Efficacy of acupuncture on cancer pain:A systematic review and meta-analysis

Background:Pain associated with cancer is one of the greatest causes of reduced quality of life in patients.Acupuncture is one of the treatments used to address this issue,with the great advantage of having little or no side effects,especially when compared with pharmacological pain-killers.objective:The aim of this systematic review and meta-analysis was to evaluate the current evidence regarding the efficacy of acupuncture for cancer pain.Search strategy:Six electronic databases(PubMed,EBSCo,Cochrane Library,Scielo,b-on and Scopus)were searched for relevant articles about pain relief in cancer patients from their beginning until 2022using MeSH terms such as“acupuncture,”“electroacupuncture,”“ear acupuncture,”“acupuncture analgesia,”“oncological pain,”and“cancer pain.”Inclusion criteria:Studies included were randomized controlled trials(RCTs)where acupuncture was compared with no treatment,placebo acupuncture or usual care.Data extraction and analysis:Three independent reviewers participated in data extraction and evaluation of risk of bias,and a meta-analysis was conducted.The primary outcome was pain intensity,measured with the visual analog scale,numeric rating scale,or brief pain inventory.Secondary outcomes also assessed were quality of life,functionality,xerostomia,pain interference,and analgesic consumption.Results were expressed as standardized mean difference(SMD)with 95%confidence interval(CI).Results:Sixteen RCTs with a total of 1124 participants were included in the meta-analysis,with the majority of the studies presenting a low or unclear risk of bias.Acupuncture was more effective in reducing pain than no treatment(SMD=–0.90,95%CI[–1.68,–0.12]),sham acupuncture(SMD=–1.10,95%CI[–1.59,–0.61])or usual care(SMD=–1.16,95%CI[–1.38,–0.93]).Conclusion:The results of this study suggest that acupuncture may be an effective intervention to reduce pain associated with cancer.Despite some limitations due to the low quality and small sample size of some included studies,as well as the different types and stages

Sorption has been proposed as a promising approach towards a sustainable water decontamination and recently to the recycling of rare earth elements(REE).Although living seaweed have been shown to be capable of removing REE from contaminated solutions,no studies have yet compared the effects of REE competition on sorption onto different groups of seaweed.These effects were analysed in the present study by exposing six living seaweeds(Ulva lactuca,Ulva intestinalis,Fucus spiralis,Fucus vesiculosus,Gracilaria sp.and osmundea pinnatifida)(3 g/L,fresh weight) to mono-element and multi-element solutions of Y,La,Ce,Pr,Nd,Eu,Gd,Tb and Dy(1 μmol/L).Results show a preference towards light REE in mono-element solutions,which is reduced when in competition with other REE.The competitive effect is less pronounced in heavy REE indicating that these are still able to bind to the macroalgae despite the presence of competing ions.Contrary to water content(%),seaweed specific surface area(cm^(2)/g) shows to be an important factor in the sorption of REE,since larger surface area is associated with higher removal and larger competitive effect.

Minimally invasive surgery started spreading worldwide in 1987,when the first laparoscopic cholecystectomy was performed.Meanwhile,improvement of endoscopic equipment and instruments allowed gastroenterologists to attempt more aggressive endoluminal interventions,even beyond the wall barrier.The first transgastric peri-toneoscopy,in 2004,brought to light the concept of natural orifice transluminal endoscopic surgery(NoTES).The idea of incisionless surgery is attractive and has become a new goal for both surgeons and other people interested in this field of investigation.The authors present a review of all developments concerning NoTES,including animal studies and human experience.

Can Persistent Children Hyperglycemia Be Induced by Causes other Than Pancreas Failure?

In this work, children’s persistent hyperglycemia has been revised using the available literature to support the proposed reasoning. Based on this study, we have shown that the human glycemic management system must be seen as coupled and integrated by four subsystems, namely, production system, consumption system, distribution system, control system, and also it should be seen as coupled to external noxious factors/stressors, if not we show that the glycemic homeostasis analysis might be defective and might induce, in many cases, a misdiagnosis of the causes of the persistent hyperglycemia under consideration. Also, in this work, some considerations were presented to show that anomalies in the cerebral glycemic control through the glucose sensor neurons might be a possible cause/origin of some of the glycemic abnormalities and dysfunctions (however, not only the known related hypoglycemia but also hyperglycemia) that occur in childhood. Finally, it is shown that persistent novel external noxious factors of modernity or noxious factors already known, but amplified by modernity, such as persistent stress, media induced fears, and phobias, environmental pollution, and electromagnetic pollution, can and should also be considered as possible precursors for the development of anomalies in the juvenile homeostatic glycemic system that might well be, if intense and persistent, the driver of the worldwide observed T1DM epidemic events.

Acute diverticulitis in younger patients:Any rationale for a different approach?

AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHoDS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,2007.Diverticulitis was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group B>50 years(n=126) .Mean patient follow-up was 15 mo. RESULTS:The median age was 60 years.A significantly higher proportion of patients in group B presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .Recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency surgery.In multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CoNCLUSIoN:Based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.

Arbutus unedo L.(strawberry tree,Ericaceae)is a woody species with a circum-Mediterranean distribu-tion.It has considerable ecological relevance in southern European forests due to its resilience to abiotic and biotic stressors.Its edible red berries are used in the production of traditional products,including an expensive spirit.Several compounds extracted from the species have bioactive prop-erties used by the cosmetic and pharmaceutical industries.The strawberry tree has gone from a neglected species to become a highly valuable crop with large cultivated areas in southern European and North African countries.Due to an increasing demand from farmers for plants with improved features,researchers have been trying to improve this spe-cies through conventional and biotechnological tools,focus-ing mainly on population analysis using molecular markers,in vitro cloning,tolerance to biotic and abiotic stresses,and intraspecific crosses to obtain genotypes with new char-acteristics.The objective of this review is to gather and update information about the species and make it available to researchers and stakeholders.Future research areas that are considered a priority for this species are highlighted.

The broke of a tailings dam from Vale S.A. destroyed a gigantic area killing 270 persons in Brumadinho/Brazil. organizing activities after a mass disaster is a complex process that requires the involvement of many people and resources in the laboratory. To DNA identification of the victims, a daunting work had to be made by DNA laboratory staff in a collaborative effort with many partners. Efforts to implement good practice guidelines in Disaster Victim Identification (DVI) have revealed several important aspects that need to change in the forensic DNA laboratory. This article highlights the challenges of implementing DVI best practice guidelines in resource-poor settings, but with professionals from different sectors engaged in the same goal of briefly identifying victims and helping families and society.

A Vulnerability Assessment Framework for Cultural Heritage Sites:The Case of the Roman Ruins of Tróia

This article contributes to developing an indicator-based vulnerability assessment framework for cultural heritage sites.It provides a vulnerability index for heritage sites potentially exposed to multiple hazards,including sudden-onset and slow-onset hazards,while considering climate change influences.Through determining particular criteria and indicators,the Cultural Heritage Vulnerability Index incorporates structural and non-structural factors of the heritage site and its local and national settings.The assessment procedure was applied to the case of the Roman Ruins of Tróia in Portugal.The findings highlight those areas of sensitivity(e.g.,the existing deterioration patterns and types of foundation)and coping and adaptive capacities(e.g.,institutional setting and response plan)that significantly contribute to the level of vulnerability and risk.The results of vulnerability assessment will further enable determining priorities and developing risk mitigation and preparedness measures,in particular reducing structural sensitivity and promoting coping capacities.

Intimate partner violence against women. Does violence decrease after the entry of the alleged offender into the criminal justice system?

Intimate partner violence(IPV)is simultaneously assumed as a serious crime and a major public health issue,having recurrences as one of its main characteristics and,consequently,re-entries of some alleged offenders in the criminal justice system(CJS).The main goal of this study is to assess if in cases of female victims of IPV,violence decreases after the first entry of the alleged offender in the CJS.A retrospective study was performed based on the analysis of police reports of alleged cases of IPV during a 4-year period.The final sample(n=1488)was divided into two groups according to the number of entries in the CJS(single or multiple)followed by a comparative approach.Results suggest that violence decreases after the first entry of alleged offenders in the CJS.Re-entries were found in only 15.5%of the cases but they were accountable for 3.3 times more crimes on average.Besides,victims of recidivism presented more injuries and required more medical care.Thus,a small group of alleged offenders seems to be more violent and accountable for most of the IPV crimes registered in the CJS suggesting that regardless of legal sanctions aiming to deter violence,these measures may not be enough for a certain group of offenders.This study sustains the need for a predictive model to quantify the risk of repeated IPV cases within the Portuguese population.

Intraoperative cell salvage with autologous transfusion in liver transplantation

Liver transplant(LT) is the primary treatment for patients with end-stage liver disease. About 25000 LTs are performed annually in the world. The potential for intraoperative bleeding is quite variable. However, massive bleeding is common and requires blood transfusion. Allogeneic blood transfusion has an immunosuppressive effect and an impact on recipient survival, in addition to the risk of transmission of viral infections and transfusion errors, among others.Techniques to prevent excessive bleeding or to use autologous blood have been proposed to minimize the negative effects of allogeneic blood transfusion.Intraoperative reinfusion of autologous blood is possible through previous selfdonation or blood collected during the operation. However, LT does not normally allow autologous transfusion by prior self-donation. Hence, using autologous blood collected intraoperatively is the most feasible option. The use of intraoperative blood salvage autotransfusion(IBSA) minimizes the perioperative use of allogeneic blood, preventing negative transfusion effects without negatively impacting other clinical outcomes. The use of IBSA in patients with cancer is still a matter of debate due to the theoretical risk of reinfusion of tumor cells. However, studies have demonstrated the safety of IBSA in several surgical procedures, including LT for hepatocellular carcinoma. Considering the literature available to date, we can state that IBSA should be routinely used in LT, both in patients with cancer and in patients with benign diseases.
