Comparison of laparoscopic and open surgery for pyogenic liver abscess with biliary pathology

AIM:To investigate the feasibility and therapeutic effect of laparoscopic surgery for pyogenic liver abscess(PLA) with biliary ***:From January 2004 to October 2010,31 patients with PLA combined with biliary pathology meeting entry criteria received surgical management in our *** the 31 patients,13 underwent laparoscopic surgery(LS group) and 18 underwent open surgery(OS group).Clinical data including operation time,intraoperative blood loss,postoperative complication rate,length of postoperative hospital stay,and abscess recurrence rate were retrospectively analyzed and compared between the two ***:All patients received systemic antibiotic *** patients underwent ultrasound-guided percutaneous catheter drainage before *** complications occurred in 5 patients(16.1%,5/31) including 2 in the LS group and 3 in the OS *** patient had retained calculus in the common bile duct and another had liver abscess recurrence in the OS *** retained calculus and liver abscess recurrence occurred in the LS *** the two groups,there was no mortality during the perioperative *** were no significant differences in operation time,intraoperative blood loss and transfusion,postoperative complication rate and abscess recurrence rate between the two *** intake was earlier(1.9 ± 0.4 d vs 3.1 ± 0.7 d,P < 0.05) and length of postoperative hospital stay was shorter(11.3 ± 2.9 d vs 14.5 ± 3.7 d,P < 0.05) in the LS group than in the OS ***:Laparoscopic surgery for simultaneous treatment of PLA and biliary pathology is feasible in selected patients and the therapeutic effect is similar to that of open surgery.

An integrated MINLP model for multi-party coordination in downstream oil supply chain

Cooperation among enterprises can bring overall and individual performance improvement,and a smooth coordination method is ***,due to the lack of customized coordination methods,cooperation in the downstream oil supply chain cannot be carried out *** paper intends to propose a multi-party coordination method to promote cooperation between oil shippers and pipeline operator by optimizing oil transportation,oil substitution and pipeline pricing *** integrated game-theoretic modeling and analysis approach is developed to characterize the operation behaviors of all stakeholders in the downstream oil supply *** proposed mixed integer nonlinear programming model constrains supply and demand capacity,transportation routes,oil substitution rules and pipeline freight *** transformation and price discretization are introduced for model linear *** experiments are carried out in the oil distribution system in South *** results show that compared to the business-as-usual scheme,the new scheme saves transportation cost by 3.48%,increases pipeline turnover by 5.7%,and reduces energy consumption and emissions by 7.66%and 6.77%.It is proved that the proposed method improves the revenue of the whole system,achieves fair revenue distribution,and also improves the energy and environmental benefits of the oil supply chain.

Mechanisms of fracture propagation from multi-cluster using a phase field based HMD coupling model in fractured reservoir

Natural fractures(NFs)are common in shale and tight reservoirs,where staged multi-cluster fracturing of horizontal wells is a prevalent technique for reservoir *** NFs and stress interference are recognized as significant factors affecting hydraulic fracture(HF)propagation,the combined influence of these factors remains poorly *** address this knowledge gap,a novel coupled hydromechanical-damage(HMD)model based on the phase field method is developed to investigate the propagation of multi-cluster HFs in fractured *** comprehensive energy functional and control functions are established,while incorporating dynamic fluid distribution between multiple perforation clusters and refined changes in rock mechanical parameters during hydraulic *** HMD coupled multi-cluster HF propagation model investigates various scenarios,including single HF and single NF,reservoir heterogeneity,single HF and NF clusters,and multi-cluster HFs with NF *** results show that the HMD coupling model can accurately capture the impact of approach angle(θ),stress difference and cementation strength on the interaction of HF and *** criterion of the open and cross zones is not *** NF angle(a)is not a decisive parameter to discriminate the *** to the relationship between approach angle(θ)and NF angle(a),the contact relationship of HF can be divided into three categories(θ=a,θa).The connected NF can increase the complexity of HF by inducing it to form branch fracture,resulting in a fractal dimension of HF as high as2.1280 at angles of±45°.Inter-fracture interference from the heel to the toe of HF shows the phenomenon of no,strong and weak ***,under the influence of NFs,distant HFs from the injection can become dominant ***,as a gradually increases,inter-fracture stress interference becomes the primary factor influencing HF propagation,gradually superseding the dominance of NF

This study investigated degradation behaviors of a nonsteroidal anti-inflammatory drug Nabumetone(NMT)and its major metabolite 6-methoxy-2-naphthylacetic acid(MNA)in the coupling process of ultraviolet and monochloramine(UV/NH2Cl).The second-order rate constants of the contaminants reacting with reactive radicals(HO·,Cl·,Cl_(2)·-,and CO_(3)·^(-))were determined by laser flash photolysis ***·and Cl·contributed predominantly with 52.3%and 21.7%for NMT degradation and 60.8%and 22.3%for MNA *** presence of chlorides retarded the degradation of NMT,while promoted the destruction of MNA,which was ascribed to the photosensitization effects of MNA under UV *** functional theory(DFT)calculations revealed that radical adduct formation(RAF)was dominant pathway for both HO·and Cl·reacting with the contaminants,and hydrogen atom transfer(HAT)preferred to occur on side chains of NMT and *** reacted with NO_(2)·through single electron transfer(SET)with the second-order rate constant calculated to be 5.35×10^(7)(mol/L)^(-1)sec^(-1),and the contribution of NO_(2)·was predicted to be 13.0%of the total rate constant of NMT in pure water,which indicated that NO_(2)·played a nonnegligible role in the degradation of *** acute toxicity and developmental toxicity of NMT were enhanced after UV/NH_(2)Cl treatment,while those of MNA were *** transformation products of both NMT and MNA exhibited higher mutagenicity than their parent *** study provides a deep understanding of the mechanism of radical degradation of NMT and MNA in the treatment of UV/NH2Cl.

OBJECTIVE: To better understand mucobilia as well as its diagnosis and treatment. METHODS: The etiological factors, diagnosis, and treatment of 8 patients with mucobilia were discussed. RESULTS: Mucobilia characterized by copious mucin secretion in the extrahepatic bile duct resulted in obstructive jaundice and cholangitis. Four patients receiving curative resection of primary, lesions were free from jaundice and cholangitis while the other 4 who had had palliative biliary drainage showed persistent symptoms. CONCLUSIONS: Mucobilia is attributable to biliary mucous metaplasia, and benign or malignant biliary tumors. Cholangioscopy and biopsy can offer precise information about the location and extension of the primary lesion. The best choice of treatment is curative resection of the primary lesion.

BACKGROUND Appendectomy is an acute abdominal surgery that is often accompanied by severe abdominal *** probiotics are one of the postoperative treatments for rapid ***,there is a lack of prospective studies on this topic after *** To investigate whether the postoperative probiotics can modulate the inflammatory response and restore intestinal function in patients following *** This was a prospective,randomized trial.A total of 60 emergency patients were randomly divided into a control group(n=30)and a probiotic group(n=30).Patients in the control group started to drink some water the first day after surgery,and those in the probiotic group were given water supplemented with Bacillus licheniformis capsules for 5 consecutive days *** indices of inflammation and postoperative conditions were recorded,and the data were analyzed with RStudio 4.3.2 *** A total of 60 participants were *** with those in the control group,the C-reactive protein(CRP),interleukin 6 and procalcitonin(PCT)levels were significantly lower in the probiotic group at 2 d after surgery(P=2.224e-05,P=0.037,and P=0.002,respectively,all Pfurthermore,probiotics0.028,both P<0.05).CONCLUSION Postoperative oral administration of probiotics may modulate the gut microbiota,benefit the recovery of the early inflammatory response,and subsequently enhance recovery after appendectomy.

Housing vacancy identification in shrinking cities based on multi-source data:A case study of fushun city in Northeast China

Urban shrinkage has attracted the attention of many geographers and urban planners in recent ***,there are fewer studies on vacant housing in shrinking ***,this study combines multi-source remote sensing images and urban building data to assess the spatiotemporal variation patterns of housing vacancy in a typical shrinking city in *** following points were obtained:(1)We developed an evaluation model to identify vacant residential buildings in shrinking cities by removing the contribution of nighttime lights from roads and non-residential buildings;(2)The residential building vacancy rate in fushun city significantly increased from 2013 to 2020,resulting in a significant high-value clustering *** impact of urban shrinkage on vacant residential buildings was higher than that on vacant non-residential buildings;(3)The World Pop population data demonstrated consistent spatial distribution and trend of population change in fushun with the residential building vacancy rate results,suggesting good reliability of the constructed evaluation model in this *** housing vacancies can help the local government to raise awareness of the housing vacancy problem in shrinking cities and to propose reasonable renewal strategies.

tumor vaccine therapy offers significant advantages over conventional treatments,including reduced toxic side ***,it currently functions primarily as an adjuvant treatment modality in clinical oncology due to limitations in tumor antigen selection and delivery *** vaccines often fail to elicit a su±ciently robust immune response against progressive tumors,thereby limiting their clinical e±*** this study,we developed a nanoparticle-based tumor vaccine,OVA@HA-PEI,utilizing ovalbumin(OVA)as the presenting antigen and hyaluronic acid(HA)and polyethyleneimine(PEI)as adjuvants and *** formulation significantly enhanced the proliferation of immune cells and cytokines,such as CD3,CD8,interferon-,and tumor necrosis factor-,in vivo,effectively activating an immune response against B16–F10 *** vivofluorescenceflow cytometry(IVFC)has already become an effective method for monitoring circulating tumor cells(CTCs)due to its direct,noninvasive,and long-term detection *** study utilized a laboratory-constructed IVFC system to monitor the immune processes induced by the OVA@HA-PEI tumor vaccine and an anti-programmed death-1(PD-1)*** results demonstrated that the combined treatment of OVA@HA-PEI and anti-PD-1 antibody significantly improved the survival time of mice compared to anti-PD-1 antibody treatment ***,this combination therapy substantially reduced the number of CTCs in vivo,increased the clearance rate of CTCs by the immune system,and slowed tumor *** greatly enhance the clinical application prospects of IVFC and tumor vaccines.
