In an effort to minimize the limitations of laparoscopy,a robotic surgery system was introduced,but its role for gastric cancer is still *** objective of this article is to assess the current status of robotic surgery for gastric cancer and to predict future *** the current study was limited by its small number of patients and retrospective nature,robot-assisted gastrectomy with lymphadenectomy for the treatment of gastric cancer is a feasible and safe procedure for experienced laparoscopic *** studies have reported satisfactory results for postoperative short-term coutcomes,such as:postoperative oral feeding,gas out,hospital stay and complications,compared with laparoscopic surgery;the difference is a longer operation ***,robotic surgery showed a shallow learning curve compared with the familarity of conventional open surgery;after the accumulation of several cases,robotic surgery could be expected to result in a similar operation ***-assisted gastrectomy can expand the indications of minimally invasive surgery to include advanced gastric cancer by improving the ability to perform ***," total" robotic gastrectomy can be facilitated using a robotsewing technique and gastric submucosal tumors near the gastroesophageal junction or pylorus can be resected safely by this novel *** conclusion,robotassisted gastrectomy may offer a good alternative to conventional open or laparoscopic surgery for gastric cancer,provided that long-term oncologic outcomes can be confirmed.

The cell-type continuous electromagnetic radiation system is a demonstration device capable of generating high-power millimeter electromagnetic waves of a specific wavelength and observing their effects on living *** irradiates a biological sample placed in a 30×30×50 cm^(3)cell with electromagnetic waves in the 3.15-mm-wavelength region(with an output of≥1 W)and analyzes the temperature change of the sample.A vacuum electronic device-based coupled-cavity backward-wave oscillator converts the electron energy of the electron beam into radiofrequency(RF)energy and radiates it to the target through an antenna,increasing the temperature through the absorption of RF energy in the *** system causes pain and ultimately reduces combat power.A cell-type continuous electromagnetic radiation system consisting of four parts—an electromagnetic-wave generator,a highvoltage power supply,a test cell,and a system controller—generates an RF signal of≥1 W in a continuous waveform at a 95-GHz center frequency,as well as a chemical solution with a dielectric constant similar to that of the skin of a living *** increase of 5°C lasting approximately 10 s was confirmed through an experiment.

The issue of CCF （common cause failure） in digital I ＆ C （instrumentation and control） systems is of great interest because an increasing number of such systems are implemented in nuclear power plants. For the mitigation of ATWS （anticipated transients without scram） as well as CCF within the PPS （plant protection system） and the ESF-CCS （engineered safety feature-component control system）, the ADPS （advanced diverse protection system） has been developed by KEPCO E ＆ C （KEPCO Engineering and Construction） Company for new nuclear units in Korea. As compared to the DPS （diverse protection system） design of APR1400, the ADPS has a diverse safety injection function considering a LBLOCA （large break loss of coolant accident） concurrent with the CCF of the PPS and ESF-CCS. Besides the function of SIAS （safety injection actuation signal） initiation, several CCF avoidance features, such as the changes of software design classification, communication methods, equipment platform, and man-machine interfaces, are introduced to enhance the reliability of the ADPS. In addition, the ADPS has recently incorporated four redundant channels with 2-out-of-4 voting logics to enhance its fault tolerant capability. Therefore, it is expected that the ADPS can provide an enhanced reliability regarding possible CCFs in the safety-grade digital I ＆ C systems as well as the ADPS itself.

在共价键合多糖类衍生物的手性固定相(Chiralpak IA,Chiralpak IB,Chiralpak IC)和涂敷型多糖衍生物的手性固定相(Chiralpak AD,Chiralcel OD)上,对非甾体抗炎药物(non-steroidal anti-inflammatory drugs,NSAIDs)进行了对映体拆分。色谱条件:流动相为10%异丙醇/正己烷(V/V)含0.1%三氟乙酸(ChiralpakAD,2%异丙醇/正己烷(V/V)含0.1%三氟乙酸(ChiralpakOD),异丙醇(四氢呋喃、乙酸乙酯、二氯甲烷)/正己烷/0.1%三氟乙酸(Chiralpak IA,Chiralpak IB,Chiralpak IC);流速为1.0mL/min;紫外检测波长为254nm。比较了非甾体抗炎药在直链淀粉-三(3,5-二甲基苯基氨基甲酸酯)衍生的手性固定相Chiralpak IA和Chiral-cel AD,纤维素-三(3,5-二甲基苯基氨基甲酸酯)衍生的手性固定相Chiralpak IB和Chiralcel OD上的对映体拆分。结果表明,非甾体抗炎药在Chiralpak IA和Chiralpak IB上的分离效果普遍低于Chiralpak AD和Chiral-cel OD。

AIM To determine adiponectin expression in colonic tissue of murine colitis and systemic cytokine expression after melatonin treatments and sleep *** The following five groups of C57BL/6 mice were used in this study:(1) group Ⅰ,control;(2) group Ⅱ,2% DSS induced colitis for 7 d;(3) group Ⅲ,2% DSS induced colitis and melatonin treatment;(4) group Ⅳ,2% DSS induced colitis with sleep deprivation(SD) using specially designed and modified multiple platform water baths; and(5) group V,2% DSS induced colitis with SD and melatonin treatment. Melatonin(10 mg/kg) or saline was intraperitoneally injected daily to mice for 4 d. The body weight was monitored daily. The degree of colitis was evaluated histologically after sacrificing the mice. Immunohistochemical staining and Western blot analysis was performed using anti-adiponectin antibody. After sampling by intracardiac punctures,levels of serum cytokines were measured by ELISA. RESULTS Sleep deprivation in water bath exacerbated DSS induced colitis and worsened weight loss. Melatonin injection not only alleviated the severity of mucosal injury,but also helped survival during stressful condition. The expression level of adiponectin in mucosa was decreased in colitis,with the lowest level observed in colitis combined with sleep deprivation. Melatonin injection significantly(P < 0.05) recovered the expression of adiponectin. The expression levels of IL-6 and IL-17 were increased in the serum of mice with DSS colitis but decreased after melatonin injection. CONCLUSION This study suggested that melatonin modulated adiponectin expression in colonic tissue and melatonin and adiponectin synergistically potentiated antiinflammatory effects on colitis with sleep deprivation.

建立用高效液相色谱拆分α-氨基酸的邻苯二甲酰衍生物对映体的方法。用多糖类手性固定相ChiralcelOD,ChiralpakAD,ChiralpakAS柱;5%异丙醇/正己烷(V/V)含0.1%三氟醋酸溶液为流动相;流速为1mL/min;检测波长:UV=254nm。考察了α-氨基酸的邻苯二甲酰衍生物在不同手性固定相(ChiralcelOD,ChiralpakAD,ChiralpakAS柱)上的对映体的手性分离。结果表明,α-氨基酸的邻苯二甲酰衍生物对映体在ChiralcelOD柱上的分离效果最佳(α=1.342.11),并且全部得到了基线分离。

Optimal initiation of Helicobacter pylori eradication in patients with peptic ulcer bleeding

AIM:To evaluate when Helicobacter pylori(***)eradication therapy(ET)should be started in patients with peptic ulcer bleeding(PUB).METHODS:Clinical data concerning adults hospitalizedwith PUB were retrospectively collected and ***,sex,type and stage of peptic ulcer,whether endoscopic therapy was performed or not,methods of *** detection,duration of hospitalization,and specialty of the attending physician were *** influencing the confirmation of *** infection prior to discharge were determined using multiple logistic regression *** *** eradication rates of patients who received ET during hospitalization and those who commenced ET as outpatients were ***:A total of 232 patients with PUB were evaluated for *** infection by histology and/or rapid urease *** these patients,53.7%(127/232)had confirmed results of *** infection prior to *** multivariate analysis,duration of hospitalization and ulcer stage were factors independently influencing whether *** infection was confirmed before or after *** the patients discharged before confirmation of *** infection,13.3%(14/105)were lost to *** the patients found to be ***-positive after discharge,41.4%(12/29)did not receive *** was no significant difference in the *** eradication rate between patients who received ET during hospitalization a n d t h o s e w h o c o m m e n c e d E T a s o u t p a t i e n t s[intention-to-treat:68.8%(53/77)vs 60%(12/20),P=0.594;per-protocol:82.8%(53/64)vs 80%(12/15),P=0.723].CONCLUSION:Because many patients with PUB who were discharged before *** infection status was confirmed lost an opportunity to receive ET,we should confirm *** infection and start ET prior to discharge.
