Introducing Ni in Ru oxide is a promising approach to enhance the catalytic activity for the oxygen evolution reaction(OER).However,the role of Ni(which has a poor intrinsic activity)is not fully ***,a Ru NiO_(x)electrode fabricated via a modified dip coating method exhibited excellent OER performance in acidic media,and neutral media for CO_(2)reduction *** combined in-situ/operando X-ray absorption near-edge structure and on-line inductively coupled plasma mass spectrometry studies to unveil the role of the Ni introduced in the Ru *** propose that the Ni not only transforms the electronic structure of the Ru oxide,but also produces a large number of oxygen vacancies by distorting the oxygen lattice structure at low overpotentials,increasing the participation of lattice oxygen for *** work demonstrates the real behavior of bimetallic oxide materials under applied potentials and provides new insights into the development of efficient electrocatalysts.

Predictors of poor outcomes in patients with wild mushroom-induced acute liver injury

To identify early predictive markers of poor outcomes in patients with acute liver injury from wild mushroom *** observational, retrospective record review involved adults aged ≥ 18 years admitted to emergency department with mushroom intoxication from January 2005 to December 2015. The diagnosis of mushroom intoxication was based on the following: (1) a positive history of recent wild mushroom intake (either raw or cooked); (2) the onset of gastrointestinal symptoms, such as watery diarrhea, vomiting, and/or abdominal pain, after ingestion; and (3) the exclusion of other possible causes of acute liver injury. Acute liver injury was defined by a > 5-fold elevation of liver enzymes or moderate coagulopathy [international normalized ratio (INR) > 2.0]. Clinical and laboratory findings were compared in survivors and *** 93 patients with mushroom intoxication, 23, 11 men (47.8%) and 12 women (52.2%), of median age 61 years, developed acute liver injury. The overall in-hospital mortality rate was 43.5% (10/23). Case of inappropriate ADH syndrome:Hyponatremia due to polyethylene glycol bowel preparation

Colonoscopic screening has been reported to reduce deaths from colorectal *** bowel preparation is essential for this and safety is an important issue in choosing the *** glycol(PEG)is regarded as a safe method for cleansing,especially compared with oral sodium ***,we present a case of hyponatremia caused by the syndrome of inappropriate antidiuretic hormone(ADH)syndrome after PEG precolonoscopic cleansing resulting in generalized tonic-clonic seizures.A 62-year-old women had ingested PEG for precolonoscopic bowel *** waiting for the colonoscopy,she developed a stuporous mentality and generalized tonic-clonic seizures,which did not correlate with brain magnetic resonance *** serum sodium level was 113 mEq per liter and laboratory analyses were consistent with inappropriate ADH *** thyroid and adrenal functions were *** were no malignancies,infections,respiratory disorders or central nervous disorders and she had no history of taking either diuretics or other medications,which might have caused inappropriate ADH *** was treated with 3%hypertonic saline and showed a complete neurological recovery as her sodium levels ***-up visits showed the patient to have a normal sodium level without neurologic *** case shows that inappropriate ADH syndrome can be caused by PEG preparation,which implies that physicians have to be aware of the possible side effects of this colonic cleansing approach and mindful of the possible ensuing symptoms.

Enterovenous fistulization: A rare complication of Crohn's disease

The presence of hepatic portal venous gas (HPVG) is associated with numerous diseases, and has been regarded as a serious, even catastrophic condition. However, anecdotal reports mention that some patients with inflammatory bowel disease (IBD), who developed HPVG after diagnostic examinations of the colon, were successfully managed with antibiotic therapy and have followed benign courses. In contrast, among IBD patients, the development of HPVG is rarely caused by enterovenous fistula. We describe a 32-year-old man with Crohn's ileocolitis who presented with hypotension and fever associated with HPVG, as well as superior mesenteric vein thrombosis, possibly caused by enterovenous fistula, who was successfully managed by surgery. We also review the literature concerning portal venous gas associated with Crohn's disease.

Is a split-dose regimen of 2 L polyethylene glycol plus ascorbic acid tolerable for colonoscopy in an early morning visit to a comprehensive medical check-up?

To evaluate the effectiveness and tolerability of a split-dose 2 L polyethylene glycol (PEG)/ascorbic acid (AA) regimen for healthy examinees who visited for comprehensive medical check-up in the early *** February 2015 to March 2015, examinees of average risk who were scheduled for a colonoscopy in the morning were retrospectively *** 189 examinees were divided into split-dose and non-split-dose groups. The adequacy of bowel preparation for the split-dose group vs the non-split-dose group was 96.8% vs 85.2%, respectively, P < 0.001, and the compliance of the last meal restriction was 74.6% vs 58.2%, respectively, P < 0.001. The sleep disturbance (P < 0.001) was more prevalent in the split-dose group, however the willingness to repeat the same preparation method (P = 0.243) was not different in both groups. The split-dose regimen was the most important factor influencing adequate bowel preparation in multivariate analysis (HR = 10.89, 95%CI: 6.53-18.17, P < 0.001).CONCLUSIONA split-dose regimen of 2 L PEG/AA for an early morning colonoscopy was more effective and showed better compliance for diet restriction without any difference in satisfaction and discomfort. Introducing a split-dose regimen of 2 L PEG/AA to morning colonoscopy examinees is effective and tolerable in a comprehensive medical check-up setting.

Review of recent progresses on flexible oxide semiconductor thin film transistors based on atomic layer deposition processes

The current article is a review of recent progress and major trends in the field of flexible oxide thin film transistors（TFTs）, fabricating with atomic layer deposition（ALD） processes. The ALD process offers accurate controlling of film thickness and composition as well as ability of achieving excellent uniformity over large areas at relatively low temperatures. First, an introduction is provided on what is the definition of ALD, the difference among other vacuum deposition techniques, and the brief key factors of ALD on flexible devices. Second, considering functional layers in flexible oxide TFT, the ALD process on polymer substrates may improve device performances such as mobility and stability, adopting as buffer layers over the polymer substrate, gate insulators, and active layers. Third, this review consists of the evaluation methods of flexible oxide TFTs under various mechanical stress conditions. The bending radius and repetition cycles are mostly considering for conventional flexible devices. It summarizes how the device has been degraded/changed under various stress types（directions）. The last part of this review suggests a potential of each ALD film, including the releasing stress, the optimization of TFT structure, and the enhancement of device performance. Thus, the functional ALD layers in flexible oxide TFTs offer great possibilities regarding anti-mechanical stress films, along with flexible display and information storage application fields.

Long-term outcomes of endoscopic submucosal dissection and surgery for undifferentiated intramucosal gastric cancer regardless of size

BACKGROUND The clinical outcomes of endoscopic submucosal dissection(ESD)for undifferentiated(UD)intramucosal early gastric cancer(EGC)compared with those of surgery,regardless of lesion size,are not well ***,there is a concern regarding the treatment plan before and after ESD in cases of UD intramucosal EGC within expanded *** To evaluate clinical outcomes of ESD compared with those of surgery in UD intramucosal EGC patients regardless of tumor *** We enrolled patients with UD intramucosal EGC after ESD with complete resection or surgery from January 2005 to August 2020 who met the within or beyond expanded indications with lesion size>2 cm(the only non-curative factor).Overall,123 and 562 patients underwent ESD and surgery,*** propensity-score matching,clinical and long-term outcomes,i.e.,recurrencefree survival(RFS)and overall survival(OS),were *** multivariable Cox proportional hazard model with treatment modality and ESD indication was used to evaluate the recurrence *** After matching,119 patients each were finally enrolled in the ESD and surgery *** median length of hospital stay was shorter in the ESD group than surgery group(4.0 vs 9.0 days,P<0.001).Four cases of recurrence after ESD were local recurrences,all of which occurred within 1 *** recurrence was seven(5.9%)and two(1.7%)in the ESD and surgery groups,*** difference was observed between the two groups with respect to OS(P=0.948).However,the ESD group had inferior RFS compared with the surgery group(P=0.031).ESD was associated with the risk of recurrence after initial treatment in all enrolled patients(hazard ratio,5.2;95%confidence interval:1.0-25.8,P=0.045).CONCLUSION Although OS was similar between the two groups,surveillance endoscopy was important for the ESD than for the surgery group because RFS was inferior and local recurrence was an issue.

AIM: To examine whether the reversibility of chronic cyclosporine A (CsA) nephrotoxicity is associated with apoptotic cell death and its regulatory factors. METHODS: Chronic CsA nephrotoxicity was induced in Sprague- Dawley rats by administering CsA (15 mg/kg, sc) for 5 weeks, and then withdrawing it for 5 or 10 weeks. The effect of CsA withdrawal on apoptotic cell death was evaluated by an in situ TdT-mediated deoxyuridine triphos- phate-biotin nick end-labeling (TUNEL) assay and the expression of pro-apoptotic [(transforming growth factor- beta1 (TGF-β1) and Fas] and anti-apoptotic [epidermal growth factors (EGF) and Bcl-2] factors. RESULTS: Discontinuation of CsA induced significant decreases in TUNEL-positive cells in a time-dependent manner and the reduction in TUNEL-positive cells was correlated with the tubulointerstitial fibrosis score (r=0.919, P<0.01). Upregulation of TGF-β1 and Fas expression in CsA-treated rat kidneys was decreased significantly after with- drawal of CsA. In contrast, downregulated EGF and Bcl-2 expression returned to normal or supernormal levels. CONCLUSION: CsA withdrawal is associated with a decrease in apoptotic cell death and with changes in the expression of pro-apoptotic and anti-apoptotic molecules involved in renal wound repair. This may constitute one of the mechanisms underlying the reversibility of chronic CsA nephrotoxicity.

Comparison of CT and MRI for presurgical characterization of paraaortic lymph nodes in patients with pancreatico-biliary carcinoma

AIM:To determine the accuracy of computed tomography (CT) and magnetic resonance (MR) for presurgical characterization of paraaortic lymph nodes in patients with pancreatico-biliary carcinoma. METHODS:Two radiologists independently evaluated CT and MR imaging of 31 patients who had undergone lymphadenectomy (9 metastatic and 22 non-metastatic paraaortic nodes). Receiver operating characteristic (ROC) curve analysis was performed using a five point scale to compare CT with MRI. To re-define the morphologic features of metastatic nodes, we evaluated CT scans from 70 patients with 23 metastatic paraaortic nodes and 47 non-metastatic ones. The short axis diameter, ratio of the short to long axis, shape, and presence of necrosis were compared between metastatic and non-metastatic nodes by independent samples t-test and Fisher's exact test. P 5.3 mm), irregular margin, and presence of central necrosis are the suggestive morphologic features of metastatic paraaortic nodes.

目的:分析富亮氨酸胶质瘤失活蛋白1(LGI-1)抗体阳性自身免疫性脑炎(AE)(LGI-1 AE)并发睡眠障碍和认知障碍患者的临床资料,探讨其可能的病理机制。方法:患者,男性,68岁。因记忆力减退2个月,抽搐1个月入院,临床诊断LGI-1 AE,给予人免疫球蛋白联合甲泼尼龙琥珀酸钠治疗后症状好转。在排除任何药物影响下,分别于急性期及恢复期对患者进行神经心理学评估及包括睡眠多导图(PSG)检查的睡眠评估。结果:急性期评估提示患者存在严重认知障碍,简易精神状态检查量表(MMSE)评分22分,蒙特利尔认知评价量表(MoCA)评分19分。PSG检查,总睡眠时间明显缩短(265 min),睡眠全程碎片化,睡眠效率降低,N3和快速眼动期(REM)睡眠完全缺失。恢复期评估,患者认知功能改善(MMSE评分30分,MoCA评分26分),PSG检查总睡眠时间正常,睡眠潜伏期13.5 min,睡眠碎片化明显改善,睡眠效率提高(84.3%),N3期睡眠26 min(5.1%),REM期睡眠69 min(13.6%)。结论:LGI-1 AE患者睡眠结构的异常与认知障碍在发病和转归方面同步,可能是LGI-1 AE患者认知障碍的病因之一。睡眠障碍的病理起源可能为下丘脑。下丘脑分泌素和lim同源框(lhx6)通路可能成为纠正睡眠结构同时治疗认知障碍的新靶点。
