BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

The Yangtze River is the mother river of *** promote the aquatic ecosystem protection of the great river,the Project of Yangtze Fisheries Resources and Environment Investigation(2017-2021)supported by the Ministry of Agriculture and Rural Affairs,*** carried out by 24 institutes and universities that located in the Yangtze River basin surveys the status of(1)fish species composition and spatial distribution,(2)current fish abundance,(3)endangered fishes,(4)Yangtze finless porpoise,(5)aquatic eco-environments,(6)water-level fluctuation areas,(7)capture fisheries and recreational fisheries of the Yangtze River mainstream and 10 of its main tributaries,including Yalong River,Heng River,Min River(including Dadu River),Chishui River,Tuo River,Jialing River,Wu River,Han River,Dongting Lake and Poyang *** results showed that there were 443 fish species(belonging to 163 genus,37 families,and 18 orders)before 2017,but only 323 fish species(including 15 new recorded exotic species)were recorded in the project of *** them,Cyprinus carpio,Carassius auratus,Hypophthalmichthys molitrix,Pelteobagrus fulvidraco,Coilia brachygnathus,Silurus asotus,Saurogobio dabryi,Ctenopharyngodon idellus,Pelteobagrus nitidus,Hemiculter leucisculus,Siniperca chuatsi,Coreius heterodon,Culter alburnus,Parabramis pekinensis,and Aristichthys nobilis were the dominant species across the whole Yangtze River *** is estimated that there were 886 million individuals weighing 124.8 million kg,merely equivalent to 27.3%of the resources in 1950s,30.9%of the resources in 1960s,or 58.7%of the resources in *** the new list of protected fishes that recorded in the Yangtze River system,only 15 of 29 were collected in this project of *** gladius has been affirmed to be Extinct by International Union for Conservation of Nature(IUCN).The wild individuals of Tenualosa reevesii and Luciobrama macrocephalus have disappeared for many years and maybe have been extinct

　　Diaryl ether moiety is found in a pool of naturally occurring and medicinally important compounds.[1] As a consequent, considerable efforts have been devoted to the assembly of this framework.[2] Recently, we have developed a microwave heating version of the synthesis of diaryl ethers as well as aryl sulfides. Under our conditions, even the extremely electron-poor 4-nitrophenol works well and its reaction with 1-halo-4-nitrobenzenes produces 4-(nitrophenoxy)-benzonitriles in satisfactory yield. The scope of the present protocol has been expanded to hydroxylated six-membered heterocycles as well as 2-pyrimidinethiol with mildly activated aryl halides, affording heteroaryl ethers and respectively sulfides. The advantages of the present method include the wide substrate scope, no use of any metal catalysts, the ease of product isolation and high yields.……

干燥综合征是一种慢性自身免疫性疾病,以免疫细胞浸润,唾液腺和泪腺的进行性损伤为主要特征,除了影响泪腺和唾液腺,导致眼睛干涩和口干症状外,还会累及人体心、肺、肾和中枢神经等多个系统,严重影响人体身心健康。虽然近年来西医在该病的诊断和治疗方面有了广泛和深入的研究,但目前还没有针对潜在病因的治疗方法。中医药在整体观的基础上,能够辨证施治,随证遣方,具有多靶点、多层次、多途径的治疗特点。目前随着中医药现代化研究的进展,发现其在治疗本病方面有着独特的优势,在调节免疫、降低炎症反应及上调水通道蛋白等方面有着姣好的作用,既能缓解临床症状,延缓病情,又能够减轻激素等药物的不良反应。通过回顾近年来关于中医药治疗干燥综合征(SS)的相关文献,对其发病机制及中医药作用的相关机理做了进一步的总结,本文拟从调节免疫、上调水通道蛋白及抗氧化应激等方面就中医药治疗本病的相关机制做一研究归纳,以期为相关实验研究及临床提供更多的理论依据。

Benign prostatic hyperplasia (BPH) is highly prevalent among older men, impacting on their quality of life, sexual function, and genitourinary health, and has become an important global burden of disease. Transurethral plasmakinetic resection of prostate (TUPKP) is one of the foremost surgical procedures for the treatment of BPH. It has become well established in clinical practice with good efficacy and safety. In 2018, we issued the guideline “2018 Standard Edition”. However much new direct evidence has now emerged and this may change some of previous recommendations. The time is ripe to develop new evidence-based guidelines, so we formed a working group of clinical experts and methodologists. The steering group members posed 31 questions relevant to the management of TUPKP for BPH covering the following areas: questions relevant to the perioperative period (preoperative, intraoperative, and postoperative) of TUPKP in the treatment of BPH, postoperative complications and the level of surgeons’ surgical skill. We searched the literature for direct evidence on the management of TUPKP for BPH, and assessed its certainty generated recommendations using the grade criteria by the European Association of Urology. Recommendations were either strong or weak, or in the form of an ungraded consensus-based statement. Finally, we issued 36 statements. Among them, 23 carried strong recommendations, and 13 carried weak recommendations for the stated procedure. They covered questions relevant to the aforementioned three areas. The preoperative period for TUPKP in the treatment of BPH included indications and contraindications for TUPKP, precautions for preoperative preparation in patients with renal impairment and urinary tract infection due to urinary retention, and preoperative prophylactic use of antibiotics. 在甲醇溶液中,水杨醛和牛磺酸经缩合反应合成了水杨醛缩牛磺酸Schiff碱(1);1用硼氢化钠还原制得对应的其还原Schiff碱(2)。1和2的结构经1H NMR,IR和元素分析表征。X-射线单晶衍射分析表明,2属单斜晶系,空间群P21/c,晶胞参数a=10.787 4(12),b=8.989 7(10),c=11.098 3(13),β=103.180(2)°,V=1 047.9(2)3,Z=4,Dc=1.466 ***-3,μ=0.303 mm-1,F(000)=488,R1=0.032 0,wR2=0.080 9。用荧光光谱初步探讨了1和2的荧光性质,结果表明其荧光的λmax分别位于500 nm和430 nm。1的整个发射峰在480 nm～560 nm,是一种绿光的单体。

目的研究天南星科石柑子[Pothos chinensis（Raf.）Merr.]全草的脂溶性化学成分。方法采用硅胶、Sephadex LH-20、半制备HPLC等色谱技术和重结晶等方法进行分离纯化,根据波谱学方法与化合物的理化性质及文献对照鉴定化合物的结构。结果分离并鉴定了17个化合物,分别为和厚朴酚（1）、扁柏脂素（2）、24-丙基胆甾-7,22-二烯-3-醇（3）、24-丙基胆甾-7-烯-3-醇（4）、β-谷甾醇（5）、胡萝卜苷（6）、正三十一烷（7）、棕榈酸（8）、油酸（9）、亚油酸（10）、α-亚麻酸（11）、13-苯基十三烷酸（12）、13-（3＇,4＇-亚甲二氧基苯基）-十三烷酸（13）、单硬脂酸甘油酯（14）、1-棕榈酸单甘油酯（15）、苯甲酸（16）和香草酸（17）。结论化合物1~16均为首次从该植物中分离得到。
