过去的20年里,在美国实施的肩关节置换术的数量迅速增长,其中包括部分或全肱骨头表面置换术、半肩置换术、全肩关节置换术和反式全肩关节置换术。影像学检查在诊断这些肩关节置换术引起的并发症方面起到了重要作用。本综述分为两个部分。第一部分阐述了对各种影像学检查模式的总体探讨,包括X线片、计算机断层扫描、核磁共振成像、超声、核医学以及它们提供有益的以治疗为导向的信息时所发挥的作用。第二部分重点研究最佳影像学方法的选择,用于肩关节置换术的并发症,包括无菌性松动、感染、骨折、肩袖肌腱撕裂和神经损伤。在本报告中所提供的美国放射学院适宜性标准循证指南,是通过广泛分析来自同行评审期刊的现有医学文献,并应用行之有效的方法学而完成的(兰德/加州大学洛杉矶分校的适宜性方法以及推荐评价、发展和评估分级),它们用于特殊临床情况下的影像学检查和治疗措施的适宜性评价。多学科专家小组对指南进行了进一步的分析和审查。由于用于推荐合适成像算法的数据会出现不足或模棱两可,在这种情况下,专家意见可能对现有证据进行补充。为此,将美国《放射学院适宜性标准:肩关节置换术后影像学检查》翻译成中文,供同道参考。

目的:采用统一的分析方案,评估全球多个国家/地区的二氧化氮(NO_(2))与总死亡率、心血管和呼吸系统疾病死亡率之间的短期关联。研究设计:采用两阶段的时间序列分析方法、过度离散的广义线性模型和多水平meta分析。研究地点:22个低到高收入国家/地区的398个城市。主要结局指标:1973—2018年逐日总死亡人数(6280万人)、心血管疾病死亡人数(1970万人)和呼吸系统疾病死亡人数(550万人)。结果:平均而言,NO_(2)浓度在滞后1天(前1天)每增加10μg/m^(3),会导致总死亡率、心血管和呼吸系统疾病死亡率分别增加0.46%(95%可信区间0.36%~0.57%)、0.37%(0.22%~0.51%)、0.47%(0.21%~0.72%)。在对共污染物(PM_(10)、PM_(2.5)、臭氧、二氧化硫和一氧化碳)进行调整后,这些关联仍然很稳定。所有3种死因的暴露-反应曲线几乎是线性的,没有明显的阈值。在398个城市中,可归因于高过假定零水平的NO_(2)浓度造成的死亡比例为1.23%(95%可信区间0.96%~1.51%)。结论:这项多中心研究提供了关于NO_(2)短期暴露与总死亡率、心血管和呼吸系统死亡风险之间的独立和线性关联的关键证据,说明通过加强NO_(2)的控制和监管限制标准,可获得人群水平的健康收益。

近年来实效性临床试验受到更多重视,尤其在传统医学临床评价研究领域。为了提高实效性临床试验报告的质量,CONSORT声明组和医疗保健体系中的实效性随机对照临床试验(pragmatic randomized controlled trials in health care systems,PRACTIHC)小组对《CONSORT声明》进行了修改,以帮助读者评估试验质量和结果的可靠性。实效性试验报告指南,作为《CONSORT声明》的一个特殊扩展,能在报告、评价和应用实效性试验方面帮助编辑、系统评价者、试验设计者和决策者评估实效性试验报告中的信息,判定其结论是否适用,以及干预措施是否可行、可否被接受。2005年1月和2008年3月,在加拿大多伦多分别举行了为期两天的会议来探讨提高随机对照试验对卫生保健决策贡献的方法,焦点集中于实效性试验。2005年会,后扩展版草案修订清单发送至编写组成员,经过几次修订,编写组起草了一份摘要草案。在2008年讨论并修改了这一草案。这份草案发给CONSORT声明组以供修改并发表。这份声明的扩展版提出了对8个条目的补充。及时向国内读者和临床试验研究人员介绍该指南有重要的现实意义和学术价值。北京中医药大学循证医学中心刘建平教授等在短时间内翻译了该指南。本刊希望通过介绍中文版"提高实效性临床试验报告的质量——《CONSORT声明》的扩展"以提高国内实效性临床试验报告的质量,相信也有助于改进试验设计的方法学质量。

Objective: To compare the perioperative outcomes of two cesarean section methods, the finger- assisted stretching technique (FAST), based on a modified joel- Cohen method, with the traditional technique. Methods: A retrospective review of the records of 416 women who underwent cesarean sections at Guro Hospital, Seoul, Korea, between May 1993 and December 2001 was performed. Of the 416 women, 283 underwent cesarean sections with FAST and 133 with the traditional technique. Results: Operative time was significantly shorter with FAST (15.3 vs. 42.6 min, P < .05), and FAST was associated with lower blood loss (601 vs. 928 mL, P < .05) and shorter hospital stay (3.7 vs. 6.5 days, P < .05). There were no significant differences in wound infection, voiding difficulty, and postoperative adhesions between the two methods. Conclusion: These results suggest that FAST may be the better technique.

Diabetic retinopathy(DR)affects approximately one-third of diabetic patients and,if left untreated,progresses to proliferative DR(PDR)with associated vitreous hemorrhage,retinal detachment,iris neovascularization,glaucoma and irreversible *** vitreous samples of human patients with PDR,we found elevated levels of hypoxia inducible factor 1 alpha(HIF1α).HIFs are transcription factors that promote hypoxia adaptation and have important functional roles in a wide range of ischemic and inflammatory *** recreate the human PDR phenotype for a preclinical animal model,we generated a mouse with neuroretinal-specific loss of the von Hippel Lindau tumor suppressor protein,a protein that targets HIF1αfor *** found that the neuroretinal cells in these mice overexpressed HIF1αand developed severe,irreversible ischemic retinopathy that has features of human *** progression of retinopathy in these mutant mice should facilitate the evaluation of therapeutic agents for ischemic and inflammatory blinding *** addition,this model system can be used to manipulate the modulation of the hypoxia signaling pathways,for the treatment of non-ocular ischemic and inflammatory disorders.
