BACKGROUND Acetabular anterior wall fracture with preservation of the pelvic brim is extremely *** is different from anterior wall fracture classified by Judet and *** studies have reported cases treated by open reduction and internal fixation via the Smith-petersen or iliofemoral *** SUMMARY We report a 48-year-old Chinese woman who had difficulty moving her right hip from abduction and external rotation after falling from 3 *** radiograph and three-dimensional reconstruction of computed tomography revealed acetabular anterior wall fractures combined with fractures of the anterior inferior iliac spine and the iliac wing but not involving the pelvic ***,the patient underwent interim management by closed reduction of the hip dislocation and skin traction for 6 ***,we used a modified pararectus approach for treatment to fix the acetabular fractures with a reconstruction plate and nonlocking T-shape *** the 9-mo follow-up,the patient could walk painlessly without necrosis of the femoral head or heterotopic ossification,and the X-rays and computed tomography scan reconstructions showed good bone *** The modified pararectus approach described here can facilitate exposure,reduction,and osteosynthesis for atypical acetabular fracture with less invasiveness.

两种遗忘型轻度认知损害患者的随访比较

目的比较单纯遗忘型轻度认知功能损害（aMCI-s）和多个认知领域损害遗忘型轻度认知功能损害（aMCI-m）患者的2年随访转归结果并分析其神经心理学变化.方法采用MMSE、听觉词语学习测验（AVLT）、逻辑回忆测验（LMT）、连线测验（TMT）... 详细信息

目的比较单纯遗忘型轻度认知功能损害（aMCI-s）和多个认知领域损害遗忘型轻度认知功能损害（aMCI-m）患者的2年随访转归结果并分析其神经心理学变化.方法采用MMSE、听觉词语学习测验（AVLT）、逻辑回忆测验（LMT）、连线测验（TMT）、Stroop色词测验（CWT）、Rey复杂图形测验（CFT）、五点连线测验（FPT）、画钟测验（CDT）、言语流畅性测验（VF）和临床痴呆量表（CDR）等一系列神经心理测验评估记忆障碍门诊130例遗忘型轻度认知功能损害（aMCI）就诊者（其中aMCI-s 66例和aMCI-m 64例）,并进行平均2年的随访,MCI及AD的诊断标准分别参照美国petersen等及美国神经病学、语言障碍和卒中-老年性痴呆和相关疾病学会（NINCDS-ADRDA）有关诊断标准.结果 aMCI的阿尔茨海默病（AD）总转化率为33.8%（44/130）;aMCI-s和aMCI-m的AD转化率分别为26.2%（17/65）和42.9%（27/63）,差异有统计学意义（x2=3.957,P=0.047）.随着aMCI发展为AD,aMCI-s和aMCI-m组在记忆和执行功能方面的减退幅度接近,aMCI-s组的视空间结构能力相对保留,语言和注意力的减退更快,aMCI-m组的视空间结构能力衰退更为显著.结论 aMCI-m比aMCI-s更容易发展为AD,aMCI分为aMCI-s和aMCI-m对判断认知功能减退的速度是有必要的,有助于判断预后.

BACKGROUND: Mild cognitive impairment is a transitional state between the cog nitive changes of normal aging and early Alzheimer’ s disease. METHODS: In a do uble- blind study, we evaluated subjects with the amnestic subtype of mild cogn itive impairment. Subjects were randomly assigned to receive 2000 IU of vitamin E daily, 10 mg of donepezil daily, or placebo for three years. The primary outco me was clinically possible or probable Alzheimer’ s disease; secondary outcomes were cognition and function. RESULTS: A total of 769 subjects were enrolled, an d possible or probable Alzheimer’ s disease developed in 212. The overall rate of progression from mild cognitive impairment to Alzheimer’ s disease was 16 pe rcent per year. As compared with the placebo group, there were no significant di fferences in the probability of progression to Alzheimer’ s disease in the vita min E group (hazard ratio, 1.02; 95 percent confidence interval, 0.74 to 1.41; P =0.91) or the donepezil group (hazard ratio, 0.80; 95 percent confidence interva l, 0.57 to 1.13; P=0.42) during the three years of treatment. Prespecified analy ses of the treatment effects at 6- month intervals showed that as compared with the placebo group, the donepezil group had a reduced likelihood of progression to Alzheimer’ s disease during the first 12 months of the study (P=0.04), a fin ding supported by the secondary outcome measures. Among carriers of one or more apolipoprotein E e 4 alleles, the benefit of donepezil was evident throughout th e three- year follow- up. There were no significant differences in the rate of progression to Alzheimer’ s disease between the vitamin E and placebo groups a t any point, either among all patients or among apolipoprotein E e 4 carriers. C ONCLUSIONS: Vitamin E had no benefit in patients with mild cognitive impairment.
