Sirolimus improves the prognosis of liver recipients with hepatocellular carcinoma:A single-center experience

Background:Tumor recurrence after liver transplantation(LT)for selective patients diagnosed with hepatocellular carcinoma(HCC)in the setting of cirrhosis is the greatest challenge effecting the prognosis of these *** aim of this study was to evaluate the efficacy of sirolimus on the prognosis for these ***:The data from 193 consecutive HCC patients who had undergone LT from January 2015 to December 2019 were retrospectively *** patients were divided into the sirolimus group[patients took sirolimus combined with calcineurin inhibitors(CNIs)(n=125)]and non-sirolimus group[patients took CNI-based therapy without sirolimus(n=68)].Recurrence-free survival(RFS)and overall survival(OS)were compared between the two *** prognostic factors and independent risk factors for RFS and OS were further ***:Non-sirolimus was an independent risk factor for RFS(HR=2.990;95%CI:1.050-8.470;P=0.040)and OS(HR=3.100;95%CI:1.190-8.000;P=0.020).A higher proportion of patients beyond Hangzhou criteria was divided into the sirolimus group(69.6%vs.80.9%,P=0.030).Compared with the non-sirolimus group,the sirolimus group had significantly better RFS(P<0.001)and OS(P<0.001).Further subgroup analysis showed similar ***:This study demonstrated that sirolimus significantly decreased HCC recurrence and prolonged RFS and OS in LT patients with different stage of HCC.

Enhanced hepatic metabolic perturbation of polystyrene nanoplastics by UV irradiation-induced hydroxyl radical generation

The environmental behavior of and risks associated with nanoplastics(NPs)have attracted considerable ***,compared to pristine NPs,environmental factors such as ultraviolet(UV)irradiation that lead to changes in the toxicity of NPs have rarely been *** evaluated the changes in morphology and physicochemical properties of polystyrene(PS)NPs before and after UV irradiation,and compared their hepatotoxicity in *** results showed that UV irradiation caused particle size reduction and increased the carbonyl index(CI)and negative charge on the particle ***-aged PS NPs(aPS NPs)could induce the generation of hydroxyl radicals(·OH),but also further promoted the generation of·OH in the Fenton reaction *** pathological damage was more severe in mice exposed to aPS NPs,accompanied by a large number of vacuoles and hepatocyte balloon-like changes and more marked perturbations in blood glucose and serum lipoprotein,alanine aminotransferase and aspartate aminotransferase *** addition,exposure to PS NPs and aPS NPs,especially aPS NPs,triggered oxidative stress and significantly damaged the antioxidant capacity of mice *** with PS NPs,exposure to aPS NPs increased the number of altered metabolites in hepatic and corresponding metabolic pathways,especially glutathione *** research suggests that UV irradiation can disrupt the redox balance in organisms by promoting the production of·OH,enhancing PS NPs-induced liver damage and metabolic *** study will help us understand the health risks of NPs and to avoid underestimation of the risks of NPs in nature.

Microstructural Evolutions and Mechanical Properties of Energetic Al_(1) (TiZrNbTaMoCr)_(15) High-Entropy Alloys

The present study focuses on investigating the microstructural evolutions and mechanical properties of energetic Al_(1)(TiZrNbTaMoCr)_(15) refractory high-entropy alloys with the different heat treatments at low Al content state. It is found that even with a reduction in the Al content, the strength of these alloys remains unaffected at room temperature and high temperature, while the plasticity improves significantly. In particular, the coherent BCC/B_(2) microstructure with needle-like B_(2) nanoprecipitates dispersed into the BCC matrix is formed in 873 K-aged Al_(1)Ti_(6)Zr_(2)Nb_(3)Ta_(3)Mo_(0.5)Cr_(0.5) alloy. Therefore, this alloy exhibits the highest compression yield strength (σ_(YS) = 1333 and 717 MPa) at room temperature and 1073 K, respectively. After 973 K-aged, the coherent BCC/B_(2) microstructure underwent destabilization, and the B_(2) phase transforms into the brittle Zr5Al3 phase which then coarsens and dominates the microstructure of S3-AlTa_(3) alloy after 1073 K-aged. Moreover, these current alloys exhibit exceptionally high theoretical exothermic enthalpy (ΔH), surpassing 11606 J·g^(−1), which highlights their significant potential as innovative high-performance energetic structural materials.

Simultaneous liver, pancreas-duodenum and kidney transplantation in a patient with hepatitis B cirrhosis, uremia and insulin dependent diabetes mellitus

Simultaneous liver,pancreas-duodenum,and kidney transplantation has been rarely reported in the literature. Here we present a new and more efficient en bloc technique that combines classic orthotopic liver and pancreas-duodenum transplantation and heterotopic kidney transplantation for a male patient aged 44 years who had hepatitis B related cirrhosis,renal failure,and insulin dependent diabetes mellitus(IDDM). A quadruple immunosuppressive regimen including induction with basiliximab and maintenance therapy with tacrolimus,mycophenolate mofetil,and steroids was used in the early stage post-transplant. Postoperative recovery was uneventful and the patient was discharged on the 15 th postoperative day with normal liver and kidney function. The insulin treatment was completely withdrawn 3 wk after operation,and the blood glucose level remained normal. The case findings support that abdominal organ cluster and kidney transplantation is an effective method for the treatment of end-stage liver disease combined with uremia and IDDM.

Marginal donor liver versus standard donor liver: A single-center observational study

Background:The effectiveness and safety of marginal donor livers remain *** study aimed to investigate the clinical efficacy of marginal donor livers in patients with liver transplantation(LT).Methods:This study included 199 liver donors(including 16 split donors)and 206 liver recipients from January 1,2018 to January 27,2020,with case follow-up until July 31,*** data of donors and recipients were retrospectively analyzed and were divided into the marginal donor and standard donor groups according to the criteria of marginal donor *** of liver and kidney functions,complications,and survival curves of the two groups were ***:Compared with the standard donor group,the blood creatinine levels were significantly higher in the marginal donor group in the first week after operation(P0.05);there was no significant difference in the incidence of complications after LT(P>0.05);there was also no significant difference in the survival curve(P=0.335).Conclusions:There were no significant differences in liver and kidney function and survival curve between the standard donor and marginal donor *** marginal donor liver appears safe and reliable for LT and may be an important strategy to expand the donor pool and solve the shortage of organs.

Shor's algorithm does not factor large integers in the presence of noise

We consider Shor's quantum factoring algorithm in the setting of noisy quantum gates. Under a generic model of random noise for(controlled) rotation gates, we prove that the algorithm does not factor integers of the form pq when the noise exceeds a vanishingly small level in terms of n—the number of bits of the integer to be factored, where p and q are from a well-defined set of primes of positive density. We further prove that with probability 1-o(1) over random prime pairs(p, q), Shor's factoring algorithm does not factor numbers of the form pq, with the same level of random noise present.

To the Editor:During the ongoing coronavirus disease 2019(COVID-19)pan-demic globally,patients with chronic liver diseases(CLD),par-ticularly cirrhosis,hepatobiliary malignancies,candidates for liver transplantation(LT),and immunosuppressed LT recipients appear to be at increased risk of infections,which leads to an increase in mortality[1-6].Apart from physical distancing,quarantine and isolation,vaccination is crucial for the restraining of the epidemic and the protection from severe acute respiratory syndrome coron-avirus 2(SARS-CoV-2)infection and aggravation of COVID-19[7,8].A recent prospective,multicenter,open-label study in China has demonstrated the safety of inactivated whole-virion SARS-CoV-2 vaccines in patients with CLD.
