Background:Helicobacter pylori(H.pylori)infection is an infectious disease with a prevalence rate of up to 50%worldwide.It can cause indigestion,gastritis,peptic ulcer,and gastric cancer.H.pylori eradication treatment can effectively control disease progression and reduce the risk of the above conditions.However,the escalating trend of antibiotic resistance presents a global challenge for H.pylori eradication.We aim to provide guidance on pharmacological treatment of H.pylori infection.Methods:This clinical practice guideline is developed following the World Health Organization’s recommended process,adopting Grading of Recommendations Assessment,Development and Evaluation in assessing evidence quality,and utilizing Evidence to Decision framework to formulate clinical recommendations,minimizing bias and increasing transparency of the clinical practice guideline development process.We used the Reporting Items for practice Guidelines in HealThcare(RIGHT)statement and The Appraisal of Guidelines for Research and Evaluation Ⅱ(AGREE Ⅱ)as reporting and conduct guides to ensure the guideline’s completeness and transparency.Results:Though decreasing in developed countries,the prevalence of H.pylori remains high in developing countries,causing a major public health burden.This clinical practice guideline contains 12 recommendations concerning pharmacological treatment for H.pylori eradication.Among them,it is worth highlighting that bismuth preparations are inexpensive,safe,and effective,consequently making bismuth quadruple therapy a preferred choice for initial and rescue treatment.In empirical treatment,high-dose dual therapy is equally effective compared with bismuth quadruple therapy.Conclusions:The 12 recommendations in this clinical practice guideline are formed with consideration for stakeholders’values and preferences,resource use,feasibility,and acceptability.Recommendations are generalizable to resource limited settings with similar antibiotic resistance pattern as China,and lower middle-income countries facing

Research and evidence are critical for the formulation of policies and practices in support of health care.In the past two decades,the Chinese Clinical Epidemiology Network has been promoting evidence-based policy making in China.Evidence-based policy has become a major part of the government’s approach to policy making.The current article addresses the translation of evidence into health policies based on the expansion of evidence-based medicine in China.It also discusses the opportunities and challenges for certain evidence to be considered in policy making and practice in the future.

The aim of this paper is to investigate the relationship between hepatitis B virus(HBV)DNA levels during the course and the progression to cirrhosis with chronic hepatitis B.A total of 239 chronic hepatitis B patients conﬁrmed by liver biopsy between 2001 and 2007 were followed up for a median of 28 months.Compared with the patients without cirrhosis,the patients progressed to cirrhosis were older and with higher HBV-DNA levels at end point.However,there was no signiﬁcant difference in cirrhosis progression between different HBV-DNA groups at baseline(P=0.531).Kaplan-Meier analysis showed higher HBV-DNA level at endpoint had increasing risk of cirrhosis(P=0.019).The results of Cox model indicated that HBV-DNA levels at endpoint,stage ofﬁbrosis,negative hepatitis B e antigen,andγ-glutamyl transpepti-dase at baseline were independent risk factors of cirrhosis.The relative risk ratios were 1.898,1.918,8.976,and 1.006,respectively.Progression to cirrhosis in chronic hepatitis B patients is correlated with HBV-DNA levels during follow-up.

Comprehensive treatment of acute-on-chronic liver failure in a patient with hepatitis B： a case report

The clinical data of a patient with acute-on-chronic liver failure were analyzed retrospectively. The patient has suffered from hepatitis B for 30 years. His liver function deteriorated, yielding Child-Pugh grade C and reaching a model for end-stage liver disease score of 33 points within a short period; this condition was complicated with highly active variceal bleeding and coagulation system failure （PT 〉 100 s）. The patient also presented hepatocellular carcinoma. Comprehensive treatments included effective inhibition of hepatitis B virus replication and intensive care support. Piggyback orthotopic liver transplantation was performed as the final treatment. The patient recovered uneventfully and was discharged after surgery.

This study aims to demonstrate that blocking the receptor-interacting protein2(Rip2)expression can decrease inflammatory cytokine production by macrophage and protect mice from endotoxin lethality.Murine Rip2 small interfering RNA(siRNA)plasmids were constructed and transfected into macrophage and Rip2 expression was detected with reverse transcription-polymerase chain reaction(RT-PCR)and western blot.Cell proliferation was assayed withMTT.TNF-a concentration was assayed with ELISA and high-mobility group box 1 protein(HMGB1)level with semi-quantitative western blot after lipopolysaccharide(LPS)stimulation.LPS challenge was given after the plasmids were injected into mice and the survival rate was calculated.Rip2 siRNA plasmid could block the mRNA and protein expression of Rip2 and promote cell proliferation.Blocking Rip2 could attenuate LPS-induced TNF-a and HMGB1 production.The HMGB1 expression in the liver decreased to(40.21±11.03)pg/g,and serum TNF-a level decreased to(300.43±59.26)ng/L(P<0.05).The survival rate of mice from endotoxemia was also improved(P<0.05).The results demonstrate that Rip2 siRNA plasmid can block the expression of Rip2,decrease the production of TNF-a and HMGB1 and protect mice from fatal endotoxemia.

为了推动超声振动致裂煤岩体技术的发展,提高矿山煤层气增产及硬岩掘进的效率,回顾了超声波技术的发展历程,综述了超声振动在致裂煤岩体方面的应用现状与研究进展,明确了超声振动在煤岩致裂领域存在的关键阻碍性技术难题及发展趋势。(1)总结了传统技术在矿山煤层气增产和硬岩掘进领域存在的技术问题,介绍了超声波技术的独特优势及应用领域,阐述了超声波发生器和超声波换能器的发展阶段及相应性能;(2)归结了超声振动煤层气促解增渗装置及流程,分析了超声振动煤层气促解增产现状,阐明了超声空化效应、机械振动效应、热效应作用下煤层气降压、加速、位能及动能增加进而促进其解吸增渗的机制;(3)归纳了超声振动破岩装置及流程,阐述了超声振动破碎硬岩的进展,揭示了超声振动产生的机械振动效应与热效应作用下疲劳损伤、微裂纹亚临界扩展、矿物颗粒不均匀热膨胀或矿物相变、岩石物理力学特性劣化导致硬岩破碎的内在机理;(4)针对超声波发生器、换能器及超声振动在矿山致裂煤岩体中的局限度,对未来超声振动致裂煤岩体在现场高效应用的研究重点提出了4点建议:高性能防爆型超声波发生器和换能器的研发,超声振动促解增渗机理的深入分析及管线技术框架设计,多功能及多场耦合室内超声振动致裂煤岩体设备研制与试验,井下抗干扰超声振动设备的研发应用。

Endodontic diseases are a kind of chronic infectious oral disease. Common endodontic treatment concepts are based on the removal of inflamed or necrotic pulp tissue and the replacement by gutta-percha. However, it is very essential for endodontic treatment to debride the root canal system and prevent the root canal system from bacterial reinfection after root canal therapy(RCT). Recent research, encompassing bacterial etiology and advanced imaging techniques, contributes to our understanding of the root canal system’s anatomy intricacies and the technique sensitivity of RCT. Success in RCT hinges on factors like patients, infection severity, root canal anatomy, and treatment techniques. Therefore, improving disease management is a key issue to combat endodontic diseases and cure periapical lesions. The clinical difficulty assessment system of RCT is established based on patient conditions, tooth conditions, root canal configuration, and root canal needing retreatment, and emphasizes pre-treatment risk assessment for optimal outcomes. The findings suggest that the presence of risk factors may correlate with the challenge of achieving the high standard required for RCT. These insights contribute not only to improve education but also aid practitioners in treatment planning and referral decision-making within the field of endodontics.

Effects of Fuzheng Huayu Decoction on plasma proteome in cirrhosis:preliminary experimental study with rats

The aim of this paper is to study the effects of Fuzheng Huayu Decoction on the plasma proteome in cirrhotic rats.Twenty-six male Sprague-Dawley(SD)rats were randomly divided into three groups:cirrhotic model group(n510),treated with CCl_(4)(CCl_(4)/olive oil:v/v51:1);Fuzheng Huayu Decoction intervention group(n510),treated with CCl_(4)+Fuzheng Huayu Decoction;and normal control group(n56),treated with olive oil only.After 8 weeks,blood samples were collected from the inferior vena cava to undergo bi-dimensional electrophoresis(2DE)and analysis by PDQuest 7.3 software.Differential protein spots were cut,enzyme hydrolysis was conducted,and peptide fragments extracted from the mixture underwent mass spectrometry(MS)with MALDI-TOF-TOF-MS.The liver fibrogenesis was assessed using a digital image analysis instrument of Masson’s trichrome stained sections.The fibrosis area of the Fuzheng Huayu Decoction was(8.9±3.7)%,significantly smaller than that of the cirrhotic model group[(12.4±4.7)%,P<0.05].Ten markedly changed protein spots were identified by MALDI-TOFTOF-MS.Eight of the 10 proteins,including plasma glutathione peroxidase,plasma glutathione peroxidase precursor,prealbumin,haptoglobin,apolipoprotein A-IV precursor,complement C4,inter-alpha-inhibitor H4 heavy chain,and serine/threonine-protein kinase microtubuleaffinity regulating kinase 1(MARK1)were expressed very lowly in the cirrhotic model group while they were expressed highly in the Fuzheng Huayu Decoction group.The expression of liver regeneration-related protein LRRG03 and vimentin increased in the cirrhotic model group,and reduced in the FuzhengHuayu Decoction group.Some proteins related to oxidative stress,cell proliferation and transformation have changed in the plasma of cirrhosis induced by CCl_(4).Fuzheng Huayu Decoction promotes protein synthesis and plays an anti-fibrotic role by antioxidation and accommodation of cell proliferation and transformation.

Joint Participant Selection and Learning Optimization for Federated Learning of Multiple Models in Edge Cloud

To overcome the limitations of long latency and privacy concerns from cloud computing,edge computing along with distributed machine learning such as federated learning(FL),has gained much attention and popularity in academia and industry.Most existing work on FL over the edge mainly focuses on optimizing the training of one shared global model in edge systems.However,with the increasing applications of FL in edge systems,there could be multiple FL models from different applications concurrently being trained in the shared edge cloud.Such concurrent training of these FL models can lead to edge resource competition(for both computing and network resources),and further affect the FL training performance of each other.Therefore,in this paper,considering a multi-model FL scenario,we formulate a joint participant selection and learning optimization problem in a shared edge cloud.This joint optimization aims to determine FL participants and the learning schedule for each FL model such that the total training cost of all FL models in the edge cloud is minimized.We propose a multi-stage optimization framework by decoupling the original problem into two or three subproblems that can be solved respectively and iteratively.Extensive evaluation has been conducted with realworld FL datasets and models.The results have shown that our proposed algorithms can reduce the total cost efficiently compared with prior algorithms.
