Secondary Hypertension in Sub-Saharan African Populations: A Retrospective Study between 2011 and 2016 at Regional Hospital of Saint-louis, Senegal

Introduction: High blood pressure (HBP) is a worldwide health issue responsible of high cardiovascular morbidity and mortality. Even though essential HBP is far the most frequently reported in patients, secondary causes must be known because of their severity and the possibility of aetiological treatment. No recent epidemiological data are available about secondary causes of HBP in black African populations. The aim of this study was to describe aetiological patterns of secondary HBP in patients followed at Saint-louis Hospital. Patients and Method: We conducted a retrospective and descriptive study in regional hospital of Saint-louis. All patients aged ≥15 years old admitted from January 1st 2011 to January 31st 2015 in internal medicine, nephrology, emergency and cardiology departments were included. Clinical, paraclinical data and patients outcomes were collected from medical records. Hypertension was defined according to JNC8 criteria. Secondary HBP was considered if explorations identified a clear aetiology to hypertension. Statistical analysis was done with Excel 2010 and STATA 12.0. Results: We included 9253 patients with mean age of 35 ± 12 years (15 - 83 years) and sex-ratio of 1.6. Overall 67.5% of patients had hypertension and secondary causes were found in 10.5% of them. The majority of patients presented clinical symptoms suggesting a secondary cause of HBP and first-line laboratory explorations were normal in half of cases. Renal diseases were responsible for 79.1% of secondary HBP cases mainly dominated by glomerulonephritis (22.6%), vascular nephropathies (18.7%) and autosomal dominant polycystic kidney disease (5.8%). They were followed by preeclampsia (13.6%) and endocrinal aetiologies such as hyperthyroidism (5.8%), hypercorticism (0.5%), pheochromcytoma (0.5%), primary hyperparathyroidism (0.4%) and Conn’s adenoma (0.1%). Schistosomiasis,one of the 17 neglected tropical diseases listed by the World Health Organization,presents a substantial public health and economic *** the 261 million people requiring preventive chemotherapy for schistosomiasis in 2013,92%of them lived in sub-Saharan Africa and only 12.7%received preventive ***,in 2010,the WHO reported that schistosomiasis mortality could be as high as 280000 per year in Africa *** May 2012 delegates to the sixty-fifth World Health Assembly adopted resolution WHA65.21 that called for the elimination of schistosomiasis,and foresees the regular treatment of at least 75%of school age children in at-risk *** resolution urged member states to intensify schistosomiasis control programmes and to initiate elimination campaigns where *** this,in June 2015,schistosomiasis was indicated to have the lowest level of preventive chemotherapy implementation in the spectrum of neglected tropical *** was also highlighted as the disease most lacking in *** is perhaps unsurprising,given that it was also the only NTD with access to drug donations but without a coalition of stakeholders that collaborates to boost commitment and *** a consequence,and to ensure that the WHO NTDs Roadmap Targets of 2012 and World Health Assembly Resolution WHA65.21 are met,the Global Schistosomiasis Alliance(GSA)has been set *** and representative,the GSA aims to be a partnership of endemic countries,academic and research institutions,international development agencies and foundations,international organizations,non-governmental development organizations,private sector companies and advocacy and resource mobilisation ***,the GSA calls for a partnership to work for the benefit of endemic countries by addressing health inequity and rural poverty.

Sex Differences in Delaying Family＂ Effects on Job and Life Satisfaction

Individuals in the workplace can have a difficult time advancing both career and family, and this remains especially true for women. The decision to forego or delay a relationship in favor of advancement at work can have a detrimental impact on satisfaction. A national sample (N = 3504) was examined for sex differences in delaying relationships and the resulting impact on life and job satisfaction. An interaction was found between sex and delaying relationship commitments in relation to pay satisfaction. Men who had not delayed committing to a relationship were significantly more satisfied with pay than those who delayed. However, women did not show this difference. Men and women who delayed a relationship displayed significantly lower levels of life satisfaction. Women also reported lower earnings and more family stress, but also reported higher levels of job satisfaction. Overall, delay of marriage and family can lead a decrease in pay satisfaction, life and family satisfaction and an increase in stress.

Background: Neuroblastoma is the most common extracranial solid tumor in children, and treatment options for recurrent neuroblastoma are limited. Using molecular profiling to target the molecular vulnerabilities of neuroblastoma with existing therapeutic agents may result in a rational, data-driven approach with potential to improve clinical outcomes. Methods: The primary objective of this pilot study was to evaluate the feasibility of supporting real-time treatment decisions through predictive modeling of genome-wide mRNA gene expression data from neuroblastoma tumor biopsies. Feasibility was defined as completion of tumor biopsy, histopathological evaluation, RNA extraction and quality control, gene expression profiling within a CLIA-certified laboratory, bioinformatic analysis, generation of a drug predicttion report, molecular tumor board review yielding a formulated treatment plan, and independent medical monitor review within a 2-week period. Results: Five patients with multiply relapsed or refractory neuroblastoma were enrolled between April and June 2010. All biopsies passed histopathology and RNA quality control. Generation of gene expression data and its analysis (3-7 days), reports which linked this data into medically actionable drug candidates (1-5 days), molecular tumor board (1-3 days) and independent medical monitor review (1 day) were all completed in real-time. The average time was 10.5 days for all patients. Conclusion: This study shows that it is feasible to create therapeutic treatment plans based on genomic profiling in less than 12 days. This warrants further testing in a Phase I study to determine safety of predicted treatments and evaluate whether the information obtained in these analyses would result in patient benefit.

Sentinel lymph node mapping for malignant melanoma of the external auditory canal

We describe a novel technique for sentinel lymph node mapping and biopsy of a primary cutaneous malignant melanoma in the medial portion of the external auditory canal. The approach is illustrated through a case report and technical description of a procedure performed under general anesthesia on a 19-year-old female patient. Due to the hidden and sensitive location of the primary tumor in the medial external auditory canal, the lymphoscintigraphy injection had to be performed by the surgeon immediately prior to the resection of her c T2 a N0M0 lesion. Final pathology revealed clear margins at the primary site resection and 2 intraparotid sentinel lymph nodes with microscopic foci of metastatic malignant melanoma, which led to further surgical management. A completion left parotidectomy and neck dissection yielded no additional metastatic disease in the fifty-five nodes that were evaluated. Using this technique, sentinel lymph node mapping and biopsy accurately predicted the highest risk lymph nodes for the primary lesion of the medial portion of the external auditory canal.

Multimodality imaging of ovarian cystic lesions: Review with an imaging based algorithmic approach

Ovarian cystic masses include a spectrum of benign, borderline and high grade malignant neoplasms. Imaging plays a crucial role in characterization and pretreatment planning of incidentally detected or suspected adnexal masses, as diagnosis of ovarian malignancy at an early stage is correlated with a better prognosis. Knowledge of differential diagnosis, imaging features, management trends and an algorithmic approach of such lesions is important for optimal clinical management. This article illustrates a multi-modality approach in the diagnosis of a spectrum of ovarian cystic masses and also proposes an algorithmic approach for the diagnosis of these lesions.

Annual rings from 30 year old vines in a California rootstock trial were measured to determine the effects of 15 different rootstocks on Chardonnay and Cabernet Sauvignon *** traits measuring vegetative growth,yield,berry quality,and nutrient uptake were collected at the beginning(1995 to 1999)and end(2017 to 2020)of the lifetime of a vineyard initially planted in 1991 and removed in 2021.X-ray Computed Tomography(CT)was used to measure ring widths in 103 *** width was modeled as a function of ring number using a negative exponential *** and late wood ring widths,cambium width,and scion trunk radius were correlated with 27 *** of annual ring width shows that scions alter the width of the first rings but that rootstocks alter the decay of later rings,consistently shortening ring width throughout the lifetime of the *** index,juice pH,photosynthetic assimilation and transpiration rates,and instantaneous water use efficiency are correlated with scion trunk ***,our research indicates that rootstocks modulate secondary growth over years,altering physiology and agronomic *** act in similar but distinct ways from climate to modulate ring width,which borrowing techniques from dendrochronology,can be used to monitor both genetic and environmental effects in woody perennial crop species.

Despite high initial response rates,acute myeloid leukemia(AML)treated with the BCL-2-selective inhibitor venetoclax(VEN)alone or in combinations commonly acquires *** performed gene/protein expression,metabolomic and methylation analyses of isogenic AML cell lines sensitive or resistant to VEN,and identified the activation of RAS/MAPK pathway,leading to increased stability and higher levels of MCL-1 protein,as a major acquired mechanism of VEN resistance.
