Can patients determine the level of their dysphagia?

AIM To determine if patients can localise dysphagia level determined endoscopically or radiologically and association of gender, age, level and *** Retrospective review of consecutive patients presenting to dysphagia hotline between March 2004 and March 2015 was carried out. Demographics, clinical history and investigation findings were recorded including patient perception of obstruction level (pharyngeal, mid sternal or low sternal) was documented and the actual level of obstruction found on endoscopic or radiological examination (if any) was noted. All patients with evidence of obstruction including oesophagealcarcinoma, peptic stricture, Schatzki ring, oesophageal pouch and cricopharyngeal hypertrophy were included in the study who had given a perceived level of dysphagia. The upper GI endoscopy reports (barium study where upper GI endoscopy was not performed) were reviewed to confirm the distance of obstructing lesion from central incisors. A previously described anatomical classification of oesophagus was used to define the level of obstruction to be upper, middle or lower oesophagus and this was compared with patient perceived *** Three thousand six hundred and sixty-eight patients were included, 42.0% of who were female, mean age 70.7 ± 12.8 years old. Of those with obstructing lesions, 726 gave a perceived level of dysphagia: 37.2% had oesophageal cancer, 36.0% peptic stricture, 13.1% pharyngeal pouches, 10.3% Schatzki rings and 3.3% achalasia. Twenty-seven point five percent of patients reported pharyngeal level (upper) dysphagia, 36.9% mid sternal dysphagia and 25.9% lower sternal dysphagia (9.5% reported multiple levels). The level of obstructing lesion seen on diagnostic testing was upper (17.2%), mid (19.4%) or lower (62.9%) or combined (0.3%). Systemic risk research is gaining traction across diverse disciplinary research communities, but has as yet not been strongly linked to traditional, well-established risk analysis research. This is due in part to the fact that systemic risk research focuses on the connection of elements within a system, while risk analysis research focuses more on individual risk to single elements. We therefore investigate how current systemic risk research can be related to traditional risk analysis approaches from a conceptual as well as an empirical point of view. Based on Sklar's Theorem, which provides a one-to-one relationship between multivariate distributions and copulas, we suggest a reframing of the concept of copulas based on a network perspective. This provides a promising way forward for integrating individual risk(in the form of probability distributions) and systemic risk(in the form of copulasdescribing the dependencies among such distributions)across research domains. Copulas can link continuous node states, characterizing individual risks, with a gradual dependency of the coupling strength between nodes on their states, characterizing systemic risk. When copulas are used for describing such refined coupling between nodes,they can provide a more accurate quantification of a system's network structure. This enables more realistic systemic risk assessments, and is especially useful when extreme events(that occur at low probabilities, but have high impacts) affect a system's nodes. In this way, copulas can be informative in measuring and quantifying changes in systemic risk and therefore be helpful in its management. We discuss the advantages and limitations of copulas for integrative risk analyses from the perspectives of modeling, measurement, and management.

The Process and Benefits of Developing Prototype Climate Services——Examples in China

Changes in climate pose major challenges to society, and so decision-makers need actionable climate information to inform their planning and policies to make society more resilient to climatic changes. Climate services are being developed to provide such actionable climate information. The successful development and use of climate services benefits greatly from close engagement between developers, providers, and users of the services. The Climate Science for Service Partnership China(CSSP China) is a China–UK collaboration fostering closer engagement between climate scientists, providers of climate services, and users of climate services. We describe the process within CSSP China of co-developing climate services through trials with users to revise and improve a prototype. Examples are provided covering various scientific capabilities, user needs, and parts of China. The development process is yielding many benefits, such as increasing the engagement between providers and users, making users more aware of how climate information can be of use in their decision-making, giving the climate service providers a better understanding of the users’ requirements for climate information, and shaping future scientific research and development. In addition to the benefits, we also document some challenges that have emerged, along with ways of alleviating them. We have two key recommendations from our experiences: make the time and space for effective engagement between the users and developers of any climate service;bring the needs of the users in to the design and delivery of the climate service as early as possible and throughout the development cycle.

Free and open access to the landsat archive has enabled the implementation of national and global terrestrial monitoring ***,we summarize a project characterizing the change history of Canada’s forested ecosystems with a time series of data representing *** the Composite2Change approach,we applied spectral trend analysis to annual best-available-pixel(BAP)surface reflectance image composites produced from landsat TM and ETM+imagery.A total of 73,544 images were used to produce 29 annual image composites,generating∼400 TB of interim data products and resulting in∼25 TB of annual gap-free reflectance composites and change *** average,10%of pixels in the annual BAP composites were missing data,with 86%of pixels having data gaps in two consecutive years or *** detection overall accuracy was 89%.Change attribution overall accuracy was 92%,with higher accuracy for standreplacing wildfire and *** were assigned to the correct year with an accuracy of 89%.Outcomes of this project provide baseline information and nationally consistent data source to quantify and characterize changes in forested *** methods applied and lessons learned build confidence in the products generated and empower others to develop or refine similar satellite-based monitoring projects.
