Effect of neoadjuvant chemoradiotherapy on prognosis and surgery for esophageal carcinoma

AIM:To investigate the role of neoadjuvant chemoradiotherapy in prognosis and surgery for esophageal carcinoma by a ***:PubMed and manual searches were done to identify all published randomized controlled trials(RCTs) that compared neoadjuvant chemoradiotherapy plus surgery(CRTS) with surgery alone(S) for esophageal *** to the test of heterogeneity,a fi xed-effect model or a random effect model was used and the odds ratio(OR) was the principal measure of ***:Fourteen RCTs that included 1737 patients were selected with quality assessment ranging from A to C(Cochrane Reviewers' Handbook 4.2.2).OR(95% CI,P value),expressed as CRTS vs S(values>1 favor CRTS arm),was 1.19(0.94-1.48,P=0.28) for 1-year survival,1.33(1.07-1.65,P=0.69) for 2-year survival,1.76(1.42-2.19,P=0.11) for 3-year survival,1.41(1.06-1.87,P=0.11) for 4-year survival,1.64(1.28-2.12,P=0.40) for 5-year survival,0.82(0.39-1.73,P<0.0001) for rate of resection,1.53(1.33-2.84,P=0.007) for rate of complete resection,1.78(1.14-2.78,P=0.79) for operative mortality,1.12(0.89-2.48,P=0.503) for all treatment mortality,1.33(0.94-1.88,P=0.04) for the rate of adverse treatment,1.38(1.23-1.63,P=0.0002) for local-regional cancer recurrence,1.28(0.85-1.58,P=0.60) for distant cancer recurrence,and 1.27(0.86-1.65,P=0.19) for all cancer recurrence.A complete pathological response to chemoradiotherapy occurred in 10%-45.5% of *** 5-year survival benefi t was most pronounced when chemotherapy and radiotherapy were given concurrently(OR:1.45,95% CI:1.26-1.79,P=0.015) instead of sequentially(OR:0.85,95% CI:0.64-1.35,P=0.26).CONCLUSION:Compared with surgery alone,neoadjuvant chemoradiotherapy can improve the long-term survival and reduce local-regional cancer *** administration of neoadjuvant chemoradiotherapy was superior to sequential chemoradiotherapy.

Long-term efficacy of perioperative chemoradiotherapy on esophageal squamous cell carcinoma

AIM: To investigate the role of perioperative chemoradiotherapy (CRT) in the treatment of locally advanced thoracic esophageal squamous cell carcinoma (ESCC). METHODS: Using preoperative computed tomography (CT)-based staging criteria, 238 patients with ESCC (stage ⅡⅢ ) were enrolled in this prospective study between January 1997 and June 2004. With informed consent, patients were randomized into 3 groups: preoperative CRT (80 cases), postoperative CRT (78 cases) and surgery alone (S) (80 cases). The 1-, 3-, 5and 10-year survival were followed up. Progressionfree survival (PFS) was chosen as the primary endpoint by treatment arm measured from study entry until documented progression of disease or death from any cause. The secondary endpoint was overall survival (OS) determined as the time (in months) between the date of therapy and the date of death. Other objectives were surgical and adjuvant therapy ***: With median follow-up of 45 mo for all the enrolled patients, significant differences in the 1-, 3-, 5-, 10-year OS (91.3%, 63.5%, 43.5%, 24.5% vs 91%, 62.8%, 42.3%, 24.4% vs 87.5%, 51.3%, 33.8%, 12.5%, P = 0.0176) and PFS (89.3%, 61.3%, 37.5%, 18.1% vs 89.1%, 61.1%, 37.2%, 17.8% vs 84.5%, 49.3%, 25.9%, 6.2%, P = 0.0151) were detected among the 3 arms. There were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). For the patients who had radical resection, significant differences in median PFS (48 mo vs 61 mo vs 39.5 mo, P = 0.0331) and median OS (56.5 mo vs 72 mo vs 41.5 mo, P = 0.0153) were detected among the 3 arms, but there were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). The local recurrence rates in the preoperative CRT, postoperative CRT group and S group were 11.3%, 14.1% and 35%, respectively (P < 0.05). The Advanced Space-based Solar Observatory(ASO-S)is a mission proposed for the 25 th solar maximum by the Chinese solar *** scientific objectives are to study the relationships between the solar magnetic field,solar flares and coronal mass ejections(CMEs).Three payloads are deployed:the Full-disk vector Magneto Graph(FMG),the Lyman-αSolar Telescope(LST)and the Hard X-ray Imager(HXI).ASO-S will perform the first simultaneous observations of the photospheric vector magnetic field,non-thermal imaging of solar flares,and the initiation and early propagation of CMEs on a single ***-S is scheduled to be launched into a 720 km Sun-synchronous orbit in *** paper presents an overview of the mission till the end of Phase-B and the beginning of Phase-C.

目的:研究瘤体内植入125Ⅰ粒子治疗兔VX2肝移植癌的疗效及其病理变化,探讨125Ⅰ粒子组织间植入治疗肝癌的可行性。方法:建立荷瘤兔肝移植癌动物模型。对照组(A组)植入空白剂量(0mCi)125Ⅰ粒子,B组植入1.0mCi125Ⅰ粒子,C组植入0.7mCi125Ⅰ粒子,D组植入0.4mCi125Ⅰ粒子。观察植入前后各组肿瘤体积并计算抑瘤率,切除肿瘤组织进行常规病理检查。结果:各治疗组肿瘤大小在治疗前后比较具有显著差异(P<0.01),均小于同期对照组(P<0.01)。在不同观察时期抑瘤率差别均有统计学意义(P<0.05),治疗1周后抑瘤率变化显著;各个组间抑瘤率差异在治疗后2周最为明显(P<0.01),以后减小,但均高于D组(P<0.01)。病理切片显示靠近125Ⅰ粒子处肿瘤细胞坏死,但远离粒子处仍可见存活肿瘤细胞。1.0mCi粒子对正常肝组织损伤较大。结论:125Ⅰ粒子组织间植入治疗肝癌有效,单个粒子活度以0.7mCi左右较为适宜。

目的探讨静脉留置针行股静脉穿刺输液在神经外科脑疝患者术前抢救中的意义。方法将我科334例单侧瞳孔散大的脑疝患者采用随机数字表法随机分为三组。A组(外周静脉输液组):静脉留置针行外周静脉穿刺输液120例。B组(中心静脉输液组):中心静脉穿刺置管输液102例。C组(静脉留置针行股静脉输液组):静脉留置针行股静脉穿刺输液112例。比较三组患者穿刺成功所需时间(T1)、输注20%甘露醇250ml所需时间(T2)、术前瞳孔回缩例数。结果 T1:A组为(2.16±0.82)min,B组为(11.73±4.43)min,C组为(2.29±0.92)min。经t检验:PAB0.05。T2:A组为(35.78±1.69)min,B组为(19.74±1.97)min,C组为(19.81±2.03)min。经t检验:PAB0.05,PAC0.05,χ2=4.341,PBC<0.05,χ2=9.533,PAC<0.01。结论静脉留置针行股静脉穿刺置管输液能快速建立输液通道,且输液速度快,能够快速有效输注20%甘露醇等高渗脱水降颅内压抢救药物,迅速降低颅内压,缓解脑疝,为进一步手术治疗赢得时间,效果优于外周静脉输液及中心静脉输液,且并发症低,在神经外科脑疝患者的抢救中意义明显。

The ^(232)Th(n,f)cross section is very important in basic nuclear physics and applications based on the Th/U fuel *** the time-of-flight method and a multi-cell fast-fission ionization chamber,a novel measurement of the^(232)Th(n,f)cross sec-tion relative to^(235)U in the 1–200 MeV range was performed at the China Spallation Neutron Source Back-n white neutron source(Back-n).The fission event-neutron energy spectra of^(232)Th and^(235)U fission cells were measured in the single-bunch *** 232Th/235U fission cross-sectional ratios were obtained,and the measurement uncertainties were 2.5–3.7%for energies in the 2–20 MeV range and 3.6–6.2%for energies in the 20–200 MeV ***^(232)Th(n,f)cross section was obtained by introducing the standard cross section of^(235)U(n,f).The results were compared with those of previous theoreti-cal calculations,measurements,and *** measured 232Th fission cross section agreed with the main evaluation results in terms of the experimental uncertainty,and 232Th fission resonances were observed in the 1–3 MeV *** present results provide^(232)Th(n,f)cross-sectional data for the evaluation and design of Th/U cycle nuclear systems.

Back-streaming neutrons from the spallation target of the China Spallation Neutron Source(CSNS)that emit through the incoming proton channel were exploited to build a white neutron beam facility(the so-called Back-n white neutron source),which was completed in March *** Back-n neutron beam is very intense,at approximately 29107 n/cm2/s at 55 m from the target,and has a nominal proton beam with a power of 100 kW in the CSNS-I phase and a kinetic energy of 1.6 GeV and a thick tungsten target in multiple slices with modest moderation from the cooling water through the *** addition,the excellent energy spectrum spanning from 0.5 eV to 200 MeV,and a good time resolution related tothe time-of-flight measurements make it a typical white neutron source for nuclear data measurements;its overall performance is among that of the best white neutron sources in the *** with advanced spectrometers,detectors,and application utilities,the Back-n facility can serve wide applications,with a focus on neutron-induced cross-sectional *** article presents an overview of the neutron beam characteristics,the experimental setups,and the ongoing applications at Backn.

We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,NICER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by NICER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.
