2 paradigm used to sub-classify microglia and macrophages has proven to be incomplete at best, with the growing amount of in vivo and genomic data incompatible with this dichotomy. Finally, a number of studies have already established that in the setting of the GBM tumor microenvironment, both microglia and macrophages are complicit in tumor progression. This review highlights the differences between microglia and macrophages, particularly in the context of GBM, and discusses at length several potential therapeutic strategies made possible by understanding specific pro-tumor and anti-tumor pathways in these myeloid populations. Ultimately, investigating the differences between microglia and macrophages offers insight into the progression of GBM, its marked resistance to current immunotherapy regimens, and future directions for new treatment modalities.

Jejunal diverticular disease complicated by enteroliths:Report of two different presentations

Jejunal diverticula are quite rare.Furthermore,small bowel diverticular disease resulting in enteroliths can lead to complications necessitating surgical intervention.In this manuscript,we report two presentations of jejunal diverticulum with complications from enteroliths followed by a review of the literature.The first case was that of a 79-year-old male who presented with abdominal pain and was found,on computed tomography scan,to have evidence of intestinal perforation.A laparotomy showed that he had perforated jejunal diverticulitis.The second case was that of an 89-year-old female who presented with recurrent episodes of bowel obstruction.A laparotomy showed that she had an enterolith impacted in her jejunum in the presence of significant diverticular disease.Although a rare entity,familiarity with jejunal diverticular disease,its complications,and its management,should be part of every surgeon's base of knowledge when considering abdominal pathology.

The field K ---- Q（x/-AT） is the only imaginary quadratic field with class number 1, in which the prime 2 splits, and we fix one of the primes p of K lying above 2. The modular elliptic curve X0 （49） has complex multiplication by the maximal order O of K, and we let E be the twist of Xo （49） by the quadratic extension K（v/M）/K, where M is any square free element of O with M -- i mod 4 and （M, 7） = 1. In the present note, we use surprisingly simple algebraic arguments to prove a sharp estimate for the rank of the MordeII-Weil group modulo torsion of E over the field F∞=k（Ep∞ ）, where Epic denotes the group of p∞-division points on E. Moreover, writing B for the twist of X0（49） by K（C/ET）/K, our Iwasawa-theoretic arguments also show that the weak form of the conjecture of Birch and Swinnerton-Dyer implies the non-vanishing at s =1 of the complex L-series of B over every finite layer of the unique Z2-extension of K unramified outside p. We hope to give a proof of this last non-vanishing assertion in a subsequent paper.

CoolVox:Advanced 3D convolutional neural network models for predicting solar radiation on building facades

Data-driven models have become increasingly prominent in the building,architecture,and construction industries.One area ideally suited to exploit this powerful new technology is building performance simulation.Physics-based models have traditionally been used to estimate the energy flow,air movement,and heat balance of buildings.However,physics-based models require many assumptions,significant computational power,and a considerable amount of time to output predictions.Artificial neural networks(ANNs)with prefabricated or simulated data are likely to be a more feasible option for environmental analysis conducted by designers during the early design phase.Because ANNs require fewer inputs and shorter computation times and offer superior performance and potential for data augmentation,they have received increased attention for predicting the surface solar radiation on buildings.Furthermore,ANNs can provide innovative and quick design solutions,enabling designers to receive instantaneous feedback on the effects of a proposed change to a building's design.This research introduces deep learning methods as a means of simulating the annual radiation intensities and exposure level of buildings without the need for physics-based engines.We propose the CoolVox model to demonstrate the feasibility of using 3D convolutional neural networks to predict the surface radiation on building facades.The CoolVox model accurately predicted the radiation intensities of building facades under different boundary conditions and performed better than ARINet(with average mean square errors of 0.01 and 0.036,respectively)in predicting the radiation intensity both with(validation error=0.0165)and without(validation error=0.0066)the presence of boundary buildings.

Improving the capacitance and energy density is a significant challenge while developing practical and flexible energy storage system(ESS).Redox mediators(RMs),as redox-active electrolyte additives,can provide additional energy storing capability via electrochemical faradaic contribution on electrodes for high-performance flexible ESSs.Particularly,determining effective material combinations between electrodes and RMs is essential for maximizing surface faradaic redox reactions for energy-storage performance.In this study,an electrode-RM system comprising heterostructured hybrid(carbon fiber(CF)/MnO_(2)) faradaic electrodes and iodine RMs(I-RMs) in a redox-active electrolyte is investigated.The CF/MnO_(2)with the 1-RMs(CF/MnO_(2)-I) induces dominant catalytic faradaic interaction with the I-RMs,significantly enhancing the surface faradaic kinetics and increasing the overall energy-storage performance.The CF/MnO_(2)-I ESSs show a 12.6-fold(or higher) increased volumetric energy density of 793.81 mWh L^(-1)at a current of 10 μA relative to ESSs using CF/MnO_(2)without I-RMs(CF/MnO_(2)).Moreover,the CF/MnO_(2)-I retains 93.1% of its initial capacitance after 10,000 cycles,validating the excellent cyclability.Finally,the flexibility of the ESSs is tested at different bending angles(180° to 0°),demonstrating its feasibility for flexible and high-wear environments.Therefore,CF/MnO_(2)electrodes present a practical material combination for high-performance flexible energy-storage devices owing to the catalytic faradaic interaction with I-RMs.
