A cost-effectiveness assessment of the operational parameters of central HVAC systems during pandemics

The present study develops a cost-effectiveness assessment model to analyze the performance of major operational parameters of central HVAC systems in terms of airborne transmission risk,energy consumption,and medical and social cost.A typical multi-zone building model with a central HVAC system is built numerically,and the effect of outdoor air(OA)ratio(from 30%to 100%)and filtration level(MERV 13,MERV 16,and HEPA)are assessed under the conditions of five climate zones in China.Compared with the baseline case with 30%OA and MERV 13 filtration,the airborne transmission risk in zones without infector is negligibly reduced with the increase in OA ratio and the upgrade of filtration level,owing to their slight modification on the equivalent ventilation rate of virus-free air.However,depending on climate zone,a 10%increase in OA ratio results in 12.5%-78.6%and 0.1%-8.6%increase in heating and cooling energy consumption,respectively,while an upgrade of filtration level to MERV 16 and HEPA results in an increase of 0.08%-0.2%and 1.4%-2.6%,respectively.Overall,when compared to the use of 100%OA ratio and HEPA filtration,the application of 30%or 40%OA ratio and MERV 13 filtration would save annually an energy and facility related cost of$29.4 billion in China,though giving an increase of approximately$0.1 billion on medical and social cost from the increased number of confirmed cases.This study provides basic method and information for the formulation of cost-effective operational strategies of HVAC systems coping with the airborne transmission,especially in resource-limited regions.

Airborne transmission during short-term events: Direct route over indirect route

Numerous short-term exposure events in public spaces were reported during the COVID-19 pandemic,especially during the spread of Delta and Omicron.However,the currently used exposure risk assessment models and mitigation measures are mostly based on the assumption of steady-state and complete-mixing conditions.The present study investigates the dynamics of airborne transmission in short-term events when a steady state is not reached before the end of the events.Large-eddy simulation(LES)is performed to predict the airborne transmission in short-term events,and three representative physical distances between two occupants are examined.Both time-averaged and phase-averaged exposure indices are used to evaluate the exposure risk.The results present that the exposure index in the short-term events constantly varies over time,especially within the first 1/ACH(air changes per hour)hour of exposure between occupants in close proximity,posing high uncertainty to the spatial and temporal evolutions of the risk of cross-infection.The decoupling analysis of the direct and indirect airborne transmission routes indicates that the direct airborne transmission is the predominated route in short-term events.It suggests also that the general dilution ventilation has a relatively limited efficiency in mitigating the risk of direct airborne transmission,but determines largely the occurrence time of the indirect one.Given the randomness,discreteness,localization,and high-risk characteristics of direct airborne transmission,a localized method that has a direct interference on the respiratory flows would be better than dilution ventilation for short-term events,in terms of both efficiency and cost.

This study aims to evaluate airborne transmission risk in university towns during the COVID-19 pandemic based on surveys of indoor environmental quality(IEQ).Both on-site measurements and questionnaire surveys were carried out in public buildings in university towns in Changsha,China.Air temperature,relative humidity,and CO 2 concentration in one library,ten classrooms,eight canteens,seven restaurants,and sixteen malls were mea-sured.2220 valid questionnaires concerning occupants’sensation on thermal environment,air movement,and indoor air quality were collected.A 3-level evaluation method of airborne transmission risk that is dependent on building type and indoor CO 2 concentration was developed.Excessive CO 2 concentration is found in library(1045 ppm),classrooms(1151 ppm),restaurants(1242 ppm),and malls(1057 ppm).The percentage time of“high risk”accounts for 18-100%in these buildings.The results reveal a serious problem:numerous public buildings in China and probably other resource limited countries are not basically prepared and equipped to cope with airborne transmission.This fact should be taken into account when developing guidelines and formu-lating mitigation measures.Real-time monitoring and displaying IEQ and thus the transmission risk level should be an important way to be widely implemented in public buildings.

Previous studies have shown that PRDX5 and Nrf2 are antioxidant proteins related to abnormal reactive oxidative species(ROS).PRDX5 and Nrf2 play a critical role in the progression of inﬂammations and tumors.The combination of PRDX5 and Nrf2 was examined by Co-immunoprecipitation,western blotting and Immunohistochemistry.H2O2 was applied to affect the production of ROS and induced multi-resistant protein 1(MRP1)expression in NSCLC cells.The zebraﬁsh models mainly investigated the synergistic effects of PRDX5 and Nrf2 on lung cancer drug resistance under oxidative stress.We showed that PRDX5 and Nrf2 form a complex and signiﬁcantly increase the NSCLC tissues compared to adjacent tissues.The oxidative stress improved the combination of PRDX5 and Nrf2.We demonstrated that the synergy between PRDX5 and Nrf2 is positively related to the proliferation and drug resistance of NSCLC cells in the zebraﬁsh models.In conclusion,our data indicated that PRDX5 could bind to Nrf2 and has a synergistic effect with Nrf2.Meanwhile,in the zebraﬁsh models,PRDX5 and Nrf2 have signiﬁcant regulatory impacts on lung cancer progression and drug resistance activities under oxidative stress.

目的探究局部放疗联合化疗或免疫治疗寡转移性食管鳞癌(ESCC)患者的预后及其影响因素。方法回顾性分析2018-03-01-2022-10-31就诊于南京医科大学附属肿瘤医院的转移性ESCC患者,共纳入96例新发寡转移患者作为研究对象。男78例,女18例;年龄47~85岁,中位年龄65岁;在食管肿瘤病灶无进展的情况下,所有患者均接受了局部放疗联合化疗或免疫治疗。96例患者共有106处寡转移病灶,其中64例患者出现器官转移,42例患者发生非区域淋巴结转移,有20例患者既出现器官转移也有非区域淋巴结转移。59例(61.5%)接受图像引导放射治疗(IGRT),37例(38.5%)接受立体定向放射治疗(SBRT)。评估局部控制率(LCR)、无进展生存期(PFS)和总生存期(OS),以及与PFS和OS相关的预后因素。结果中位随访时间为31.1(18.2~44.0)个月。1和2年LCR分别为94.3%和86.8%。中位PFS是21.1(95%CI为15.3~27.0)个月。1和2年的PFS率分别为65.0%和43.4%。中位OS为26.9(95%CI为20.0~33.9)个月。1和2年的OS率分别为80.4%和56.1%。单因素分析中ECOG评分和病灶长径是OS的影响因素,多因素结果显示ECOG评分、病灶长径、放疗方式和转移位置是OS的独立预后因素,均P<0.05。最常见的不良反应是化疗引起的白细胞降低,无治疗相关的死亡病例。结论局部放疗联合全身治疗寡转移ESCC是一种安全且有效的治疗方案。建议临床中将寡转移与多发转移ESCC患者进行分层治疗。

老年腰椎间盘突出症患者心理负荷和自我效能对术后康复影响的分析

目的研究老年腰间盘突出症(LDH)患者心理负荷和自我效能对其术后康复的影响。方法选取2022年5月—2023年5月在淮安市第一人民医院接受经皮内镜下腰椎间盘切除术治疗的108例老年LDH患者为对象,分别采用腰椎间盘突出患者遵医行为调查问卷、主观负荷评价技术量表(SWAT)、慢性疼痛自我效能感量表(CPSS)和Oswestry功能障碍指数(ODI)评价患者的遵医行为、心理负荷、自我效能和腰椎功能,比较不同遵医行为患者的SWAT评分、CPSS评分和ODI指数;采用二元Logistic回归分析方法,分析SWAT评分、CPSS评分与术后康复遵医行为的关系;采用相关性分析法,分析SWAT评分、CPSS评分与术后腰椎功能改善的关系。结果本研究问卷回收有效率为97.22%(105/108),105例老年LDH患者的遵医行为总评分为(55.04±8.96)分,其中遵医组(遵医行为总评分>54分)54例,非遵医组(遵医行为总评分≤54分)51例。遵医组SWAT评分、术后ODI指数均低于非遵医组(t=19.906、4.869,P<0.05),而CPSS疼痛管理、躯体功能、症状应对及总评分均高于非遵医组(t=5.552、4.650、3.864、6.020,均P<0.05)。2组患者文化程度、配偶情况比较,差异均有统计学意义(P<0.05)。二元Logistic多因素回归分析结果显示,高SWAT评分(OR=5.140,95%CI 2.082~12.687)是老年LDH患者术后非遵医行为的危险因素(P<0.05);高CPSS评分(OR=3.888,95%CI 1.690~8.944)、高文化程度(OR=3.068,95%CI 1.474~6.386)、有配偶(OR=2.257,95%CI 1.037~4.914)均为患者术后非遵医行为的保护因素(P<0.05)。相关性分析结果显示,SWAT评分与术后ODI指数呈正相关(r=0.735,P<0.05),CPSS评分与术后ODI指数呈负相关(r=-0.681,P<0.05)。结论老年LDH患者心理负荷越重,可能出现其遵医行为越差,术后腰椎功能改善越差;但自我效能越好、文化程度越高或有配偶的患者遵医行为相对更佳,其中自我效能越好的老年LHD患者术后腰椎功能也更好,值得临床重视。

Single crystal-single crystal bonding of langasite exhibited high strength of 23.28 MPa

Langasite(LGS,La_(3)Ga_(5)SiO_(14))is a promising material in high-temperature piezoelectric devices due to its excellent thermal stability,piezoelectricity,and electrical property.A major challenge in the development of LGS-based devices is to form high-strength bonding of the brittle LGS.Here,we report that the single crystal-single crystal(dual-SC)bonding of LGS is realized through thermal activation under a low com-pression of 50 kPa for the first time.A record bonding strength of 23.28 MPa is achieved within the dual-SC bonded LGS/LGS structure(with a high bonding ratio exceeding 93%),which is 5 times higher than that of the recent reported LGS/LGS bonding structure(in which a relatively fragile amorphous interface layer is formed between the two LGS samples).The smooth and void-free dual-SC bonding interface of LGS is verified via the cross-sectional transmission electron microscopy(TEM)observations.

The effects of attached flexible tail length on the flow structure of an oscillating cylinder

Particle Imaging Velocimetry(PIV) is utilized to investigate the flow structures of an oscillating cylinder attached to a flexible tail.At the same oscillation frequency and amplitude,the mean streamwise velocity along the wake central-line and the mean vertical velocity around the trailing edge of the flexible tail can be greatly increased with the tail length.Meanwhile,the longer the flexible tail is,the larger its deformation is.In order to study the influence of flexible tail length on the wake pattern of the experimental model,the relationships between the swirling strength Λ ci of vortex structure near the tail end and the velocity of tail trailing edge have been revealed.Moreover,the convection tracks and the Λci of vortex cores for different flexible tails are discussed.

Alternative splicing and cancer: a systematic review

The abnormal regulation of alternative splicing is usually accompanied by the occurrence and development of tumors,which would produce multiple different isoforms and diversify protein expression.The aim of the present study was to conduct a systematic review in order to describe the regulatory mechanisms of alternative splicing,as well as its functions in tumor cells,from proliferation and apoptosis to invasion and metastasis,and from angiogenesis to metabolism.The abnormal splicing events contributed to tumor progression as oncogenic drivers and/or bystander factors.The alterations in splicing factors detected in tumors and other mis-splicing events(i.e.,long non-coding and circular RNAs)in tumorigenesis were also included.The findings of recent therapeutic approaches targeting splicing catalysis and splicing regulatory proteins to modulate pathogenically spliced events(including tumor-specific neo-antigens for cancer immunotherapy)were introduced.The emerging RNA-based strategies for the treatment of cancer with abnormally alternative splicing isoforms were also discussed.However,further studies are still required to address the association between alternative splicing and cancer in more detail.
