Background:Traumatic pancreatic injuries are rare,and guidelines specifying management are controversial and difficult to apply in the acute clinical *** to sparse data on these injuries,we carried out a retrospective review to determine outcomes following surgical or non-surgical management of traumatic pancreatic *** hypothesize a higher morbidity and mortality rate in patients treated surgically when compared to patients treated ***:We performed a retrospective review of data from four trauma centers in New York from 1990–2014,comparing patients who had blunt traumatic pancreatic injuries who were managed operatively to those managed *** compared continuous variables using the Mann-Whitney U test and categorical variables using the chi-square and Fisher’s exact *** analysis was performed to determine the possible confounding factors associated with mortality in both treatment ***:Twenty nine patients were managed operatively and 32 *** was a significant difference between the operative and non-operative groups in median age(37.0 vs.16.2 years,P=0.016),grade of pancreatic injury(grade I;30.8 vs.85.2%,P value for all comparisons<0.0001),median injury severity score(ISS)(16.0 vs.4.0,P=0.002),blood transfusion(55.2 vs.15.6%,P=0.0012),other abdominal injuries(79.3 vs.38.7%,P=0.0014),pelvic fractures(17.2 vs.0.00%,P=0.020),intensive care unit(ICU)admission(86.2 vs.50.0%,P=0.003),median length of stay(LOS)(16.0 vs.4.0 days,P<0.0001),and mortality(27.6 vs.3.1%,P=0.010).Conclusions:Patients with traumatic pancreatic injuries treated operatively were more severely injured and suffered greater complications than those treated *** greater morbidity and mortality associated with these patients warrants further study to determine optimal triage strategies and which subset of patients is likely to benefit from surgery.

Does Chemo-Radiation Therapy Influence Outcomes in Unresectable Locally Advanced State IV Rectal Cancer?

Introduction: The median survival for patients with stage IV rectal cancer is approximately 20 months. Therapy should focus not only on improving survival but also on quality of life. The aim of our study was to determine if chemoradiation (C-RT) would improve palliation for metastatic unresectable locally advanced disease compared to patients receiving palliative chemotherapy alone (C) for stage IV cancer. Methods: Retrospective review of a prospectively maintained database at a single institution was carried out under IRB approval. From January 2004 to December 2008, 43 patients presenting with unresectable stage IV rectal cancer were identified with a median follow-up of 12 months. Patients with evidence of locally advanced disease or bulky disease received infusional 5-FU ± bevacizumab and 3D conformed mega voltage photon therapy (5400 cGy). Patients without evidence of bulky disease received either FOLFOX or FOLFIRI ± bevacizumab. Data on demographics, investigations, treatment, complications, metastasis, number of blood transfusions, days of hospitalization, and surgical intervention were analyzed using SPSS statistical software. p < 0.05 was considered statistically significant. Results: There were 25 and 18 patients in the C and C-RT groups respectively. There was no difference in mean age, sex or overall survival. Three patient (12%) in the C group developed hydronephrosis compared to 8 patients (44%) in the C-RT group (p < 0.05). Six patients (24%) developed bowel obstructions requiring an ostomy in the C group compared to 9 patient (50%) in the C-RT group (p = 0.07). In the C arm, 80% of patients required multiple hospitalizations for symptoms consistent with progression of disease compared to 61% of patients in the C-RT arm (p < 0.01). Conclusion: Chemoradiation in patients with locally advanced unresectable stage IV cancer has not been extensively investigated. Giant Cystic and Mediastinal Parathyroid Adenoma with Non-Uptake in Scintigraphy

Introduction: In 85% of patients, the cause of Primary Hyperparathyroidism (PHPT) is an adenoma. Parathyroid adenoma localization is usually simple. 96% of localizations prior to surgery are achieved with ultrasound and scintigraphy combination. Difficulties can appear. If the gland is not located in a cervical position and has no uptake in scintigraphy but there is high suspition of an adenoma causing the PHPT, some extra tests should be considered. Case Report: We present a case of giant cystic mediastinal adenoma, a quite rare condition, which was diagnosed as a thyroid colloid cyst by ultrasound, and was not localized by scintigraphy. We report the CT scan and the magnetic resonance imaging (MRI) images that show the tumour. It was surgically resected by a cervical approach. After the excision, the patient recovered normal levels of Calcium and Parathormone (PTH). Conclusion: We review literature about cystic and mediastinal adenomas and diagnostic methods when the standard ones do not give an accurate localization diagnosis.

Does Adult Intestinal Invagination Need Surgery? Report of a Case and Review of Literature

Benign intestinal tumors are the most frequent cause of small bowel intussusception in adults. This report presents a case of a 67-year-old male with abdominal pain, leukocytosis and C-reactive protein elevation. After ultrasonography and double contrast CT-scan consistent with ileal intussusception patient underwent laparoscopy, which confirmed diagnose. Intestinal resection and anastomosis were performed via minilaparotomy. Further histological study showed a fibroid inflammatory polyp or Vanek’s tumor as intussusception’s lead point.

Isolated Gross Hematuria or Associated to Acute Retention of Urine as a Sign of Urologic Malignant Pathology

The causes of IGH (Isolated gross hematuria), as the only symptom or associated with ARU (acute retention of urine) caused by clots in the urinary tract in patients who do not have a previous diagnosis of urological malignant pathology and to study the immediate mortality in patients with that sign are described. A descriptive observational study of the etiologies of patients shows the above mentioned symptomatology from the 1st of January to the 30th of June of 2008 in the Emergency Department which received follow-up treatment up to December 2010. It also describes the mortality before the definitive diagnosis and the days between receiving attention in the Emergencies Department and the diagnostic tests. One hundred and four cases with the criteria described above were evaluated. Of these, 20.0% turned out to have bladder, prostate or kidney cancer; the bladder being the most affected organ. Of all the pathology, benign and malignant, the most frequent one is the benign hypertrophy of the prostate (49.8% of the total). Of the total, 4.8% of the patients died in the following weeks before finishing the diagnostic study. Isolated gross hematuria is a clinical symptom closely related to urological malignant pathology and it does not have an insignificant mortality rate in a short period of time. For these reasons it is necessary to accelerate the diagnostic tests as soon as the patient presents the symptom.
