维生素逆转胃癌的癌前病变——初期临床干预试验

为探索逆转胃癌的癌前病变以预防其发病,本文观察叶酸加维生素B_(12)、合成β-胡萝卜素及天然β-胡萝卜素对56例中重度萎缩性胃炎牯膜病理和DNA甲基化水平的作用。结果:①叶酸加维生素B_(12)对胃牯膜萎缩和肠化的逆转率达77.7％,高于文献统计:②天然β-胡萝卜素显著逆转不典型增生(P=0.04),推测与其所含异构体有关;③血浆叶酸、隐黄素、α-胡萝卜素水平与胃癌癌前病变呈显著负相关:④α-胡萝卜素可使胃粘膜DNA甲基化水平显著提高(P<0.01～0.0001)。本文提示某些维生素可能通过提高胃粘膜细胞DNA甲基化水平,调节某些基因的活性,因而具有逆转胃癌癌前病变的作用。

Edible oils are obtained by two procedures: by pressing the fruit and with organic solvents. However, there are reports that indicate that the use of solvents affect the quality of the fatty acids inducing the formation of trans isomers, which are banned in many countries because these consumed in large quantities are cause of cardiovascular diseases and increase the cholesterol. The purpose of this work is to apply FTIR to study the trans fatty acids in the avocado oils of the varieties Hass, Fuerte and Criollo extracted by centrifugation (AHC, AFC and ACC, respectively) and with hexane (AHH, ACH and AFH, respectively). The Infrared spectra were obtained with Fourier Transform Infrared (FTIR) in the mid region (4000-600 cm-1). The AHC, AFC and ACC Oils IR spectra show a very intensity band at the frequency 723 cm-1, which corresponds to the functional group cis with a bending vibration mode. For the AFH the trans functional group was identified in the band sited at 968 cm-1 with a strong intensity. The centrifuge method is an alternative for obtaining avocado oil, because the degradation of the fatty acids is minimum and the green color is maintained.

目的：观察青光眼手术患者虹膜切除标本的组织病理学特征。比较拉坦前列腺素治疗组与非拉坦前列腺素治疗组（对照组）组织学差异。方法：虹膜切除标本及患者病史资料呈送拉坦前列腺素病理学中心。标本由3位眼科病理学家以盲法分别观察。评估内容包括：癌变。癌前改变及其他病变：色素沉着水平差异，细胞构成差异。炎症，血管异常情况及虹膜色素上皮改变。结果：所收集标本中，449例曾接受拉坦前列腺素治疗，142例无拉坦前列腺素或其衍生物治疗史。所有标本均未发现癌或癌前病变。拉坦前列腺素治疗组，虹膜痣发生率显著高于对照组（P=0．001）。基质成纤维细胞和黑色素细胞数为对照组2倍（P〈0．001）。采用免疫组化染色技术分析2002年6月收集的各亚组标本，结果显示：拉坦前列腺素治疗组与非拉坦前列腺素治疗组黑色素细胞均数无显著差异（P=0．35）。结论：本研究结果支持已有观点，即：拉坦前列腺素诱发的眼部颜色的改变是由于虹膜基质黑色素细胞内黑色素数量增多而引起，虹膜痣数量增加可能是此效应的局部表现。

This paper presents a method for determining the percentage of coarse aggregate in concrete specimens by image processing. The test pieces were produced with the aim of obtaining images of their cross sections through a scanner table. In order to increase the contrast between mortar and coarse aggregate the sliced surfaces were treated with the phenolphthale in solution. The images obtained in the scanner were processed in a program developed with MATLAB (matrix laboratory). The average coarse aggregate in each section and the mean of coarse aggregate per test body were calculated. With the results, it was revealed that the method returned satisfying results when compared to the original trace of the concrete.

[背景]关于空气污染对中老年人——一个易感的、低储存量的人群——的肺功能影响的研究很少,其中的机制以及潜在效应的易感因素尚不清楚。[目的]在一个表征明确的老年男性队列中评估空气污染物与肺功能关联的滞后结构,以及9个候选基因的26个CpG位点上DNA甲基化（〈或≥中位数）的潜在效应修饰作用。[方法]1999至2009年间,对标准老龄化研究中的776名男性测量1至4次用力肺活量（FVC）、1秒用力呼气量（FEV1）和血液DNA甲基化。在肺功能测试前的4 h至28 d里,通过固定的监视仪测量空气污染。使用线性混合效应模型来估计空气污染物的主要影响以及DNA甲基化的效应修饰作用。[结果]黑炭、总的和非交通源颗粒物（空气动力学直径≤2.5μm,PM2.5）、一氧化碳和二氧化氮的亚慢性暴露（累计3~28 d）每增加一个四分位数间距（IQR）,与FVC和FEV1下降1%~5%有关联（P〈0.05）,但与急性暴露（前4 h内、当天或前一天）没有关联。FVC的斜率估计值比FEV1的大,并随累积暴露而增加。在TLR2位点2和位点5低甲基化（〈中位数）和GCR高甲基化（≥中位数）的参与者中,空气污染物（累计28 d）的斜率估计值都更高。[结论]亚慢性暴露于交通源污染物与老年人肺功能降低有显著的关联;非交通源污染物（颗粒物和臭氧）的关联性较弱。与炎症和免疫有关的表观遗传机制可能会影响这些关联。

背景：有关药物和介入治疗对ST段抬高型和非ST段抬高型急性冠脉综合征（non—ST-segment elevation acute coronary syndrome，NSTE ACS）患者的作用，随机试验已经提供了足够的证据，但是是否已经转化为相应的临床变化还不清楚。目的：确定ST段抬高型心肌梗死（ST-segment elevation myocardial infarction，STEMI）和NSTE ACS患者的院内治疗改变是否伴随临床结果的改善。设计、地点及患者：全球急性冠脉事件注册研究（Global Registry of Acute Coronary Events，GRACE）是一项多国家队列研究。自1999年7月1日至2006年12月31日，该研究共入选并随访了来自14个国家、113家医院的44372例ACS患者。主要观测指标：当前循证药物和介入治疗应用的趋势及患者预后（死亡、充血性心力衰竭、肺水肿、心源性休克、卒中和心肌梗死）。结果：随着研究时间的进展，相关药物的应用增加（β-受体阻滞剂、他汀类药物、血管紧张素转换酶抑制剂、行或未行经皮冠状动脉介入治疗[percutaneous coronary intervention，PCI]患者应用噻氯毗啶、糖蛋白Ⅱb／Ⅲa抑制剂和低分子肝素；P〈0．001）。STEMI患者药物再灌注治疗下降了22个百分点（95％可信区间[confidence interval，CI]，-27～-17），而直接PCI增加了37个百分点（95％CI，33—41）。在NSTEMI患者中，PCI的比例显著升高了18个百分点（95％CI，15～20）。充血性心力衰竭和肺水肿在STEMI患者中降低了9个百分点（95％CI，-12～-6），在NSTEACS患者中降低了6．9个百分点（95％CI，-8．4～-4．7）。STEMI患者的院内死亡率降低了18个百分点（95％CI，-5．3～-1．9），心源性休克降低了24个百分点（95％CI，-4．3～-0．5）。校正了危险评分后，NSTEACS患者的院内死亡率降低了0．7个百分点（95％CI，-1．7～0．3）；STEMI患者6个月随访时卒中发生率降低了0．8个百分点（95％CI，-1．7～0．1），心肌梗死发生率降低了2．8个百分点（95％CI，-6．4～0．9）。NSTEACS患者6个月死亡率降低了1．6个百分点（95％CI，-3．0～-0．1），卒中发生率降低了0．7个百分点（95％CI，-1．4～0．1）。结论：在本项多国家观察性研究中，ACS患者治疗的改善与6个月新发心力衰竭、死亡率、卒中和心肌梗死发生率显著降低相关。
