嗜酸性粒细胞性食管炎是由过敏原驱动、免疫介导的慢性疾病。近年来，大家逐步认识到该病是成人和儿童发生吞咽困难及上消化道症状的重要原因。时该病的遗传及环境因素、相关的炎症背景、食管改变的远期并发症等方面的资料逐步丰富。本文主要总结了成人嗜酸性粒细胞性食管炎的最新进展，讨论通过微侵入的方法替代反复活检来判断疾病的活动程度的尝试，对采用现有方法如阻抗和腔内成像等技术检测黏膜完整性及食管功能改变的研究进行总结，探讨质子泵抑制剂时嗜酸性粒细胞性炎症的疗效差异。一临床治疗研究仍然支持应用局部糖皮质激素和食物剔除疗法作为该病的一线治疗方法。本文还将讨论优化食物剔除疗法以缩短饮食限制时间并降低饮食限制的程度。展望未来，目前对嗜酸性粒细胞性食管炎发病机制的理解已有可能支持进一步研发特异性的生物治疗。

A series of numerical sinmlations about a small scale （aspect ratio： 63.2） flexible pipe undergoing forced harmonious oscillation and vortex-induced vibration （VIV） have been taken into account. The wake hydrodynamics and pipe deformation were accomplished by ANSYS MFX solution strat- egy designed for fluid-structure interaction （FSI） problem with well-performed LES model. The configuration of structured mesh, multi-domain design, different mesh stiffness admeasured by User Fortran ensured that the numerical task was competent to deal with large deformation related to this case. The introduction of instantaneous amplitude definition and modeless component decom- position method （chen and Kim, 2008） was helpful to reveal much more information from modal analysis. Most results from numerical simulation are generally consistent with those from model test （Choi and Hong, 2000） via the comparison between them. As supplementary to model test, visualization of the vortex wake was also provided. It has been proved that the forced oscillation doesn＇t only excite a complicated dumbbell-like wake pattern around the outer thimble, but also results in inner flow inside the PVC pipe. The velocity of the inner flow increases with the frequency of forced oscillation.

A peer-to-peer (P2P) multimedia conferencing service is operating that users share their resources to each other on the Internet. It can solve the problem in the centralized conferencing architecture, such as the centralized loading, single point error, and expensive infrastructure. However, P2P networks have the problem that a peer has a difference between the physical location and logical location in the overlay network. In the viewpoint of P2P networks, the nearest conference resource may be far away geographically. The P2P-session initiation protocol (P2P-SIP) multimedia conference is to construct an application-based logical multicast network efficiently according to physical network information. Thus, this paper proposes a real-time streaming relay mechanism for P2P conferences on hierarchical overlay networks. The real-time streaming relay mechanism can improve the transportation efficiency of conferencing stream exchange well based on the application-layer multicast (ALM) structure and the hierarchical overlay networks.

SREBP-1 is a novel mediator of TGFβ1 signaling in mesangial cells

Glomerular matrix accumulation is a hallmark of diabetic nephropathy. Recent studies showed that overexpression of the transcription factor SREBP-1 induces glomerutoscterosis. TGFI31 is a key profibrotic mediator of glomerutoscterosis, but whether SREBP-1 regulates its effects is unknown. In kidney mesangial cells and in vivo, TGFβI activates SREBP-1. This requires SCAPo SIP0 and PI3K/Akt signaling, but is independent of Smad3. Activation of the TGFβ1-responsive reporter plasmid p3TP-lux requires SREBP-la, but not SREBP-lc, binding to an E-box adjacent to a Smad-binding element. SREBP-la overexpression atone activates p3TP-tux. Smad3 is required for SREBP-la transcriptional activation and TGFβ induces association between the two transcription factors. SREBP-la K333 acetylation by the acetyltransferase CBP is required for Smad3 association and SREBP-1 transcriptional activity, and is also required for Smad3 transcriptional activity. Thus, both Smad3 and SREBP-la activation cooperatively regulate TGFβ transcriptional responses. SREBP-1 inhibition provides a novel therapeutic strategy for diabetic kidney disease.

This paper provides a convergent validity test of two types of multinomial choice questions vis-a-vis a dichotomous choice question by formally testing whether these stated preference elicitation question formats provide comparable welfare estimates. In particular, a dichotomous choice question, a traditional multinomial choice question, and a modified multinomial choice question suggested by Carson and Groves （2007） were applied in split samples to assess the influence of the informational and incentive properties on the respondents＇ annual willingness to accept compensation for adopting costly conservation practices in agriculture that benefit the environment. Our findings suggest that the two multinomial choice question formats elicit a similar mean willingness to accept distributions, but they are both different from a standarddichotomous choice question. Further, the willingness to accept distributions derived from the multinomial choice question formats are more dispersed than those from the dichotomous choice question.

It was shown by flow cytometry analysis that crystalized preparation of Cordyceps sinensis (Cs-Cr) caused significant elevation of the number of T helper cells and Lyt-1 /Lyt-2 (T helper to T suppressor cell) ratio both in peripheral blood and the treated mice spleen. The spleen weight, phagocyte counts and phagocytic activity were also elevated in the treated group. In addition, Cs-Cr could protect T helper cells from the immnnosuppressive effects of prednisolone acetate and cyclophosphamide. These results further substantiate the fact that Cs-Cr is an immunoregulator/biological response modifier of cellular immunity and may be potentially useful in handling immunodeficient or immunosuppressed patients.

Carbon monoxide （CO） can act as an anti-inflammatory effector in mouse models of lung injury and disease, through the downregulation of pro-inflammatory cytokines production, though the underlying mechanisms remain unclear. The nucleotide-binding oligomerization domain-, leucine-rich region-, and pyrin domain-containing-3 （NLRP3） inflammasome is a protein complex that regulates the maturation and secretion of pro-inflammatory cytokines, including interleukin-1β （IL-1β）. In this report, we show that the CO-releasing molecule （CORM-2） can stimulate the expression of pyrin, a negative regulator of the NLRP3 inflammasome. CORM-2 increased the transcription of pyrin in the human leukemic cell line （THP-1） in the absence and presence of lipopolysaccharide （LPS）. In THP-1 cells, CORM-2 treatment dose-dependently reduced the activation of caspase-1 and the secretion of IL-lp, and increased the levels of IL-IO, in response to LPS and adenosine 5＇-triphosphate （ATP）, an NLRP3 inflammasome activation model. Genetic interference of IL-10 by small interfering RNA （siRNA） reduced the effectiveness of CORM-2 in inhibiting IL-1β production and in inducing pyrin expression. Genetic interference of pyrin by siRNA increased IL-lp production in response to LPS and ATP, and reversed CORM-2-dependent inhibition of caspase-1 activation. CO inhalation （250 ppm） in vivo increased the expression of pyrin and IL-10 in lung and spleen, and decreased the levels of IL-1β induced by LPS. Consistent with the induction of pyrin and IL-10, and the downregulation of lung IL-1β production, CO provided protection in a model of acute lung injury induced by intranasal LPS administration. These results provide a novel mechanism underlying the anti-inflammatory effects of CO, involving the IL-10-dependent upregulation of pyrin expression.
