Ba.KGROUND: Dia.nostic ima.ing is a. integra. a.pect of ca.e tha. is often insufficient, if not a.together a.sent, in rura. a.d remote regions of low to middle income countries(LMICs) such a. Ta.za.ia. The introduction of ultra.ound ca. significa.tly impa.t trea.ment in these countries due to its porta.ility, low cost, sa.ety, a.d usefulness in va.ious medica. a.sessments. This study reviews the implementa.ion of a.four-week ultra.ound course a.ministered a.nua.ly from 2013–2016 in a.hea.thca.e professiona. school in Mwa.za. Ta.za.ia.by first-yea. a.lopa.hic US medica. ***: Pa.ticipa.ts(n=582, over 4 yea.s) were recruited from the Ta.da.ui Institute of Hea.th Sciences a.d Technology to ta.e the ultra.ound course. Subjects were predomina.tly clinica. officer students, but other pa.ticipa.ts included other hea.thca.e professiona. students, pra.ticing hea.thca.e professiona.s, a.d school employees. Da.a.collected includes pre-course exa.ina.ion scores, post-course exa.ina.ion scores, course quiz scores, demogra.hic surveys, a.d postcourse feedba.k surveys. Da.a.wa. a.a.yzed using two-ta.led t-tests a.d the single fa.tor a.a.ysis of va.ia.ce(a.OVa..RESULTS: For a.l pa.ticipa.ts who completed both the pre-a.d post-course exa.ina.ions(n=229, 39.1% of the tota. recruited), there wa. a.significa.t mea. improvement in their ultra.ound knowledge of 42.5%, Pa.a.suggests tha. tra.ned first-yea. medica. students ca. effectively tea.h a.point of ca.e ultra.ound course to hea.thca.e professiona. students within four weeks in Ta.za.ia. Future investiga.ion into the level of long-term knowledge retention, impa.t of ultra.ound tra.ning on knowledge of huma. a.a.omy a.d dia.nostic ca.a.ilities, a.d how expa.sion of a. ultra.ound curriculum ha. impa.ted a.cess to ca.e in rura. Ta.za.ia.is wa.ra.ted.

Synovia. ma.ropha.es a.e crucia. in the development of joint infla.ma.ion a.d bone da.a.e;however, the pa.hwa.s tha. controlma.ropha.e remodeling in infla.ma.ory M1 cells or bone-eroding osteocla.ts a.e not fully understood. We determined tha.eleva.ed IL-7R/CD127 expression is the ha.lma.k of rheuma.oid a.thritis (Ra. M1 ma.ropha.es a.d tha. these cells a.e highlyresponsive to interleukin-7 (IL-7)-driven osteocla.togenesis. We esta.lished tha. lipopolysa.cha.ide (LPS), interferon-γ (IFNγ), a.dtumor necrosis fa.tor-α (TNFα), the cla.sic M1 ma.ropha.e media.ors, enha.ce IL-7R expression in Ra.a.d murine *** loca. expression of IL-7 provokes a.thritis, predomina.tly through esca.a.ing the number of F480^(+)iNOS^(+) cells ra.her tha. CD3^(+)T cells. Ectopic LPS injection sta.ilizes IL-7-induced a.thritis by increa.ing myeloid IL-7R expression, in pa.t via.IFNγ ***, in Ra.−/− mice, IL-7-media.ed a.thritis is suppressed beca.se of the reduction in myeloid IL-7R expression due to the la.kof IFNγ. Moreover, the a.eliora.ion of IL-7-induced a.thritis by a.ti-TNF thera.y is due to a.decrea.e in the number of cells in theunique F480^(+)iNOS^(+)IL-7R^(+)CCL5^(+) subset, with no impa.t on the F480^(+)a.gina.e^(+) cell or CD3^(+) T cell frequency. Consistent with thepreclinica. findings, the findings of a.pha.e 4 study performed with Ra.pa.ients following 6 months of a.ti-TNF thera.y revea.edtha. IL-7R expression wa. reduced without a.fecting the levels of IL-7. This study shifts the pa.a.igm by discovering tha. IL-7-induced a.thritis is dependent on F480^(+)iNOS^(+)IL-7R^(+)CCL5^(+) cell function, which a.tiva.es TH-1 cells to a.plify myeloid IL-7Rexpression a.d disea.e severity.

Ba.KGROUND: The purpose of this study wa. to a.sess the short-term effica.y of a.4-week ultra.ound curriculum ta.ght by a.erica. first-yea. medica. students to genera. pra.titioners working in public hea.th ca.e clinics, or puskesma., in Ba.dung, ***: We performed a.prospective, observa.iona. study of Indonesia. hea.th ca.e pra.titioners from public clinics in Ba.dung, Indonesia. These pra.titioners were enrolled in a.4-week ultra.ound tra.ning course ta.ght by first-yea. a.erica. medica. students. a.tota. of six sessions were held comprising of 38 ultra.ound milestones. a.pre-course a.d post-course written exa. a.d pra.tica. exa. wa. ta.en by ea.h ***: We enrolled 41 clinicia.s in the course. The a.era.e pre-course exa. score wa. 35.2% with a.2.4% pa.s ra.e, wherea. the a.era.e post-course exa. score wa. 82.0% with a.92.7% pa.s ra.e. The a.era.e pra.tica. score a. the completion of the course wa. 83.2%(SD=0.145) with 82.9% of the cla.s pa.sing(score a.ove 75.0%).CONCLUSION: Our da.a.suggests tha. first-yea. medica. students ca. effectively tea.h ultra.ound to physicia.s in Indonesia.using a.4-week intensive ultra.ound tra.ning course. Future studies a.e needed to determine the a.ount of tra.ning required for proficiency a.d to eva.ua.e the physicia.s' perceptions of the student-instructors' depth of knowledge a.d skill in point of ca.e ultra.ound.
