Current concepts on osteonecrosis of the femoral head

It is estimated that 20000 to 30000 new patients are diagnosed with osteonecrosis annually accounting for approximately 10% of the 250000 total hip arthroplasties done annually in the United States. Thelack of level 1 evidence in the literature makes it difficult to identify optimal treatment protocols to manage patients with pre-collapse avascular necrosis of the femoral head, and early intervention prior to collapse is critical to successful outcomes in joint preserving procedures. There have been a variety of traumatic and atraumatic factors that have been identified as risk factors for osteonecrosis, but the etiology and pathogenesis still remains unclear. Current osteonecrosis diagnosis is dependent upon plain anteroposterior and frog-leg lateral radiographs of the hip, followed by magnetic resonance imaging(MRI). Generally, the first radiographic changes seen by radiograph will be cystic and sclerotic changes in the femoral head. Although the diagnosis may be made by radiograph, plain radiographs are generally insufficient for early diagnosis, therefore MRI is considered the most accurate benchmark. Treatment options include pharmacologic agents such as bisphosphonates and statins, biophysical treatments, as well as joint-preserving and joint-replacing surgeries. the surgical treatment of osteonecrosis of the femoral head can be divided into two major branches: femoral head sparing procedures(FHSP) and femoral head replacement procedures(FHRP). In general, FHSP are indicated at pre-collapse stages with minimal symptoms whereas FHRP are preferred at post-collapse symptomatic stages. It is difficult to know whether any treatment modality changes the natural history of core decompression since the true natural history of core decompression has not been delineated.

Precise control of the evaporation of multiple droplets on patterned surfaces is crucial in many technological applications,such as anti‐icing,coating,and high‐throughput ***,the complex evaporation process of multiple droplets on well‐defined patterned surfaces is still poorly ***,we develop a digital twin system for real‐time monitoring of key processes on a droplet microarray(DMA),which is essential for parallelization and automation of the operations for cell ***,we investigate the evaporation of multiple nanoliter droplets under different conditions via experiments and numerical *** demonstrate that the evaporation rate is not only affected by the environmental humidity and temperature but is also strongly linked to the droplet distribution on the patterned surfaces,being significantly reduced when the droplets are densely ***,we propose a theoretical method to aid in the experimental detection of volumes and pH variation of evaporating droplets on patterned *** versatile and practical strategy allows us to achieve active maneuvering of the collective evaporation of droplets on a DMA,which provides essential implications for a wide range of applications including cell culture,heat management,microreactors,biochips,and so on.

BACKGROUND Secondary rectal linitis plastica(RLP)from prostatic adenocarcinoma is a rare and poorly understood form of metastatic spread,characterized by a desmoplastic response and concentric rectal wall infiltration with mucosal *** complicates endoscopic diagnosis and can mimic gastrointestinal *** case series underscores the critical role of magnetic resonance imaging(MRI)in identifying the distinct imaging features of RLP and highlights the importance of considering this condition in the differential diagnosis of patients with a history of prostate *** SUMMARY Three patients with secondary RLP due to prostatic adenocarcinoma presented with varied clinical *** first patient,a 76-year-old man with advanced prostate cancer,had rectal pain and *** showed diffuse prostatic invasion and significant rectal wall thickening with a characteristic"target sign"*** second,a 57-year-old asymptomatic man with elevated prostatespecific antigen levels and a history of prostate cancer exhibited rectoprostatic angle involvement and rectal wall thickening on MRI,with positron emission tomography/computed tomography PSMA confirming the prostatic origin of the metastatic *** third patient,an 80-year-old post-radical prostatectomy,presented with refractory *** revealed a neoplastic mass infiltrating the rectal *** all cases,MRI consistently showed stratified thickening,concentric signal changes,restricted diffusion,and contrast enhancement,which were essential for diagnosing secondary *** confirmed the prostatic origin of the neoplastic involvement in the *** Recognizing MRI findings of secondary RLP is essential for accurate diagnosis and management in prostate cancer patients.

The United Nations(UN)’s call for a decade of“ecosystem restoration”was prompted by the need to address the extensive impact of anthropogenic activities on natural *** ecosystem restoration is increasingly necessary due to increasing habitat degredation in deep waters(>200 m depth).At these depths,which are far beyond those accessible by divers,only established and emerging robotic platforms such as remotely operated vehicles(ROVs),autonomous underwater vehicles(AUVs),landers,and crawlers can operate through manipulators and multiparametric sensor arrays(e.g.,optoacoustic imaging,omics,and environmental probes).The use of advanced technologies for deep-sea ecosystem restoration can provide:①high-resolution three-dimensional(3D)imaging and acoustic mapping of substrates and key taxa,②physical manipulation of substrates and key taxa,③real-time supervision of remote operations and long-term ecological monitoring,and④the potential to work ***,we describe how robotic platforms with in situ manipulation capabilities and payloads of innovative sensors could autonomously conduct active restoration and monitoring across large spatial *** expect that these devices will be particularly useful in deep-sea habitats,such as①reef-building cold-water corals,②soft-bottom bamboo corals,and③soft-bottom fishery resources that have already been damaged by offshore industries(i.e.,fishing and oil/gas).

Old-age inflammatory bowel disease onset:A different problem?

Inflammatory bowel disease （IBD） in patients aged 60 accounts for 10%-15% of cases of the disease. Diganostic methods are the same as for other age groups. Care has to be taken to distinguish an IBD colitis from other forms of colitis that can mimick clinically, endoscopically and even histologically the IBD entity. The clinical pattern in ulcerative colitis （UC） is proctitis and left-sided UC, while granulomatous colitis with an inflammatory pattern is more common in Crohn’s disease （CD）. The treatment options are those used in younger patients, but a series of considerations related to potential pharmacological interactions and side effects of the drugs must be taken into account. The safety profile of conventional immunomodulators and biological therapy is acceptable but more data are required on the safety of use of these drugs in the elderly population. Biological therapy has risen question on the possibility of increased side effects, however this needs to be confirmed. Adherence to performing all the test prior to biologic treatment administration is very important. The overall response to treatment is similar in the different patient age groups but elderly patients have fewer recurrences. The number of hospitalizations in patients 65 years is greater than in younger group, accounting for 25% of all admissions for IBD. Mortality is similar in UC and slightly higher in CD, but significantly increased in hospitalized patients. Failure of medicaltreatment continues to be the most common indication for surgery in patients aged 60 years. Age is not considered a contraindication for performing restorative proctocolectomy with an ileal pouch-anal anastomosis. However, incontinence evaluation should be taken into account an individualized options should be considered

Degraded water quality from nutrient pollution,physical,biological,and other chemical factors contributes to the development and persistence of many harmful algal blooms(HAB s).The complex dynamics of the HAB s is a challenge to marine ecosystems for the toxic effects *** consequences include fish,bird,and mammal mortality,respiratory or digestive tract problems,memory loss,seizures,lesions and skin irritation in many *** review is intended to briefly summarize the recent reported information on harmful marine toxin deleterious effects over the primary organisms of the food web,namely algae,zooplankton and *** focus is made on oxidative stress status of cells and *** though in situ field research is less controlled than laboratory studies,in which the organisms are directly exposed to the toxins under consideration,both types of approaches are required to fully understand such a complex *** top of that,the contribution of the increasing water temperatures in the sea,as a consequence of the global climate change,will be addressed as a topic for further studies,to evaluate the effect on regulating algal growth,species composition,trophic structure,metabolic stress and function of aquatic ecosystems.

A fast, reliable and precise method for the determination of Ba, Zr, Th, U, La, Ce, Eu, Hf and Gd in leaching solutions containing up to 1 mol/L FeCl3 by inductively coupled plasma-atomic emission spectrometry（ICP-AES） was ***-liquid extraction with diethyl ether was applied for selective and quantitative extraction of the iron with a decontamination factor of *** study has enabled the determination of rare earth elements, U, Th Ba, and Zr, from the solution with high iron content with detection limits in the range of 1-24 ng/ml and a precision of 0.9%-4.6 % relative standard *** developed method was simple and was adopted for implementation in a glove box for handling radioactive materials.

BACKGROUND Endoscopic resection of non-invasive lesions is now the standard of care for lesions in the GI ***,resection techniques require extensive training,are not available in all endoscopy centers and are prone to *** mucosal ablation(EMA)is a combination of resection and ablation techniques and it may offer an alternative in the management of such *** SUMMARY In this case series we report the successful treatment of three flat colonic polyps using the EMA *** lesions were treatment naïve and 1 was a recurrence after an endoscopic mucosal *** sizes ranged from 2 to 4 *** three polyps were ablated successfully with no immediate or delayed *** recurrence rate at 1 year of follow up was 0%.CONCLUSION Based on this initial experience,we conclude that EMA is a safe and effective technique for the treatment of non-invasive colonic polyps when endoscopic resection techniques are not available.

Fe-dependent cellular alterations of oxidative balance in aquatic *** be ferroptosis involved?

The purpose of this review is to briefly summarize the central role of iron(Fe)in terms of cellular alterations of the oxidative/protective balance with special emphasis on its possible involvement in ferroptosis-dependent disruption in aquatic *** ferroptotic cells or tissues,the intracellular Fe level increases;meanwhile the treatment with Fe chelators limits *** algae can assimilate Fe from the environment through several mechanisms,and aquatic animals incorporate dissolved Fe and Fe bound to both inorganic particles and organic *** central role of lipid peroxidation mediating ferroptosis was demonstrated in some algae where both low and high Fe concentrations could induce oxidative stress and programmed cell *** animals have high levels of polyunsaturated fatty acids and numerous studies have analyzed Fe effects on the lipidic fraction which could be related to *** ferroptosis reaction can be regulated through the antioxidant defense system,in combination with the protein degradation structure,metabolism,and gene *** depletion of nonenzymatic antioxidants like reduced glutathione(GSH)in animals,and the reduction of both GSH and ascorbate in photosynthetic organisms,are characteristic features of ***,ferroptosis can be prevented if Fe chelators,certain antioxidants,and specifically regulating genes are ***,the global scenario for the Fe role as a toxic component in biological systems seems to be even more complicated than it was previously *** more research on this subject is needed to improve the life span and survival of aquatic organisms after exposure to natural and anthropogenic adverse conditions.

Importance of sperm morphology during sperm transport and fertilization in mammals

After natural or artificial insemination, the spermatozoon starts a journey from the site of deposition to the place of fertilization. However, only a small subset of the spermatozoa deposited achieves their goal： to reach and fertilize the egg. Factors involved in controlling sperm transport and fertilization include the female reproductive tract environment, cell-cell interactions, gene expression, and phenotypic sperm traits. Some of the significant determinants of fertilization are known （i.e., motility or DNA status）, but many sperm traits are still indecipherable. One example is the influence of sperm dimensions and shape upon transport within the female genital tract towards the oocyte. Biophysical associations between sperm size and motility may influence the progression of spermatozoa through the female reproductive tract, but uncertainties remain concerning how sperm morphology influences the fertilization process, and whether only the sperm dimensions per se are involved. Moreover, such explanations do not allow the possibility that the female tract is capable of distinguishing fertile spermatozoa on the basis of their morphology, as seems to be the case with biochemical, molecular, and genetic properties. This review focuses on the influence of sperm size and shape in evolution and their putative role in sperm transport and selection within the uterus and the ability to fertilize the oocyte.
