Utility of multiple endoscopic techniques in differential diagnosis of gallbladder adenomyomatosis from gallbladder malignancy with bile duct invasion: A case report

BACKGROUND Gallbladder adenomyomatosis(GAM) is a benign lesion, characterized by thickening of the gallbladder wall and a focal mass, which overlap with the features of gallbladder malignancy. Consequently, differential diagnosis of GAM from gallbladder cancer is difficult and approximately 20% of suspected malignant biliary strictures are postoperatively confirmed as benign ***, we report a case in which a preoperative diagnosis of GAM was made by a combination of endoscopic and imaging *** SUMMARY A 40-year-old man was referred to our hospital chiefly for a fever and right upper abdominal pain with dark urine. Enhanced computed tomography showed thickening of the gallbladder wall and a mass in the gallbladder neck with involvement of the hepatic bile ducts, which was suspected to be *** malignancy with bile duct invasion was ruled out by subsequent endoscopic examinations, including endoscopic retrograde cholangiopancreatography, intraductal ultrasound, and Spy Glass. Endoscopic examinations showed a homogeneous hyperechoic lesion with smooth margins of benign bile duct stricture suggestive of inflammatory stenosis of the bile *** patient underwent laparoscopic cholecystectomy. GAM was postoperatively diagnosed and confirmed based on the histopathology results, which are consistent with the preoperative diagnosis. Notably, no malignant event occurred in the patient during a 12-mo follow-up *** A combination of endoscopic techniques may help in the differential diagnosis of GAM from gallbladder cancer.

The U-Th-REE-Nb(Ta)-polymetallic mineralization is generally related to either the silica-undersaturated syenites,the silica-oversaturated alkaline/peralkaline granites or igneous *** this study,the authors report a new mineralization type,which is related to the magmatic-hydrothermal albitite(with mineral assemblage predominated by albite with volume content>90%),as exemplified by the Chachaxiangka deposit in Qinghai Province of *** Chachaxiangka deposit is the first albititerelated U-Th-REE-Nb deposit recognized in China and the mineralization can be divided into 3 types:the vein-type,the disseminated veinlet type and breccia type,of which the former 2 are *** mineralization stages can be identified according to the detailed mineralogical analyses,including the magmatic stage,main hydrothermal mineralization stage and post-ore *** comprehensive analyses of the mineralogical,major and trace element compositions,the authors suggest that the albitite vein is magmatic-hydrothermal in origin and both the magmatic evolution and overprint of the hydrothermal fluids play important roles in the formation of the albitite and related polymetallic *** separation between the silicate melt and carbonate/phosphate melt might take place in the magmatic stage,yet the immiscibility between the silicate melt and chloride-dominated fluids is the most important mechanism for the REE mineralization and also causes the Nb-Th re-mobilization and *** red color of the albitite aplite vein is an eye-catching prospecting mark in the field and more mineralization can be expected at depth and in the surrounding *** discovery of the new albitite type U-Th-REENb mineralization give rise to new ideas during future U-Th-REE-Nb exploration,not only in the Qaidam-Altun belt,but also other areas across China.

Effect of oxymatrine on the p38 mitogen-activated protein kinases signalling pathway in rats with CCl4 induced hepatic fibrosis

Background Recent studies have suggested that p38 mitogen-activated protein kinases （MAPK） signalling pathway plays an important role in hepatic fibrosis. This study explored the antifibrotic effect of oxymatrine on tetrachloromethane induced liver fibrosis in rats and its modulation on the p38 MAPK signalling pathway. Methods One hundred and twenty healthy male Sprague-Dawley rats were randomly assigned to six groups： normal （n=20）, induced fibrosis （n=20）, colchicine （n=20） and three treatment groups of oxymatrine （n=20x3）. We obesrved changes in deposition of collagen, hyaluronic acid （HA）, laminin （LN）, collagen type IV （CIV）, procollagen III （PCIll） and hydroxyproline （Hyp）, a-smooth muscle actin （α-SMA） and phosphor-p38 （pp38）. Results The relative indicators of changes in histopathology, HA, LN, CIV, PCIII, Hyp, a-SMA and pp38 were raised significantly in the induced fibrosis group （P〈0.01 vs normal group）. The semiquantitative hepatic fibrosis staging scores of middle dose group and high dose group were decreased （P 〈0.05 and P 〈0.01 respectively vs the induced fibrosis group）, as was the average area of collagen in rats＇ liver, the concentrations of serum HA, LN, CIV, PCIII and liver tissue homogenate Hyp. The gene expression of α-SMA mRNA was considerably decreased in the treated animals, as was the protein espression of pp38 protein. Conclusions Oxymatrine is effective in reducing the production and deposition of collagen in the liver tissue of experimental rats in ways which relate to modulating the fibrogenic signal transduction via p38 MAPK signalling pathway.

The fatigue resistance of casting polyurethane(CPU)is crucial in various sectors,such as construction,healthcare,and the automotive *** its importance,no studies have reported on the fatigue threshold of *** study employed an advanced Intrinsic Strength Analyzer(ISA)to evaluate the fatigue threshold of CPUs,systematically exploring the effects of three types of isocyanates(PPDI,NDI,TDI)that contribute to hard segment structures based on the cutting *** multiple advanced characterization techniques(XRD,TEM,DSC,AFM),the results indicate that PPDI-based polyurethane exhibits the highest fatigue threshold(182.89 J/m^(2))due to a highest phase separation and a densely packed spherulitic structure,although the hydrogen bonding degree is the lowest(48.3%).Conversely,NDI-based polyurethane,despite having the high hydrogen bonding degree(53.6%),exhibits moderate fatigue performance(122.52 J/m^(2)),likely due to a more scattered ***-based polyurethane,with the highest hydrogen bonding degree(59.1%)but absence of spherulitic structure,shows the lowest fatigue threshold(46.43 J/m^(2)).Compared to common rubbers(NR,NBR,EPDM,BR),the superior fatigue performance of CPU is attributed to its well-organized microstructure,polyurethane possesses a higher fatigue threshold due to its high phase separation degree and orderly and dense spherulitic structure which enhances energy dissipation and reduces crack propagation.

AIM:To evaluate the effect of femtosecond laser small incision lenticule extraction(SMILE)on the binocular visual function in myopic patients with glasses-free threedimensional(3D)***:Totally 50 myopic patients(39 females and 11 males)with SMILE were enrolled in this prospective *** glasses-free 3D technique was used to evaluate the binocular visual function in these subjects including static stereopsis,dynamic stereopsis,foveal suppression,and binocular balance point of signal to noise ratio(s/n ratio).All subjects received measurements in 1d before operation,and 1d,1wk,and 1mo ***:Both static and dynamic stereopsis showed no significant difference after *** foveal suppression improved significantly 1wk and 1mo after SMILE(P=0.005 and P=0.007 respectively).The binocular balance point of signal to noise ratio showed a significant improvement 1d,1wk and 1mo after SMILE for both eyes(P<0.001 for each eye respectively).CONCLUSION:Glasses-free 3D technique can be used to evaluate the effect of SMILE on the binocular visual function in myopic patients perceptively,and SMILE can improve both foveal suppression and binocular imbalance in these patients.

Background:Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality. While liver transplantation (LT) provides the best long-term survival, it is constrained by organ scarcity and strict criteria. Liver resection (LR) is often the initial treatment for patients with solitary tumors and preserved liver function. The high recurrence rates associated with LR has prompted the exploration of sequential living donor liver transplantation (seq LDLT) after LR as a strategy for HCC patients with high-risk of ***:We analyzed data from 27 adult patients who underwent seq LDLT after LR for HCC at Kaohsiung Chang Gung Memorial Hospital (KCGMH) between June 1994 and December 2023. Patients were selected based on high-risk histopathological features post-LR or as part of downstaging strategy. Outcomes measured included overall survival (OS) and disease-free survival (DFS).Results:Among 765 HCC patients who underwent LDLT, 204 received LR before LDLT, and 27 underwent seqL DLT. Five patients (19%) underwent living donor liver transplantation (LDLT) following LR as a downstaging strategy while the rest received seqL DLT as a preemptive strategy. The median age was 53.5 years with 85%males. Chronic hepatitis B was the predominant underlying disease (74%). The 1-, 3-, and 5-year OS and DFS rates were 100%, 96.0%, 96.0%and 100%, 96.2%, 96.2%, respectively, with two patients experiencing HCC recurrence. One patient died from HCC recurrence. High-risk histopathological features included microvascular invasion (52%), satellite nodules (15%), multiple tumors (26%), tumors> 5 cm(19%), and a total tumor diameter> 10 cm (7%).Conclusions:Seq LDLT offers a promising, tailored approach for managing HCC with adverse histopathologic features. Combining seq LDLT, downstaging strategies, and multidisciplinary treatments can achieve satisfactory OS and DFS in carefully selected patients, highlighting the need for refined criteria to identify the best candidates.

Background Successful revascularization of coronary artery disease, especially ST-elevation myocardial infarction （STEMI）, does not always mean optimal myocardial reperfusion in a portion of patients because of no-reflow phenomenon. We hypothesized that statins might attenuate the incidence of myocardial no-reflow when used before percutaneous coronary intervention （PCI）. The purpose of this study was to summarize the evidence of pre-procedural statin therapy to reduce myocardial no-reflow after PCI. Methods We searched the MEDLINE, Cochrane, and *** databases from inception to October 2012 for clinical trials that examined statin therapy before PCI. We required that studies initiated statins before PCI and reported myocardial no-reflow. A DerSimonian-Laird model was used to construct random-effects summary risk ratios. Results In all, 7 studies with 3086 patients met our selection criteria. The use of pre-procedural statins significantly reduced post-procedural no-reflow by 4.2% in all PCI patients （risk ratio （RR） 0.56, 95% confidence interval （CI） 0.35 to 0.90, P=0.016）, and attenuated by 5.0% in non-STEMI patients （RR 0.41, 95% CI 0.18 to 0.94, P=0.035）. This benefit was mainly observed in the early or acute intensive statin therapy populations （RR 0.43, 95% CI 0.26 to 0.71, P=0.001）. Conclusions Acute intensive statin therapy before PCI significantly reduces the hazard of post-procedural no-reflow phenomenon. The routine use of statins before PCI should be considered.
