Personalized treatment of well-differentiated gastric neuroendocrine tumors based on clinicopathological classification and grading:A multicenter retrospective study

Background:The incidence of well-differentiated gastric neuroendocrine tumors(G-NET)is increasing annually,and while they have a good prognosis and low mortality rate,their high recurrence rate makes treatment options controversial.This study aims to determine the relationship between individualized treatment plans and the recurrence of G-NET.Methods:We performed a multicenter,retrospective study of 94 patients with highly differentiated G-NET and treated at Peking Union Medical College Hospital,Yantai Yuhuangding Hospital,and Beijing Zhong-Neng-Jian Hospital from November 2015 to September 2023.Risk factors for recurrence of G-NETs were investigated using chi-squared test and multifactorial logistic regression analysis.Results:After a median follow-up of 49 months,the overall recurrence rate among the 94 G-NET patients was 14%(13/94).The recurrence rates of endoscopic mucosal resection(EMR),endoscopic submucosal dissection(ESD),somatostatin analog(SSA)therapy,and surgery were 43%(6/14),10%(5/49),5%(1/22),and 11%(1/9),respectively.Post-treatment recurrence rates were significantly different(P=0.014)among four treatments(EMR,ESD,SSA,and surgery),and further subgroup comparisons revealed lower recurrence rates in the ESD and SSA groups than in the EMR group.From the second month onward,SSA therapy considerably reduced the gastrin levels from 1081.0(571.5,2472.8)pg/mL to 461.5(255.3,795.0)pg/mL(Z=-3.521,P<0.001).Both chi-squared test and multifactorial logistic regression analysis suggested that among the clinicopathological parameters studied,only the pre-treatment gastrin level(P=0.018 and 0.005)and the type of treatment(P=0.014 and 0.017)were significantly associated with G-NET recurrence.Conclusions:Individualized treatment strategies may reduce the risk of relapse after G-NET treatment.Long-term SSA therapy may be a secure and efficacious treatment option for type 1 G-NET with more than six lesions,and it substantially decreases the incidence of post-treatment recurrence.

Markovian Quadratic BSDEs with an Unbounded Sub-quadratic Growth

This paper is devoted to the solvability of Markovian quadratic backward stochastic differential equations(BSDEs for short) with bounded terminal conditions. The generator is allowed to have an unbounded sub-quadratic growth in the second unknown variable z. The existence and uniqueness results are given to these BSDEs. As an application, an existence result is given to a system of coupled forward-backward stochastic differential equations with measurable coefficients.

Phytosterol esters can effectively decrease serum cholesterol concentration in the human body and prevent cardio-cerebrovascular diseases.It was found that phytosterol esters exhibited better solubility and bioavailability than free phytosterols.In recent years,phytosterol esters have attracted increasing attention.However,during food processing,phytosterol esters are susceptible to degradation at high temperatures,resulting in certain losses and formation of potentially harmful substances for humans.This paper reviews the relevant literatures and updates on the thermal oxidation stability of phytosterol esters in recent years from the following aspects:(i)Sources,physiological activities,and applications of phytosterol esters;(ii)Oxidation mechanism of phytosterol esters;(iii)Effects of phytosterols species,the volume of addition,food matrix,heating temperature and time,and antioxidants on the thermal loss and oxidation stability of phytosterol esters.The research progress on the safety of phytosterol esters is also discussed in detail.Additionally,the prospects for future research are highlighted.

The catalytic activities are generally believed to be relevant to the electronic states of their active center, but understanding this relationship is usually difficult. Here, we design two types of catalysts for electrocatalytic urea via a coordination strategy in a metal–organic frameworks: Cu^(Ⅲ)-HHTP and Cu^(Ⅱ)-HHTP. Cu^(Ⅲ)-HHTP exhibits an improved urea production rate of 7.78 mmol h^(−1)g^(−1) and an enhanced Faradaic efficiency of 23.09% at-0.6 V vs. reversible hydrogen electrode, in sharp contrast to Cu^(Ⅱ)-HHTP.Isolated CuⅢspecies with S = 0 spin ground state are demonstrated as the active center in Cu^(Ⅲ)-HHTP, different from Cu^(Ⅱ) with S = 1/2 in Cu^(Ⅱ)-HHTP. We further demonstrate that isolated Cu^(Ⅲ)with an empty dx2-y20orbital in Cu^(Ⅲ)-HHTP experiences a single-electron migration path with a lower energy barrier in the C–N coupling process, while Cu^(Ⅱ)with a single-spin state( d_(x2-y2)^(1)) in Cu^(Ⅱ)-HHTP undergoes a two-electron migration pathway.

An efficient labeled memory system for learned indexes

The appearance and wide use of memory hardware bring significant changes to the conventional vertical memory hierarchy that fails to handle contentions for shared hardware resources and expensive data movements.To deal with these problems,existing schemes have to rely on inefficient scheduling strategies that also cause extra temporal,spatial and bandwidth overheads.Based on the insights that the shared hardware resources trend to be uniformly and hierarchically offered to the requests for co-located applications in memory systems,we present an efficient abstraction of memory hierarchies,called Label,which is used to establish the connection between the application layer and underlying hardware layer.Based on labels,our paper proposes LaMem,a labeled,resource-isolated and cross-tiered memory system by leveraging the way-based partitioning technique for shared resources to guarantee QoS demands of applications,while supporting fast and low-overhead cache repartitioning technique.Besides,we customize LaMem for the learned index that fundamentally replaces storage structures with computation models as a case study to verify the applicability of LaMem.Experimental results demonstrate the efficiency and efficacy of LaMem.

Alzheimer's disease(AD)is a common cause of dementia,characterised by cerebral amyloid-βdeposition,pathological tau and neurodegeneration.The prodromal stage of AD(pAD)refers to patients with mild cognitive impairment(MCl)and evidence of AD's pathology.At this stage,disease-modifying interventions should be used to prevent the progression to dementia.Given the inherent heterogeneity of MCl,more specific biomarkers are needed to elucidate the underlying AD's pathology.Although the uses of cerebrospinal fluid and positron emission tomography are widely accepted methods for detecting AD's pathology,their clinical applications are limited by their high costs and invasiveness,particularly in low-income areas in China.Therefore,to improve the early detection of Alzheimer's disease(AD)pathology through cost-effective screening methods,a panel of 45neurologists,psychiatrists andgerontologistswas invited to establish a formal consensus on the screening of pAD in China.The supportive evidence and grades of recommendations are based on a systematic literature review andfocus group discussion.National meetings were held to allow participants to review,vote and provide their expert opinions to reach a consensus.A majority(two-thirds)decision was used for questions for which consensus could not be reached.Recommended screening methods are presented in this publication,including neuropsychological assessment,peripheral biomarkers and brain imaging.In addition,a general workflow for Screening pAD in China is established,which will help clinicians identify individuals at high risk and determine therapeutic targets.

Tuberculosis(TB)is a chronic infectious disease,which is caused by the pathogen Mycobacterium tuberculosis(Mtb)and reemerged as a global health risk with a significant proportion of multi-drug resistant and extensively drug resistant TB cases.It is very urgent to find some novel high-confidence drug targets in Mtb for discovering the effective anti-TB agents.Thioredoxin reductase(TrxR)has been identified to be a highly viable target for anti-TB drugs for its important role in protecting the pathogen from thiol-specific oxidizing stress,regulating intracellular dithiol/disulfide homeostasis and DNA replication and repair.In the present work,a near-infrared(NIR)fluorescent probe DDAT was developed for the detection of TrxR activity and used to high-throughput screen the TrxR inhibitors from natural products.Two screened TrxR inhibitors from Sappan Lignum and microbial metabolites that were further used to inhibit Mycobacterium tuberculosis.All the results indicate that DDAT is a practical fluorescent molecular tool for the discovery of potential anti-TB drugs.

Development of new metal-free heterogeneous catalysts has long been the focus of intense research interest. The integration of multifunctional monomers into the skeletons of porous organic polymers(POPs)provides an efficient pathway to achieve this goal. Herein, we rationally designed and successfully prepared a new Tr?ger’s base(TB)-derived POPs by insertion of pillar[5]arene macrocycle as a positively auxiliary group. Combined the both merits of pillar[5]arene macrocycle and TB moiety, the as-prepared polymer was further explored as an effective metal-free heterogeneous catalyst and exhibited promoted catalytic performance in Knoevenagel condensation and CO2conversion. This work provides a new strategy to fabricate metal-free heterogeneous catalysts based on macrocyclic POPs.

Sensitive humidity sensor based on moisture-driven energy generation

The emergence of novel self-powered humidity sensors has attracted considerable attention in the fields of smart electronic devices and personal healthcare.Herein,self-powered humidity sensors have been fabricated using a moisture-driven energy generation(MEG)device based on asymmetric tubular graphitic carbon nitride(g-CN)films prepared on anodized aluminum(AAO)template.At a relative humidity(RH)of 96%,the MEG device can provide an open-circuit voltage of 0.47 V and a short-circuit current of 3.51μA,with a maximum output power of 0.08μW.With inherent self-powered ability and humidity response via current variation,an extraordinary response of 1.78×106%(41%-96%RH)can be gained from the MEG device.The possible power generation mechanism is that g-CN/AAO heterostructure can form ion gradient and diffusion under the action of moisture to convert chemical potential into electrical potential,evoking a connaturally sensitive response to humidity.Self-powered respiration monitoring device based on the sensor is designed to monitor human movement(sitting,warming up,and running)and sleep status(normal,snoring,and apnea),maintaining excellent stability during cumulative 12-h respiration monitoring.This self-powered humidity sensing technology has promising potential for extensive integration into smart electronic and round-the-clock health monitoring devices.
