The CaO-P_(2)O_(5)-SiO_(2)-MgO system presents several compounds used as biomaterials such as hydroxyapatite(HA),tricalcium phosphate(TCP)and TCP with magnesium substituting partial calcium(TCMP).Theβ-TCMP phase with whitlockite structure has interesting biological features and mechanical properties,meeting the requirements of a bioactive material for bone *** this work,the production of Mg-doped TCP,β-TCMP,has been investigated by crystallization from a glass composed of 52.75 wt%3CaO·P_(2)O_(5),30 wt%SiO_(2)and 17.25 wt%MgO(i.e.,31.7 mol%CaO,10.6 mol%P_(2)O_(5),26.6 mol%MgO and 31.1 mol%SiO_(2))using heat treatments between 775℃and 1100℃for up to 8 *** devitrification process of the glass has been accompanied by differential scanning calorimetry(DSC),high-resolution X-ray diffraction(HRXRD),relative density and bending strength *** characterization by HRXRD and DSC revealed the occurrence of whitlockite soon after the bulk glass preparation,a transient non-cataloged silicate between 800℃and 1100℃,and the formation of diopside in samples treated at 1100℃as crystalline *** overall crystalline fraction varied from 26%to 70%depending on the heat ***,contraction of the a-axis lattice parameter and expansion of the c-axis lattice parameter of the whitlockite structure have been observed during the heat treatments,which were attributed to theβ-TCMP formation with the partial substitution of Ca^(2+)by Mg^(2+).Relative densities near 99%and 97%for the glass and glass-ceramics respectively indicated a discrete reduction as a function of the devitrification *** strengths of 70 MPa and 120 MPa were determined for the glass and glass-ceramic material crystallized at 975℃for 4 h,respectively.

Arginine-glycine-aspartic acid （RGD） dendrimer-based nanopatterns on poly（L- lactic acid） were used as bioactive substrates to evaluate the impact of the RGD local surface density on the chondrogenic induction of adult human mesenchymal stem cells. During chondrogenic commitment, active extracellular matrix （ECM） remodeling takes place, playing an instructive role in the differentiation process. Although three-dimensional environments such as pellet or micromass cultures are commonly used for in vitro chondrogenic differentiation, these cultures are rather limited with respect to their ability to interrogate cells in celI-ECM interactions. In the present study, the nanopatterns of the tunable RGD surface density were obtained as a function of the initial dendrimer concentration. The local RGD surface density was quantified through probability contour plots for the minimum interparticle distance, constructed from the corresponding atomic force microscopy images, and correlated with the cell adhesion and differentiation response. The results revealed that the local RGD surface density at the nanoscale acts as a regulator of chondrogenic commitment, and that intermediate adhesiveness of cells to the substrates favors mesenchymal cell condensation and early chondrogenic differentiation.

Appropriateness of the study of iron deficiency anemia prior to referral for small bowel evaluation at a tertiary center

AIM To evaluate the adequacy of the study of iron deficiency anemia(IDA)in real life practice prior to referral to a gastroenterology department for small bowel *** All consecutive patients referred to a gastroenterology department for small bowel investigation due to iron deficiency anemia,between January 2013 and December2015 were *** patients referred from general practitioners or directly from different hospital departments were *** clinical information regarding prior anemia workup was retrospectively collected from medical *** appropriate pre-referral study was considered the execution of esophagogastroduodenoscopy(EGD)with Helicobacter pylori(***)investigation,colonoscopy with quality standards(recent,total and with adequate preparation)and celiac disease(CD)screening(through serologic testing and/or histopathological investigation).RESULTS A total of 77 patients(58.4% female,mean age 67.1±16.7 years)were ***(53.2%)patients were referred from general practitioners,41.6% from other hospital specialties and 5.2% directly from the emergency *** mean pre-referral hemoglobin concentration was 8.8±2.0 g/d L and the majority of anemias had microcytic(71.4%)and hypochromic(72.7%)characteristics.77.9%of patients presented with an incomplete pre-referral study:EGD in 97.4%,with *** investigation in 58.3%,colonoscopy with quality criteria in 63.6%,and CD screening in 24.7%.Patients with an appropriate study at the time of referral were younger(48.7±17.7 vs 72.3±12.3 years,P<0.001).Small bowel evaluation was ultimately undertaken in 72.7%of patients,with a more frequent evaluation in patients with a quality colonoscopy at referral(78.6%vs 23.8%);P<0.001(OR=11.7,95%CI:3.6-38.6).The most common diagnosis regarded as the likely cause of IDA was small bowel angioectasia(18.2%)but additional causes were also found in the upper and lower gastrointestinal tracts of near 20% of *** bowel stud

BACKGROUND：Liver ischemia reperfusion（IR）injury triggers a systemic inflammatory response and is the main cause of organ dysfunction and adverse postoperative outcomes after liver ***（PTX）and hypertonic saline solution（HTS）have been identified to have beneficial effects against IR *** study aimed to investigate if the addition of PTX to HTS is superior to HTS alone for the prevention of liver IR ***： Male Wistar rats were allocated into three groups. Control rats underwent 60 minutes of partial liver ischemia, HTS rats were treated with 0.4 mL/kg of intravenous 7.5% NaCl 15 minutes before reperfusion, and HPTX group were treated with 7.5% NaC1 plus 25 mg/kg of PTX 15 minutes before reperfusion. Samples were collected after reperfusion for determination of ALT, AST, TNF-α, IL-6, IL-10, mitochondrial respiration, lipid peroxidation, pulmonary permeability and myeloperoxidase. RESULTS： HPTX significantly decreased TNF-α 30 minutes after reperfusion. HPTX and HTS significantly decreased ALT,AST, IL-6, mitochondrial dysfunction and pulmonary myelo- peroxidase 4 hours after reperfusion. Compared with HTS only, HPTX significantly decreased hepatic oxidative stress 4 hours after reperfusion and pulmonary permeability 4 and 12 hours after reperfusion. CONCLUSION： This study showed that PTX added the beneficial effects of HTS on liver IR injury through decreases of hepatic oxidative stress and pulmonary permeability.

A sensing system in the near infrared region has been developed for ammonia sensing based on the wavelength modulation spectroscopy （WMS） principle. The WMS is a rather sensitive technique for detecting atomic/molecular species, presenting the advantage that it can be used in the near-infrared region by using the optical telecommunications technology. In this technique, the laser wavelength and intensity were modulated by applying a sine wave signal through the injection current, which allowed the shift of the detection bandwidth to higher frequencies where laser intensity noise was typically lower. Two multi-pass cells based on free space light propagation with 160 cm and 16 cm of optical path length were used, allowing the redundancy operation and technology validation. This system used a diode laser with an emission wavelength at 1512.21nm, where NH3 has a strong absorption line. The control of the NH3 gas sensing system, as well as acquisition, processing and data presentation was performed.

Endoscopic balloon dilation of crohn's disease stricturessafety,efficacy and clinical impact

AIM To evaluate the incidence of anastomotic strictures after intestinal resection in Crohn's disease(CD), demonstrate long-term efficacy and safety of endoscopic balloon dilation(EBD) in CD strictures and its impact on the diagnosis of subclinical postoperative endoscopic recurrence. METHODS Retrospective single tertiary center study based on prospectively collected data between 2010 and 2015including anastomotic and non-anastomotic strictures. RESULTS29% of 162 CD patients included developed an anastomotic stricture. 43 patients with anastomotic strictures and 37 with non-anastomotic strictures underwent EBD; technical success was 97.7% and 100%, respectively, however, 63% and 41% needed repeat dilation during the 4.4-year follow-up. Longer periods between surgery and index colonoscopy and higher lactoferrin levels were associated with the presence of stricture after surgery. Calprotectin levels > 83.35 μg/g and current or past history of smoking were associated with a shorter time until need for dilation(HR = 3.877, 95%CI: 1.480-10.152 and HR = 3.041, 95%CI: 1.213-7.627). Anastomotic strictures had a greater need for repeat dilation(63% vs 41%, P = 0.047). No differences were found between asymptomatic and symptomatic cohorts. Disease recurrence diagnosis was only possible after EBD in a third of patients. CONCLUSION EBD is an effective and safe alternative to surgery, with a good short and long-term outcome, postponing or even avoiding further surgery. EBD may allow to diagnose disease recurrence in patients with no clinical signs/biomarkers of disease activity.

Impact of Drug-Drug Interaction between CDK4/6 Inhibitors and Proton Pump Inhibitors on Survival Outcomes in the Treatment of Metastatic Breast Cancer—Real World Data from a Portuguese Center

Introduction: Proton pump inhibitors (PPi) are widely prescribed, including in patients undergoing treatment for advanced breast cancer (ABC). Due to the pharmacokinetic characteristics of the CDK4/6 inhibitor (Ci) palbociclib a drug interaction with PPi was hypothesized. It was shown in a retrospective study that this association was an independent predictive factor for worse progression-free survival (PFS). Objective: To verify the impact of concomitant administration of PPi with Ci on overall survival (OS) and PFS. Material and Methods: This is a retrospective cohort study of patients treated with Ci for HR+HER2-ABC in the period from Feb/2017 to Aug/2020. SPSS software was used for data processing. Univariate analysis was done by the Kaplan-Meier method and log-rank test, and multivariate analysis by COX regression. P-value < 0.05 was considered significant. Results: 80 patients were included. The median age at diagnosis of ABC was 56 years (25 - 75). Treatment with Ci was 1st line for ABC in 68.8%. Choice of Ci was palbociclib in 73.8% (n = 59) and ribociclib in 26.3% (n = 21). The hormone partner was a nonsteroidal aromatase inhibitor in 45.0%, and fulvestrant in 55.0% of cases. 37.5% of patients were on PPi, and 70.0% of them were during the entire treatment (23.3% omeprazole, 73.4% pantoprazole, 3.3% others). Patients taking concomitant PPi and Ci had lower OS (OS-3 years 42.6% vs. 63.4%, p = 0.254) and PFS (PFS med 15 m. vs. 21 m., p = 0.733), although with no statistically significant difference. Discussion: In the sample, there was a numerical difference, without the statistical significance in the use of PPi in the survival of patients under Ci. This difference could be more evident with a longer follow-up and a larger sample size. This study intends to alert to the growing importance of checking for drug interactions. Large-area polycrystalline diamond(PCD)coatings are important for fields such as thermal management,optical windows,tribological moving mechanical assemblies,harsh chemical environments,biological sensors,*** plasma chemical vapor deposition(MPCVD)is a standard technique to grow high-quality PCD films over large area due to the absence of contact between the reactive species and the filament or the chamber ***,the existence of temperature gradients during growth may compromise the desired uniformity of the final diamond *** the present work,a thick PCD coating was deposited on a 100-mm silicon substrate inside a 915-MHz reactor;the temperature gradient resulted in a non-uniform diamond *** attempt was made to relate the local temperature variation during deposition and the different properties of the final *** was found that there was large instability inside the system,in terms of substrate temperature(as high asΔT=212℃),that resulted in a large dispersion of the diamond coating’s final properties:residual stress(-15.8 GPa to+6.2 GPa),surface morphology(octahedral pyramids with(111)planes to cubo-octahedrals with(100)flat top surfaces),thickness(190μm to 245μm),columnar growth of diamond(with appearance of variety of nanostructures),nucleation side hardness(17 GPa to 48 GPa),quality(Raman peak FWHM varying from 5.1 cm^(-1) to 12.4 cm^(-1) with occasional splitting).This random variation in properties over large-area PCD coating may hamper reproducible diamond growth for any meaningful technological application.
