Peripancreatic collections in acute pancreatitis: Correlation between computerized tomography and operative fi ndings

AIM: To evaluate the ability of contrast-enhanced computerized tomography (CECT) to characterize the nature of peripancreatic ***: Twenty five patients with peripancreatic collections on CECT and who underwent operative intervention for severe acute pancreatitis were retrospectively studied. The collections were classified into (1) necrosis without frank pus; (2) necrosis with pus; and (3) fluid without necrosis. A blinded radiologist assessed the preoperative CTs of each patient for necrosis and peripancreatic fluid collections. Peripancreatic collections were described in terms of volume, location, number, heterogeneity, fluid attenuation, wall perceptibility, wall enhancement, presence of extraluminal gas, and vascular ***: Fifty-four collections were identif ied at operation, of which 45 (83%) were identif ied on CECT. Of these, 25/26 (96%) had necrosis without pus, 16/19 (84%) had necrosis with pus, and 4/9 (44%) had fluid without necrosis. Among the study characteristics, fluid heterogeneity was seen in a greater proportion of collections in the group with necrosis and pus, compared to the other two groups (94% vs 48% and 25%, P = 0.002 and 0.003, respectively). Among the wall characteristics, irregularity was seen in a greater proportion of collections in the groups with necrosis with and without pus, when compared to the group with fluid without necrosis (88% and 71% vs 25%, P = 0.06 and P < 0.01, respectively). The combination of heterogeneity and presence of extraluminal gas had a specif icity and positive likelihood ratio of 92% and 5.9, respectively, in detecting pus. CONCLUSION: Most of the peripancreatic collections seen on CECT in patients with severe acute pancreatitis who require operative intervention contain necrotic tissue. CECT has a somewhat limited role in differentiating the different types of collections.

CO_(2)capture and conversion has been prospected as an auspicious technology to simultaneously tackle the rise in global CO_(2)emission and produce valueadded fuels with the goal of accomplishing carbon neutrality.A sustainable route to achieve this is via the utilization of solar energy,thereby harnessing the abundant and nonexhaustive resource to shift our reliance away from rapidly depleting fossil *** carbon nitride(g-C_(3)N_(4))and its allotrope have earned its rank as a fascinating metal-free photocatalyst due to its superior stability,high surface-area-to-volume ratio,and tunable surface *** leveraging these properties,robust carbon nitride-based nanostructures are engineered for photocatalytic CO_(2)conversion to energy-rich C_(1)-C_(2) product,which is indispensable in the chemical ***,this review presents the latest panorama of experimental and computational research on tuning the local electronic,surface chemical coordination environment,charge dynamics and optical properties of low-dimensional carbon nitride and its allotropes toward highly selective and efficient CO_(2)*** name a few,structural engineering,point-defect engineering,heterojunction construction,and cocatalyst *** advance this frontier,critical insights are elucidated to establish the structure-performance relationship and unravel primary factors dictating the selectivity of C_(1)-C_(2) molecules from CO_(2)***-field assisted photocatalysis such as with electric(photoelectro-)and heat(photothermal)is discussed to uncover the synergistic contributions that drive the development in ***,future challenges and prospects are outlined for the potential application of solar-driven CO_(2)conversion,along with the scale-up strategy from the economic viewpoint toward the rational development of high-efficiency carbon nitride catalysts.

目的临床数据分析旨在比较“8针法”针灸治疗与经典的中医针灸治疗LDH,得出的治疗方法有更高的症状改善腰椎间盘突出症的治疗以及试图找到一个更有效的LDH的针灸技术。方法收集2020年4月至2020年11月被确诊为腰椎间盘突出症(LDH)的患者,60例。随机分为组1(n=30)和组2(n=30)。组1采用“8针法”针灸治疗。组2采用传统针刺疗法治疗。两组治疗频率均为每周2次,间隔3～4天,共6周。12个疗程为一个疗程。每次治疗持续时间为30分钟。采用视觉模拟评分(Visual Analogue Scale,VAS)、日本骨科协会评分(Japan Orthopedic Association Score,JOA)、Roland-Morris 残障问卷工作天数(Roland-Morris Disability Questionnaire Work days,RMDQ)、Oswestry 残障指数问卷(Oswestry Disability Index,ODIQ)和MRI对两组的结果进行评估。数据分析在患者完成一个疗程后进行。结果本研究显示,经过一个疗程后,两组患者疼痛、功能和生活质量水平(VAS、JOA、RMDQ、ODIQ)均有统计学意义的改善(p<.001)。然而,发现接受8针治疗方法的患者,疼痛显著降低(M=.20,SE=.20)(p=.008),功能水平、完成生理和社会活动(ODIQ)(M=.07,SE=.01)(p=.001)较接受传统治疗的高(M=1.03,SE=.23)(p=.008),ODIQ(M=.10,SE=0.2)(p=.001)。试验还显示,在VAS中,8针法治疗在6次治疗后达到最佳结果VAS(M=.20,SE=.20)(p<.001),而传统治疗仅在12次治疗后达到最佳结果VAS(M=1.03,SE=.23)(p<.001)。8针法疗法各指标均有较高的得分:VAS(M=.20,SE=.20)(p<.001);JOA(M=28.60,SE=.40);RMDQ(M=.27,SE=.27):ODIQ(M=.07,SE=.01)(p<.001),优于传统治疗(M=1.03,SE=.23)(p<.001);JOA(M=27.57,SE=.52);RMDQ(M=.53,SE=.53);ODIQ(M=.10,SE=.02)(p<.001)。两组间JOA和RMDQ差异无统计学意义结论对于腰椎间盘突出症(LDH)轻中度患者,“八针法”是更有效的非手术方法。

Modified activated carbon with interconnected fibrils of iron-oxyhydroxides using Mn^(2+) as morphology regulator, for a superior arsenic removal from water

The arsenic removal efficiency of iron-modified activated carbons depends greatly on the number of available iron oxide surface sites, which are given by the surface area of the anchored particles. In this sense, aiming the generation of an adsorbent with superior arsenic adsorption capacity, we developed a protocol to anchor interconnected fibrils of iron oxyhydroxides, using Mn^(2+) as a morphology regulator. The protocol was based on a microwave-assisted hydrothermal method, using bituminous based activated carbon and both Fe^(2+) and Mn^(2+) ions in the hydrolysis solution. The elemental analysis of modified carbons revealed that Mn does not anchor to the carbon. However, when Mn is included in the hydrolysis solution, the iron content in the activated carbon increased up to 3.5 wt%,without considerable decreasing the adsorbent surface area. Under specific hydrothermal conditions, the Mn^(2+) promoted the formation of iron oxide nanoparticles shaped as interconnected fibrils. This material showed a superior arsenic adsorption capacity in comparison to similar iron modified activated carbons(5 mg As/g carbon, at 2 mg As/L),attributed to the increase in quantity and availability of active sites located on the novel interconnected fibrils of iron oxyhydroxides nanostructures.

The capability of embedded piezoelectric wafer active sensors(PWAS)to perform in-situ nondestructive evaluation(NDE)for structural health monitoring(SHM)of reinforced concrete(RC)structures strengthened with fiber reinforced polymer(FRP)composite overlays is ***,the disbond detection method were developed on coupon specimens consisting of concrete blocks covered with an FRP composite *** was found that the presence of a disbond crack drastically changes the electromecfianical(E/M)impedance spectrum lneasurcd at the PWAS *** spectral changes depend on the distance between the PWAS and the crack ***,large scale experiments were conducted on a RC beam strengthened with carbon fiber reinforced polymer(CFRP)composite *** beam was subject to an accelerated fatigue load regime in a three-point bending configuration up to a total of 807,415 *** these fatigue tests,the CFRP overlay experienced disbonding beginning at about 500,000 *** PWAS were able to detect the disbonding before it could be reliably seen by visual *** correlation between the PWAS readings and the position and extent of disbond damage was *** preliminary results demonstrate the potential of PWAS technology for SHM of RC structures strengthened with FRP composite overlays.

Adult celiac disease with acetylcholine receptor antibody positive myasthenia gravis

Celiac disease has been associated with some autoimmune disorders. A 40-year-old competitive strongman with celiac disease responded to a gluten-free diet, but developed profound and generalized motor weakness with acetylcholine receptor antibody positive myasthenia gravis, a disorder reported to occur in about 1 in 5000. This possible relationship between myasthenia gravis and celiac disease was further explored in serological studies. Frozen stored serum samples from 23 acetylcholine receptor antibody positive myasthenia gravis patients with no intestinal symptoms were used to screen for celiac disease. Both endomysial and tissue transglutaminase antibodies were examined. One of 23 (or, about 4.3%) was positive for both IgA-endomysial and IgA tissue transglutaminase antibodies. Endoscopic studies subsequently showed duodenal mucosal scalloping and biopsies confirmed the histopathological changes of celiac disease. Celiac disease and myasthenia gravis may occur together more often than is currently appreciated. The presence of motor weakness in celiac disease may be a clue to occult myasthenia gravis, even in the absence of intestinal symptoms.

Effects of intermittent sprint and plyometric training on endurance running performance

Purpose:The purpose of this study was to compare the effects of intermittent sprint training and plyometric training on endurance running ***:Fourteen moderately trained male endurance runners were allocated into either the intermittent sprint training group(n = 7)or the plyometric training group(n = 7).The preliminary tests required subjects to perform a treadmill graded exercise test,a countermovement jump test for peak power measurement,and a 10-km time *** included 12 sessions of either intermittent sprint or plyometric training carried out twice per *** completion of the intervention,post-tests were ***:Both groups showed significant reduction in weekly training mileage from pre-intervention during the intervention *** were significant improvements in the 10-km time trial performance and peak *** was also significant improvement in relative peak power for both *** 10-km time trial performance and relative peak power showed a moderate inverse ***:These findings showed that both intermittent sprint and plyometric training resulted in improved 10-km running performance despite reduction in training *** improvement in running performance was accompanied by an improvement in peak power and showed an inverse relationship with relative peak power.

Practical starter pig amino acid requirements in relation to immunity,gut health and growth performance

Immune system activation begins a host of physiological responses. Infectious agents are recognized by monocytes and macrophages which in turn stimulate cytokine production. It is the hormone-like factors called cytokines that orchestrate the immune response. The classic responses observed with immune system activation and cytokine production include： anorexia, fever, lethargy, recruitment of other immune cells, and phagocytosis. While production of immune system components is known to require some amino acids, increases in amino acid requirements are more than offset by the associated decrease in protein accretion and increased muscle protein degradation that also accompanies immune system activation. However, the biggest impact of cytokine production is a decrease in feed intake. Therefore, as feed intake decreases, the energy needed to drive protein synthesis is also decreased. This suggests that diets should still be formulated on a similar calorie：lysine ratio as those formulated for non-immune challenged pigs. The evidence is sparse or equivocal for increasing nutrient requirements during an immune challenge. Nutritionists and swine producers should resist the pressure to alter the diet, limit feed, or add expensive feed additives during an immune challenge. While immune stimulation does not necessitate changes in diet formulation, when pigs are challenged with non-pathogenic diarrhea there are potential advantages on gut health with the increased use of crystalline amino acids rather than intact protein sources （i.e., soybean meal）. This is because reducing crude protein decreases the quantity of fermentable protein entering the large intestine, which lowers post weaning diarrhea. It also lowers the requirement for expensive specialty protein sources or other protein sources such as soybean meal that present immunological challenges to the gut.
