OBJEcTIVE To examine fatigue and quality of life (QOL) inbreast cancer patients undergoing chemotherapy or *** A self-report survey derived from the chineseversion of Brief Fatigue Inventory, the Functional Assessmentof chronic Illness Therapy for Breast cancer, and the Medicaloutcomes Study Social Support Survey. Descriptive statisticswas used to examine the intensity of fatigue and the prevalenceof severe fatigue. Multivariate analysis of variance was used todetermine factors that affect the five domains of QOL among *** The majority of the participants (n = 261) perceiveda mild level of fatigue, but 35.6% of them suffered severe *** had a significantly negative association with all domainsof QOL except social/family wellbeing. The participants whowere receiving chemotherapy, undergoing curative treatment andhaving inadequate social support were more likely to have poorerQOL in all five domains (after adjustment for age).cONcLUSION Although the majority of the participantsexperienced a mild level of fatigue, there was a substantial groupof breast cancer patients who perceived their fatigue as severe. Thefindings of this study showed that fatigue had a detrimental effecton the various aspects of the participants' QOL. Demographic andclinical characteristics of breast cancer patients who were at riskof getting poorer QOL were identified. The results of the studydemonstrate that we should enhance healthcare professionals'awareness of the importance of symptom assessment, andprovide them with information for planning effective symptom-management strategies among this study population.

How can Practitioners Support citizen Volunteers in Disaster Risk Reduction? Insight from “Good and Ready” in Aotearoa New Zealand

Global and national policy frameworks emphasize the importance of people's participation and volunteers' role in disaster risk reduction. While research has extensively focused on volunteers in disaster response and recovery, less attention has been paid on how organizations involved in disaster risk management can support volunteers in leading and coordinating communitybased disaster risk reduction. In 2019, the New Zealand Red cross piloted the Good and Ready initiative in Auckland,Aotearoa New Zealand, with the objective to empower local people in resilience building with a focus on volunteers and community participation. This research examined the positive and negative outcomes of Good and Ready and investigated volunteers' experiences in the disaster resilience initiative. It involved the codesign of a questionnaire-based survey using participatory methods with Good and Ready volunteers, the dissemination of the survey to gather volunteers' viewpoints, and a focus group discussion with participatory activities with Red cross volunteers. The findings highlight that a key challenge lies in finding a balance between a program that provides flexibility to address contextual issues and fosters communities' ownership, versus a prescriptive and standardized approach that leaves little room for creativity and self-initiative. It pinpoints that supporting volunteers with technical training is critical but that soft skills training such as coordinating, communicating,or facilitating activities at the local level are needed. It concludes that the sustainability of Good and Ready requires understanding and meeting volunteers' motivations and expectations and that enhancing partnerships with local emergency management agencies would strengthen the program.

Integrins and their potential roles in mammalian pregnancy

Integrins are a highly complex family of receptors that, when expressed on the surface of cells, can mediate reciprocal cell-to-cell and cell-to-extracellular matrix(EcM) interactions leading to assembly of integrin adhesion complexes(IAcs) that initiate many signaling functions both at the membrane and deeper within the cytoplasm to coordinate processes including cell adhesion, migration, proliferation, survival, differentiation, and metabolism. All metazoan organisms possess integrins, and it is generally agreed that integrins were associated with the evolution of multicellularity, being essential for the association of cells with their neighbors and surroundings, during embryonic development and many aspects of cellular and molecular biology. Integrins have important roles in many aspects of embryonic development, normal physiology, and disease processes with a multitude of functions discovered and elucidated for integrins that directly influence many areas of biology and medicine, including mammalian pregnancy, in particular implantation of the blastocyst to the uterine wall, subsequent placentation and conceptus(embryo/fetus and associated placental membranes) development. This review provides a succinct overview of integrin structure, ligand binding, and signaling followed with a concise overview of embryonic development, implantation, and early placentation in pigs, sheep, humans, and mice as an example for rodents. A brief timeline of the initial localization of integrin subunits to the uterine luminal epithelium(LE) and conceptus trophoblast is then presented, followed by sequential summaries of integrin expression and function during gestation in pigs, sheep, humans, and rodents. As appropriate for this journal, summaries of integrin expression and function during gestation in pigs and sheep are in depth, whereas summaries for humans and rodents are brief. The role of systemic therapy in borderline resectable and locally advanced pancreatic ductal adenocarcinoma

Pancreatic ductal adenocarcinoma(PDAc)remains a deadly disease,even in patients whose cancer is localized and *** resection provides the only option for cure,but long-term survival rates remain *** patients with borderline resectable(BR)disease who undergo upfront resection,many patients are either too unwell for subsequent adjuvant systemic therapy,develop recurrence soon after,or are found to have unresectable disease *** is increasing evidence for a neoadjuvant approach,using more conventional multi-agent chemotherapy regimens,which have demonstrated higher activity in the metastatic setting compared to single *** patients with locally advanced(LA)disease,which is unresectable by current definitions,there is mounting evidence that effective neoadjuvant systemic therapy is able to convert some patients’disease to a resectable state,offering the potential for long-term survival and *** we present a review of key trials focusing on prospective,randomized studies to provide high-level evidence supporting a neoadjuvant approach to both BR and LA ***,many knowledge gaps exist,such as the optimal neoadjuvant multi-agent chemotherapy regimen,the role of radiotherapy,and the safety and efficacy of adding immunotherapy to chemo/radiation *** challenges in determining the optimal approach to patients with BR or LA PDAc include not only overcoming the inherent difficulties in conducting complex,multidisciplinary,multicentre randomized trials in patients with a high-morbidity and mortality disease,but also trying to standardize disease definitions,treatment regimens,and outcome measures.

The convention on Biological Diversity seeks to conserve at least 30%of global land and water areas by 2030,which is a challenge but also an opportunity to better preserve biodiversity,including flowering plants(angiosperms).Herein,we compiled a large database on distributions of over 300,000 angiosperm species and the key functional traits of 67,024 *** this database,we constructed biodiversity-environment models to predict global patterns of taxonomic,phylogenetic,and functional diversity in terrestrial angiosperms and provide a comprehensive mapping of the three diversity *** further evaluated the current protection status of the biodiversity centers of these diversity *** results showed that geographical patterns of the three facets of plant diversity exhibited substantial spatial mismatches and nonoverlapping conservation *** centers of functional diversity,particularly of herbaceous species,were primarily distributed in temperate regions and under weaker protection compared with other biodiversity centers of taxonomic and phylogenetic *** global assessment of multifaceted biodiversity patterns and centers highlights the insufficiency and unbalanced conservation among the three diversity facets and the two growth forms(woody ***),thus providing directions for guiding the future conservation of global plant diversity.

Influence of the Nature of the Incoming Sludge on the Performance of a Vertical Flow Reed Beds in Dakar-Senegal

This work investigates the influence of the type sludge on drainage, plant development, purification performances and biosolids quality. Drainage properties were measured through the frequency of clogging, the percentage of leachate recovered and the dryness of accumulated sludge. Plant development was measured through the density, the height and the stem diameter. Purification performance was evaluated from the reduction rate. Biosolids quality was measured after 3 months of maturation. The results show that the clogging frequencies were 9.5%;0% and 3.7%;the volume of leachate recovered was 42.2%;20.4% and 24.7% and, the dryness was 33.4%;61.1% and 52.4% for FS-ST, FS-STT and SS respectively. Plants densities were about, with densities 197.1, 171.3 and 178.3 plants/m2 in beds fed respectively with FS-ST, FS-STT and SS. Despite the high removal rates, the concentrations of pollutants in the leachates are above the Senegalese standard NS 05-061 for discharge into the environment. The biosolids are all mature with c/N and NH4+/NO3?ratios lower than 12 and 1 respectively. The biosolids are also rich in organic and mineral elements. The concentrations of Ascaris eggs are higher than the WHO recommendations. These biosolids should be stored for additional time or composted.

Watson-Jones Anatomical Approach for Open Reduction and Internal Fixation of Proximal Femoral Fractures without Image Intensifier in a low-Resource Setting

Introduction: Standard procedures for surgical fixation of proximal femoral fractures (PFF) require an image intensifier which in developing countries remains a luxury. We hypothesized that, with a well-codified technique, the Watson Jones approach (WJA) without image intensifier nor traction table, can allow open reduction and internal fixation (ORIF) of PFF using Dynamic hip screw (DHS), with satisfactory outcome. Patients and methods: Forty one consecutive patients (mean age 59.5 ± 21.6 years, 61% males) who were followed in a Teaching Hospital for PFF treated by ORIF using the WJA and DHS from January 2016 to December 2020 were reassessed. The outcome measures were the quality of the reduction, the positioning of the implants, the tip-apex distance (TAD), the rate and delay of consolidation, the functional results using Postel Merle d’Aubigné (PMA) score, the rate of surgical site infection (SSI) and the overall mortality. logistic regression was used to determine factors associated with mechanical failure. Results: The mean follow-up period was 33.8 ± 15.0 months. Fracture reduction was good in 31 (75.6%) cases and acceptable in 8(19.5%) cases. Implant position was fair to good in 37 (90.2%) patients. The mean TAD was 26.1 ± 3.9 mm. Three patients developed SSI. consolidation was achieved in 38 (92.6%) patients. The functional results were good to excellent in 80.5% of patients. The overall mortality rate was 7.3%. There were an association between mechanical failure and osteoporosis (p = 0.04), fracture reduction (p = 0.003), and TAD (p = 0.025). In multivariate logistic regression, no independent factors were predictive of mechanical failure. conclusion: This study shows that ORIF using DHS for PFF via the Watson-Jones approach without an image intensifier can give satisfactory anatomical and functional outcomes in low-resource settings.
